Abstract

Although thought to be uncommon, it is believed that cervicofacial actinomycosis is underreported due to frequent misdiagnosis. It has been called the “most misdiagnosed disease by experienced clinicians”. A wide range of dental procedures or trauma, including tooth eruption, may precipitate actinomycosis by enabling microorganisms to penetrate the deeper tissue layers via an entry point. This infective disease is significant due to its potentially aggressive and locally destructive nature. This case describes a patient diagnosed with cervicofacial actinomycosis. Her history prior to this was significant for proliferative verrucous leukoplakia, and T4aN0M0 (AJCC 7th Edition) squamous cell carcinoma of the right hard palate for which she received surgery and post-operative radiotherapy. The mechanism of actinomycotic infection, its presentation, differential diagnosis, and management is discussed, with review of the relevant literature. This case highlights an unusual case of cervicofacial actinomycosis and demonstrates the complexities in reaching a definitive diagnosis in cases of osteomyelitis and osteoradionecrosis with significant bacterial burden.

Abstract

The prevalence and clinical characteristics of temporomandibular disorders (TMD) in the Mediterranean region of Türkiye have not yet been thoroughly investigated. Objective: This study aimed to determine the prevalence and severity of TMD in a sample of the population in this region and to characterize the clinical findings related to TMD. Methods: Four hundred and one participants were included in this study. “Presence of TMD” in the participants was evaluated using the Fonseca Anamnestic Index. Through clinical examination, the findings in the participants were classified as limited mouth opening, deviation, temporomandibular joint (TMJ) sounds, TMJ pain, and muscle pain. Results: The prevalence of TMD was found to be 66.8%, with “Mild TMD” being the most frequent diagnosis among the participants. Clinical findings related to TMD were detected in 48.6% of the participants, and TMJ sounds were the most common clinical finding. TMD was most commonly found in participants over the age of 52, and clinical findings related to TMD were most common in participants between the ages of 25 and 38. In addition, a relationship was found between “Presence of clinical findings” and the gender and age of the participants (p < 0.001 for both comparisons). Conclusion: The results show that females may be more prone to TMD than males. During clinical examinations, it should be considered that “Presence of clinical findings” may be related to gender and age.

Abstract

Objective: This review aimed to seek insights into the challenges faced by caregivers and children with autism spectrum disorder (ASD) when performing and accessing oral care and their recommendations to overcome the matter. Methods: Six electronic databases were used to search for published English full articles using a standardised strategy and grouped using thematic analysis as findings to the study’s goal. Results: The initial screening found 46 relevant articles, but 21 were deemed eligible. Studies were mostly observational (18 crosssectional surveys and 3 case-controls), retrieving data from online or face-to-face surveys and insurance health info. The perceptions and potential solutions made by parents, teachers, doctors, and individuals with ASD were diverse but mostly consistent in most studies with a focus on issues of oral health as low priority, incompetency in oral health care performance at home, children’s cooperation and involuntary behaviours in dental settings, and constraints in related resources. Conclusion: Carers’ lack of awareness and prioritisation of treatment for children with ASD, challenges in performing oral care at home, concerns over oral health care provided by caregivers in educational institutions and dental clinics, and challenges in accessing more supportive oral health care services were highlighted as barriers to oral care.

Abstract

Objective: This study aimed to investigate the effect of the COVID-19 pandemic on the anxiety levels and career satisfaction of dentistry students. Methods: A total of 220 dentistry students were included in the study. A questionnaire was mailed to dentistry students at Trakya University between June 2021 and January 2022. Beck Anxiety Inventory (BAI) was used to evaluate anxiety levels. Data were analyzed, and p < 0.05 was considered statistically significant. Results: The mean BAI scores of the participants were 14.6 ± 13.6. The anxiety levels of the females were significantly higher than the males (p = 0.004). There was a significant relationship between the level of knowledge about COVID-19 and anxiety (p = 0.032). More than half of dentistry students were anxious about the pandemic conditions and concerned about being infected during treatment. Moreover, most participants were anxious about infecting their patients and families, yet 86.8% do not regret choosing dentistry as a profession. Conclusion: An increase was observed in the anxiety levels caused by the pandemic in dentistry students. The outcomes of the current study revealed that the COVID-19 pandemic affected female dental students more. Although the COVID-19 pandemic negatively affected the psychological health of dental students, most participants were satisfied with their careers.

Abstract

Herbal products are gaining popularity due to their diverse biological action and safety. Mangosteen peel extract has antibacterial and anti-inflammatory properties and can be used to treat periodontal diseases. Objective: To compare the efficacy of mangosteen peel extract and tetracycline gel as an adjunct to non-surgical management of periodontal abscess in type-II diabetes patients. Methods: This was a double blinded, randomized clinical trial included 20 Patients with Type-II diabetes. Both test and control groups received NSPT followed by adjunctive local application of mangosteen peel extract and tetracycline gel. Clinical parameters i.e., Probing depth (PPD), Clinical attachment level (CAL), Bleeding Index (BI) and Landry’s healing index (HI) were evaluated at base line and 1 month. Results: In intra and intergroup analysis, both the test and control groups had a better reduction in PPD, BI and CAL with a statistical significance. However, there was no statistical significance between the groups in healing index and showed no change in patient perception in both the groups. Conclusion: 4% mangosteen gel has significantly reduced the BI and PPD with increased CAL and acceptable subjective perception with no recurrence of periodontal abscess over the traditionally used tetracycline gel.

Abstract

Resolving substandard oral healthcare data quality requires identifying the influencing factors. However, no known instrument for such assessment can be used locally. Objective: To develop and validate the HAI-DQ questionnaire for assessing human factors that influence oral healthcare data quality among primary oral healthcare workers in Malaysia. Methods: The HAI-DQ questionnaire was developed based on literature reviews, focus group discussions, and focus observations. Six experts assessed its content validity using the Content Validity Index (CVI), and the Face Validity Index (FVI) was used to establish its face validity through a pre-test involving 30 primary dental personnel. Results: Four domains with 57 items were derived during the HAI-DQ development. The I-CVI values between 0.83 to 1.00 and the S-CVI value of 0.97 indicated acceptable content validity. Excellent face validity was denoted by the I-FVI values of 0.93 to 1.00 and an S-FVI value of 0.99. Conclusion: HAI-DQ is relevant and comprehensible for assessing human aspects that influence data quality, indicating its potential to be used as an investigation tool in future research related to oral healthcare data quality. Nevertheless, other psychometric assessments are required to further support the instrument’s validity.

Abstract

The geometric design of an endodontic file includes cross-sectional shape, taper, and pitch. NiTi endodontic files of OneCurve (MicroMega) are made of two variations of cross-sectional shape, that is, double-s shaped and triple helix. Various kinds of modifications were made to increase the resistance of the NiTi endodontic files from fracture. Cyclic fatigue is the most common factor that causes a fracture of the endodontic file. Objective: The purpose of this study was to obtain a NiTi endodontic file design that has the best cyclic resistance value based on its taper size and number of pitch. Methods: This study used OneCurve endodontic file size 25.06, then scanned with MicroCT Scan and modified the geometric design. The number of file modifications was nine. Taper size modifications were 4%, 6%, and 8%; the number of pitch modifications was reduced by three, fixed, and added by three. Cyclic fatigue measurement simulations were carried out using the finite element method three times each. The result data were analyzed using a two-way ANOVA test and then continued with LSD. Results: The results of the statistical test showed that there was an effect of the size of the taper on the cyclic fatigue values. Endodontic files with a size of a taper 4% had the lowest cyclic fatigue value. Conclusion: The conclusion of this research was that the taper size affects the value of cyclic fatigue in continuous endodontic files made of nickel-titanium with variable cross sections. The number of pitch did not show a significant effect on the cyclic value.

Abstract

Objective: This study aimed to evaluate the smile esthetics of individuals and the factors affecting it using the smile esthetic index (SEI) and compare them with the patient’s perception of smile. Methods: Seventy-six patients consisting of 44 females and 32 males who presented to for routine periodontal treatments were included in study. The smile esthetics were evaluated by three researchers on the photographs taken from the patients. The visual analog scala was used for the subjective evaluation of smile esthetics. Results: According to the results obtained from the study, the average of the SEI total scores was 6.38; the mean subjective evaluation of smile aesthetics was 6.33. There was no statistically significant difference depending on gender and age in the subjective and objective evaluations of the patients’ smile aesthetics. A statistically significant correlation was found between the physician’s scoring for objective smile aesthetics and the age of the patients. Conclusion: The SEI is an index that can be applied in the clinic and is used to evaluate the smile esthetics revealing objective values. Different esthetical outcomes may occur with objective and subjective evaluation. The use of these indices for diagnosis and treatment planning will allow for developing appropriate treatments.
