Abstract

Introduction: Congenital syphilis is a multi-organ infection caused by Treponema pallidum and transmitted from mother to fetus through uteroplacental circulation. Congenital syphilis is one of the main contributors to neonatal deaths in the United States. Case: A male infant was born by cesarean delivery from a G1P0 mother at 28 weeks of gestational age because of premature rupture of the membrane and anhydramnion. The mother had an untreated latent syphilis infection. The birthweight was 1,400 grams, the head circumference was 25 cm, and the body length was 35 cm. His APGAR score was 3-3-5 with severe asphyxia and respiratory distress syndrome. Multiple abnormalities were found, and the VDRL titre was 1/128. Cardiac-pulmonary resuscitation started immediately with 0.15 mg epinephrine through an intraendotracheal tube and continued with 0.02 mg epinephrine through intravenous access every 3 minutes. The baby was declared dead after 30 minutes of resuscitation. Conclusion: Congenital syphilis can cause lethal complications due to untreated latent syphilis suffered by his mother. The mother should undergo antenatal screening to prevent congenital syphilis.

Abstract

Atopic dermatitis (AD), characterized by recurrent eczematous lesions and pruritus, is a major contributor to the total global burden of disability. The common management practice includes patient education and topical emollients and is heavily reliant on the use of anti-inflammatory agents with systemic corticosteroids. Monoclonal antibodies provide a novel, targeted approach towards specific cytokines. This study aims to highlight the novel systemic therapeutic drug aimed at specific targets. A comprehensive literature review utilizes databases including EMBASE, EBSCOhost, PubMed, Scopus, and Wiley Online Library, using keywords: “monoclonal antibody” AND “therapy” AND “atopic dermatitis”. Relevant papers containing different updates in atopic dermatitis management and the utilization of monoclonal antibodies are included in this review. The current regimen of monoclonal antibodies offers better management of moderate-to-severe AD. Monoclonal antibodies have become a promising therapy as they demonstrate better therapeutic effects in terms of skin barrier protection and anti-inflammatory and pruritusreducing effects. However, its application still faces various challenges such as side effects and infections. The heterogeneity and relapsing nature also add to the complexity and risk profile of AD management.

Abstract

Introduction: HIV infection and malnutrition are strongly linked and correlated. Although the exact mechanism is unknown, malnutrition is suspected to disrupt antiretroviral (ART) drug absorption. This research is aimed at determining the relationship between nutritional status and immunological response in children with HIV/AIDS taking ART. Methods: A cross-sectional retrospective analytic study was conducted on 30 children with HIV/AIDS in the pediatric clinic at Wangaya Hospital, using medical record data. The sample was taken with purposive sampling. Nutritional status was defined as poor (weight per height [WHZ] < -2 for children <5 years old and BMI for age [BAZ] <-2 for children ≥5 years old) and good nutrition (WHZ ≥-2 and <+1 for <5 years old children and BAZ ≥-2 and <+1 for ≥5 years old). Immunological response was seen by the increase in CD4 level through a certain time span and categorized into poor (did not increase or increase below the average) and good response (increase above the average). The data was analyzed using Chi-Square. Results: There is no significant relationship between nutritional status and immunological response in children with HIV/AIDS (p = 0.660). There was an almost equal amount of each immunological response category for both good and poor nutritional status. Conclusion: This study showed no difference in immunological response among children with either good or poor nutritional status. Further research on other confounding factors that may affect immunological responses is needed.

Abstract

With its locally invasive nature, cholesteatoma has the potential to cause fatal complications. However, many remain undetected until advanced stages. Otalgia is suggested as the indicator of impending intratemporal or intracranial complication and is commonly presented before episodes of characteristic foul-smelling purulent discharge. As an earlier diagnosis of this condition is needed, this study aims to find whether the presence of otalgia has a prognostic value of cholesteatoma formation in adult patients with chronic suppurative otitis media. Literature searching using Pubmed, Cochrane, and ScienceDirect was done from the 12th to 16th of March 2022. Two articles were selected and appraised using the Center of Evidence-Based Medicine Oxford tool for prognostic study. Both studies retrospectively found no statistically significant patient characteristics or clinical presentation findings including otalgia that might guide the distinction between chronic suppurative otitis media on first hospital admission with and without cholesteatoma. Even though all studies have good validity, applicability, and precise prognostic estimates owing to their narrow 95%CI, considerable discrepancies between the two studies exist, therefore clinical interpretation remains questionable. Otalgia does not have a significant prognostic role in cholesteatoma formation in patients with chronic suppurative otitis media.

Abstract

Atopic dermatitis, also known as atopic eczema, is a common chronic, recurrent inflammatory condition of the skin. This condition is most commonly experienced in childhood, especially aged 0 to 12 months. The most frequent symptom is itching. This disease greatly affects the patients’ quality of life, psychology and productivity, requiring long-term treatment. To this day, the management of atopic eczema refers to conventional treatment using generic medicines such as oral anti-allergies (antihistamines) and both topical and oral corticosteroids. This article examines potentials of virgin coconut oil as an alternative treatment for atopic dermatitis, various methods of virgin coconut oil production, as well as the composition of the active compounds of virgin coconut oil and their roles in the management of atopic dermatitis.

Abstract

The mechanism of preeclampsia is still unknown, so early diagnosis and termination of pregnancy are definitive therapies. A prediction model to be implemented in a low-resource setting is needed to predict the risk of pre-eclampsia in pregnant women. Pre-eclampsia prediction models also play a role in clinical decision-making, assisting with information, education, and communication (IEC) and considering the administration of aspirin prophylaxis. This study aims to systematically review the development of pre-eclampsia prediction models in a low-resource setting. PubMed, ScienceDirect, and Wiley Online Library databases between January 2019 and June 2023 were systematically reviewed. Article identification, screening, and selection of relevant articles, as well as data extraction, were carried out independently by the authors following PRISMA guidelines. Of the six articles that met the requirements, models that used maternal characteristics, risk factors, and physical examination. Laboratory tests improved the accuracy of pre-eclampsia prediction models in a low-resource setting. Examination with Doppler ultrasound and biomarkers could significantly improve the sensitivity and specificity of prediction models but could not be universally applied in a low-resource setting.

Abstract

Introduction: Maxillofacial trauma is related to head trauma, mostly caused by motorcycle traffic accidents in Sumba. The limitations in diagnostic evaluation at rural hospitals present unique challenges to healthcare providers. This research describes some characteristics of maxillofacial trauma cases in Waikabubak General Hospital, West Sumba, East Nusa Tenggara. Methods: A retrospective descriptive study based on medical records between 2018 – 2021. Results: A total of 37 patients were included; the majority were in the 16 to 64 year-old age group (78%), with males (81%). Motorcycle road traffic accidents (73%) were the main cause, and the most common fracture site was the mandibula (35%). Skull AP lateral plain radiography was the most commonly used x-ray technique (49%). Cases were mostly discharged against medical advice (54%). Conclusion: Motorcycle road traffic accidents were the main cause of maxillofacial injuries, and the mandibula was the most common site of fractures.

Abstract

Introduction: West Sumba, East Nusa Tenggara, is known as one of the areas with limited healthcare facilities. Burn injury can happen to anyone from all sorts of backgrounds. This research was conducted to observe the pattern of burn injury in Waikabubak General Hospital. Methods: A retrospective descriptive study was conducted using medical records data at Waikabubak General Hospital from 2019-2021. Results: Thirty-five burn injuries patients were evaluated. The majority of patients were male (71%) and ranging from the age of < 2 - 12 years old (54%). Scald burns by hot water were the most common cause (54%). The extremities were the common site of injury (34%), with the mean total body surface area (TBSA) was 16.8%, and the most common complications were severe dehydration (9%) and death (9%). The main comorbid disease was epilepsy (11%), most had a length of stay of 5-10 days, with an average of 6 days, and the most frequent intervention was debridement (54%). Most patients were discharged with improved condition (74%). Conclusion: Children and patients with epilepsy were at risk. It is important to equip caregivers and family members with specific preventive strategies and first aid training programs.

Abstract

Background: Stunting is defined as low height-for-age caused by chronic or recurrent undernutrition. Undernutrition is the leading risk factor for tuberculosis (TB) globally. It is usually associated with poverty, poor maternal health and nutrition, frequent illness and/or inappropriate feeding and care in early life. Stunting prevents children from reaching their physical and cognitive potential, so appropriate management is needed. Case: weight for age but severely stunted and suggestive TB. Blood study result showed anemia with Mentzer index >13. Bone age showed the age of A 4 year-old girl was referred with breathing difficulty since 2 weeks accompanied with decreased appetite and weight. She was in normal 3 years and below chronological age. Management was focused on TB, nutritional, and anemia treatment. Conclusion: Nutritional assessment should be integrated into standard TB care and also simultaneously treat iron deficiency anemia.

Abstract

A case of a 58-year-old woman with lepromatous leprosy and severe ENL reaction is reported. The patient has symptoms of pustules for 1 year and the nodules were getting worse in the last week, accompanied by fever, joint pain, nausea, and vomiting. Physical examination revealed madarosis, infiltrates of both ears, and multiple painful erythematous nodules on the extremities and trunk. Slit skin smear examination showed bacterial index +3 and morphological index of 15%. Histopathological examination revealed neutrophil infiltration and vasculitis in the upper dermis. The patient was diagnosed with lepromatous leprosy and severe ENL reaction. The patient received multidrug therapy and methylprednisolone (62.5 mg/day), which was gradually reduced. The ENL reaction is caused by the deposition of M. leprae antigens and antibodies in small blood vessels, resulting in vasculitis and the release of enzymes that damage tissues. ENL reactions are commonly found in multibacillary MH and occur before, during, and after treatment. This reaction is mediated by cytokines produced by T-helper-2 (Th2). ENL reactions can cause disability.
