Abstract

Introduction: Tuberculosis (TB) remains a public health issue. The high number of cases highlights the need for early detection through spatial analysis. Objective: This study aimed to analyze TB case distribution using QGIS as a basis for early detection of transmission risk in Bantul Regency. Methods: This quantitative descriptive study used TB case data for 2022–2024 obtained from the Bantul District Health Office through the Tuberculosis Information System (SITB), comprising 4,052 cases. Preprocessing included data cleaning, data selection, data normalization, and data transformation before spatial analysis using QGIS. Spatial data were obtained from the Special Region of Yogyakarta Geoportal, which provides shapefiles of sub-district areas. Processed data were presented on a dashboard as tables, charts, and case distribution maps. Results: A total of 4,052 TB cases were recorded from 2022 to 2024. Sewon had the highest number of cases, with 547 cases, followed by Bantul with 371 cases and Banguntapan with 363 cases. Cases were concentrated in the central to northern regions, with consecutive increases in Sewon, Banguntapan, and Bambanglipuro. The mapping results were developed into a web-based dashboard. Conclusion: Spatial analysis using QGIS can illustrate TB case distribution patterns and help identify areas at risk of increased transmission.   Keywords: QGIS, spatial analysis, tuberculosis

Abstract

Introduction: Ultra-processed foods contain additives such as artificial sweeteners, emulsifiers, and preservatives that may contribute to oxidative stress and endothelial dysfunction, key mechanisms in cardiovascular disease. Objective: This review evaluated the role of food additives in inducing oxidative stress and endothelial dysfunction and summarized their potential cardiovascular mechanisms. Methods: A PRISMA-based literature search was conducted in PubMed, Scopus, ScienceDirect, SpringerLink, and Google Scholar for studies published between 2019 and 2026. Studies examining artificial sweeteners, emulsifiers, or preservatives in relation to oxidative stress, inflammation, endothelial dysfunction, or cardiovascular outcomes were included. Thirteen eligible studies were analyzed using narrative thematic synthesis. Results: Food additives may increase reactive oxygen species production, mitochondrial dysfunction, gut dysbiosis, chronic inflammation, lipid peroxidation, and impaired antioxidant defenses. Artificial sweeteners were associated with reduced nitric oxide bioavailability, emulsifiers with intestinal dysbiosis and inflammation, and preservatives with oxidative and inflammatory activation. These alterations may promote endothelial dysfunction and increase cardiovascular risk. Conclusion: Oxidative stress appears to be a central mechanism linking food additive exposure to endothelial dysfunction and adverse cardiovascular outcomes. Abstract

Pendahuluan: Personalized medicine berkembang sebagai pendekatan inovatif yang bertujuan meningkatkan pencegahan, diagnosis, dan terapi penyakit dengan mempertimbangkan karakteristik biologis individu. Dalam beberapa tahun terakhir, gut microbiome semakin mendapat perhatian sebagai salah satu faktor penting yang memengaruhi kerentanan terhadap penyakit dan respons terapi melalui interaksi kompleks dengan metabolisme inang, fungsi imun, serta jalur pensinyalan molekuler. Tujuan: Tinjauan sistematis ini bertujuan untuk mengevaluasi peran gut microbiome dalam personalized medicine dari perspektif molekuler dan fisiologis serta mengeksplorasi potensinya sebagai biomarker dan target terapi. Metode: Tinjauan sistematis dilakukan dengan mengikuti pedoman Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Pencarian literatur dilakukan pada basis data PubMed dan ScienceDirect menggunakan kombinasi kata kunci yang berkaitan dengan gut microbiome, personalized medicine, metabolomik, dan precision medicine. Sebanyak 929 artikel teridentifikasi melalui pencarian awal. Setelah proses penyaringan dan penilaian kelayakan berdasarkan kriteria inklusi dan eksklusi yang telah ditetapkan, sebanyak 21 artikel diikutsertakan dalam sintesis akhir. Hasil: Hasil tinjauan menunjukkan bahwa gut microbiome berperan sebagai regulator molekuler yang memengaruhi ekspresi gen, metabolisme, respons imun, dan berbagai jalur pensinyalan seluler melalui produksi metabolit bioaktif. Selain itu, gut microbiome berkontribusi terhadap variasi antarindividu dalam luaran terapi melalui mekanisme farmakomikrobiomik serta menunjukkan potensi yang signifikan sebagai biomarker diagnostik, prognostik, dan prediktif pada berbagai penyakit. Berbagai intervensi berbasis mikrobioma, termasuk modulasi pola makan, senyawa bioaktif, dan fecal microbiota transplantation, juga menunjukkan potensi terapeutik yang menjanjikan dalam mendukung strategi precision medicine. Kesimpulan: Gut microbiome memiliki peran penting dalam personalized medicine melalui regulasi proses molekuler dan fisiologis yang terlibat dalam perkembangan penyakit dan respons terapi. Integrasi profil mikrobioma dengan pendekatan multi-omics berpotensi mendukung pengembangan strategi diagnosis dan terapi yang lebih presisi pada masa mendatang. Abstract

Background: Blood transfusion safety depends on the ability of screening methods to detect transfusion-transmitted infections (TTIs), including human immunodeficiency virus (HIV). Rapid tests are widely used because they are practical and economical; however, they have limitations in detecting HIV infection during the early stage due to the window period. Electrochemiluminescence immunoassay (ECLIA), a fourth-generation method, can simultaneously detect HIV antibodies and p24 antigen, thereby potentially improving early HIV detection. Objective: This study aimed to analyze the limitations of rapid tests compared with ECLIA in HIV screening among blood donors. Methods: This study used a retrospective descriptive comparative design based on HIV screening data from blood donors at the Indonesian Red Cross Blood Donation Unit in Yogyakarta. Rapid test results were compared with ECLIA results as the reference method and analyzed based on sensitivity, specificity, predictive values, and diagnostic accuracy. Results: ECLIA showed higher sensitivity than rapid tests, particularly during the early stage of infection. Rapid tests may produce false-negative results because of their limited detection ability during the window period, whereas both methods demonstrated relatively high specificity. Conclusion: ECLIA has better diagnostic performance than rapid tests for HIV screening among blood donors. Fourth-generation methods are recommended to improve blood transfusion safety through earlier HIV detection.   Keywords: ECLIA; HIV; rapid test
