Abstract

An aging population has occurred in DKI Jakarta Province, as data show the elderly population has exceeded 10% of the total population since 2020. Human aging occurs naturally, leading to declines in organ function and functional capacity. Innovative health promotion efforts are needed to effectively maintain the independence of the elderly and keep them healthy. The "Elderly School" (Sekolah Lansia/SL) is the intended innovation, initiated by Indonesia Ramah Lansia (IRL) and implemented as policy by the BKKBN, the leading sector in population. This study aims to evaluate the robustness of the elderly health promotion program implemented through the SMART Elderly School in DKI Jakarta since 2022. A mixed-method approach (quantitative and qualitative) was used to assess the implementation of the SMART Elderly School program, describe the characteristics of the elderly school participants in DKI Jakarta, and measure the quality of life of the elderly. The research population consisted of 129 elderly individuals who met the inclusion criteria. Data collection utilized questionnaires and the WHOQOL instrument to measure the quality of life of the elderly. Qualitative data were obtained through document studies, in-depth interviews, and participant observation. Qualitative data validation applied source and method triangulation techniques. Univariate quantitative data analysis was used to describe the characteristics and quality of life of the elderly. Qualitative data analysis used content analysis of in-depth interview transcripts and field notes. The results showed that the quality of life of the elderly school participants was in the high and very high categories, with characteristics varying across education, age, sex, marital status, and medical history over the past 2 (two) years. The implementation of the SMART Elderly School program in DKI Jakarta, as a robust elderly health promotion effort that adopts the concept of lifelong learning, is proven effective in maintaining the quality of life of the elderly.

Abstract

Background: While physical inactivity remains a major, modifiable risk factor for stroke and long-term disability in older adults, sustaining conventional exercise routines often proves challenging. Consequently, gardening and urban agriculture are gaining traction as accessible, sustainable physical activities that actively promote healthy aging. Objective: To synthesize existing evidence, this systematic review evaluates the impact of gardening and urban agriculture as physical activity interventions for stroke prevention in aging populations. Methods: Adhering to PRISMA guidelines, we conducted comprehensive searches across the PubMed, Scopus, and Dimensions databases for literature published between 2015 and 2026. Eligibility was restricted to studies of adults aged 60 and older who engaged in horticultural therapy, community or home gardening, or urban agriculture, with reported outcomes related to cardiovascular health and active aging. Results: Out of 322 initial records, five studies qualified for full qualitative synthesis. The reviewed literature indicates that gardening interventions reliably enhance mobility, physical exertion, cardiovascular health, psychological resilience, and social interaction among older adults. By naturally mitigating sedentary behaviors and improving modifiable cardiovascular risk factors, these activities indirectly contribute to stroke prevention. Conclusion: Although methodological heterogeneity across the studies and a scarcity of controlled experimental trials preclude definitive causal conclusions, the current evidence is highly promising. Gardening and urban agriculture offer viable, cost-effective, and community-integrated strategies to advance active aging, elevate overall well-being, and reduce stroke vulnerability among older adults.

Abstract

Background: Falls among older adults are a major global health concern associated with disability, hospitalization, mortality, and reduced independence. Recent advances in artificial intelligence (AI), wearable Internet of Things (IoT), and machine learning have shifted fall prevention from reactive detection toward proactive risk prediction. Objective: This scoping review mapped current evidence on AI-powered wearable IoT devices for predicting falls in older adults, focusing on sensor technologies, AI models, predictive performance, and implementation challenges. Methods: Following PRISMA-ScR guidelines, literature searches were conducted in PubMed, Scopus, IEEE Xplore, Web of Science, and Cochrane Library databases for studies published between 2008 and 2026. Eligible studies involved older adults, wearable IoT sensors, and AI or machine learning models for fall-risk prediction or pre-impact fall detection. Results: Of 1,100 identified records, 170 full-text articles were screened, and 13 studies met the final eligibility criteria. IMUs, accelerometers, gyroscopes, plantar-pressure sensors, and surface electromyography (sEMG) were the most frequently used sensors. Random Forest, XGBoost, Support Vector Machine (SVM), and Long Short-Term Memory (LSTM) were the dominant analytical approaches. Several laboratory-based systems achieved accuracies above 95%; however, most studies relied on simulated falls in younger individuals rather than on real-world data from older adults. Conclusion: AI-powered wearable IoT systems show strong potential for proactive fall-risk prediction in older adults. However, translational barriers remain, including limited ecological validation, small sample sizes, and the persistent simulation gap. Future research should prioritize real-world validation, explainable AI, and the integration of multimodal sensing.

Abstract

The rapid deployment of Large Language Models (LLMs) across healthcare and conversational technology directly impacts the expanding global older population, a demographic that increasingly relies on these systems for cognitive framing and daily emotional interaction. However, tech governance frameworks largely neglect algorithmic ageism. While developers routinely update codebases to mitigate racial and gender biases, age discrimination remains unaddressed. This study quantifies the depth of this systemic oversight. Through a systematic evaluation of 12 core papers published between 2023 and 2026 in accordance with the PRISMA-ScR guidelines, we identified a critical dual-layered manifestation of AI ageism: explicit ageist values embedded in token-prediction engines and a condescending conversational style that defines benevolent ageism. Younger users generate the vast majority of training data for Reinforcement Learning from Human Feedback (RLHF) loops. Consequently, LLMs replicate and amplify these youth-centric perspectives. Current standard testing benchmarks fail to detect these interactional nuances. To address this methodological blind spot, this paper introduces a dual-stage debiasing framework that implements age-empathetic constraints during preprocessing, data cleaning, and real-time generation. Integrating social gerontology with digital ethics, this model provides a verifiable framework for inclusive conversational AI.

Abstract

Children with chronic conditions are at increased risk of emergency hospital readmission because of disease complexity, recurrent exacerbations, technology dependence, fragmented continuity of care, and socioeconomic vulnerability. Emergency readmissions contribute substantially to healthcare utilization, caregiver burden, and poorer pediatric health outcomes. This scoping review aimed to map and synthesize evidence regarding determinants, utilization patterns, and prevention strategies associated with emergency readmission among children with chronic conditions. The review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews (PRISMA-ScR) guideline. Literature searches were conducted across PubMed/MEDLINE, Scopus, Web of Science, CINAHL, Cochrane Library, SpringerLink, and ScienceDirect for studies published between 2010 and 2025. Of 412 identified records, 28 eligible reports were included following screening, eligibility assessment, and data charting. The included studies consisted predominantly of retrospective cohort studies, administrative database analyses, systematic reviews, and observational studies conducted mainly in high-income countries. Major factors associated with emergency readmission included complex chronic conditions, prolonged length of stay, technology dependence, prior healthcare utilization, inadequate discharge planning, weak care-transition continuity, socioeconomic disadvantage, race and insurance disparities, and limited caregiver readiness. Children with asthma, diabetes, sickle cell disease, neurological disorders, cancer, respiratory illness, and pediatric intensive care unit exposure demonstrated the highest readmission risk. Multifaceted discharge interventions, transitional-care programs, family-centered education, telehealth follow-up, and integrated care coordination showed potential to reduce avoidable readmissions. Overall, pediatric emergency readmission represents a multidimensional healthcare challenge requiring integrated clinical, family-centered, and healthcare-system interventions to strengthen continuity of care and reduce preventable rehospitalization.

Abstract

Quorum sensing (QS) is a bacterial cell-to-cell communication system that plays a central role in regulating virulence, biofilm formation, pathogen adaptation, and infection persistence. The global rise in antimicrobial resistance, driven by conventional antibiotic use, has spurred the development of alternative, non-lethal strategies, including antivirulence approaches. This review aims to examine quorum sensing as an antivirulence target and to evaluate the potential of quorum-sensing inhibition (QSI) to control bacterial infection. A narrative literature review was conducted using scientific publications from 2016 to 2026 retrieved from reputable databases. The review highlights that QS regulates multiple pathogenic determinants, including virulence factor production, secretion system control, biofilm development, colonization processes, and bacterial survival within host environments. QSI-based interventions can be achieved through inhibition of signal molecule synthesis, enzymatic degradation of autoinducers, and receptor competition. This strategy potentially reduces selective pressure that causes resistance since it does not directly affect bacterial viability. Beyond clinical applications in chronic and biofilm-associated infections, QSI also has promising prospects in food safety, environmental management, and medical technologies. However, challenges related to compound stability, in vivo effectiveness, and potential bacterial adaptation require further investigation before broader clinical implementation can be achieved.

Abstract

The prevalence of Pulmonary Tuberculosis in the Municipality of Bobonaro, Timor Leste, is still relatively high and requires serious attention in efforts to control infectious diseases. This study aims to analyze risk factors related to the incidence of Pulmonary Tuberculosis in adults. This study uses a quantitative approach with a case-control design and was conducted in June–July 2025. The sample consisted of 95 cases and 95 purposively selected controls. Data were analyzed using SPSS version 30. The results showed that smoking history was the most dominant factor with a risk of 6.685 times (p < 0.001), followed by occupancy density (OR = 4.313; p < 0.001), firewood use (OR = 3.515; p = 0.003), alcohol consumption (OR = 3.494; p = 0.002), inadequate home ventilation (OR = 3.061; p = 0.007), and history of close contact (OR = 2.667; p = 0.018). Nutritional status factors (OR = 1.916) and access to health services (OR = 2.135) showed risky but not significant trends. These findings emphasize the importance of interventions on behavioral and environmental factors in TB control strategies in Bobonaro.
