Abstract

The presence of Escherichia coli bacteria or what is often called E.coli is an indicator of contaminated food and water which shows that there is contamination in human feces. 16 Research results show that consuming food contaminated with Escherichia coli bacteria can cause symptoms of diarrhea, pain, fever, and vomiting6. Escherichia coli bacteria can be transmitted through contact with infected food handlers when processing food16. Escherichia coli can be transmitted from feces through contact with human fingers, flies, soil and water that comes into direct contact with food and eating utensils19. Diarrhea in Indonesia is an endemic disease and a potential disease that is associated with a significant mortality rate. In that year, there were 21 extraordinary events (KLB) which spread across 12 provinces and 17 districts/cities. 12. An increase in diarrhea occurred in 2018 with the number of cases reaching 4,504,524 people. In 2019, diarrhea cases decreased slightly to 4,485,513 people. The national incidence rate of diarrhea is 270 per 1,000 population12. In 2020, diarrhea in Bandar Lampung City reached 16,989, in 2021 there was an increase with the number of cases 22,371 (Central Statistics Agency for Bandar Lampung City, 2023) 4. Knowledge and updates regarding E. Coli as a cause of diarrhea are important for medical personnel.

Abstract

Recombinant DNA technology is the fusion of genetic material from two different sources. Recombinant DNA refers to combining genetic material from two different sources to create a new organism or to produce a specific protein. The main purpose of genetic recombination is to produce more evolved and adapted organisms, as well as develop biotechnology strategies, such as recombinant protein production. This technology is applied in clinical and medicine such as vaccines. Recombinant vaccines are made by utilizing recombinant DNA technology, which is a gene cloning technique that is modified to produce the specific protein product sought. Gene cloning is a procedure by which foreign DNA or genes are inserted into a host cell using a vector. Gene cloning involves the insertion of a gene of interest into a vector, resulting in the creation of recombinant DNA that can replicate within the host cell, an example being the SARS-CoV-2 DNA vaccine. Oncolytic virus, Gendicine, Modified Virus Ankara (MVA) are recombinant DNA technologies for cancer therapy. The advantages of this therapy are its ability to form new combinations of genetic material by insertion of molecules; DNA recombination technology allows the production of large quantities of certain protein molecules, which can be used in the manufacture of drugs; the production of therapeutic substances by strains of organisms can be achieved through the use of recombinant DNA technology; drugs that have unique properties that are difficult or impossible to obtain through traditional approaches can be made through DNA recombination; and DNA recombination plays an important role in drug manufacturing by facilitating the synthesis of therapeutic molecules important for human health. The disadvantages are that it has a negative impact on the environment, requires strict supervision to ensure safety and security, and ethical controversies. This technology has many benefits, but it is necessary to consider some of the disadvantages and adverse effects that can be caused.

Abstract

Asthma is one of the health problems that the world is fighting against, according to WHO in 2019 the global prevalence of asthma sufferers reached 262 million people with high mortality, causing 455,000 deaths. Apart from the high mortality rate, asthma also has a high morbidity rate. One of the reasons for the high morbidity rate in asthma is the high recurrence rate of asthma, which affects the productivity of asthma sufferers. In 2020, the asthma recurrence rate in adults was 40.7% and in children the asthma recurrence rate was divided into two groups based on age, namely in the under 18 age group it was 42.7% and in the under 5 year age group it was 52. 9%. There are many factors that influence the rate of asthma recurrence, namely allergic and non-allergic factors. One non-allergic factor that can trigger an asthma recurrence is an infection in the sinuses or sinusitis. The aim of this study was to determine the relationship between chronic rhinosinusitis and the rate of asthma recurrence in children. The method used was a review article by searching for articles using the keywords rhinosinusitis, asthma, asthma recurrence rate via Google Scholar and Pubmed, then the articles were reviewed and analyzed. The results of this review article are that there is a relationship between chronic rhinosinusitis and the rate of asthma recurrence in children, especially influencing the degree of asthma attacks in children.

Abstract

Tuberculosis (TB) continues to be the leading cause of disease and mortality caused by chronic pulmonary infection. The World Health Organization (WHO) considers that one-third of the global population is infected with Mycobacterium tuberculosis (Mtb).  Mycobacterium tuberculosis infection results in persistent lung damage, which is characterized by ongoing inflammation that damages lung tissue. The patient additionally experienced a cough for almost one month, with clear mucous, absence of blood, and mild shortness of breath. Patients had decreases in body weight and loss of appetite for about three weeks. Night sweats appeared within one month. There was no specific complaint of gastrointestinal symptoms. The patient's nutritional management was focused primarily on oral intake, with a total daily consumption of 1690 kcal. The daily protein, carbohydrate, and fat consumption is divided into 42.45 grams of protein, 253.5 grams of carbohydrates, and 37.5 grams of fat. There was no history of food allergy from the patient, therefore we decided not to impose any dietary restrictions. The improvement in body weight was used to monitor the outcome of the nutritional intervention. Nutritional issues have emerged as a significant contributor to an increasing percentage of individuals with low hemoglobin (Hb), influencing TB-related morbidity. The most prevalent causes of anemia in tuberculosis are nutritional deficiencies and malabsorption as a result of appetite loss. Poor intake of food has also been linked to an increased risk of disseminated TB. To improve treatment outcomes, nutritional support should be provided in addition to tuberculosis medicine.

Abstract

Hypertension is one of the most common problems that occurs in the population, whether diagnosed or not. Cholesterol levels are closely related to hypertension and its complications. The characteristic of geriatric patients is multipathology. In hypertensive patients with hypercholesterolemia, it is very likely to cause vertigo. Vertigo or in Latin, Vertere, means twisting. In general, vertigo is known as a syndrome that occurs due to disturbances in the balance system. Implement family doctor services based on evidence-based medicine holistically and comprehensively for patients by identifying risk factors, clinical problems, and patient management based on a framework for solving patient problems using a patient-centered, family approach, and community-oriented approach. The study carried out a case report by carrying out an overall assessment, namely history taking, physical examination, and visits to the patient's home to assess the environment. Secondary data was obtained from the patient's medical records. Assessment is based on a holistic diagnosis of the beginning, process, and end of the study qualitatively and quantitatively. Based on an examination of a 70-year-old male patient with complaints of dizziness, nausea, vomiting, and blurred to dark vision. The patient also admitted that he had soreness in his neck and shoulders. The patient admitted that the complaints came and went. On physical examination, it was found to be within normal limits. On supporting examinations, high cholesterol levels were found. Holistic and comprehensive management was carried out. Services with a family medicine approach in pharmacological and non-pharmacological therapy can solve health problems and improve the patient's quality of life. The role and support of the family is very much needed in monitoring.

Abstract

Herpes zoster is a manifestation of reactivation of latent endogenous infection of the varicella zoster virus in the neurons of the dorsal root sensory ganglion, cranial nerve ganglion or autonomic nerve ganglion which spreads to nerve tissue and skin in the same segment. Herpes zoster occurs sporadically throughout the year without knowing the season. The incidence of herpes zoster reaches two to three cases per thousand population each year. The incidence and severity of the disease increases with age. More than half of all cases reported occur in people over 60 years of age and complications occur in almost 50% of people in old age. Herpes zoster is rarely found at an early age (children and young adults), but when it occurs, it is likely due to maternal varicella during pregnancy. The incidence of herpes zoster in men and women is the same. The risk of disease increases with the presence of malignancy, or with bone marrow/kidney transplantation or HIV infection. There is no gender predilection. Herpes zoster is contagious but its transmission capacity is small compared to varicella. The treatment received by the patient was observing bleeding, checking vital signs periodically, and providing pharmacological treatment in the form of acyclovir 400mg tablets and acyclovir cream. Patients are advised to check in a week later. The prognosis for this patient is dubia ad bonam.

Abstract

Primary amenorrhea is a condition where menstruation does not occur in women aged 16 years. This situation occurs in women of reproductive age 0.1-2.5%. Meanwhile, secondary amenorrhea is the absence of menstruation for three cycles or 6 cycles after previously having a normal menstrual cycle. The incidence rate is around 1 to 5% in women of reproductive age. Treatment or management of amenorrhea depends on the cause. If the cause is drastic weight loss or obesity, sufferers are advised to follow an appropriate diet. If the cause is excessive exercise, sufferers are advised to reduce it. If the cause is a tumor, then surgery is performed to remove the tumor. So basically treating amenorrhoea always requires the help of a doctor to help diagnose or find the cause. The patient came to the Pertamina Bintang Amin Husada Hospital polyclinic on February 6 2024 with complaints of no menstruation since 4 months accompanied by lower abdominal and back pain. The treatment obtained by the patient was observation of bleeding, checking vital signs regularly, norelut tab twice a day, 2 tablets for 5 days, the patient was advised to diet. The prognosis for this patient is dubia ad bonam. The working diagnosis in this case is secondary amenorrhea based on clinical considerations, physical examination findings, and supporting examinations.

Abstract

Myasthenia gravis is an autoimmune disease involving the neuromuscular junction (NMJ) of skeletal muscles. It clinically manifests as muscle weakness that worsens with activity or fatigue and improves after rest. It typically involves muscles of the eyes, throat, and extremities. The global epidemiology of myasthenia gravis (MG) affects 30 per 1 million people per year, with an incidence rate of 20 per 100,000 population. In men, the disease often occurs at the age of 60 years. Various conditions can trigger it such as infections, immunizations, surgeries, and medications. Pneumonia is a serious form of acute lower respiratory tract infection in lung parenchyma, encountered in about 15-20% of cases. Case: A 60-year-old male patient presented with complaints of shortness of breath, difficulty swallowing, difficulty opening eyelids, and productive cough. From the history, physical examination, and diagnostic tests, the diagnosis of myasthenia gravis and community-acquired pneumonia was established. The patient received therapy with pyridostigmine bromide 5 x 60mg, methylprednisolone injection 125mg every 12 hours, and levofloxacin injection 750mg every 24 hours.

Abstract

Video-assisted thoracic surgery (VATS) lobectomy provides a minimally invasive approach for the management of early-stage lung cancer. This systematic review and meta-analysis is aim to provide the outcomes between VATS and thoracotomy lobectomy. The research is aim to compare between video-assisted thoracoscopis surgery and thoracotomy in non small cell lung cancer patient. Five articles databases were searched via PubMed. Primary outcomes were operative time, postoperative hospital stay, chest drainage volume, chest drainage time, and number of lymph node dissesction. Review Manager (RevMan) [computer program] version 5.4 was used to pool the data. There were significant results VATS has shorter postoperative length of stay (95% CI 1.08 to 4.83) and chest drainage volume (CI 25.83 to 344.98). There were not significant results in terms of operative time (95%CI -30.88 to 40.10), chest drainage time (95%CI -0.95 to 2.50), and number of lymph node rdissection (95%CI -0.67 to 1.740). VATS lobectomy is superior to thoracotomy lobectomy in terms of shorter postoperative length of stay (95% CI 1.08 to 4.83) and chest drainage volume (CI 25.83 to 344.98). The results also showed no significant differences between the two treatment modalities regarding the operative time, chest drainage time, and number of lymph nodes dissected.

Abstract

Hoarse voice (dysphonia) is one of the reasons for consultation (1%) in all primary health services. Vocal cord paralysis is reduced movement or paralysis of the vocal cords. This situation is a manifestation of disease that occurs in the cranial cavity, larynx, thorax and mediastinum which causes paralysis of the vagus nerve and recurrent laryngeal nerve. Mr. I, a 56 year old male, came with complaints of sudden loss of voice. Complaints appeared 11 days before admission to the hospital (SMRS). Another complaint that is felt is that patients choke more easily when drinking. Complaints are felt to be getting worse day by day. The patient had previously been given antibiotics and anti-inflammatory drugs given at the puskesmas but there was no improvement either. The patient was previously hospitalized due to complaints of excessive vomiting. The patient's general status is within normal limits. LFO results showed adductor type unilateral paramedian vocal fold paralysis in the dextra paramedian position. The patient was given a physiotherapy plan and given steroids and vitamin B12. Voice therapy can be an effective treatment option in patients with unilateral paresis of the vocal folds and should be considered individualized according to the patient's age.
