Abstract

Tinitus merupakan persepsi suara tanpa adanya sumber suara eksternal. Walaupun tidak berbahaya, tinitus sangat memengaruhi kualitas hidup. Sampai saat ini belum ada pengobatan untuk tinitus, namun konseling dan edukasi yang tepat dapat membantu pasien menangani stres yang berhubungan dengan tinitus.   Tinnitus is the perception of sound without external source. Although not considered serious, tinnitus greatly affects quality of life. There is no established cure for tinnitus, but appropriate counseling and education could help patients dealing with stress associated with tinnitus.

Abstract

Herpes zoster adalah penyakit neurokutan yang disebabkan oleh virus varicella zoster laten. Lansia dengan diabetes melitus lebih rentan terkena infeksi ini. Kasus: Pria, usia 65 tahun, dengan keluhan lenting disertai nyeri seperti ditusuk di punggung dan dada kiri sekitar 5 hari lalu. Pasien mengidap diabetes melitus tipe 2 (DM tipe 2), rutin mengonsumsi obat hipoglikemik oral (OHO). Diagnosis berdasarkan anamnesis dan temuan klinis yang khas; terapi dengan antivirus, analgetik, dan tetap mengonsumsi OHO.   Herpes zoster is a neurocutaneous disease caused by latent varicella zoster virus. Elderlies with diabetic melitus are more susceptible to this infection. Case: A 65-year old male patient with grouped vesicle and stabbing pain in left side of his back and chest since 5 days. The patient had type 2 diabetes mellitus (type 2 DM) treated with oral hypoglycemic agent. Diagnosis was based on anamnesis and typical clinical findings, and treated with antiviral, analgetic, and continued OAD.

Abstract

Latar Belakang: Keganasan pada ekstremitas bawah, kulit penis, vulva/vagina, dan anus berisiko mengalami metastasis KGB inguinal. Diseksi KGB inguinal dapat memberikan hasil disease-free and overall survival yang baik. Tujuan: Melihat hasil pasca-operasi diseksi KGB inguinal pada kasus serial. Cara: Diseksi KGB inguinal dilakukan pada kasus melanoma maligna TxN2bM0, melanoma maligna TxN1M0, dan cutaneous apocrine carcinoma et phenoscrotal T3N1M0. Diskusi: Diseksi KGB inguinal pada kasus keganasan bermetastasis dari ekstremitas bawah, kulit penis, vulva/vagina, dan anus, memberikan hasil disease-free and overall survival yang baik, namun dengan komplikasi pasca-operasi berkurangnya kualitas hidup.   Introduction: Malignancy in lower extremities, penis skin, vulva/vagina, and anus may spread to inguinal lymph node. Inguinal lymph node dissection can provide good disease-free and overall survival results. Objective: To review postoperative results of inguinal lymph node dissection (ILND) in case series. Method: Inguinal lymph node dissection was done in cases of malignant melanoma TxN2bM0, malignant melanoma TxN1M0, and cutaneous apocrine carcinoma er phenoscrotal T3N1M0. Discussion: ILND can improve disease-free and overall survival results in malignancies cases from lower extremities, skin of the penis, vulva/vagina, and anus with metastases, albeit with postoperative complications that can decrease the quality of life.

Abstract

During the menopause transition, several symptoms such as muscle and joint pain, back pain, vasomotor symptoms, urogenital symptoms, and psychological disorders may occur due to declining estrogen levels. Approximately a third of women would experience severe symptoms with a need for treatment. Currently, menopausal hormonal therapy (MHT) is still considered to be the first-line therapy despite its risk of cancer. Other options, such as pharmacotherapy or herbal formulas, have their limitation, urging the need for an effective and safe treatment. Standardized purified cytoplasmic pollen (PCP) extract, otherwise known as PureCyTonin® is a novel, non-hormonal, non-phytoestrogenic option that is proven to be effective in reducing menopausal symptoms while being neutral to the proliferation and apoptosis of breast cancer cells. Recent evidence shows promising results of PureCyTonin® for menopausal symptoms treatment.

Abstract

Streptococcus Grup A (SGA) atau Streptococcus pyogenes, merupakan bakteri yang sering menyebabkan infeksi saluran napas dan infeksi kulit pada anak. Organisme ini juga dapat menimbulkan infeksi invasif, yaitu sindrom syok toksik streptococcal dan necrotizing fasciitis; serta berbagai komplikasi dan sekuele, di antaranya demam rematik akut, glomerulonefritis akut pasca-Streptococcus, artritis reaktif pasca-Streptococcus dan pediatric autoimmune neuropsychiatric disorders associated with Streptococcus pyogenes (PANDAS). Peningkatan kasus infeksi SGA, baik invasif maupun non-invasif, dengan puncaknya di akhir tahun 2022 lalu, khususnya pada populasi anak, telah menjadi perhatian di beberapa negara. Klinisi sangat perlu mengenali berbagai spektrum klinis infeksi SGA, khususnya pada populasi anak.

Abstract

Varicose veins are characterized by the elongation, widening, and twisting of the venous system, accompanied by impaired blood circulation. It usually occurs in lower extremities, with a prevalence of 10%-30%. It affects mainly women, and the prevalence rate increases with age. The CEAP (Clinical, Etiological, Anatomical, and Pathophysiological) classification allows for a more precise diagnosis and provides better results. Treatment includes conservative therapy and intervention. With appropriate management, postoperative care, and rehabilitation, the prognosis of varicose veins is generally good.

Abstract

Hyperglycaemia is a medical condition of increased blood glucose levels, which is characteristic of several diseases, especially diabetes mellitus. Diabetes mellitus is currently a global threat to health. A fixed-ratio combination a insulin glargine and lixisenatide (iGlarLixi) is a productcontaining glargine insulin (a long-acting insulin) and lixisenatide (a glucagon-like peptide-1 receptor agonist) in one single injection. Fixed ratio combination offers many clinical benefits for adults with type 2 diabetes mellitus.

Abstract

Background: Traumatic brain injury (TBI) is a condition resulting from physical trauma to the head which can cause structural damage to the brain and interfere with cognitive, motor and emotional functions. Citicoline is a natural compound that plays an important role in phospholipid metabolism and neurotransmitter synthesis in the brain. This evidence-based case report (EBCR) study aims to provide scientific evidence of the potential benefits of citicoline in improving clinical outcomes in TBI patients. Method: Literature search using the PICO format. The formulation of clinical questions was arranged by case. Search for articles on March 6–7th 2023 using the Publish or Perish (PoP) software with the target of searching articles for the last 10 years (2013-2023) on the Scopus, Pubmed, Google Scholar databases, and manual searches on the Cochrane database, using the keyword 'traumatic brain' injury' AND 'therapy' AND 'citicoline' AND 'neuroprotective' AND 'treatment' AND 'outcome' along with synonyms and related terms. Results: Four studies were obtained, with the level of evidence 1a which resulted from a meta-analysis or systematic review of various randomized controlled trials, which were then critically reviewed. Two studies reported no significance of clinical assessment and the Glasgow Coma Scale (GCS); two other studies showed significant results at the level of independence. Conclusion: This study shows that citicoline is beneficial in the independence of TBI patients but does not produce a significant difference in the Glasgow Coma Scale assessment.

Abstract

Citicoline has a neuroprotectant effect, especially in mild-to-moderate stroke. The presence of the ε4 allele in the ApoE gene is associated with a severe acute ischemic stroke (AIS) phenotype, in addition to neurodegenerative risk This study aims to determine the effect of citicoline on cognitive and motoric recovery in severe AIS biomolecularly, such as ApoE4 rs429358 and rs7412 polymorphisms. A 64-year-old woman was diagnosed with AIS with a 6-hour onset and with a NIHSS score of 25 and MMSE score of 21. The patient’s AIS was biomolecularly severe as proven by PCR-RFLP (polymerase chain reaction-restriction fragment length polymorphism) ApoE4 which yielded 482 bp at rs429358 (codon 112) and 249 bp 167 bp at rs7412 (codon 158) on electrophoretic visualization. The patient received 3x 500 mg intravenous citicoline for 4 days followed by another 1000 mg of citicoline intravenous during polyclinic control for 5 consecutive days. On the 9th day, the patient experienced improvement in cognitive function with an MMSE score of 29 and superior (4/5) and inferior (5/5) motoric strength. Citicoline is a neuroprotectant agent that is beneficial for the recovery of cognitive and motoric function in biomolecularly severe SIA (ApoE4, C/C, Arg112/Arg158) because it can save the penumbra area from infarction.

Abstract

Glaucoma is one of the most common causes of blindness in the world. The most common type is open-angle glaucoma. In open-angle glaucoma, intraocular pressure slowly rises, causing nerve damage which will gradually narrow the visual field. Treatment involves surgical as well as medical options. Several classifications of drugs with different mechanisms of action can be combined. Several treatment options will be discussed in this article.
