Abstract

Prosedur Sectio Caesarea (SC) kerap menimbulkan rasa nyeri pascaoperasi yang membuat ibu lebih memilih beristirahat di tempat tidur dan enggan melakukan gerakan tubuh. Padahal, aktivitas mobilisasi awal berperan penting dalam mempercepat proses pemulihan serta penyembuhan luka operasi. Penelitian ini bertujuan menganalisis faktor-faktor yang memengaruhi tingkat pemahaman ibu pasca SC mengenai mobilisasi dini di Ruang Ranukumbolo RSUD Dr. Saiful Anwar Malang. Desain penelitian menggunakan pendekatan observasional analitik dengan rancangan potong lintang (cross sectional). Populasi terdiri atas seluruh ibu post SC pada Desember 2024 sebanyak 27 orang, yang diambil menggunakan teknik total sampling. Pengumpulan data dilakukan melalui kuesioner terstandar yang telah diuji validitas dan reliabilitasnya, sedangkan analisis data memanfaatkan uji Chi-square. Hasil menunjukkan mayoritas responden berusia 20–35 tahun (59,3%), berpendidikan rendah (55,6%), bekerja (63%), tinggal di lingkungan kurang mendukung (66,7%), memiliki latar sosial budaya tidak mendukung (74,1%), terpapar media massa (51,9%), dan merupakan multipara (55,6%). Pengetahuan ibu pasca SC sebagian besar tergolong baik (37%). Faktor yang berpengaruh signifikan terhadap tingkat pengetahuan adalah pendidikan (p=0,001), pekerjaan (p=0,018), paparan media massa (p=0,040), dan pengalaman melahirkan (p=0,007), sedangkan usia (p=0,675), kondisi lingkungan (p=0,852), serta sosial budaya (p=0,863) tidak berpengaruh. Kesimpulannya, tingkat pengetahuan ibu post SC umumnya baik, dipengaruhi oleh aspek pendidikan, pekerjaan, media, dan pengalaman, sehingga perlu peningkatan edukasi berkesinambungan melalui tenaga kesehatan serta media informasi untuk memperkuat kesadaran pentingnya mobilisasi dini pascaoperasi.

Abstract

Abstract Placenta accreta is a severe obstetric complication characterized by abnormal adherence of the placenta to the uterine wall, which may lead to massive hemorrhage and maternal death. The incidence of placenta accreta continues to rise in parallel with increasing rates of cesarean sections and other uterine surgical procedures. This study aimed to identify the determinant factors influencing the occurrence of placenta accreta. This research employed a retrospectiv analytic design with a cross-sectional approach. The sample consisted of 90 pregnant women selected through random sampling, based on medical records from 2022 to 2024. Data collection used an observation sheet based on the Placenta Accreta Index (PAI) and analysis was performed using the Chi-Square test. The results showed a significant association between placenta accreta and history of cesarean section (p = 0.004), maternal age >35 years (p = 0.024), and history of other uterine surgical procedures (p = 0.028). Meanwhil parity, placenta previa, and gestational hypertension were not significantly associated. It is concluded that maternal age, cesarean history, and previous uterine procedures are key determinants of placenta accreta. These findings are expected to contribute to risk screening and delivery planning for high-risk pregnant women.

Abstract

Abstract Back pain is a common physically distressing side effect reported by cervical cancer patients undergoing chemotherapy, impacting their adherence to treatment and quality of life. This study aims to determine the effectiveness of Friction massage in reducing post-chemotherapy back pain among cervical cancer patients at the Singkarak Room of Dr. Saiful Anwar Hospital, Malang. A Quasi-Experimental design with a One-Group Pretest-Posttest approach was employed. A sample of 25 respondents was selected using accidental sampling based on specific inclusion criteria. Data on pain intensity were measured using the Numeric Rating Scale (NRS) before and immediately after a 20-minute Friction massage intervention, then analyzed using a paired t-test. The results showed that before the intervention, 76% of respondents experienced mild pain and 24% had moderate pain. After the Friction massage, 68% reported no pain and 32% reported mild pain. The paired t-test obtained a significant value (p = 0.001, < 0.05), indicating that H1 is accepted. In conclusion, Friction massage is an effective non-pharmacological complementary modality for reducing post-chemotherapy back pain in cervical cancer patients. Clinical implementation of this intervention as a standard nursing care protocol is highly recommended to improve patient comfort.

Yanuarti, Poppy Indah. 2025. External and Internal Factors of Post-Cesarean Section Patients Affecting Breastfeeding Success at Dr. Saiful Anwar Regional General Hospital Malang. Breastfeeding rates in Indonesia have decreased significantly in recent years, prompting UNICEF and WHO to call for greater efforts to protect, promote, and support breastfeeding practices. A preliminary study at Dr. Saiful Anwar Regional General Hospital Malang revealed that 68.5% of mothers breastfed their infants from days 1-5 post-cesarean section. Post-cesarean section mothers who did not breastfeed their infants until the postnatal control visit accounted for 15.1%, with reasons including insufficient milk production and mothers undergoing medical treatment. The remaining 16.2% of mothers did not breastfeed due to infant mortality. The aim of this study was to analyze the external and internal factors of post-cesarean section patients affecting breastfeeding success at Dr. Saiful Anwar Regional General Hospital Malang in 2025. This study employed an observational analytical research design with a cross-sectional approach. The research utilized questionnaire sheets as the research instrument and employed accidental sampling with 62 respondents during November-December 2024. Data processing in this study used computerized techniques assisted by SPSS and employed Chi-Square formula. Based on the research conducted to analyze external factors in post-cesarean section patients among 62 respondents, it was found that respondents who received family support and achieved successful breastfeeding accounted for 45.7%, which was higher compared to those who received support but did not succeed in breastfeeding (37.0%). The Chi-Square statistical test showed a p-value of 0.01, indicating statistical significance (p-value <0.05). Regarding internal and external factors in post-cesarean section patients among 62 respondents, it was found that respondents who produced abundant breast milk and succeeded in breastfeeding accounted for 54.2%, higher than respondents who produced abundant milk but did not succeed in breastfeeding (45.8%). The Chi-Square statistical test showed a p-value of 0.04, indicating statistical significance (p-value <0.05). Healthcare professionals should enhance services by motivating and educating mothers on exclusive breastfeeding, implementing rooming-in practices to enable mothers to easily observe, care for, and breastfeed their infants, and evaluate breastfeeding success at home with follow-up education. Abstract

Abstract Infant mortality rates can be significantly reduced if newborns receive breast milk immediately. Breastfeeding within the first hour—supported by programs such as Early Initiation of Breastfeeding (IMD) and exclusive breastfeeding—can save millions of newborn lives. However, many postpartum mothers experience difficulties with breast milk expression. Oxytocin massage is one solution to address issues with the flow of breast milk. Using an oxytocin massage device (STIPRO) can further assist postpartum mothers with milk expression. This study aimed to determine the difference in effectiveness between manual oxytocin massage and non-manual massage (using the STIPRO device) regarding the speed of breast milk expression among postpartum mothers at Dr. Saiful Anwar Regional General Hospital (RSUD) in Malang, East Java. A pre-experimental design with a post-test-only control group was employed. Purposive sampling was used to select 30 participants: 15 respondents received manual oxytocin massage, and 15 received non-manual massage (STIPRO). Research instruments included the STIPRO device and an observation checklist for breast milk expression. The t-test results showed a p-value of 0.000 (less than a = 0.05) and a calculated t-value (5.551) greater than the t-table value (2.048), indicating a significant difference in effectiveness between manual and non-manual (STIPRO) oxytocin massage regarding the speed of breast milk expression. The average volume of expressed breast milk in the non-manual massage group (intervention group) was 17.6 cc, which was higher than that of the manual massage group (control group) at 5.0 cc. It is concluded that there is a difference in effectiveness between manual and non-manual (STIPRO) oxytocin massage; breast milk expression was higher in the intervention group than in the control group. Respondents are encouraged to practice non-manual oxytocin massage (STIPRO) and proper breast care at home to ensure continued milk production and smooth expression.
