Abstract

Background: Neonatal hypoglycemia is a metabolic problem that is often found in the NICU and is one of the causes of permanent brain damage in infants. The diagnosis of persistent hypoglycemia and its causes are still controversial. Case: A male baby was born to a mother with severe preeclampsia at 36 weeks of gestation, with a birth weight of 1600 grams. At 1 hour of age, he experienced hypoglycemia that lasted more than 72 hours with the lowest blood sugar of 18 mg/dL, and experienced hyperkalemia (potassium level 6.1 mmol/L). The baby is diagnosed as persistent hypoglycemia. The clinical condition improved after being given dextrose and dexamethasone, and was discharged in stable condition. Conclusion: Management of persistent hypoglycemia depends on the cause. Diagnosis refers to risk factors before or after birth.

Abstract

Stroke is a major cause of disability and cognitive impairment, accounting for 5.2% deaths worldwide. Antiplatelets and anticoagulants have been widely used as prevention of recurrent stroke. Recently, lumbrokinase has been used in stroke and other cardiovascular patients, but further research is needed.

Abstract

Carbon monoxide (CO) poisoning is one of the leading environmental emergencies with high mortality and morbidity. The brain and heart are especially at-risk organs of being affected. Clinical manifestations may closely mimic various nonspecific viral illnesses. Diagnosis is made based on of clinical symptoms, patient history, and the location’s circumstances. Current therapy recommendations for CO poisoning are 100% normobaric oxygen (NBO) or hyperbaric oxygen (HBO). Non-invasive ventilation CPAP (continuous positive airway pressure) method is effective as a therapy for CO poisoning in the emergency room because it can provide tissue oxygenation and eliminate CO quickly.

Abstract

Pituitary apoplexy defined as bleeding or infarct on pituitary gland often occurs in undiagnosed pituitary tumour. Mechanisms thought to trigger pituitary apoplexy are fluctuations in blood pressure, hormonal stimulation of the pituitary gland, coagulation disorders, or vascular disorders. The case is a male, aged 73 years, from India, with complaints of chest pain in the last four hours. Electrocardiography (ECG) and laboratory examination results lead to an acute coronary syndrome (ACS) event. Patients are given fondaparinux, aspirin, and clopidogrel. On the 4th day of hospitalization, the patient experienced a severe headache, projectile vomiting, and double vision. Radiological examination reveals a pituitary tumour with signs of diffuse haemorrhage compressing the optic chiasma, suggesting a pituitary tumour apoplexy. Hormone function tests show a non-functioning pituitary tumour. The blood thinners were discontinued, vitamin K and dexamethasone were given to reduce the patient symptoms. After a few days, the headaches and double vision began to improve. Head MRI showed stable mass of bleeding. Pituitary tumour may be a relative contraindication to dual antiplatelets and anticoagulants in acute coronary syndromes especially in group with renal or liver comorbidities.

Abstract

Background: Otitis media with effusion (OME) is the presence of middle ear fluid without acute ear infection signs or symptoms. OME is one of the causes of hearing loss. Chronic asymptomatic OME could be managed by careful waiting for 3 months because it is a self-limiting disease. During surveillance, autoinflation is a beneficial intervention with minimal side effects and high adherence. Objective: To evaluate the effectiveness of autoinflation to improve hearing in otitis media with effusion and hearing loss. Methods: Literature searching was conducted through PubMed, Cochrane, ScienceDirect, Scopus, and EBSCOHost with inclusion and exclusion criteria. The articles were selected and underwent critical appraisal using rapid critical appraisal of systematic review and level of evidence by the Oxford Centre for Evidence-based Medicine in 2011. Result: One systematic review and meta-analysis, or RCT, met the criteria. This article revealed a pooled estimate of RR 1.74; 95% CI [1.22-2.50] with a Z-score of 3.02 (p=0.0025). This result showed substantial significant (p=0.01) heterogeneity (I2=69%). Conclusion: OME patients with hearing loss who performed autoinflation >1 month had a 1.74 times greater chance of achieving hearing improvement than controls. Autoinflation with the Politzer method gives significant results in improving hearing loss. Large-scale RCT research is recommended to evaluate the effectiveness of autoinflation with the Politzer method or other available methods in OME patients with hearing loss.

Abstract

People with diabetes mellitus have a dysregulated immune system and are more susceptible to infection, which poses a higher risk of severe influenza complications. Current evidence for influenza vaccine effectiveness in people with diabetes mellitus indicates a beneficial reduction of morbidity and mortality. In addition, serological studies show antibody levels elicited from influenza vaccination in diabetes patients show comparable results with healthy adults. Although more controlled clinical trials in diabetes mellitus patients with influenza are needed to reflect the effect of vaccination, current data indicate the need for routine influenza vaccination in diabetes mellitus patients, as recommended by guidelines. Efforts should be made to increase vaccination coverage in this risk group.

Abstract

Latar Belakang: Mahasiswa Fakultas Kedokteran dengan kecerdasan emosional dan perilaku perfesional yang baik akan dapat memberikan kualitas pelayanan terbaik, sehingga memaksimalkan pelayanan kesehatan. Kecerdasan emosional juga diperlukan untuk pengembangan perilaku profesional. Tujuan: Mengetahui hubungan kecerdasan emosional dengan perilaku profesional mahasiswa Fakultas Kedokteran Universitas Tanjungpura angkatan 2019. Metode: Studi analitik observasional jenis cross-sectional. Populasi penelitian merupakan mahasiswa Fakultas Kedokteran angkatan 2019. Teknik pengumpulan sampel, yaitu total sampling dengan subjek penelitian 171 orang. Analisis bivariat menggunakan uji korelasi Spearman. Hasil: Mahasiswa Program Studi Kedokteran Fakultas Kedokteran Universitas Tanjungpura angkatan 2019 sebagian besar memiliki tingkat kecerdasan emosional sedang dan memiliki perilaku profesional di bawah rata-rata. Terdapat hubungan positif bermakna antara variabel pengendalian diri, empati, dan keterampilan sosial pada kecerdasan emosional terhadap perilaku profesional mahasiswa Fakultas Kedokteran Universitas Tanjungpura angkatan 2019. Background: Faculty of medicine students with high emotional control will manage their feelings and emotions well to provide the best quality of service. Students need to develop their professional behavior from the beginning of educational process. Purpose: To determine the relationship between emotional intelligence and professional behavior among Faculty of Medicine students, Tanjungpura University batch 2019. Method: A cross-sectional observational analytical study. The respondents were Faculty of Medicine students, Tanjungpura University batch 2019 with a total sampling of 171 respondents. Bivariate analysis used the Spearman correlation test. Result: Students of the Medical Study Program, Faculty of Medicine, Tanjungpura University batch 2019 mostly have a moderate level of emotional intelligence and below-average professional behavior. There is a significant positive relationship between emotional intelligence and professional behavior on the variables of self-control, empathy, and social skills.

Abstract

Sindrom Tolosa Hunt adalah nyeri periorbital atau orbital unilateral serta paresis satu atau lebih saraf kranial III, IV, dan/atau VI, yang disebabkan oleh peradangan sinus kavernosus, fisura orbital superior, dan rongga orbital. Gejala berkurang dalam 72 jam setelah terapi corticosteroid yang adekuat. Diagnosis yang tepat memungkinkan intervensi segera, dengan demikian dapat mencegah keparahan termasuk kehilangan penglihatan. Tolosa Hunt syndrome is unilateral periorbital or orbital pain and paresis of one or more cranial nerves III, IV, and/or VI caused by inflammation of the cavernous sinus, superior orbital fissure, and orbital cavity. Symptoms will relieve within 72 hours after adequate corticosteroid therapy. Appropriate diagnosis of the disease can allow for immediate intervention and thus prevent severity including loss of vision.

Abstract

Otak beroperasi dalam lingkungan yang sangat rumit dan membutuhkan pengaturan elektrolit secara tepat. Salah satu elektrolit utama dalam tubuh yang juga penting untuk aktivitas otak adalah natrium. Hiponatremia dapat menyebabkan pembengkakan otak yang berbahaya bagi fungsi otak yang dapat dikaitkan dengan keluaran pasien yang lebih buruk. Hiponatremia dapat mengancam jiwa, sehingga membutuhkan tata laksana segera dan tepat. The brain operates in a very complicated environment and requires precise regulation of electrolytes. One of the main electrolytes in the body that is also essential for brain activity is sodium. Hyponatremia can cause brain swelling which is harmful to brain function and can be associated with poorer patient outcomes. Hyponatremia can be life-threatening, thus requiring immediate and appropriate management.

Abstract

Chronic obstructive pulmonary disease (COPD) is one of the leading causes of death worldwide. Pulmonary rehabilitation is an essential component of COPD management and can be delivered through telerehabilitation programs. COPD telerehabilitation consists of exercise testing and training that can be performed at home with supervision from a physiotherapist or remotely using a smartphone application. Exercise training includes cardiorespiratory endurance, resistance, flexibility, and respiratory muscle training. COPD telerehabilitation is effective in improving exercise tolerance, reducing the incidence of acute exacerbations, and improving patients’ quality of life.
