Abstract

Background: Immunity status is a system that protect the human body against foreign substances. Good immunity status can prevent various diseases. Several risk factors that make a person susceptible to infection is less nutritional status. Immunonutrition intake consists of some specific nutrient components that can form the immune system. This study aims to determine the relationship between nutritional status and intake of immunonutrition (proteins, zinc and iron) with the immunity status. Method: This study is an analytic observational study with cross sectional design at 78 students  of medical faculty in Lampung university. Sampling was taken by proportionated random sampling. Nutritional status was obtained from anthropometric examination using the body mass index criteria, the immunonutrition intake taken through 3x24 h food recall and the immunity status was taken by lymphocyte numbers. Result: The results showed that in general, the immunity status are good (58.97%), nutritional status are normal (66.67%), protein intake are adequate (62.82%), zinc and iron intake classified inadequate (71,79%; 65,38%). There is a significant association between the intake of protein and iron with the immunity status (p=0.004; p=0.014) and there is not significant association between nutritional status and intake of zinc with the immunity status (p=0.192; p=0.599). From the four independent variables that have a more meaningful relationship is the intake of protein and iron. Conclusion: The intake of immunonutrition (protein and iron) was associated with the immunity status. [JuKe Unila 2014; 4(8):158-165]

Abstract

Background: World Health Organization through tuberculosis (TB) control averted up to six million deaths and cured thirty-six million people in 1995-2008, but had less success in reducing TB incidence, especially in thirteen high burden countries, including Indonesia. Therefore, TB control will need to have more emphasis on the issues of social determinants, as social determinants affect TB’s incidence directly and/-or through TB’s risk factors. This study aimed to identify social determinants characteristics of TB patients in Bandar Lampung, during period January–July 2012. Methods: The research setting was at twenty-seven primary health centers and one hospital that have implemented the Directly Observed Treatment Shortcourse (DOTS) strategy in Bandar Lampung. Respondents of this research were 238 TB patients and 238 patients without TB. Research variables consisted of social determinants, which measured through its indicators (education, occupation, family income, and social class). Data have been collected by using questionnaire and then was analyzed with Stata 11. Results: Tuberculosis patients had lower education, occupation, family income, and social class compare to patients without TB. Conclusion: The knowledge of social determinants characteristics of TB patients can be used to support TB control program, particularly to implement DOTS strategy together with improving social determinants. Abstract

Background: One of Diabetes Melitus (DM) complication to the eye that may result as visual disturbances and blindness is cataract. According to The Framingham Eye Study and the Health and Nutrition Examination Survey shows that patient with DM before age of 65 years is easier 3-4x to get cataract and 2x easier than the age after 65 years compared to non DM. The research about risks factors for diabetic cataract in type 2 DM in Indonesia is rare, so that it is necessary to do the research about these risk factors. Method: This study involves observational analytic study with case control study design. Subject of the case is type 2 DM with cataract (73 people) and type 2 DM without cataract as control (73 people) at Dr. Kariadi Hospital Semarang. Then data followed retrospectively about risk factors that influence incidence of cataract in type 2 DM. The stages of analysis used in this study is univariate analysis, bivariate analysis and multivariate analysis. The statistics test used in multivariate analysis is Multiple Logistic Regression. Result: The multivariate analysis shows that 5 years Duration of DM or longer (P=0,002) and Trigliserida value ≥150 mg/dl (P=0,015) are the risks factors for cataract in type 2 DM. Conclusion: 5 years duration of DM or longer and trigliserida value ≥150 mg/dl are factors that proven as risk factors for cataract in type 2 DM. [Juke Unila 2014; 4(8):173-179]

Abstract

Background: Based on the Health Law No. 36 year 2009 part 164(1) and (2) about the Occupational Health mandates that every workplace should hold an Occupational Health and Safety (OHS) to prevent health problems for workers. This is also include for workers in the agricultural sector that uses pesticides. The behavior of using pesticide must be on the rules set by the Ministry of Health of the Republic of Indonesia. Study about the behavior of using pesticides on farmers at the RJ village of Bandar Lampung. Method: Design of this study was a cross sectional study. Sample of this study 50 farmers. Data were collected by using interviews and questionnaire. The behavior of using pesticides include: the use of Personal Protective Equipment (PPE), the direction of spraying, washing hands after spraying, changing clothes after spraying, taking a bath after spraying, and smoking after spraying. Result: The use of PPE was complete about 8%, spraying pesticide in the direction of the wind about 80%, washing hands after spraying about 66%, changing clothes after spraying about 82%, taking a bath after spraying about 66% and smoking after spraying about 50%. Conclusion: Behavior on using PPE and smoking after spraying was low. All sectors need to support farmers’ behavior to use pesticides wisely. The impact is to increase farmers’ health status. Abstract

Background: Indonesia is one of countries which have inhabitant of elderly highest ever. In 2020 Indonesia will be the fifth highest of elderly. East Java province has included in groups the area having structure aging population. East Java have the number of elderly second highest after Yogjakarta (10.4%). The growing number of elderly would have become complex problem for senior family and community including the physical aspects, biological, mental, or socially economy. Along with the matter will affect on nutrition status for elderly. The aim of this studi is to determine correlation between depression symptom with nutritional status of elderly in Jember regency. Method: This research was observational with cross sectional design. Subjects were elderly in Sumbersari sub-district, Jember regency who fulfilled inclusion criteria. Research subjects consisted of 120 elderly. Depression symptom was measured with Geriatric Depression Scale-15 (GDS-15), dietary intake with semi quantitative food frequency questionnaire (SQFFQ) method, and nutritional status was determined based on body mass armspan (BMA). Data were analyzed with chi squaretest and multiple logistic regression. Result: The proportion of depression symptom in Sumbersari sub-district, Jember egency was by 27,5%. Most of the subjects who suffer depression symptom have more and less nutritional status 69,7% (23 elderly), and have a good nutritional status for as many as 10 elderly (30.3%). The greatest distribution of subjects with depression as much as 31 elderly (33.7%) and the subject with no depression as many as 61 elderly (66.3%) female subjects in the group scattered. Abstract

Background: Peer-Assisted Learning (PAL) has been widely used in the clinical skills training in medical education. The study of this area shows that PAL-scheme program has benefits both in student tutors and tutees in mastering their clinical skills. However, student’s motivations in this PAL-scheme program haven’t much been explored. The aim of this study is to explore student’s motivations in a peer-assisted clinical skills training program. Method: This study used exploratory focus group discussion (FGD) of 4 groups of 3rd year students (2 tutors and 2 tutees) Faculty of Medicine, Universitas Gadjah Mada and in-depth interview to 6 faculty staffs which were chosen purposively. The data were then transcribed and analyzed for themes. Result: The results of this study showed that when joining a PAL-scheme program, tutees were motivated extrinsically as a preparation for the end of year Objective Structured Clinical Examination (OSCE), whereas tutors were motivated more intrinsically. Various spectrums of motivations were founds, student’s motivations in a peer-assisted clinical skills training program were different between tutors and tutees. Tutors were motivated intrinsically whereas tutees extrinsically. Conclusion: Improvements in organizing PAL program need to be done to enhance the tutees motivation more intrinsically and also to improve the quality and quantity of clinical skills training. Abstract

Background: Stress has been shown can change the structure of the brain, especially in the cornu ammonic (CA) I region of hippocampus. Centella asiatica or pegagan as herbal neuroprotector was investigated. The study aimed to determine the effect of ethanolic extract of C. asiatica to the thickness of pyramidal layer in the CA1 region in rats subjected to chronic restrain stress (CRS). Method: This experimental study used post test only group design. Thirty five rats were divided into 6 groups: non stress group (control) and stress only group pulvis gum arabicum (PGA), positive control group (treated with fluoxetin 10mg/kgBW), and 3 treatment groups are treated with C. asiatica 150, 300, and 600mg/kgBW respectively. All groups except non stress group were subjected to CRS, for 6 hours per day for 21 days. Fluoxetin, PGA, and C. Asiatica were applied 30 minutes before CRS done. Rats were sacrificed at the end of 24th day and were perfused transcardially. Processed brain tissue stained with toluidine blue, and measured with Image Raster program. Result: The thickness of pyramidal layer in the CA1 region are increasing significantly in treatment groups which treated with 150 and 300 kg/BW of C. asiatica, but not significant which treated with 600mg/kgBW ethanolic extract of C. asiatica. Conclusion: Ethanolic extract of C. asiatica can improve thickness of pyramidal layer in the CA1 region of hippocampus of Sprague Dawley rats after chronic restrain stress. Abstract

Developmental dysplasia of the hip (DDH) is a spectrum of pathology ranging from acetabular dysplasia with minor instability until acetabular dysplasia with hip dislocation. The incidence of DDH that require treatment is 5.1 per 1000 live birth. Early detection is important to deliver good outcome and lessen complication. Careful history taking, physical examination and ultrasonography, have increased the sensitivity in establishing diagnose of DDH. Pavlik harness is the mainstay of treatment for infant who has not commenced crawling. Despite the fact of high success rate, the use of Pavlik harness is still uncommon in Indonesia. This paper reports a case of DDH treated with Pavlik harness and assessed regularly with ultrasonography. A 3 months girl with developmental dysplasia of the left hip. The initial ultrasound shows α angle 49.40 and β angle was 50.20, femoral head subluxated to superolateral aspect. The patient had Pavlik harness and followed up with serial ultrasound at 1st, 3rd and 4th month’s time. Having had 4 months of Pavlik harness, the α angle of the left hip became 630, and β angle 490 with good structure of ilium. Femoral head was smooth and no signs of avascular necrosis. The hip has full range of motion. Parents have no difficulty in taking care of their daughter daily activities such as taking a bath, changing napkin, and clothes. This implicates, patient treated using Pavlik harness showed good outcome clinically and ultrasonographically. Pavlik harness has been proven to be a safe, simple and user friendly method for treatment of DDH before crawling age. The good result obtained from this case could initiate application of pavlik harness in larger population of early detected DDH in Indonesia. [JuKe Unila 2014; 4(8):208-217]

Abstract

The prevalence of peripheral arterial disease (PAD) is high in the HIV-infected population and is much higher than expected in the general population. A 27 years old male patient came to Cipto Mangunkusumo General Hospital (RSCM) with chief complaint of pain on right leg which worsened since 2 days prior admission. Patient was diagnosed HIV positive 8 months prior admission and had already on ARV treatment Duviral (AZT/3TC) and Alluvia (lopinavir/ritonavir) since then. CT angiography showed stenosis of femoral communis artery with total stenosis of right posterior tibialis artery. Patient was diagnosed with criticl limb ischemia and got amputation. Some theories of PAD mechanism in HIV patient have been proposed. Highly ac tive antiretroviral therapy (HAART) has decreased the mortality and illnesses related to HIV infection. However, a variety of atherogenic metabolic abnormalities, including dyslipidemia, lipodystrophy, and also thickened intima-media thickness have been observed after the introduction of HAART, especially using Protease Inhibitors. Human Imunodeficiency Virus itself is also thought to have role in injury to the arterial wall such as via chemokine CCL2/MCP-1, a critical mediator  of  atherosclerosis. All HIV-infected patients candidate to antiretroviral therapy and patients already under treatment should undergo an assessment that includes the evaluation of the cardiovascular risk. Abstract

The incidence of heart valve disease, especially mitral valve prolapse was first reported in hyperthyroid heart disease in 1981. Incidence of heart valve disease in the form of mitral regurgitation (MR) or tricuspid regurgitation (TR) reached 23% in patients with hyperthyroidism. 45 years old male with a chief complaint tightness, worsened since 1 week before entering the hospital. Since 2 years ago patient often feels palpitations and breathless if activity. About 5 months ago, patient was sweating a lot and tremble, increase in appetite but weight going down. Defecation 5-6 times a day. Visible and palpable lump in the neck diameter ±5 cm, soft, ned, part moves while swallowing. Laboratory showed free T4: 7.550, sensitive TSH: 0.010. Thoracic X-ray showed cardiomegaly with cardiac waist disappeared. ECG showed atrial fibrillation. Echocardiography showed LA + LV dilatation TAPSE 1.91, AR Moderate, Severe MR, AML prolapse in A2 and PML with EF: 62.7%. Ultrasonography showed Throid hyperthyroidism and thyroid scan results with the impression difusa bilateral goitre within crease of total thyroid uptake. Patients received digoxin, furosemide, PTU. Patients then performed MV Repair and AV Repair. The mechanism of valvular heart disease relationship in hyperthyroidism has not been completely understood. In patients with MPV, some studies have shown a high prevalence of HLA-Bw35. Increased production and secretion of acid polymukosakarida in Graves can accumulate all the heart valves, especially the mitral valve, causing the valve to thicken (myxomatous). MR in hyperthyroidism is said to be a consequence of mitral annulus dilatation due to the left ventricle is dilated. It has been reported also a gernline mutations of thyrotropin receptor gene in Chinese families with thyrotoxicosis and MVP. In hyperthyroid patients is important to explore the patient's cardiovascular condition, including the possibility of valve disorders. [JuKe Unila 2014; 4(8):225-232]
