Abstract

Kelainan uterus dan tuba Falopi merupakan penyebab utama infertilitas. Patologi uterus dan tuba dapat berupa malformasi kongenital atau yang didapat. Beberapa metode pencitraan seperti laparoskopi, fluoroskopi, sonografi infus salin, dan histerosalpingografi (HSG) telah digunakan untuk menilai patologi uterus dan tuba. Meskipun laparoskopi adalah modalitas pilihan untuk menyelidiki patensi tuba dan struktur panggul, HSG merupakan metode diagnostik awal infertilitas karena kemudahan, akurasi, dan risiko komplikasi. Tinjauan ini mengilustrasikan beberapa fitur radiografi kelainan struktur pada uterus dan tuba Falopi.   Abnormalities of the uterus and fallopian tubes are major causes of infertility. Uterine and tubal pathology may be congenital or acquired malformations. Several imaging methods such as laparoscopy, fluoroscopy, infusion saline sonography, and hysterosalpingography (HSG) have been used in the assessment of uterine and tubal pathology. Although laparoscopy is the modality of choice for investigating tubal patency and pelvic structures, HSG is the initial diagnostic method for infertility screening because of its ease of performance, accuracy, and risk of complications that benefit from the size, contour, and surface anatomy of the uterus and fallopian tubes. Abstract

Background: Malaria remains a major health problem in Indonesia; P. falciparum is responsible for the majority of severe cases and malaria-related deaths. This report presents a case of severe tropical malaria complicated with acute kidney injury treated in a remote area with limited resources. Case: A 25-year-old male with a 9-day history of fever presented to the emergency department with severe headache, nausea, jaundice, abdominal pain, and hemoglobinuria. Microscopic examination of the blood revealed P. falciparum parasites. Laboratory results also showed anemia and thrombocytopenia, bilirubinuria, proteinuria, elevated liver enzymes, and pre-renal azotemia. The patient received antimalarial treatment (dihidroartemisinin/piperaquine once daily for 3 days and a single dose of primaquine) and supportive therapy. He was discharged in a stable condition on the fifth day of hospitalization. Conclusion: Early detection and management is crucial for the patient’s prognosis.

Abstract

Cellulite is a topographic change of the skin commonly found in the buttocks and thighs in the majority of post-pubertal women. Cellulite in males is usually accompanied by hormonal disorders. Histological changes were found in the dermis, adipose tissue, and septa. The pathophysiology of cellulite is yet to be determined, several factors are suspected.

Abstract

Hydroa vacciniforme (HV) is a rare photodermatoses with the most frequent onset in childhood. The exact pathogenesis of HV is still not known. This disease is a spectrum with almost similar clinical symptoms, but with a very different prognosis. Classic HV has the best prognosis, but affects the quality of life due to scar formation, while hydroa vacciniforme-like lymphoma (HVLL) is the variant with the worst prognosis. HV needs to be considered as a differential diagnosis, especially in childhood-onset photodermatoses. Classical HV must be distinguishes from hydroa vacciniforme-like lymphoproliferative disorders (HVLPD) because it is related to prognosis. There is still no consensus or guidelines on diagnosis and management for HV.

Abstract

Introduction:  Detection of hypokalaemia by ECG accompanied by detailed history and physical examination will greatly facilitate diagnosis, initiate potassium correction therapy, and prevent further complications due to various cardiac arrhythmias and neurological dysfunctions. Case: A 40-year-old man with weakness in both leg muscles since last night, starting from the legs up to the thighs. Initial ECG showed sinus rhythm with a heart rate at 80 bpm, first-degree heart block (PR interval 240 ms); Q wave in III lead; “apparent” prolonged QTc interval at 551 ms; ST segment depression in I, II, and V3-V6 leads; ST segment elevation in aVR lead; T-wave inseparable from large U wave formed giant U wave that remarkedly seen in V2-V4 leads. Laboratory tests revealed very low serum potassium at 1.94 mmol/L Conclusion: In severe hypokalaemia, T and U waves will merge and form a giant U wave. Emergency and primary physicians should be familiar with the ECG changes of hypokalaemia, especially the giant U wave.

Abstract

Craniopharyngioma is a benign neuroepithelial tumor around the pituitary gland. Two types of craniopharyngioma based on their morphology are the more common adamantinomatous type (ACP) and the papillary type (PCP). The most common symptoms at diagnosis are hypothalamic/pituitary deficiency, visual disturbances, and increased intracranial pressure. The goal of surgery, whether total or subtotal resection, is decompression of the optic tract and ventricle. Radiotherapy after subtotal resection gives better results; it can be done in children >3 years old, but in practice, it is more often done in children >5 years old. Management of craniopharyngioma requires a multidisciplinary approach.

Abstract

Sunlight contains ultraviolet rays such as ultraviolet A (UVA) and ultraviolet B (UVB) rays have adverse effects on the skin such as photoaging, immunosuppression, photocarcinogenesis, and exacerbation of photodermatosis. Photoprotection is an important preventive measure. The use of sunscreen is an important and essential component; development of ultraviolet (UV) filters, sunscreens with topical antioxidants, and others, are current sunscreen innovations. However, sunscreens do not prevent visible light radiation hence the development of nontopical oral and subcutaneous photoprotection that has systemic effect. Development of physical photoprotection is by adjusting the type, color, and thickness of material. Other photoprotection is from ozone layer, weather, and selection of fabric or glass used as shades.

Abstract

Introduction Crusted scabies is more often found in immunocompromised conditions. This disease is also associated with various problems, such as delayed diagnosis, secondary infection, and mortality due to sepsis. Case A 17-year-old boy with itching and scaly skin between fingers and toes for one month. Antifungal therapy resulted in no improvement. The patient has had a history of leprosy reactions when treated with steroids since 2019. Crusted lesions were covered with extensive scales and thick hyperkeratosis between the fingers and toes. There is also leucocytosis. The clinical diagnosis was crusted scabies with a secondary infection. Patient was given clindamycin, cetirizine, 0.9% NaCl compresses, coconut oil, and 5% permethrin. On day 14, lesions improved.

Sepsis is a life-threatening organ dysfunction caused by dysregulation of the immune response to infection. Antithrombin III is a single plasma bound glycoprotein and include a class of serine protease (serpine) inhibitors; its synthesis in the liver decreases during sepsis.

Abstract

Approximately 15-20% of breast cancers harbor human epidermal growth factor receptor 2 (HER2) overexpression and/or amplification. This subtype of breast cancer is associated with aggressive behavior, high recurrence rates, and worse prognosis. However, with the introduction of HER2-targeted therapy, trastuzumab, the survival rates have greatly improved. In patients with HER2 positive advanced disease, taxane in combination with trastuzumab and pertuzumab is given in the first-line setting. In the second-line setting and beyond, several emerging anti-HER2 therapies are becoming available including trastuzumab deruxtecan, tucatinib, T-DM1, neratinib, lapatinib, pyrotinib, margetuximab.
