Abstract

Background: Neonatal asphyxia remains the main cause of neonatal mortality as well as permanent neurological abnormality. With informed consideration about risk factors, more than half fetuses who need resuscitation could be identified prior to birth. Objectives: The study was aimed to identify antepartum and intrapartum risk factors in neonatal asphyxia of full term babies.Study Design: Case control.Material and Method: Data were collected from medical record of babies hospitalized in Subdivision of Perinatology, Department of Child Health in Dr. Sardjito Hospital in January to December 2004. The risk factors of neonatal asphyxia were identified before and during delivery. Data were analized using chi square, bivariate and multivariate analysis. Results: In multivariate analysis, antepartum factor which increases the risk of neonatal asphyxia was small for gestational of age (SGA) baby (OR: 3,43; CI 95%: 1,81-6,53). Intrapartum factors that increased the risk of neonatal asphyxia were meconium stained of amnion fluid (OR:  49,02; CI 95%: 12,34- 143,67), prolonged second stage of delivery (OR: 9,73; Cl 95%: 3,99-23,99) and Caesarean section delivery with general anesthesia (OR:  8,62; CI    95%: 2,16-34,44).Conclusion: SGA baby, meconium stained of the amniotic fluid, prolonged second stage of delivery and Caesarean section delivery with general anesthesia increased the risk of neonatal asphyxia in full term baby.Key words: risk factors - neonatal asphyxia - aterm babies

Abstract

Key words : biochemical genetic trait - glucose-6-phosphatedehydrogenase deficiency - ovalositosis--p. thalassemia

Abstract

Defects in the ventricular septum with obstruction to right ventricular outflow encompass a wide anatomic, physiologic and clinical spectrum. Large ventricular septal defects occur with pulmonic stenosis that varies from mild to severe to complete (pulmonary atresia). Small ventricular septal defects occur with mild to severe pulmonic stenosis. The most familiar and prevalent combination is tetralogy of Fallot with the incidence of approximately 0.4 to 0.6 per 1000 live births. The following illustrates four cases of Fallots tetralogy with a wide anatomic, physiologic and clinical variations. The first case is a small ventricular septa defect along with severe pulmonic stenosis, the fourth case, however, comprises a large VSD and pulmonary atresia, while in the others the anatomic variations are in between.Hypoxic spells are important aspects of the history in Fallots tetralogy. Spells are an alarming episode that should be treated as an emergency. Knee-chest position, squatting or sitting with legs drawn underneath can afford to relief dyspnea. The best treatment, however, is surgical treatment, indicated in all patients with Fallots tetralogy. The result of this surgical treatment up to now is very satisfactory.Key Wards: tetralogy of Fallot - pulmonic stenosis - ventricular septal defect - pulmonary atresia - bicuspid a pulmonalis

Abstract

Background: Hemorrhagic disease of the newborn (HDN) is a hemorrhage at the neonatal period. The most dangerous form of HDN is intracranial bleeding which may be fatal. The most frequent cause of HDN is deficiency of vitamin K dependent factors or prothrombin group. Objective: The aim of the study is to know the effect of the administration of vitamin K2 to the pregnant women on the activities of prothrombin gourp in cord blood. Methods: This was experimental design. Forty pregnant women were enrolled in this study. Vitamin K2 was given orally at the dosage of 10 mg twice daily. At delivery the cord blood was collected for the measurement of prothrombin group activities. Results: In the treatment group the median activities of prothrombin were 37.3%, F VII were 62.4%, F IX were 29.5%, and F X were 34.9%, while in the control group the median activities of prothrombin were 33.5%, F VII were 47.3%, F IX were 23.7%, and F X were 29.0%. The difference of the activities of vitamin K dependent factors between treatment group and control group was statistically significant. Conclusion: The administration of vitamin K2 to the pregnant women increases the activities of vitamin K dependent factors in the cord blood.Key words: hemorrhagic disease of the newborn - oral vitamin K2 - cord blood - vitamin K dependent factor activity

Abstract

Cutaneous larva migrans (CLM) is caused by animal hookworm larvae in man. The conventional diagnosis and treatment of CLM will injure the skin of the patients, and the results are not satisfying. Therefore it should be necessary to do the serological test to diagnose CLM.The antigens were prepared by extracting the hookworm larvae in CTAB detergent, and injected into Balb/C mice. Characterization of antigens were done, quantitatively (by spectrophotonieter) and qualitatively (by SDS PAGE electrophoresis). The results showed that the titre rate of hookworm larvae antigens was 145 ug/ml. The antigens consist of 4 protein fractions with molecular weight of 25 KD, 55 KD, 66 KD and above 66 KD. Those antigents apparently generated high immune response in immunized miceKey Words; Ancylostoma spp. - CTAB detergent - SDS PAGE electrophoresis - immunology - parasitology
