Abstract

Extended-spectrum beta-lactamase (ESBL)-producing Gram-negative bacteria were an emerging cause of neonatal infections. However, local data from resource-limited hospitals in eastern Indonesia remained scarce. This study determined the proportion of ESBL production among Gram-negative isolates and analyzed the association of demographic and clinical characteristics with ESBL positivity in neonates. A retrospective cross-sectional study was conducted at a referral hospital in East Lombok, Indonesia, between January 2023 and December 2024. Secondary data were obtained from 117 neonates with Gram-negative isolates. Antimicrobial susceptibility testing was performed using the VITEK 2 Compact system. Data were analyzed using chi-square and multivariable logistic regression. Of 117 neonates, 56 (47.9%) harbored ESBL-producing isolates, most commonly Klebsiella pneumoniae and Serratia marcescens. Early neonatal age was the only factor independently associated with ESBL positivity (adjusted OR 49.40; 95% CI 14.86-164.17), whereas sex, gestational age, and birth weight were not. Early neonatal age was strongly associated with ESBL positivity, suggesting a possible role of maternal or perinatal colonization. These findings supported heightened vigilance and judicious empirical antibiotic use, particularly in the early neonatal period in resource-limited settings.

Abstract

Plastic waste from coastal settlements, fisheries, and marine activities has become a major source of microplastic pollution in the Tamarunang Waters of Jeneponto Regency, where filter-feeding brown mussels (Modiolus modulaides) readily accumulate microplastics. This study evaluated the effectiveness of microplastic depuration using a pump-circulation system with a natural bioadsorbent. A quantitative experimental approach with a completely randomized design (CRD) was employed, comprising five treatments: a control (no depuration) and depuration periods of 1, 2, 3, and 4 days, each replicated three times. Mussels were collected using purposive sampling from sites with high mussel abundance and potential plastic contamination. Of 810 mussels collected, 405 individuals were analyzed (5 treatments × 3 replications × 27 mussels). Data were analyzed using Analysis of Variance (ANOVA) and linear regression. The novelty of this study lies in the use of mashed kepok banana peels as a natural bioadsorbent. The lowest microplastic content was observed after four days of depuration. Although three- and four-day periods showed no significant difference, both were significantly more effective than shorter depuration periods. Depuration efficiency increased from 47.825% at day 0 to 94.132% after four days. Fragments, fibers, and films were the dominant types of microplastics. These findings demonstrate that a banana peel bioadsorbent, combined with a circulation system, effectively reduces microplastic contamination and provides a sustainable, low-cost depuration method. Future studies should validate this approach under commercial-scale and field conditions while assessing its effects on mussel quality and food safety.

Abstract

Reproductive health among young women is influenced by physical, nutritional, and psychosocial factors; however, most interventions address these aspects separately. This study aimed to evaluate the effectiveness of integrating acupressure and nutritional support within a Positive Youth Development (PYD)-based midwifery care model to improve reproductive health among young women. This study employed a quasi-experimental design with a pretest–posttest control group approach. A total of 60 young women aged 15–24 years were selected using purposive sampling and assigned to intervention and control groups. Data were collected using a validated and reliable questionnaire, and the intervention was conducted over six weeks. Statistical analysis was performed using paired-sample t-tests and independent-samples t-tests. The results showed that the mean reproductive health score in the intervention group increased from 60.27 to 82.30, whereas the control group increased from 58.43 to 65.50. A statistically significant difference was observed between the two groups (p = 0.000), indicating that the integrated intervention was more effective than standard reproductive health education. These findings suggest that a midwifery care model integrating acupressure, nutritional support, and Positive Youth Development effectively improves reproductive health among young women. This approach offers a holistic, youth-friendly strategy that may be incorporated into reproductive health promotion and preventive care programs.

Abstract

Renal cell carcinoma (RCC) in Indonesia is frequently diagnosed at an advanced stage. Although immune checkpoint inhibitor-based combinations are recommended as first-line therapy, access remains limited in many public healthcare settings, and pazopanib continues to be widely used in routine clinical practice. This single-center observational study evaluated real-world outcomes of pazopanib in adult patients with stage IV RCC treated at a tertiary referral center in Indonesia between January 2020 and December 2025; patients treated for less than one month were excluded. Overall survival (OS) was estimated using the Kaplan–Meier method, while disease progression and treatment exposure were analyzed descriptively. Sixteen patients were included. Median age was 54 years (range, 24–69), and clear-cell RCC was the predominant histological subtype (93.8%). Disease progression occurred in 8 patients (50.0%), most commonly in the lungs. At the end of follow-up, 5 patients (31.3%) were alive, 5 (31.3%) had died, and 6 (37.5%) were lost to follow-up. Mean OS was 29.5 months (95% CI, 18.2–40.7 months), while median OS was not reached. Pazopanib remains a feasible treatment option for selected patients with stage IV RCC in resource-constrained settings where access to immunotherapy is limited.

Abstract

Adolescent fruit and vegetable (FV) intake remains below recommended levels. In Indonesia, only 2.3% of adolescents aged 10–14 years met recommended intake in the 2023 Indonesian Health Survey. Low nutrition literacy and limited psychosocial support contribute to this problem, while school interventions rarely integrate digital gamification with behaviour-change theories. This study develops a conceptual model linking digital gamification, nutrition literacy, and psychosocial pathways to adolescent FV intake and provides empirical evidence through a school-based intervention. Grounded in the ANIMOE health-promotion model and integrating the Transtheoretical Model, PRECEDE-PROCEED framework, and Social Cognitive Theory, this mixed-method quasi-experimental pretest-posttest control-group study involved 103 junior high school students in Jambi City, Indonesia (intervention n=52; control n=51). FV intake was assessed using a Food Frequency Questionnaire. Within-group changes were analysed using the Wilcoxon signed-rank test (p<0.05). The intervention significantly increased nutrition knowledge (13.62 to 18.88), attitude (89.03 to 110.42), and FV intake (3.00 to 4.91 servings/day; all p<0.05), while controls showed no significant changes. Behavioural intention progressed across TTM stages, with the modal stage shifting from contemplation to action. Parental support and teacher reinforcement also improved. The theory-based gamified intervention improved cognitive, motivational, and behavioural determinants of adolescent FV intake. The proposed model provides a framework for structural validation, while schools represent settings for nutrition promotion.

Abstract

Type 2 diabetes mellitus (T2DM) remains a major public health challenge in primary care, particularly in rural areas where medication adherence, lifestyle practices, family support, diabetes education, and access to healthcare may affect glycemic control and quality of life. This study aimed to identify determinants of blood glucose stability and examine its association with quality of life among patients with T2DM receiving care at Community Health Centers in West Lombok Regency, Indonesia. This quantitative observational analytical study used a cross-sectional design and was conducted from January to April 2026 among 362 patients enrolled in the Chronic Disease Management Program (Prolanis). Data were collected using structured questionnaires, clinical records, and the Diabetes Quality of Life Clinical Trial Questionnaire and analyzed using chi-square tests and multiple logistic regression. Medication adherence (OR = 4.72; 95% CI: 2.91–7.66), family support (OR = 3.58), physical activity (OR = 2.89), diabetes education (OR = 2.41), diet (OR = 2.07), and access to healthcare (OR = 1.62) were significant determinants of blood glucose stability. Stable blood glucose was strongly associated with good quality of life (OR = 7.18; p < 0.001). These findings highlight the importance of integrated primary care interventions that strengthen medication adherence, family involvement, lifestyle management, diabetes education, and healthcare access to improve glycemic stability and quality of life among patients with T2DM in rural settings.

Abstract

The increasing prevalence of chronic kidney disease requiring hemodialysis highlights the importance of understanding patients’ experiences in maintaining quality of life during long-term treatment. Hemodialysis imposes substantial physical, psychological, social, and spiritual challenges that require continuous adaptation. This study aimed to explore the lived experiences of hemodialysis patients in maintaining quality of life. A qualitative study with a phenomenological approach was conducted involving 13 hemodialysis patients selected through purposive sampling. Data were collected through in-depth interviews and analyzed using Interpretative Phenomenological Analysis (IPA). Trustworthiness was ensured through member checking and systematic thematic interpretation. Three major themes emerged: (i) lived experiences and changes in the meaning of life, including initial reactions of shock, fear, denial, and eventual acceptance of hemodialysis; (ii) coping strategies and social support during treatment, encompassing personal coping mechanisms, family and social support, and spirituality as a source of resilience; and (iii) perceptions of quality of life and future expectations, highlighting subjective interpretations of well-being and aspirations to remain healthy, independent, and meaningful in daily life. The findings revealed that hemodialysis patients sustained their quality of life through a dynamic process of adaptation. This process involved managing physical challenges, developing psychological resilience, maintaining supportive social relationships, and strengthening spiritual beliefs, all of which contributed to their overall well-being.

Abstract

This study aimed to develop a head nurse leadership model based on Bureaucratic Caring Theory in Bhayangkara Level III Hospital, Manado. A qualitative action research design was applied involving head nurses selected through purposive sampling. Data were collected through in-depth interviews, reflective leadership practice, and field notes. The data were analyzed using thematic analysis to identify patterns in leadership experiences. Four main themes were generated: bureaucratic structure in nursing units, leadership functions of head nurses including planning, organizing, directing, and controlling, application of caring values in leadership practice, and organizational culture and work ethic related to communication and leadership autonomy. The findings showed that head nurse leadership was not only a structural role but also a dynamic process integrating managerial functions, decision-making, communication, and caring values. The study concluded that an integrated leadership model combining organizational structure, caring values, and work culture was essential to enhance leadership effectiveness and nursing service quality.

Abstract

Type 2 Diabetes Mellitus requires continuous management. Health-Related Quality of Life (HRQoL) and medication adherence are important indicators of therapy success that are often affected by a decline in ongoing understanding. Analyze the effect of INTEGRA (Blood Glucose Telemonitoring Intervention) on HRQoL and medication adherence of Type 2 Diabetes Mellitus patients at the Cilacap Tengah I Community Health Center. This quasi-experimental pretest-posttest study with a control group involved 138 respondents (69 intervention, 69 control) selected by purposive sampling and simple randomization. The intervention group received the INTEGRA program for 12 weeks; the control group received standard care. HRQoL was measured using the WHOQOL-BREF and complemented by the Summary of Diabetes Self-Care Activity (SDSCA). Analysis used Mann-Whitney, Wilcoxon, and ANOVA. The intervention group showed an increase in SDSCA scores; the control did not change significantly. The WHOQOL-BREF intervention score increased, whereas the control remained stable. ANOVA showed INTEGRA increased confounding variables. INTEGRA effectively improves HRQoL and treatment adherence in patients with Type 2 Diabetes Mellitus. The program is recommended for services in primary health facilities.

Abstract

Percutaneous nephrolithotomy (PCNL) is the standard treatment for large renal calculi, yet clinically significant hemorrhage necessitating transfusion remains a critical perioperative challenge. This PRISMA-guided systematic review and meta-analysis aimed to consolidate the evidence on predictors of blood transfusion or severe bleeding after PCNL. Pooled analyses identified diabetes mellitus, hypertension, urinary tract infection, renal anomalies, multiple or staghorn stones, solitary kidney phenotype, multiple access tracts, larger stone size, and longer operative time as significant risk factors. Notably, hydronephrosis demonstrated an inverse association. In conclusion, post-PCNL bleeding requiring transfusion is best conceptualized as a three-domain risk paradigm: patient susceptibility, stone complexity, and procedural intensity.
