Abstract

Supratentorial intracerebral hemorrhage (ICH) is the second most common stroke and can potentially have poor outcomes. Citicoline has been researched extensively for ischemic stroke yet has little evidence for hemorrhagic stroke. This study aims to critically appraise evidence regarding efficacy of citicoline in improving functional status and neurological deficit in supratentorial ICH. The literature search utilized 5 databases: PubMed, Embase, Cochrane, Scopus, EBSCOHost. From 802 studies identified at the start of the search, 5 articles were selected for critical appraisal. A study by Zang demonstrated improvement in the NIHSS score (p=0.015) and Barthel index (p=0.001). A study by Mousavi showed improvement in the Barthel index (p=0.008) when compared to the control, but no difference when compared to the baseline in both groups. A study by Iranmanesh demonstrated improvement in the motoric strength (p=0.019). A study by Chua demonstrated improvement in the Barthel index at 3 months follow-up (p<0.05). However, there are no differences in the modified Rankin scale and NIHSS score. A study by Secades demonstrated improvement in the NIHSS score (p<0.001) when compared to baseline, but no difference when compared to the control. Citicoline has the potential to improve patient mobility in supratentorial intracerebral hemorrhage stroke, but in terms of improvement in functional status and neurological impairment requires further research.

Abstract

Background: Head injuries can reduce brain function with high morbidity. Several studies showed that citicoline may improve mild, moderate, and severe head injuries. Objective: To review the effectiveness of citicoline for cognitive improvement in head injury patients. Methods: Systematic search of 2 databases, ScienceDirect and Pubmed. Selected articles were then critically reviewed and analyzed. Results: Studies by Levin (1991) and Calatayud, et al, (1991) in Secades, et al. (2021); and Shokouhi, et al, (2014) showed that citicoline may improve cognitive function in adults and elderly with mild, moderate, and severe head injuries (p <0.05). Conclusion: Citicoline is effective in improving cognitive function in head injury patients.

Abstract

Necrotizing enterocolitis (NEC) remains one of the most common gastrointestinal diseases in neonates, with high morbidity and mortality. An 8-day-old full-term baby girl was admitted with an initial diagnosis of sepsis and acute diarrhea due to a cow’s milk allergy. Her condition worsened on the 4th day of hospitalization with abdominal distention, bradypnea, and vomiting. An abdominal x-ray showed increased bowel gas. She had a slightly lowered FT4. The findings met Bell’s criteria for the diagnosis of NEC. After being given meropenem and levothyroxine for 7 days, she was discharged on the 12th day of hospitalization in good condition. NEC is allegedly to be a multifactorial disease, with predisposing factors of cow’s milk formula (CMF) feeding or not breast-fed since birth, sepsis, and hypothyroidism. The combination of clinical symptoms, radiologic findings, and risk factor tracing is important in the diagnosis and management of NEC.

Abstract

Coronary artery bypass grafting (CABG) is the most ideal and frequently performed procedure for multi-vascular coronary artery stenosis, but its long-term prognosis is unfavorable. A 64-year-old male with a history of coronary artery graft surgery seven years ago complained of recurrent chest pain radiating to the jaw, relieved by isosorbide dinitrate. Physical examination, electrocardiography, and echocardiography test results were within normal limits. Laboratory results showed critical blood urea nitrogen (BUN) values requiring hemodialysis, on angiographic examination showed stenosis in all grafted and native coronary arteries. The diagnoses were grade III coronary artery disease and renal failure. Percutaneous coronary intervention using a drug-eluting stent (DES) in the saphenous vein graft conduit of the right coronary artery was performed, along with dual antiplatelet agents, b-blockers, nitrates, and statins.

Abstract

Anthrax is a zoonotic disease caused by the bacterium Bacillus anthracis. The source of anthrax transmission to humans is usually through contact with infected products or with infected ruminants such as goats, sheep, cattle, and buffalo. Anthrax is a priority for the Ministry of Health and the Ministry of Agriculture’s zoonotic control program. Addressing the significant public health impacts, economic losses, and bioterrorism threats requires a comprehensive interdisciplinary and community response. The knowledge and skills of medical personnel for earlier diagnosis and to provide appropriate treatment need to be increased by updating guidelines and cross-sector collaboration.

Abstract

The incidence of sexually transmitted infections in man who have sex with man (MSM) is increased among human immunodeficiency virus (HIV) patients. HIV and syphilis are two sexually transmitted diseases that frequently interact and modify the symptoms and signs of the disease. A case of a 27-year-old male with generalized multiple erythematous plaques including palms and soles since 4 days ago was reported. The patient has been diagnosed with HIV since 2017. The patient is sexually active with a male sex partner and rarely uses condoms. Diagnosis of syphilis is made based on anamnesis, physical examination, and laboratory tests. The treatment was benzathine penicillin G 2.4 million units in 3 doses. The combination of HIV and syphilis needs longer therapy and requires serological follow-up for up to 24 months.

Abstract

Background: In the development of pharmacological therapy for dementia, especially Alzheimer's disease, several types of therapy are widely used: acethylcholine esterase inhibitors/AChEI, N-methyl-D-aspartate (NMDA) receptors inhibitors, and cholinergic precursor. Aim: To review the evidence on the effectiveness of citicoline supplementation to the standard therapy of dementia to prevent the progression or improve the manifestations of dementia. Methods: A literature search was performed in three following databases: Pubmed, EBSCOhost, Scopus, and Cochrane within the last ten years. Results: Of the 239 articles, five studies were eligible for critical appraisal. Four studies showed a significant mean MMSE score increase in the citicoline + standard therapy group (intervention) compared to the standard therapy group (control) during the follow up period between 3 – 12 months. One study showed a significant mean MMSE score increase of in the intervention group compared to the control group. Conclusion: Oral citicoline supplementation of 1,000 mg/day to the standard therapy showed a positive effect on cognitive function in dementia patients. Critical appraisal of validity and application of the included studies should be considered upon daily clinical application.

Abstract

Low level laser therapy (LLLT) is used in almost all areas of modern medicine, including cosmetic dermatology. LLLT has been beneficial for the treatment of wrinkles, acne vulgaris, hypertrophic scars, hair loss, cellulite, and burns. LLLT can reduce UV damage both as a treatment and as a prophylaxis. In pigmentation disorders such as vitiligo, LLLT can increase pigmentation by stimulating melanocyte proliferation and reducing depigmentation. The non-invasive nature and near absence of side effects of LLLT encourage further research in cosmetic dermatology.

Abstract

Numerous inflammatory, neoplastic, and genetic skin disorders are associated with interstitial lung disease (ILD), a fibrosing inflammation of lung parenchyma with significant morbidity and mortality. There is inconsistent data regarding the association between bullous pemphigoid and pneumonia. This article details the clinical manifestations of bullous pemphigoid (BP) and pemphigus vulgaris (PV) associated with interstitial pneumonia, specific diagnostic strategies, underlying mechanistic connections, and possible treatments.

Abstract

Latar Belakang. Acetosal dan clopidogrel merupakan agen antiplatelet yang sering digunakan dalam pencegahan stroke iskemik. Tujuan. Mengetahui kejadian rekurensi stroke iskemik dengan terapi antiplatelet di poliklinik saraf Rumah Sakit Tingkat III Brawijaya Surabaya. Metode. Penelitian cross-sectional deskriptif non-eksperimental. Data rekam medis pasien stroke iskemik dengan terapi antiplatelet periode Agustus – November tahun 2022 di Rumah Sakit Tingkat III Brawijaya Surabaya. Hasil. Didapatkan 245 pasien stroke iskemik, di mana 146 pasien (59,59%) adalah laki-laki, 140 pasien (57%) dalam rentang usia 19-64 tahun, 158 pasien (64,49%) dengan hipertensi, 80 pasien (32,65%) dengan diabetes melitus, dan 39 pasien (15,92%) dengan jantung. Politerapi kombinasi acetosal dan clopidogrel pada 5 pasien (1,6%), monoterapi antiplatelet pada 240 pasien, 123 pasien (50,2%) menggunakan acetosal. Pasien stroke pertama sebanyak 96,3%. Simpulan. Penderita stroke iskemik terutama pada laki-laki usia 19-64 tahun dengan penyakit komorbid terbanyak adalah hipertensi. Jenis obat terbanyak adalah monoterapi acetosal. Kejadian rekurensi jarang.   Introduction. Acetosal and clopidogrel are antiplatelet agents often used for ischemic stroke prevention. Objective. To observe stroke recurrence among ischemic stroke patients with antiplatelet therapy at Neurology Clinic in Brawijaya level III Hospital Surabaya, Indonesia. Methods. A cross-sectional descriptive non-experimental research on data from medical records of ischemic stroke patients with antiplatelet therapy during August-November 2022 in Tingkat III Brawijaya Hospital, Surabaya, Indonesia. Result. During the survey period, 245 ischemic stroke patients were recorded, 146 patients (59,59%) were male, and 140 patients (57%) aged between 19-64 year-old. Hypertension was found in 158 patients (64.49%), 80 patients (32.65%) with diabetes mellitus, and 39 patients (15.92%) with heart disease. Acetosal and clopidogrel combination was used by 5 patients (1.6%). The majority of 240 patients (98,3%) used monotherapy, whereas 123 patients used acetosal (51,2%). A proportion of 96.3% were first stroke patients. Conclusion. Most ischemic stroke patients are male, aged 19-64 year-old with hypertension as the most common comorbidity. The majority used acetosal monotherapy. Recurrent strokes are rare.
