Abstract

Pendahuluan: Sindrom metabolik ditandai dengan hipertensi, obesitas sentral, resistensi insulin, dan dislipidemia. Hylocereus polyrhizus dipercaya dapat mengobati sindrom metabolik. Metode: Penelitian eksperimental laboratorik dengan desain pretest dan posttest control group. Sebanyak 30 tikus dibagi menjadi 5 kelompok: K1 dan K2 kontrol negatif dan positif, kemudian K3, K4, K5 kelompok yang diberi ekstrak Hylocereus polyrhizus dengan dosis berturut-turut 60, 120, 180 mg/200 gBB. Data dianalisis dengan uji one-way ANOVA, uji Tukey HSD, uji T berpasangan, dan uji korelasi Pearson. Hasil: Kadar HDL tertinggi terdapat pada K5 dan terendah pada K1. Uji korelasi Pearson menunjukkan dosis Hylocereus polyrhizus mempunyai hubungan sangat kuat dengan peningkatan kadar HDL. Simpulan: Dosis ekstrak Hylocereus polyrhizus 60, 120, 180 mg/200 gBB dapat meningkatkan kadar HDL secara signifikan. Pada penelitian ini, dosis paling optimal adalah 180 mg/200 gBB.

Abstract

Infertility caused by the absence of the uterus or its physiological capabilities is called absolute uterine factor infertility (AUFI). One of the causes of AUFI is Mayer-Rokitansky-Küster-Hauser syndrome (MRKH), a congenital abnormality that results in the absence of or failure to develop a uterus, without any interference of secondary sex characteristics. The only proven therapy to overcome infertility in RMKH syndrome is uterine transplantation. However, there are ethical problems with the uterus transplant procedure, especially regarding the nature of the syndrome, which is not a life-saving intervention but is limited to life-enhancing purposes.

Abstract

Introduction: Most people with ureteral duplications never experience any symptoms, so the condition goes unreported despite being one of the most prevalent kidney anomalies. Case: A 31-year-old female visited our medical facility for a routine checkup. Kidney function and complete urine analysis were normal. Ultrasonography revealed what appears to be a duplex collecting system in the right kidney, with two distinct noncommunicating renal pelvices. A 3D computed tomography (CT) scan of the abdomen with contrast revealed a duplex collecting system in the right kidney. Conclusion: Radiologists play a crucial role in detecting these abnormalities.

Abstract

Candida albicans is a commensal organism that can may become invasive and fatal under certain circumstances. Several genetic variations in the human genome towards susceptibility to Candida infections have been identified. There are a number of genes associated with Candida virulence factors, host environment, and immune response. C-type lectin receptors (CLRs) are characterized by leucine-rich and nucleotidebinding domains that can detect intracellular signals through various types of cytoplasmic domains. CLRs are family of PRRs that bind glycan through recognition of extracellular carbohydrate and mediate intracellular signaling through various type of cellular domains, resulting in a decrease of neutrophils and facilitating the occurrence of fungal infections. This study review the genetic determinant of defense mechanisms and immune deficiencies against Candida infection.

Abstract

Introduction: Erythrodermic psoriasis (EP) is a rare type of psoriasis and can be life-threatening. EP requires intensive observation because the large area of the scaly skin increases the risk of secondary infection and sepsis, disrupts the regulation of body temperature, and causes loss of protein, electrolytes, and fuids. Case: Male, 37 years old, with reddish and scaly skin all over the body since 1 day, accompanied by pain and itch. The patient had a history of psoriasis with routine treatment. Discussion: Diagnosis is based on history and typical clinical fndings. The management is intravenous fuids, antibiotics, corticosteroids, antihistamines, and a combination of topical therapy. Conclusion: Erythrodermic psoriasis (EP) is a rare and potentially life-threatening type of psoriasis that requires close bservation, as it can lead to shock and organ failure.

Abstract

Fahr’s syndrome is a collection of symptoms accompanied by symmetrical and bilateral abnormal calcification in the basal ganglia, thalamus, dentate nucleus, cerebral cortex, cerebellum, and hippocampus. Fahr syndrome can occur in young to middle-aged adults. Some of the most important etiologies are endocrine disorders, infections, and autoimmune diseases. The clinical symptoms are very diverse. Diagnostic criteria are established. One of the routine imaging examinations is a head CT scan. There is no specific therapy for Fahr syndrome, the treatment focuses on the symptoms and the underlying disease.

Abstract

Background: During the COVID-19 pandemic, teaching and learning activities have to be conducted online. Many students have difficulty in dealing with online learning, and it may become a stress factor. Stress may cause several dermatological disorders such as acne. Objective: To determine the relationship between stress caused by online learning and acne severity during the COVID-19 period. Methods: A cross-sectional study with observational analytic research design. The population is Medical Study program students, Faculty of Medicine, Tanjungpura University. The sampling method uses the Proportionate Stratified Random Sampling technique with a minimum of 169 samples; 57 third-year students, 55 second-year students, and 57 first-year students. Stress level was measured with the MSSQ questionnaire, acne severity was measured with GAGS. Bivariate analysis used the Spearman correlation test. Results: A total of 62.7% of males and 46.8% of females experienced a moderate level of stress on online learning. A total of 66.7% of males and 56.4% of females had moderate acne. Spearman’s test for the relationship between online learning stress levels and acne severity obtained a value of 0.048 (p <0.05) and r = 0.152. Conclusion: The relationship between the stress level of online learning and the severity of acne among Medical Study program students at, the Faculty of Medicine, Tanjungpura University was very weak. Abstract

Testicular cancer accounts for 1% of all malignancies and 5% of urological-related neoplasms; most commonly in young males aged 15-40 years, seminoma is the most common subtype (50%-60%). A 72-year-old patient with increasing right scrotum size since 3 years ago. A right testicular tumor was found. Trans-scrotal radical orchiectomy was done as the definitive treatment. The result of the histopathological examination was a classic seminoma pT1. GCT is a testicular cancer uncommonly diagnosed in the elderly. A better prognosis is achieved through early diagnosis and appropriate treatment.

Abstract

Background: The most common extrapulmonary TB is lymph node TB in 35% cases of worldwide. Drug-induced hepatitis (DIH) is the possible adverse effect of antimycobacterial agents. Case: An 18-year-old male with multiple tender lumps on the right side of his neck for the last 2 weeks. Physical findings were multiple masses in the right anterior supraclavicular and submandibular region. Biopsy revealed suppurative granulomatous lymphadenitis. The diagnosis is TB lymphadenitis and the first regimen of anti-TB was given, but the patient developed nausea and vomiting and increased AST/ALT after 5 days. Anti-TB was temporarily discontinued. Anti-TB regimen was resumed after clinical and laboratory returned to normal. Conclusion: Patients with TB and DIH need to be treated individually depending on their tolerance to the drugs. Careful clinical and laboratory monitoring was needed to prevent adverse effects and for best results.

Abstract

Background: Acute generalized exanthematous pustulosis (AGEP) is a severe cutaneous adverse reaction most commonly triggered by medications, particularly antibiotics. AGEP is characterized by the rapid onset of numerous small, sterile, non-follicular pustules on an erythematous base. Case: A 38 year-old female with a rapidly progressing rash, fever, and malaise after taking cefxime for surgical wound. Physical examination revealed difuse, erythematous plaques with numerous small, non-follicular pustules distributed to almost the entire body except the face. Laboratory fndings were leukocytosis and neutrophilia. Patient was treated with dexamethasone and ampicillin sulbactam injections. But skin reactions were severely worsened accompanied by a worsened vital signs, necessitating Intensive Care Unit (ICU) monitoring. The total EuroSCAR score is 9, categorized as defnite AGEP. Skin resolution occurred after 5 days of corticosteroid therapy. Discussion: The use of antibiotics is one of the triggering factors for AGEP. In this case, skin reactions appeared within 24-48 hours after drug exposure. Conclusion: AGEP is a rare clinical condition. Identifying and discontinuing the causative drug is crucial for successful treatment.
