Abstract

Gestational diabetes mellitus carries significant risks to the health both of the mother and fetus. Lifestyle modifications, mainly dietary patterns and physical activity, have emerged as major modifiable factors in the prevention and management of GDM. We conducted an analysis of recent meta-analyses, umbrella reviews, and systematic reviews published between 2020 and 2025 that assessed dietary interventions, physical activity programs, and combined lifestyle approaches for GDM prevention and management. Primary outcomes included the incidence of GDM, measures of glycemic control, gestational weight gain, and maternal-neonatal clinical outcomes. Dietary interventions showed consistent protective effects against GDM, with a pooled odds ratio of 0.69 (95% CI 0.56–0.84), or approximately a 31% relative risk reduction. The physical activity effects were significantly modified by timing and intensity: first-trimester initiation (OR 0.57, 95% CI 0.43–0.75) and mild-to moderate intensity activity (OR 0.65, 95% CI 0.53–0.80), while vigorous or later-start exercise did not confer any benefit (OR 1.09, 95% CI 0.50–2.38). Combined diet-plus-exercise interventions may have a possible but inconsistent protective effect, with high heterogeneity across studies. DASH and Mediterranean dietary patterns presented favorable results in pooled comparisons.

Abstract

Background: Adolescence is a pivotal window for lifelong health, yet adolescent girls face persistent sexual and reproductive health (SRH) and mental health (MH) burdens amid stigma, access barriers, and digital-era risks, necessitating midwife-led responses. Purpose: This study aims to examine how the role of midwives with various interventions can optimise the reproductive and mental health of adolescent girls. Methods: Narrative review of studies published in the last five years. A computerised search identified 1.032 records (PubMed=666; ScienceDirect=325; Scopus=41). Titles/abstracts were screened using prespecified inclusion criteria (English; alignment with the objective). Fifteen studies involving girls aged 10–19 years in clinics, schools, communities, and primary-care settings were included. Data on midwife-led SRH–MH interventions were extracted and synthesised thematically. Results: Midwife-delivered, youth-friendly SRH–MH models were feasible and acceptable, and were associated with increased SRH service uptake, better engagement and continuity, and reductions in depressive/anxiety symptoms in several digital or hybrid interventions. Enablers included confidentiality, respectful non-judgmental counselling, trained staff, clear referral pathways, and flexible access; barriers included stigma, privacy concerns, costs/transport, and restrictive social norms. Conclusion: Midwife-led, integrated, and digitally enabled SRH–MH services show promise for optimising outcomes among adolescent girls. Implications for midwifery practice include adopting AYFHS standards, institutionalising routine PHQ-A/GAD-7 screening with referral pathways, ensuring confidential, trauma-informed counselling and contraceptive choice, leveraging blended in-person/mobile follow-up, and monitoring quality indicators (screening coverage, service use, completed referrals).

Abstract

Early childhood is a critical period for the development of fundamental movement skills (FMS). However, Indonesia continues to face challenges related to motor development, with motor delays affecting approximately 29.9% of children. This study aimed to develop an Animal-Based Physical Exercise program designed to support FMS development in toddlers through a simple, contextual, and enjoyable approach. This study employed a descriptive observational design with a limited pilot implementation. The study involved seven toddlers aged 2-5 years and two instructors at a fitness studio in Surabaya. Data were collected through direct observation of physical exercise activities and brief interviews with the instructors. The observation findings were used to develop the Animal-Based Physical Exercise program, which was subsequently implemented on a limited basis with two toddlers aged 1.5 and 4 years. The collected data were analyzed descriptively by organizing and interpreting the observation and implementation findings. The observations showed that younger children required more guidance and participated less consistently during the activities, whereas the older child was able to follow the movement sequence more independently and participated actively throughout the session. These observations suggest that children's responses to physical exercise may vary according to their developmental stage. The developed Animal-Based Physical Exercise program integrates animal imitation movements into structured play activities to encourage locomotor skills, balance, coordination, and active participation. This study provides a practical and contextually appropriate physical exercise program that can be implemented in fitness studios, early childhood education settings, and at home to support toddlers' motor development. The findings also provide preliminary information for developing age-appropriate physical activity programs and may serve as a reference for future studies involving larger samples and longer implementation periods.

Abstract

Infants age 0-11 months are in a critical period of rapid growth and development known as the golde age. Proper stimulation, particularly physical activity, plays an important role in supporting optimal motor, sensory, and cognitive development. This study aims to describe the implementation of infant physical exercise as an early simulation strategy to support growth and development outcomes. A descriptive observation method was used and conducted in three different locations. Data were collected through direct observation, documentation, and interviews with parents and midwives. The results showed that infants with prior exposure to physical exercise were more comfortable, calm, and cooperative, while those without prior experience tended to be fussy and less adaptive. In addition, the sequence of activities and infant readiness influenced the effectiveness of stimulation. Baby spa, including baby gym, gym ball exercises, and baby massage, was found to be an effective approach to support infant growth and development. In conclusion, physical stimulation should be introduced early, gradually, and adjusted to the infant’s condition to achieve optimal developmental outcomes.

Abstract

Prenatal stress and prolonged labor are important concerns in maternal health because they may affect maternal comfort, labor progression, and the need for obstetric intervention. Prenatal yoga combined with structured pregnancy exercise has been proposed as a non-pharmacological strategy to support stress regulation and physiological readiness for childbirth. This quasi-experimental study involved 26 third-trimester primigravida women from maternity clinics in Nganjuk Regency, Indonesia, with 13 participants in the intervention group and 13 participants in the control group. Participants in the intervention group received combined prenatal yoga and structured pregnancy exercise twice weekly for three weeks, whereas the control group received structured pregnancy exercise only with the same frequency and duration. Serum cortisol levels were measured before and after the intervention using ELISA. The active phase of first-stage labor was defined as the period from 4 cm cervical dilation to full dilation and was documented using a partograph by attending midwives. The intervention group showed a significantly greater mean reduction in serum cortisol than the control group (−209.67 ± 56.30 vs. −129.96 ± 186.93 ng/ml; p = 0.010, Mann-Whitney U) and a shorter active-phase labor duration (4.08 ± 1.29 vs. 5.54 ± 0.90 hours; p = 0.003, independent t-test). A significant reduction in cortisol was also observed within each group (intervention: p < 0.001, paired t-test; control: p = 0.001, Wilcoxon test). These findings indicate that combined prenatal yoga and structured pregnancy exercise was associated with improved serum cortisol regulation and shorter active-phase labor duration compared with structured pregnancy exercise alone. Delivery mode was not evaluated as an outcome; therefore, further studies are needed to assess whether this intervention influences cesarean section rates.
