Abstract

Vitiligo berpotensi menimbulkan dampak psikososial pada pasien, sehingga diperlukan terapi yang aman dan efektif. Saat ini belum ada terapi untuk menyembuhkan; tidak ada terapi tunggal yang dapat berhasil baik pada semua pasien, tetapi banyak pilihan untuk mengatasi lesi vitiligo. Modalitas terapi antara lain terapi topikal, terapi oral, terapi sinar, terapi bedah, terapi psikologis, dan terapi komplementer. Dengan meningkatnya penelitian dan pengetahuan tentang patomekanisme vitiligo, beberapa agen topikal baru dikembangkan untuk menginduksi pigmentasi.   Vitiligo has potential psychosocial impact on patients. There is currently no therapy to treat vitiligo, but many options are available to treat vitiligo lesions. No single therapy can produce good results in all patients; many treatment options are topical treatments, oral treatments, light therapy, surgical therapy, psychological therapy, and complementary therapies. Increasing research and knowledge on vitiligo patomechanism including relation with oxidative stress, skin blood flow, or a cytokine-mediated mechanisms neuropeptides lead to newly developed topical agents to induce pigmentation

Abstract

Background. Febrile seizure is the most frequent neurological disorder in childhood. Some studies report association between anemia and febrile seizure. Even though anemia is highly prevalent in Indonesia, only one study assessed this association. Objective. To compare the hemoglobin level in children with febrile seizure and febrile children without seizure, and to assess the relationship between anemia and febrile seizure. Method. This retrospective case control study used medical record of febrile children aged 6 months to 3 years old hospitalized at Landak General Hospital from May 2016 to April 2017. Hemoglobin level from first blood workup then compared between fifty children with first-time febrile seizure (case group) and a hundred febrile children without seizure (control group). Results. Hemoglobin level was lower in case group compared to control group (p =0.000). Children with febrile seizure were more likely to be anemic compared to febrile children without seizure [OR 6.73 with 95% CI 3.18 to 14.26]. Conclusion. There is significant difference of mean hemoglobin among children with febrile seizure compared to febrile children without seizure. Children with febrile seizure were six times more likely to have anemia.   Latar belakang. Kejang demam adalah gangguan neurologi yang paling sering ditemukan pada anak. Beberapa penelitian menemukan hubungan antara anemia dan kejang demam. Meskipun prevalensi anemia tinggi di Indonesia, hanya satu studi meneliti tentang hubungan ini. Tujuan. Untuk membandingkan kadar hemoglobin pada anak dengan kejang demam dan anak demam tanpa kejang, serta menilai hubungan antara anemia dan kejang demam. Metode. Penelitian retrospektif kasus kontrol, menggunakan data rekam medis anak dengan demam berusia 6 bulan hingga 3 tahun yang dirawat di RSUD Landak dari bulan Mei 2016 hingga April 2017. Kadar hemoglobin dari pemeriksaan sampel darah pertama pada 50 anak demam dengan kejang pertama kali (kelompok kasus) dibandingkan dengan pemeriksaan 100 anak demam tanpa kejang (kelompok kontrol). Hasil. Rerata kadar hemoglobin lebih rendah pada kelompok kasus dibandingkan kelompok kontrol (p =0.000). Anak demam dengan kejang cenderung dalam kondisi anemia dibandingkan anak demam tanpa kejang [OR 6.73, 95%IK 3.18 - 14.26]. Simpulan. Terdapat perbedaan signifikan rerata kadar hemoglobin antara anak demam dengan kejang dibandingkan dengan anak demam tanpa kejang. Anak demam dengan kejang memiliki kecenderungan 6 kali lipat menderita anemia.

Abstract

Osteoporosis sering ditemukan dalam praktik sehari-hari di Indonesia. Osteoporosis memiliki karakteristik berupa penurunan densitas mineral tulang/bone mineral density (BMD). Faktor risiko osteoporosis antara lain keturunan, diet rendah kalsium, merokok, konsumsi berlebihan alkohol dan kafein, penggunaan steroid jangka panjang, dan latihan fisik inadekuat. Untuk kelompok berisiko tinggi, disarankan pemeriksaan DXA untuk skrining dan diagnosis osteoporosis. Komplikasi osteoporosis antara lain fraktur vertebra, non-vertebra, dan pinggul. Salah satu terapi osteoporosis adalah bisfosfonat, khususnya yang mengandung nitrogen (alendronate, risedronate, ibandronate, zolendronate).   Osteoporosis is a common problem found in daily practice in Indonesia. Osteoporosis is characterized by the decline in bone mineral density (BMD). Risk factors of osteoporosis are family history of osteoporosis, low calcium diet, smoking, overconsumption of alcohol and caffeine, long term steroid usage, and inadequate physical activity. DXA examination for screening and diagnosis is suggested for high risk group. The complications of osteoporosis are vertebral, non-vertebral, and hip fractures. On of osteoporosis therapy is bisphosphonate, especially nitrogen containing bisphosphonates (alendronate, risedronate, ibandronate, zolendronate).

Abstract

Pendahuluan. Kondiloma akuminata raksasa (KAR) atau disebut juga tumor Buschke-Lowenstein adalah tumor akibat infeksi human papillomavirus (HPV) di daerah anorektal dan genitalia eksterna dengan diameter lebih dari 2,5 cm. Kasus. Seorang laki-laki homoseksual, 32 tahun dengan benjolan yang bertambah banyak dan makin membesar sejak 2 bulan. Pasien terinfeksi HIV sejak 2 tahun yang lalu. Kombinasi krioterapi dan larutan KOH 5% memberikan perbaikan klinis. Diskusi. Terapi kombinasi dapat dijadikan pilihan terapi KAR disertai penyulit atau jika terapi tunggal tidak memberikan perbaikan klinis.   Background. Giant condyloma acuminata also known as Buschke-Lowenstein tumor is a tumor caused by human papillomavirus (HPV) infection in anorectal area and external genitalia with diameter more than 2.5 cm. Case. A 32 year old homosexual male had multiplying and growing lumps since two months. Patient was diagnosed HIV since 2 years ago. A combination therapy with cryotherapy and KOH 5% solution resulted in clinical improvement. Discussion. Combination therapy may be an option for giant condyloma acuminata with complications or if single therapy does not provide clinical improvement.

Abstract

Background: Exclusive breastfeeding is considered as one of many crucial factors in child development. The research was conducted with the aim of to determine the relationship between exclusive breastfeeding and child development. Method: An observational analytic study with cross-sectional design; the independent variable was exclusive breastfeeding and the dependent variable was child development according to KPSP score. A total of 86 samples were obtained using consecutive sampling method. Data were analyzed using the Chi-square test. Result: A significant positive correlation between exclussive breastfeeding and child development (p=0.035). Conclusion: There is a correlation between exclussive breastfeeding and child development in children at Wangaya Denpasar Regional Hospital.

Abstract

Seiring dengan berkembangnya teknologi, laser dan intense pulsed light (IPL) menjadi pilihan modalitas terapi hiperpigmentasi. Kedalaman penetrasi sinar laser meningkat seiring meningkatnya panjang gelombang. Laser dan IPL dengan panjang gelombang pendek efektif untuk lesi hiperpigmentasi superfisial. Sedangkan lesi hiperpigmentasi dermal dan campuran lebih merespons terhadap laser dengan panjang gelombang yang lebih panjang. Pemilihan parameter laser serta persiapan sebelum dan setelah tindakan yang tepat dapat mengurangi risiko efek samping.   With the rapid evolution of technology, laser and intense pulsed light (IPL) have become one of the modalities of choice for hyperpigmentation therapy. The depth of laser beam penetration increases with increasing wavelength. Laser and IPL with short wavelength are effective to treat superficial hyperpigmentations. While dermal and mixed type hyperpigmentations have better response towards lasers with longer wavelength. Accurate laser parameter choice and good pre and post procedural preparations reduce the risk of side effects.

Abstract

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are acute life-threatening mucocutaneous reactions characterized by extensive epidermal necrosis and epidermal detachment. The incidence of SJS in Japan is 2.3 cases/100,000 people/per year. The mortality rate of SJS-TEN in Saudi Arabia is 1 per 10 cases (10%). SJS-TEN can be caused by drugs, idiopathic, infection, post-bone marrow transplant, immunization, radiotherapy, and genetics. The pathogenesis of SJS-TEN is suspected as a result of a cytotoxic reaction to keratinocyte cells which causes massive apoptosis. Clinical manifestation begins with prodromal symptoms followed by erythematous macular lesions and atypical target lesions that may confluence, then develop into bullous, oozing erosions with mucosal involvement. Vital sign observation is important to predict prognosis. A diagnostic procedure is necessary if another diagnosis is being considered. Management of SJS-TEN includes cessation of suspected drugs, early treatment such as fluid resuscitation, early nutrition, treatment and evaluation of eyes and skin lesions. Multiorgan involvement can occur in SJS-TEN so multidisciplinary management is needed for optimal therapy. Corticosteroids and cyclosporin A provide beneficial effects on SJS-TEN. SCORTEN is used to evaluate prognosis.

Abstract

Latar Belakang: Beberapa studi menunjukkan peranan asam urat dalam progresivitas pre-diabetes menuju diabetes. Tujuan: Mengetahui hubungan kadar asam urat serum dengan HbA1c pada pasien DM tipe 2 di RSUP Dr. Wahidin Sudirohusodo Makassar. Metode: Penelitian retrospektif observasional cross-sectional menggunakan data sekunder 107 pasien DM tipe 2 yang memenuhi kriteria inklusi dan menjalani pengobatan di poliklinik dan perawatan Interna RS Wahidin Sudirohusodo (RSWS) periode Januari-Desember 2016. Uji normalitas data menggunakan uji Kolgomorov Smirnov dengan hasil tidak terdistribusi normal. Korelasi kadar asam urat dengan HbA1c dianalisis menggunakan uji Spearman dengan nilai p<0,05. Hasil: Dijumpai korelasi positif yang signifikan antara kadar asam urat dan HbA1c pada pasien DM tipe 2 (r=0,229; p=0,018). Simpulan: Kadar asam urat serum memiliki korelasi positif dengan HbA1c pada pasien DM tipe 2.   Background: Some studies suggest the role of uric acid in progression of prediabetes to diabetes. Objective: To determine the correlation between uric acid serum and glycated haemoglobin (HbA1c) among type 2 diabetes mellitus patients in Wahidin Sudirohusodo Hospital. Method: A cross-sectional observational retrospective study conducted on 107 type 2 DM patients who fulfilled the inclusion criteria and underwent treatment at Wahidin Sudirohusodo Hospital during January-December 2016. Data is not normally distributed. The correlation of uric acid with HbA1c was analyzed using Spearman test with p-value <0.05. Result: A significant positive correlation between uric acid levels and HbA1c in patients with type 2 DM (r = 0.229, p = 0.018)

Abstract

Jeringau Merah (Acorus sp.) telah digunakan secara empiris oleh masyarakat suku Dayak Kalimantan Barat untuk menyembuhkan penyakit. Penelitian ini bertujuan untuk mengetahui efek antiinflamasi dan dosis optimum ekstrak etanol rimpang Jeringau Merah pada tikus jantan galur Wistar. Perlakuan pada lima kelompok, yaitu kelompok kontrol positif diberi natrium diklofenak, kontrol negatif diberi CMC-Na 1%, dan kelompok uji diberi ekstrak etanol rimpang Jeringau Merah 225 mg/kgBB, 450 mg/kgBB, dan 675 mg/kgBB. Pengujian dengan metode rat hindpaw edema pada telapak kaki kiri tikus jantan galur Wistar dengan induksi karagenan 2%. Nilai AUC digunakan untuk menghitung besarnya daya antiinflamasi tiap kelompok perlakuan dibandingkan dengan kelompok kontrol negatif. Data dianalisis dengan uji ANOVA dan uji LSD. Hasil menunjukkan ekstrak etanol rimpang Jeringau Merah mempunyai aktivitas antiinflamasi dengan dosis optimum ekstrak 450 mg/kgBB dan daya antiinflamasi sebesar 34,20%.   Red Jeringau (Acorus sp.) has been used empirically to cure diseases in West Kalimantan. This study aims to examine the antiinflammatory effects and optimum dose of ethanol extract of Red Jeringau rhizomes in Wistar male rats. The samples were divided into five groups, positive control group was given diclofenac sodium, negative control group was given CMC-Na 1%, and the extract groups were given 225 mg/bw, 450 mg/bw and 675 mg/bw ethanol extract of red Jeringau rhizomes. The test was on artificial inflammation in left foot of Wistar male rats induced with carrageenan 2%. AUC values were used calculate the antiinflammatory effects on each group compared to control group. Data were analyzed by one-way ANOVA and LSD test. The results showed that ethanol extract of Red Jeringau rhizomes has antiinflammatory activity and the optimum dose was 450 mg/kgBB with antiinflammatory percentage 34,20%.
