Abstract

Epilepsy is the most common chronic brain disorder and can affect people of all ages. Globally, an estimated 60 million people are diagnosed with epilepsy, half of whom are women, and about a third of women with epilepsy are of childbearing age. The main therapy for epilepsy is anti-epileptic drugs to control epileptic seizures. In Indonesia, first-line anti-epileptic drugs are carbamazepine, valproic acid, phenobarbital, and phenytoin, while second-line anti-epileptic drugs are lamotrigine, levetiracetam, clobazam, and topiramate. There are many concerns when women with epilepsy start planning a pregnancy, especially regarding the effect of epilepsy drugs on pregnancy and their effects on the fetus, such as teratogenic effects, neonatal outcomes, and neurodevelopmental impacts. The choice of anti-epileptic drugs (AEDs) in women of childbearing age must be considered carefully.

Abstract

Hand, foot, and mouth disease (HFMD) is an exanthematous disease with the main manifestation of a maculopapular rash eruption on the hands, feet, and mouth accompanied by fever with potentially serious complications in children under five-years-old. Indonesia has a recurring cycle of HFMD outbreaks, making it a re-emergent disease. The author conducted a systematic search in three databases, including Pubmed, Wiley Online Library, and Science Direct, with the keywords "hand, foot, and mouth disease,"enterovirus", and "therapy," published from 2000 until 2023. A total of 791 studies were included in the analysis, and their quality was assessed using the Newcastle-Ottawa Scale. The currently available comprehensive HFMD management includes prevention, therapy, and complication management. Current research has not succeeded in uncovering a definitive treatment; thus, management focused on symptomatics, followed by isolation with good oral and skin care. In severe cases, available options include antiviral therapy and intravenous immunoglobulin, along with respiratory and blood circulation system support. Research has shown that the inactivated Enterovirus A71 (EV-A71) vaccine is the most effective and cost-effective option. However, the evolution of enterovirus epidemiology caused one agent intervention that will be inadequate in the future; thus, developing definitive treatments and effective multivalent vaccines is essential.

Abstract

Introduction: Melanonychia is brown or black pigmentation of the nail plate caused by melanin accumulation. Melanonychia is a common cause of nail discoloration that can progress to malignant melanoma. Methods: A literature review on scientific databases PubMed and Science Direct. The keywords used were “melanonychia”, “early detection”, and “treatment approach”. The inclusion criteria were all scientific articles on early detection methods and approaches to melanonychia management. Results: The initial search obtained 125 scientific articles relevant to the specified keywords. Screening the title and abstract of the study resulted in 45 articles that were downloaded and underwent full-text screening. Five articles met the inclusion criteria, and qualitative analysis was carried out. Conclusion: Early detection of melanonychia is very important to prevent complications from malignant melanoma. Delayed diagnosis occurs due to low patient awareness of melanonychia lesions and their prognosis.

Abstract

Catamenial pneumothorax is a rare condition in the spectrum of thoracic endometriosis syndrome (TES), women of reproductive age experience spontaneous pneumothorax episodes that occur around the time of menstruation. Although infrequent, cases of recurrent pneumothorax accompanied by a history of pelvic surgery, menstrual pain, and infertility should raise clinical suspicion. Diagnosis involves a comprehensive history, radiological examination, and video laparoscopy as the definitive diagnostic method. Management includes symptomatic management with pleural drainage, followed by minimally invasive operative therapy and hormonal therapy for 6-12 months post-initial management. The recurrence of catamenial pneumothorax remains a problem, and its prognosis and impact are not yet fully understood.

Abstract

Hepatorenal syndrome (HRS) is a serious renal complication that frequently develops in patients with decompensated cirrhosis, causes systemic circulatory disturbance. HRS involves complex pathogenesis mechanisms and treatment. Various therapies have been developed to tackle HRS, such as the use of vasoconstrictor agents, intravenous albumin, transjugular intrahepatic portosystemic shunting (TIPS), and liver transplantation.

Abstract

Myocarditis is the inflammation of the cardiac muscle. Diagnosis can be challenging as the symptoms are not typical. Cardiac magnetic resonance imaging (MRI) can be used to diagnose myocarditis, allows more reliable quantitative measurements, and a clear discrimination from other mimicking conditions. We include 3 patients with myocarditis.

Abstract

Myocarditis is a condition characterized by inflammation in the myocardium. Myocarditis is difficult to diagnose, due to its multiple, heterogeneous manifestations with many differential diagnoses. T1 and T2 cardiac mapping were added as a criterion in the revised Lake Louise Criteria (LLC). The revised criteria have been reported to have higher sensitivity. We included two patients diagnosed with myocarditis based on the revised LLC. All patients were diagnosed using cardiac magnetic resonance (CMR), which includes late gadolinium enhancement (LGE) and T1-T2 mapping. LGE is a technique frequently used in CMR to visualize cardiac tissue, whichis most sensitive to fibrosis. Revised LLC includes quantitative measures of T1 and T2 value, and T1-T2 mapping is advantageously related to its ability to quantitatively assess myocardial abnormalities. CMR using LGE combined with T1-T2 mapping allows the diagnosis of myocarditis. Recovery of T1 and T2 values during follow-up allows quantitative measurement of patients’ condition and treatment success.

The abnormal skin wound healing process in the formation of dense fibrous tissue with excessive growth is known as a keloid. Genetic, systemic, and local factors are associated with keloid formation. Abstract

Breast cancer is the most common cancer in Indonesia and one of the biggest contributor to cancer mortality. About 70% breast cancers are detected at an advanced stage which makes the mortality rate even higher. Several measures have been taken to detect breast cancer early, including mammography screening. Mammography is X-ray imaging used to detect abnormalities in breast cancer. The American College of Radiology recommends breast cancer screening for at-risk women starting at age 40. However, whether mammography screening can reduce breast cancer mortality is still debatable.

Abstract

Introduction:  Although SLE prevalence lacks specific Indonesian data, it predominantly affects women of childbearing age. Case: A 24-yearold pregnant woman with a history of SLE experienced worsening shortness of breath, leg swelling, and fatigue since her second month of pregnancy. Hospital admission revealed severe hypertensive crisis, active SLE affecting kidneys, joints, skin, congestive heart failure, and anemia. She underwent rigorous medical treatments and ICU monitoring. Discussion: SLE affects multiple organs. Careful management and monitoring are required during pregnancy for maternal and fetal well-being. Conclusion: Pregnant women with SLE may encounter various pregnancy-related complications. Effective and comprehensive management is vital to achieve favorable outcomes.
