Abstract

Introduction: Pneumonia is still one of the common causes of death in children under 5 years old in low-middle-income countries. The common cause of pneumonia is bacteria, requiring antibiotics as treatment. This systematic review evaluates the efficacy of amoxicillin and cotrimoxazole in treating pneumonia. Methods: This systematic review used PRISMA rules to analyze Pubmed and Europe PMC articles. The dose, duration, and therapeutic failure of amoxicillin and cotrimoxazole were assessed. Results: A total of 336 articles were selected for title and abstract screening. Four articles met the requirements, and two were selected for systematic review analysis. A total of 20,646 patients were analyzed, 11,633 (56.3%) received amoxicillin and 9,013 (43.7%) received cotrimoxazole. There was no significant difference in treatment failure between amoxicillin and cotrimoxazole. Conclusion: Amoxicillin has the same efficacy as cotrimoxazole for the management of non-severe pneumonia in children under 5 years old.

Abstract

Diabetes insipidus is a part of the polyuria-polydipsia syndrome, defined as urine output greater than 50 mL/kgBW accompanied by high fluid intake. This disease is classified into 4 types: central, nephrogenic, dipsogenic, and gestational. This disease is extremely rare and can easily go unnoticed. Different types of diabetes insipidus have different therapies.

Abstract

Introduction: The increasing number of stroke survivors was accompanied by an increase of cognitive impairment incidence as one of the post-stroke long-term consequences. Post-stroke cognitive impairment needs to be highlighted because of impact on reducing the quality of life, especially in the elderly. Methods: Literature was extensively searched across ScienceDirect, PubMed, Google Scholar, and Cochrane databases, using several predetermined keywords. Results and Discussion: Nine articles were analyzed. MMSE and MoCA assessments were used as parameters of post-stroke cognitive impairment. Most patients experience post-stroke cognitive impairment ranging from mild cognitive impairment to vascular dementia or other types of dementia. Risk factors include older age, female, lower and shorter education, location of lesions in the left hemisphere with involvement of certain brain structures, uncontrolled and fluctuating blood pressure, family history of stroke, comorbid diseases, as well as an individualistic social life. Conclusion: Stroke significantly increases the risk of cognitive impairment anddementia.

Abstract

Introduction: Cycling is a popular sport and also has various health benefits. However, cycling carries health risks, including fertility problems. This research aims to explore the association between cycling and infertility in men. Methods: This systematic review was conducted according to a PROSPERO protocol (ID: CRD42024509255) and PRISMA guidelines using the PubMed, Cochrane, and ScienceDirect databases. Keywords “(Bicycle) AND (Infertility)” are used. The risk of bias was assessed based on the Cochrane risk of bias tool for cohort studies. Results: The final two cohort studies were included with a total of 487 cyclists. Regular cycling of more than 1.5 hours/week was associated with 34% (95% CI 4-55%) lower sperm concentration compared to control. Regular cycling of more than 5 hours/week was associated with lower sperm concentration (OR 1.92, 95% CI 1.03-3.56) and lower total sperm motility (OR 2.05, 95% CI 1.19-3.56) compared to the control group. Conclusion: There is a relationship between cycling and male infertility; a decrease in sperm concentration in men who cycle at least 1.5 hours/week and lower sperm motility in men who cycle for at least 5 hours/week. The direct efect of cycling on the incidence of infertility as a consequence needs to be explored further.

Abstract

Background: Tuberculosis (TB) remains a major health threat, especially in developing countries. Although the lungs are the most affected organ, systemic manifestations are quite significant. D-dimer, as one of the fibrin degradation products and also an indicator of coagulation activity, has been extensively researched for its predictive and diagnostic value in various diseases. This study aims to assess the effectiveness of D-dimer as a diagnostic biomarker for pulmonary tuberculosis (TB) in patients with pleural effusion compared to those with non-TB pleural effusion. Methods: The study was conducted as an observational case-control on 64 patients with pulmonary TB with pleural effusion were compared to 64 non-TB subjects with pleural effusion. Result: In the TB patient group, 62.5% of patients were smokers, 46.9% of patients had been exposed to TB. Patients were assessed based on the TB symptom scale and with radiological lung examinations, either x-rays or CT scans. D-dimer levels were correlated with the TB severity. D-dimer levels above the threshold value were significantly higher in patients with severe TB symptoms. Conclusion: A direct correlation was observed between the size of radiological lung lesions and D-dimer levels. The average D-dimer level in the control group was 220 ± 60 ng/mL, which increased to 450 ± 90 ng/mL in TB patients with severe lesions.

Abstract

Patient expectations in the context of modern health services are defined as patient expectations regarding the availability of information, participation in decision-making, and overall service quality. The research aim is to analyze the influence of tangibility, empathy, reliability, responsiveness, and assurance on patient expectations intervened by hospital brand image. The selection used the accidental sampling technique of 200 respondents at emergency room patients using accidental sampling technique. This study employed quantitative methodology, focusing on associative causality through a cross-sectional framework. Data were gathered via questionnaires and analyzed using structural equation modeling—partial least squares (SEM-PLS). The findings suggested that patient expectations were shaped by tangibility, empathy, and responsiveness yet were unaffected by reliability and assurance, with brand image served only to mediate the impact of responsiveness on patient expectations. These outcomes offer insights for hospital administration on the importance of enhancing systems for facility upgrades, technological advancements, and the improvement of human resources through targeted training.

Abstract

Peripartum cardiomyopathy is a cardiac disorder with shortness of breath as the main symptom but often late diagnosed due to resemblance to normal condition of late gestation. The causes of peripartum cardiomyopathy are multifactorial including ages, races, genetics, and preeclampsia. Peripartum cardiomyopathy needs early diagnosis and appropriate management to reduce the risk of cardiac complications and to improve the quality of life for both mother and child.

Abstract

Background: Syphilis infection could occur simultaneously with other diseases such as TB and HIV. Case: A 24-year-old male with red spots on his hands, body, and feet for 2 weeks without pain, itching, nor burning sensation. Physical examination revealed multiple erythematous polymorphic scaly macules with clear borders, size 0.5-1 cm, accompanied by scuama. VDRL titer 1:16, and TPHA titer 1:320. The patient was also tested positive for HIV and TB. The patient was allergic to benzathine penicillin G, so the patient was given alternative syphilis therapy doxycycline 2x100 mg for 14 days. He was clinically improved and VDRL titers decreased. Conclusion: Screening for other infectious diseases such as HIV and TB is necessary in syphilis cases due to the high incidence of co-infection with other diseases.

Abstract

Background: The infant mortality rate (IMR) is one of the indicators commonly used to determine overall public welfare. This study aims to look deeper into the profile of neonatal mortality in Maluku region, particularly at dr. M. Haulussy Regional Hospital. Methods: This study was conducted on the neonatal population in the neonatal intensive care unit (NICU) of dr. M.Haulussy Regional Hospital DR M Haulussy from January 1, 2022, to August 31, 2023. A total of 34 neonates met the inclusion criteria. Results: The main factors causing neonatal death were respiratory failure/asphyxia, LBW, and sepsis with the percentage of 25.6%, 24.4%, and 22.2% respectively. Conclusion: The most common factors of neonatal mortality in this sample population were respiratory failure/asphyxia, LBW/BBLR/BBLT, and sepsis.

Abstract

Introduction: Indonesia ranks 3rd in tuberculosis (TB) cases in the world, and data from the Ministry of Health in 2020 showed that 10%-15% of TB sufferers had diabetes mellitus (DM). TB is one of the important causes of death, and DM affects the immune system which can increase the severity and mortality of the disease. This evidence-based case report was conducted to examine the mortality risk of tuberculosis patients with DM comorbidity. Methods: An article search was conducted to answer research questions based on 3 databases: PubMed, Cochrane, and ScienceDirect with the keywords “((Tuberculosis) AND (DM type 2) AND (Mortality))”. The search was based on inclusion and exclusion criteria and a critical review was conducted according to the guidelines for prognostic studies. Result: Tuberculosis patients with type 2 DM comorbidities have a 1.51 times higher risk of mortality compared to tuberculosis patients without type 2 DM comorbidities. Conclusion: Study by Huangfu, et al, (2019) can be the best choice to answer the clinical question that the risk of death of tuberculosis patients with comorbid DM increases by 1.51 times compared to patients without DM.
