Abstract

Introduction: Pericoronitis is an inflammation of the soft tissue surrounding the crown of an impacted or partially erupted tooth, most commonly involving the mandibular third molar. This condition is caused by the accumulation of plaque and food debris beneath the operculum, which triggers bacterial infection. The management of pericoronitis can be performed either conservatively or definitively, one of which is operculectomy. Operculectomy is a minor surgical procedure to remove part of the operculum or tissue flap over an erupting tooth, especially the third molar. Operculectomy is indicated on third molars that are fully erupted, have an opposing tooth, are positioned vertically, have sufficient space for eruption, are intended as abutments for fixed prostheses, or when the patient refuses extraction. Contraindications include third molars that are not fully erupted, horizontally positioned, or lack an opposing tooth. Case Report: A 24-year-old female patient presented with a complaint of the lower right third molar being partially has operculum, causing pain during mastication. Intraoral examination showed missing teeth in two teeth, caries in six teeth, gangrene radix in tooth 36, and pre-eruption of tooth 48. The patient had no history of systemic disease. Discussion: The patient was diagnosed with pericoronitis in the operculum region of tooth 48. Clinical findings included hyperemia, swelling, bleeding, and food impaction. An operculectomy was performed after initial therapy. Follow-up evaluation showed significant improvement within two weeks post-treatment without scar formation. Conclusion: Operculectomy is an effective treatment option for managing pericoronitis of the mandibular third molar, with a good prognosis when accompanied by optimal oral hygiene maintenance.

Abstract

Introduction: Immune Thrombocytopenic Purpura (ITP) is a hematologic disorder characterized by low platelet levels (<100.000/µL) and mucocutaneous bleeding involving gingiva. It develops as an antibody-mediated reaction against platelets influenced by genetic, microbial, and  drug-related factors. It is more frequently observed in women and children. This case report highlights ITP identification through prolonged gingival bleeding during scaling. Case History: A 46-year-old male patient presented with spontaneous gingival bleeding lasting three days. Intraoral examination showed erythematous and edematous gingival margin with supragingival calculus. The patient experienced prolonged bleeding with delayed coagulation during scaling despite hemostatic gel and 500 mg tranexamic acid per-oral administration. He was referred for complete blood count which revealed severe thrombocytopenia (1.000/µL) and confirmed as ITP. Management involved platelet transfusion and pulse-dose methylprednisolone resulted in improvement of platelet number after 8 months with moon face effect. Discussion: ITP is commonly asymptomatic but dental procedure may uncover its presence through abnormal bleeding. Gingival inflammation from dental plaque accumulation increases vascular permeability that is aggravated by thrombocytopenia resulting in prolonged gingival bleeding even with local hemostatic measures. This condition reflects that he was under severe thrombocytopenic conditions during scaling. Effective plaque control is essential for ITP patients to reduce local inflammation and support systemic stability. Conclusion: Oral manifestation of ITP such as gingival bleeding during scaling emphasizes the pivotal role of general dentists in early detection of hematologic disorders to minimize the risk of further complications.

Abstract

Introduction Oral biopsy is a fundamental diagnostic procedure in dentistry, essential for the early detection of oral cancer and other pathologies. While theoretical knowledge is a core component of dental education, the translation of this knowledge into clinical confidence and practical ability is critical for patient care. Objective: : evaluate the awareness, knowledge, and self-perceived ability to perform oral biopsies Material and Methods: A cross-sectional study was conducted among dental students. Participants (n=110) comprising 14 multiple-choice questions assessing key variables: knowledge of biopsy principles, techniques, lesion identification, specimen preservation, and ability to perform the procedure Results: While theoretical awareness was high (98% knew what a biopsy is), a significant gap existed with practical application. Only 16% of students felt confident performing a biopsy independently. The primary barriers were reliance on theoretical knowledge only (47%) and a lack of clinical training (37%). A statistically significant association was found between gender and the perceived ability to perform a biopsy (p=0.002). However, no significant correlation was found between academic stage (4th vs. 5th year) and practical ability (p=0.2). Conclusion: Dental students have a strong theoretical foundation in oral biopsy but report a significant deficit in practical, hands-on training and self-confidence. The findings highlight a critical need to reform the curriculum to include more clinical and simulation-based training to bridge this theory-practice gap and better prepare future dentists for this essential diagnostic responsibility

Abstract

Introduction: The operculum surrounding the lower third molar can prompt plaque retention from accumulated food debris, and chewing trauma from opposing teeth can lead to pericoronitis. Pericoronitis typically occurs in the lower third molars. Case Report: A 17-year-old male patient and a 20-year-old female patient came with complaints of gums partially covering the crowns of their lower right tooth, which were painful when biting and interfered with eating. The patient had no systemic diseases, no drug or food allergies. Clinical and radiographic examinations revealed partial eruption of teeth 48. The operator decided to perform an operculectomy on tooth 48. Discussion: Operculectomy is the procedure to removal of the gum above the third molar. This is a surgical procedure to treat pericoronitis and prevent future recurrence. Conclusion: The operculectomy procedure using surgical blade was successful in this case because it is the gold standard and a conventional technique that can still be used for pericoronitis due to its ease of performance, affordability, and good healing process. The success of operculectomy treatment also depends on the patient's oral hygiene.
