Abstract

Suicide cases in adolescents are a concern to the World Health Organization (WHO) for immediate early detection, prevention, and management. Medical personnel should be more aware of risk factors, signs, and symptoms, and provide solutions. There are internal and external factors that underlie suicidal behavior. The level of suicidal behavior varies from person to person, early detection is needed. Early detection can be done by using screening tools that assess suicidal thoughts and attempts. The important role of the family as the first place to grow is also very much needed. The family will be a support system in the prevention of suicidal behavior. Cross-sectoral collaboration was needed to reduce the incidence of suicide in adolescents. Health workers need to have comprehensive knowledge and understanding of suicidal behavior. Suicidal behavior disorders need to be identified immediately to determine interventions that are appropriate to the patient’s condition. pharmacological and pharmacological therapies are needed to prevent suicidal behavior. Follow-up after therapy needs to be considered in terms of preventing recurrent suicidal behavior.

Abstract

Thyrotoxicosis refers to a syndrome caused by excessive secretion of thyroid hormone (hyperthyroidism). Changes in thyroid hormone levels directly affect the occurrence of abnormal changes in the structure and/or function of the heart, leading to cardiomyopathy, and increase the risk of complications such as heart failure and life-threatening arrhythmias. Global data shows approximately 1% of thyrotoxicosis patients develop dilated cardiomyopathy, leading to severe impairment of left ventricular function and cardiogenic shock. Early diagnosis and proper management are necessary for a better outcome and prognosis.

Abstract

Diabetic ketoacidosis (DKA) is a complication in type 1 diabetes pediatric patients with quite high morbidity and mortality rates. The basic treatment in pediatric DKA is fluid and electrolyte correction. Hydration can reduce counterregulatory hormones, increase renal glucose clearance, and increase insulin sensitivity. The purpose of administering fluids is to fill intravascular, interstitial and intracellular fluids. There is still debate regarding the risks of cerebral edema or even pulmonary edema.

Abstract

Junctional bradycardia occurs when electrical activity at the SA node is disrupted, or there is a conduction block originating from the SA node, or its automaticity is less than the AV node or HIS bundle. A 46-year-old female was referred because of shortness of breath and swollen limbs. The patient has a long history of diabetes and hypertension. She was diagnosed with acute pulmonary edema, junctional bradycardia, stage 4 CKD, CHF, hypoalbuminemia, anemia, and T2DM. The patient was admitted to the ICCU for 7 days before being discharged. Cardiorenal syndrome can induce sinus node dysfunction or AV node dysfunction, leading to junctional bradycardia. Subsequently, the junctional bradycardia exacerbates concomitant heart failure and renal failure.

Abstract

Rabies is a fatal zoonotic disease caused by animal bites, with the majority of cases occuring in Asia, including Indonesia. About 30% to 60% of animal bite victims are children under 15 years old. Most rabies deaths are caused by delayed treatment because it is considered harmless, whereas prevention by providing prophylaxis before and after exposure can prevent rabies deaths. Meanwhile, the treatment for rabies is still limited and has a poor prognosis.

Abstract

The incidence of diabetic ketoacidosis (DKA) in children with type 1 diabetes mellitus reaches 1–12 per 100 patients each year. The case is a boy aged 13 years 10 months with chief complaints of weakness and drowsiness. Blood sugar levels 532 mg/dL, blood ketone levels 6.6, blood pH 7.13, HbA1c 19.2%. The principles of DKA management include fluid therapy, insulin, electrolyte balance, treat the underlying disease, and monitor complications.

Abstract

Alcohol has profound effects on glucose metabolism. Alcohol-induced hypoglycemia is rare in healthy individuals with good glycogen reserve, but may be life-threatening in individuals with silent chronic liver dysfunction, due to liver inability to manage glucose uptake. A 39-year-old male was admitted to the emergency room with sudden convulsion after taking local alcoholic beverages. A very low blood glucose level and significantly elevated liver enzyme levels were detected. The patient was stable after 3 days of intensive care without repeated phase of hypoglycemia.

Abstract

Erythroderma, also known as exfoliative dermatitis, is a diffuse inflammation-related skin disorder characterized by redness and scales affecting more than 90% of the body surface area. Its incidence is around 1 per 100,000 adults, with a mortality rate of 16% each year, especially in immunodeficient patients. Several conditions as the main cause include psoriasis (23%), atopic dermatitis (16%), drug hypersensitivity reactions (15%), and cutaneous T-cell lymphoma (CTCL) or Sezary syndrome (5%). Erythroderma is a potentially life-threatening condition requiring diagnosis, identification of the etiology, and appropriate management.

Abstract

Type 2 diabetes mellitus is a progressive chronic metabolic disease characterized by an increase in blood sugar due to insulin resistance. Uncontrolled diabetes will cause various acute and chronic complications. The case is a young DM patient with complications of hyperosmolar hyperglycemia status, cerebral ischemia, and pressure ulcers.

Abstract

Interstitial lung disease (ILD) encompasses a group of heterogeneous lung disorders with varying etiologies and clinical presentations. Early diagnosis is essential for timely intervention and improved patient outcomes. This mini-review highlights the significance of early diagnosis of ILD in primary health centers, focusing on the benefits and challenges associated with early detection, and the role of primary healthcare professionals in achieving a timely diagnosis. Primary healthcare professionals play a pivotal role in recognizing early signs and symptoms of ILD, conducting preliminary evaluations, and guiding patients through the diagnostic process. Collaborative efforts between primary healthcare professionals, pulmonologists, and radiologists are crucial to overcome these challenges and ensure timely diagnosis.
