Abstract

Hiponatremia adalah gangguan elektrolit karena total air tubuh yang berlebih dibandingkan dengan total jumlah natrium. Hiponatremia dapat diklasifikasikan menurut osmolalitas, status volume tubuh, dan osmolalitas urin. Ketoasidosis diabetik dengan kadar gula darah tinggi biasanya akan menyebabkan hiponatremia dilusional. Pada laporan kasus ini akan dibahas klasifikasi hiponatremia dan kejadiannya pada pasien ketoasidosis diabetik atau pada pasien hiperglikemia.   Hiponatremia adalah gangguan elektrolit karena total air tubuh yang berlebih dibandingkan dengan total jumlah natrium. Abstract

Introduction: Distance or online learning conducted during the COVID-19 pandemic causes a lack of interaction among students which affects the level of empathy of these students. Online learning also causes stress in medical students which can lead to burnout. The purpose of this study was to determine the relationship between the level of empathy and the level of burnout among students of the Tanjungpura University Medical Study Program class of 2019. Methods: The method used in this research is an observational analytic study with a cross-sectional approach. The research population was students of the Tanjungpura University Medical Study Program class of 2019 who were taken using a sampling technique in the form of total sampling with a minimum sample size of 76 people. Bivariate analysis using Spearman correlation test. Results: The results obtained from the Spearman correlation test based on SPSS 24.0 analysis obtained a significance value of 0.768 (p>0.05) for the relationship between the level of empathy and the level of burnout. Conclusion: There is no relationship between the level of empathy and the level of burnout in students of the Medical Study Program at Tanjungpura University class 2019.

Abstract

Konsep analgesia multimodal dianjurkan untuk pengobatan nyeri. Paracetamol dan nonsteroidal antiinflammatory drugs (NSAID) umumnya menjadi analgesik dasar, serta dikombinasikan dengan opioid sesuai kebutuhan. Analgesik intravena (IV) dapat menjadi solusi bagi pasien yang tidak dapat menerima analgesik per oral. Kombinasi paracetamol dan ibuprofen IV menunjukkan efek analgesik yang signifikan, dengan manfaat opioid sparing, antipiretik, serta dengan profil keamanan yang relatif baik pada pasien nyeri akut.   The concept of multimodal analgesia is recommended for the treatment of pain. Paracetamol and nonsteroidal anti-inflammatory drugs (NSAIDs) are generally the basic analgesics, and are combined with opioids as needed. Intravenous (IV) analgesics may serve as an alternative for patients who cannot tolerate oral analgesics. The IV combination of paracetamol and ibuprofen for acute pain shows a significant analgesic effect with the benefits of opioid sparing, also with antipyretic effect, and with a relatively good safety profile.

Abstract

Perubahan tekanan, suhu, tingkat kelembapan, keadaan imobilisasi, suara bising, turbulensi, dan ekspansi udara di rongga tubuh yang berkaitan dengan perjalanan via udara dapat berdampak pada tubuh manusia. Para penumpang dengan riwayat penyakit jantung harus menyadari risikonya dan melakukan tindakan pencegahan.   Changes in pressure, temperature, humidity levels, immobilization, noise, turbulence, and expansion of air in the body cavity associated with air travel can all have an impact on the human body. Passengers with a history of heart disease must be aware of the risks and take the necessary precautions.

Abstract

Hydrops fetalis (HF) is an abnormal fluid collection in two or more fetal compartments. HF is categorized as immune HF (IHF) and non-immune HF (NIHF). IHF is due to rhesus (Rh) incompatibility and NIHF is due to various pathologies. Management is according to underlying etiologies. Prognosis is determined by early diagnosis with ultrasonography (USG), medical intervention, and appropriate delivery time.

Abstract

Introduction: Tihi-tihi and Selangan Islands, floating village areas in Bontang, East Kalimantan, are remote areas in Indonesia, which can only be accessed by sea route. The research was conducted to describe skin diseases in those islands. Method: Retrospective observational cross-sectional and descriptive analysis study on floating communities in Tihi-tihi and Selangan Island, Bontang Lestari Village, South Bontang District, Bontang City, East Kalimantan, Indonesia. Results: The study was conducted on 95 subjects, mostly female (53%), and the largest age group was 26-45 years (29%), followed by the 5-11 years group (24%). Skin infection diseases (58.96%) were dominated by scabies (67.9% of the skin infection disease group). The second highest was allergic contact dermatitis (41%) from the non-infection group. There is an association between age and disease group (p<0.05), with an odds ratio of 3.164. Conclusion: Skin diseases in Tihi-tihi and Selangan Islands are dominated by scabies, followed by allergic contact dermatitis. The pediatric age group is at a three-fold greater risk of developing skin infections than non-infection skin diseases.

Sodium-glucose Co-transporter 2 inhibitor (SGLT2i) is one of the antidiabetic classes recommended for patients with heart and kidney problems. Evidence-based studies show the benefits of SGLT2i extend to heart, kidney, or other metabolic diseases in patients without diabetes mellitus.

Abstract

Introduction: Hyponatremia (<135 mmol/L) is one of the most common electrolyte disturbances in clinical practice. Correct treatment can prevent worse outcomes. A case of hyponatremia supposedly induced by amlodipine besylate was presented. Case: A 74 year-old male with lethargy and occasional nausea and admitted to having chronic hyponatremia in the past years. He’s been taking clopidogrel, simvastatin, and amlodipine besylate regularly since 7 years ago and had a history of coronary angioplasty 7 years ago. His lowest serum sodium level was 128 mmol/L even though he already took a 500 mg of sodium chlorine once daily. His serum sodium level never reached the normal range despite increasing salt intake and fluid restriction. Discontinuation of amlodipine besylate and change to nebivolol result in increased serum sodium level to 131 mmol/L. Conclusion: This case of hyponatremia may be induced by amlodipine.    Pendahuluan: Hiponatremia (<135 mmol/L) adalah salah satu gangguan elektrolit yang paling umum dalam praktik klinis sehari-hari. Tata laksana yang tepat dapat mencegah prognosis lebih buruk. Laporan ini memaparkan kasus hiponatremia diduga disebabkan oleh amlodipine besylate. Kasus: Laki-laki berusia 74 tahun datang dengan lesu dan mual sesekali dan mengaku menderita hiponatremia kronis dalam beberapa tahun terakhir. Dia telah mengonsumsi clopidogrel, simvastatin, dan amlodipine besylate secara teratur sejak 7 tahun yang lalu dan memiliki riwayat angioplasti koroner 7 tahun yang lalu. Kadar natrium serum terendahnya adalah 128 mmol/L, meskipun sudah mengonsumsi kapsul natrium klorida 500 mg sekali sehari. Kadar natrium serumnya tidak pernah mencapai kisaran normal, meskipun asupan garam dan pembatasan cairan sudah ditingkatkan. Amlodipine besylate dihentikan dan diganti dengan nebivolol; setelah itu kadar natrium serum menjadi 131 mmol/L. Simpulan: Kasus hiponatremia ini mungkin diinduksi oleh amlodipine.

Abstract

Introduction: Tenofovir disoproxil fumarate (TDF), an antiviral nucleoside analog reverse transcriptase inhibitor, has been used for the treatment of HIV and HBV in the past decades. TDF nephrotoxicity has been reported and could lead to renal failure and Fanconi Syndrome (FS). Case: A 58-year-old female HBsAg carrier with TDF treatment since 2013 presented with osteomalacia. She had glucosuria, albuminuria, vitamin D insufficiency, hypophosphatemia, and occasional hypokalemia episodes. Laboratory results showed amino aciduria, which indicates renal tubulopathy. The diagnosis of Fanconi syndrome was confirmed. Withdrawal of TDF was followed by improvement. Conclusion: The acquired FS associated with TDF is reversible. The use of TDF-containing antiviral regimens should be followed by screening recommendations to detect early Fanconi syndrome.

Abstract

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are life-threatening mucocutaneous reactions with severe skin involvement; may affect many internal organ systems with significant psychosocial complications. Several studies have documented physical complications associated with Stevens-Johnson syndrome and toxic epidermal necrolysis (TEN). SJS/TEN patients require multidisciplinary care to deal with psychological complications and reduced quality of life.
