Abstract

Introduction: Lupus erythematosus (LE) is a combination of various interrelating autoimmune clinical diseases. Cutaneous lupus erythematosus (CLE) is a spectrum of typical autoimmune skin features of systemic lupus erythematosus (SLE). A good recognition of the typical appearance of this skin lesion (CLE) enables diagnosis and good management of SLE. Case: A 26-year-old female with SLE with acute skin lessions (ACLE) skin lesions. Suspicion of SLE became stronger when the antinuclear antibody (ANA) titer reached 1:3,200. The main therapy is methylprednisolone injection followed by a gradual dose reduction. The patient was treated for 6 days in hospital and all lesions improved. Conclusion: The patient in this case had SLE with 6 clinical domains in the EULAR/ACR 2019 criteria and a total score of 34 (>10). Earlier and more accurate diagnosis of SLE can make therapy more effective, especially if CLE is already present.

Abstract

Colorectal cancer (CRC) had the third highest cancer incidence worldwide. Recent researches are targeting specific gene expression as CRC marker. Leucine rich repeat-containing G-protein coupled receptor 5 (LGR5) is one of the genes that regulate tumor metastasis and growth. The aim of this study is to explain the correlation between LGR5, clinical pathology, and prognosis of CRC. This systematic review was arranged using PRISMA for eligible cohort studies and evaluated using Newcastle-Ottawa scale. From 63 articles, 6 articles were selected as eligible articles for data extraction and evaluation. Based on the studies, LGR5 positivity was positively associated with histopathological characteristics, TNM staging, and vascular invasion. Besides, LGR5 expressions in those studies were high and could be positively detected by this marker. LGR5 regulates Wnt/β-catenin signaling pathway promotion in normal colon stem cell. Methylation of Wnt target gene promoter is potent predictor to CRC recurrence thus the expression of LGR5 can be used as CRC marker. LGR5 plays a role as pro-oncogenic factor in colorectal carcinogenesis via prostaglandin E2 and Epidermal Growth Factor signaling. LGR5 induction also could increase cancer cells chemoresistant ability. Thus, LGR5 could determine prognosis of CRC patient and were related to certain clinicopathological features and tumor progressions.

Abstract

Background: To ensure the universal availability of high-quality health care services, the reform of health systems tried not only to improve the coverage of health care but also the overall quality of health care services. Strategies to improve the quality of health care services include the development of a national accreditation system. This study aims to find out the differences in the quality of outpatient health services based on accreditation status. Methods: The study used a cross-sectional design. The population were all outpatients visiting the GS and the KB community health center with inclusion and exclusion criteria. The sample was selected by accidental sampling. Bivariate analysis using the Mann Whitney U test. Results: A significant difference in empathy (p <0.05), but no significant difference in tangibles, reliability, responsiveness, or assurance (p > 0.05). Conclusion: A significant difference in the quality of outpatient health services in ‘paripurna’ (GS) and ‘utama’ (KB) accredited community health centers is only in the quality of empathy.

Abstract

Introduction: First-year medical faculty students are one of the populations that often experience academic stress. Academic stress can affect academic achievement. Parents’ social support will make their children able to undergo lectures and show good academic achievement. This research aims to determine the relationship between academic stress and parents’ social support on academic achievement among Medicine Faculty students, Tanjungpura University batch 2020. Methods: A cross-sectional observational analytical study. There search population was Medicine Faculty students, Tanjungpura University batch 2020. The research sample was 239 people, chosen by simple random sampling technique. Data was obtained from filling in personal data, student academic stress scale (SASS) questionnaire, parents’ social support questionnaire, and cumulative grade point average. Bivariate analysis used the Spearman correlation test. Results: The result of the correlation test for academic stress and academic achievement was p=0,146; r=0,094; and the correlation test for parents’ social support and academic achievement was p=0,016; r=0,156. Conclusion: There is an insignificant positive relationship between academic stress and academic achievement and a significant positive relationship between parents’ social support and academic achievement.

Abstract

Stroke, especially ischemic stroke, is still one of the most prioritized health problems in Indonesia with an enormous burden and cost. The main strategy to treat ischemic stroke, which is reperfusion therapy by thrombolytic therapy or thrombectomy, has various challenges in its practices, especially regarding the recommended time window due to facility and infrastructure limitations. N-butylphthalide (NBP) is a neuroprotector with the potential ability to improve cerebral microcirculation, protect the blood-brain barrier, protect neurons from apoptosis, induce neurogenesis, and maintain neuron survivability in the condition of ischemic stroke based on preclinical studies. Clinically, NBP showed benefits mainly in improving post-stroke disability. NBP is a promising neuroprotector for better clinical outcomes.

Abstract

Introduction: Acne vulgaris has the highest prevalent skin disease in the world, with food is one of the most common triggers in adolescents. Availability of fast food has influenced young adults eating pattern. This study aims to determine the relationship between frequency of fast food consumption and severity of acne vulgaris in male students. Methods: This cross-sectional study was conducted on 129 male students. Frequency of fast food consumption and severity of acne vulgaris was measured using a modified FFQ questionnaire and GAGS score. Data were processed with STATA and analyzed using Chi-square. Results: The result showed that 65.89% male students often consumed fast food and the majority of respondents experienced mild acne vulgaris (55.81%). There was a significant relationship between frequency of fast food consumption and severity of acne vulgaris (p=0.020). Conclusion: The results of this study suggest a significant relationship between frequency of fast food consumption and severity of acne vulgaris among male students.

Abstract

Background: Stunting affects intergenerational linear growth; short mothers are preceded by stunted childhood and will give birth to babies with low birth weight (LBW). Exclusive breastfeeding is known as one of the factors to prevent stunting. This study was conducted to see the effect of exclusive breastfeeding, history of low birth weight, and mother’s height on the incidence of stunting among below-two-year-old children in Tapin district. Method: Cross-sectional with a bivariate Chi-Square analysis test with odds ratio to assess the risk. Result: The results showed that the incidence of stunting was associated with the mother’s height (p=0.002, OR 8.325, 95% CI 2.117–32.741). Exclusive breastfeeding and low birth weight were not associated with the incidence of stunting (p=0.751, OR 1.303, 95%CI 0.454-3.738 for exclusive breastfeeding and p=0.201, OR 2.222, 95%CI 0.849–5.818 for LBW). Conclusion: Mother’s height affects the incidence of stunting in below-two-year-old children in Tapin district.

Abstract

Background: Syphilis infection during pregnancy can cause congenital syphilis. Syphilis in pregnant women is treated with penicillin to prevent fetal infection and congenital syphilis. A systematic literature review is conducted to determine the efficacy of penicillin G by reviewing outcome parameters. Methods: A systematic literature review following the preferred reporting items for systematic reviews and meta-analysis (PRISMA) guidelines. Results: Through searches on PubMed, Science Direct, and Cochrane using keywords ‘Antibiotics’, ‘Pregnancy’, ‘Syphilis’, and ‘Treatment’, 230 articles were found. Twenty-five articles were duplicates, 135 articles did not meet the inclusion criteria, 75 articles were literature reviews, and 58 articles were not related to pregnant women, resulting in 6 journals that met the inclusion criteria. Conclusion: Penicillin G benzathine is the most common antibiotic used to treat syphilis in pregnancy. A total of 1,190 cases (26.26%) of congenital syphilis were found among 4,531 pregnant women infected with syphilis and receiving penicillin G benzathine therapy. The incidence of congenital syphilis was 2.4% in pregnant women who received therapy since the first trimester. The incidence of congenital syphilis increased 12 times if therapy was given in the second trimester and 33 times if given in the third trimester.

Abstract

Pendahuluan: Ginekomastia merupakan kondisi pembesaran payudara pada pria akibat proliferasi kelenjar payudara yang umumnya bilateral. Ginekomastia unilateral jarang ditemukan pada dewasa muda. Kasus: Pria berusia 19 tahun dengan pembesaran payudara kanan yang disadari dalam 1 bulan terakhir. Payudara tidak nyeri, tidak mengeluarkan cairan, dan tidak teraba benjolan keras. Pada pemeriksaan fisik, ditemukan payudara kanan lebih besar dibandingkan payudara kiri, tidak tampak perubahan kulit (peau d’orange, ulserasi), retraksi puting susu, ataupun keluar cairan. Teraba pembesaran kelenjar dengan diameter +5 cm, tidak nyeri tekan, lunak, immobile, batas tegas, tidak ada galaktore, tidak teraba benjolan. Pemeriksaan laboratorium dalam batas normal. Pasien didiagnosis ginekomastia unilateral dekstra. Dilakukan mastektomi simple pada payudara kanan. Temuan histopatologi pada sediaan eksisi mammae dekstra mendukung diagnosis ginekomastia. Simpulan: Anamnesis dan pemeriksaan fisik dapat menegakkan diagnosis ginekomastia. Evaluasi laboratorium dan pencitraan diperlukan untuk membuktikan kecurigaan klinis atau kasus yang tidak jelas serta mencari penyebab.

Abstract

Pendahuluan: Apendisitis akut merupakan penyebab tersering penyakit bedah non-obstetrik pada wanita hamil. Namun, temuan klinis mungkin tidak khas selama kehamilan. Diagnosis apendisitis akut pada kehamilan tidak mudah karena tanda-tanda klinis dapat disalahartikan sebagai gejala terkait kehamilan. Kasus: Perempuan berusia 18 tahun G2P1A0 usia kehamilan 10 minggu dengan keluhan nyeri perut bawah bagian tengah terus-menerus sejak 5 jam disertai mual dan muntah 2 kali serta miksi tidak tuntas. Perdarahan vagina dan cairan ketuban disangkal. Pada pemeriksaan fsik didapatkan bising usus menurun, nyeri tekan hipogastrik, iliaka kanan, dan titik McBurney, serta tanda Dunphy, Psoas, dan Obturator positif. Dengan skor Alvarado 7 dan hasil USG, didiagnosis apendisitis akut. Dilakukan apendektomi. Temuan histopatologi kesan apendisitis akut supuratif. Simpulan: Apendisitis pada kehamilan memerlukan diagnosis yang cepat dan akurat, karena klinis sering dianggap gejala kehamilan.
