Abstract

Seven patients of open tibial fractures type III C were treated in the Emergency Unit, Sardjito hospital from 1990 to 1995. Three patients underwent early amputation and four patients ultimately required secondary amputation from three to twelve days after vascular repair intervention.Open tibial fractures associated with vascular trauma need an immediate decision between early amputation and limb salvage. Prospective studies are important before a well-defined protocol or objective criteria for early amputation can be considered.Key word : open fracture – tibial fracture – lower fracture – type Ill C – amputation

Abstract

Two groups of patients were treated with combined modality radiation and hyperthermia at the Strahlentherapeutische Klinik, Universitit Erlangen-Nurnberg, Federal Republic of Germany, from October 1987 to July 1988. The first group of 8 patients (4 cases of carcinoma of-the floor of the mouth, 3 cases of tongue carcinoma, 1 case of Ewing sarcoma) was treated with combination radiation and interstitial hyperthermia. 7 cases have complete remission (87.5%), whereas 1 case has partial remission (12.5%). The second group of 6 patients (5 breast cancer recurrences in the chest wall and supraclavicular region, 1 case with cancer of the parotid gland) were treated with combined radiation of electron beam 12 Mev by Siemens Mevatron 20 and superficial hyperthermia using Lund Buehler hyperthermia system 4010, 915 MHZ/20 W, once a week. 4 cases have complete remission (66.6%) and two cases have partial remission (33.3%). As a background of hyperthermia we also describe the principles of thermobiology, heat interaction with irradiation, physiologic mechanisms, and effects of hypertherrnia on microvasculature of tumors as well, as basic principles of physics and instrumentation of hyperthermia.key words: cancer-hyperthermia-radiation therapy-thermobiology-thermometry

Abstract

Background: Mutilation to eliminate the identity of murder victim is often conducted in crimes, for instance by cutting the head from the body. In order to determine whether the head belongs to the same individual with the body, research on the vertebral anatomy of the seventh cervical and first thoracic vertebrae is very important to be conducted.Objective: to observe the morphological congruence between the seventh cervical vertebrae and the first thoracic vertebrae.Material and Method: Thirty skeletons housed at the Laboratory of Anatomy, Embryology and Anthropology Gadjah Mada University Faculty of Medicine, Yogyakarta. The methodology is by scanning the C7 and T1 with three-dimensional scanner Umax Astra 2000, which scanned the bones into two dimensional on scanning paper. The scanned C7 is superimposed to T1, in the same individual and also between individual. Result: High degree of congruence between C7-T1 occurred at lamina, the structure that connects the pedicle and spinous process. This congruence extends into the posterio-inferior lateral border at the interior articular facets. Superimposition of C7-T1 inter-individual showed no accordance on the spinous process, lamina, inter-vertebral foramen, transverse process, and corpus vertebrae. Thus there was no two individuals had the same size and contour of C7-T1.Conclusion: This research is important in determining individuation on decapitation homicide where the body is dispersed from the head. This study also shows the contribution of anatomy in forensic sciences.Key words: decapitation, individuation, congruence, vertebrae, scanning

Abstract

Bacground: " Gurah" is a traditional treatment for nasal and lower respiratory tract diseases or for establishing clearer voice, where the extract " Srigunggu" or "Sirgunggu" is used as nasal drop. The side effect of "gurah" on the mucocilliary system has not been known yet.Objective: The study was done to know the effects of "Gurah" on the mucocilliary transport, in chronic rhinitis patients.Method: The study was performed by pretest-posttest design.The study was performed at "Gurah" location of Kyai Hisyam, Giriloyo, Wukirsari, Imogiri, Kabupaten Bantul Daerah Istimewa Yogyakarta. The mucocilliary transport time of the subjects with chronic rhinitis were tested with saccharine method for three times i.e. on the second (Wt2) and the tenth day (Wt10) after "gurah". Procedure of the "gurah" was done by KH Hisyam himself, and the measurement of mucocilliary transport time was done by the author. The paired-t test was used to test the significant difference of the mucocilliary transport time between before and after "gurah".Result: On the second day after "gurah" there was significant difference of the mucocilliary transport time between before "gurah" and after "gurah" (P< 0.05), but on the tenth day after "gurah", there was no significant difference of the mucocilliary transport time (P>0.05).Conclusion: The mucocilliary transport time was prolonged significantly on second day after "gurah", but on the tenth day the mucocilliary transport time returned to the before ''gurah" value.Key word: "Gurah" - mucociliary transport time

Abstract

Background: Nonperiodic form of B. ma/ayi was recently reported from East Kalimantan by Sudjadi as a new sub-species of filarial worm. The parasite was highly prevalent among Dayak people living from shifting cultivation of paddy rice in deep forest. Wild mosquitoes exist in the highly endemic area of Krayan, Long Ikis district, Pasir regency.Objective: To show current transmission of filariasis and its main vector species in the area of indigenous inhabitants of Dayak.Method: Wild mosquitoes were collected using human bait by standard method at night and day time in different places in the Krayan area. Wild mosquitoes landing and feeding on him were collected, than killed in local laboratory, identified and dissected to find filarial larvae.Results: Seven genera or 21 species (amount of 22.942) of mosquitoes were collected from Krayan for dissection, and they were markly predominated by Mansonia (21.960 or 95.5%) mosquitoes. Transmission of filariasis was detected currently occured in the area with the main vector of Ma. bonneae. This mosquito species was not only discovered harbouring infective larvae of the disease, but was also found as markly predominant species among Mansonia collected, particulary in locations within the environment of thick forest.Conclusion: Current transmission of filariasis with Ma. bonneae, as the main vector recorded in the highly endemic area of indigenous Dayak people in Krayan, supports the nonperiodic form of B. malayi in East Kalimantan to be zoonotic, sylvanic and phyletically they are more primitive than the other B. malayi type.Key words: filariasis - nonperiodic form of B. malayi - transmission - main vector - Ma. bonneae

Abstract

Glaukom merupakan salah satu dari lima penyebab utama kebutaan di Indonesia. Timolol merupaltan obat glaukom yang belum lama dipergunakan di Yogyakarta. Timolol tergolong betablocker yang dapat menurunkan tekanan bola mata dengan cara menghambat produksi humor aquosa pada corpus ciliare.Telah dilakukan percobaan pada orang normal kelompok muda berumur antara 10-20 tahun, kelompok normal lanjut usia berumur 40 60 tahun, clan kelompok penderita glaukom primer. Pada ketiga kelompok tersebut diberi tetesan maleat timolol 0,5%. Terjadi penurunan tekanan bola mata yang jelas setelah satu jam. Dua jam setelah tetesan masih terjadi penurunan tekanan bola mata yang nyata pada kelompok normal muda usia dan kelompok penderita glaukom primer. Dengan perhitungan statistik penurunan tersebut ternyata bermakna.Key Words: timolol maleate - beta blocker drugs - glaucoma - intraocular pressure - tonometer

Abstract

As screening tool for HIV-1 infection, anti HIV-1 ELISA kit with high sensitivity was proved to be very efficient. Because of its high sensitivity, the use of the kit to screen HIV-1 infection in low risk individuals or in seroprevalence study in population of non-endemic area will reduce its specificity. Therefore, there will be many false positive results which, in turn, will give psychosocial and economical impact to the individuals In question. To evaluate false positive results and specificity of anti HIV-1 kit, 1028 normal individuals with low risk of HIV-1 infection were studied. All positive results from HIV-1 ELISA kit confirmed with confirmative ELISA and Western-blot analysis. The results showed that HIV-1 ELISA kit gave false positive results of 1.17% and specificity of 98,38%. Ninety one point seven percent of false positive results had low-ODs ( 2 ODBP) and poor reproducibility value (8,3%). Seventy five percent of these false positive were from individuals aged 1-15 years and not related to gender as well as socioeconomical status. The probable causes of false positive results are endogenous factors in sera samples such as alteration of density of sera during storage, the presence of anti-granulocyte, anti-HLA, antibody to cultured lymphocyte h9/lllb, and polyclonal hyperglobulinemia. Confirmatory ELISA with antigen control had the same sensitivity and specificity as Western-blot. Therefore, ELISA with antigen control can be used for confirmation test of positive or questionable results obtained from anti HIV-1 ELISA. The kit is cheaper and faster, and is very practical to use, especially in laboratories equipped with ELISA system.Key Word : anti-HIV-1 ELISA - specificity - false positive - confirmatory ELISA - Western-blot.
