Abstract

Objective: This study aimed to analyze factors contributing to delayed diagnosis and treatment of UDT patients at Saiful Anwar General Hospital in Malang. Material & Methods: This retrospective study was conducted at Saiful Anwar General Hospital focusing on patients who underwent orchiopexy or orchiectomy for UDT between January 2019 to February 2024. Delays in treatment were defined as surgery performed after 18 months of age. Statistical analysis involved a comparative study between delayed and non-delayed cases, with a significance level of p<0.05. Results: This study evaluated 140 patients with UDT, aged 3.7 ± 3.87 years old, with 77.1% experiencing delayed referrals. The most common associated anomaly was hypospadias (29.3%). Factors contributing to delayed referral include undiagnosed UDT, inadequate follow-up, and advice to await spontaneous testicular descent, observed in 62% of delayed cases (p = 0.025). Parental unawareness of the risks associated with delayed surgery and misdiagnosis by healthcare providers were significant contributors, affecting 72% and 74% of delayed cases, respectively (p = 0.032 and p < 0.005). Referral sources differed significantly, with pediatricians accounting for 43% of non-delayed referrals, compared to only 8% for delayed cases. General practitioners were responsible for 70% of delayed referrals, and 22% were self-referred (p < 0.005). Conclusion: Most pediatric patients at Dr. Saiful Anwar General Hospital experienced delayed treatment of UDT, often due to referral delays and undiagnosed conditions. Training for primary health-care providers and routine testicular examinations are essential. Keywords: Delayed factors, diagnosis and treatment, undescended testis.

Abstract

Objective: This study aimed to determine the prevalence of phimosis in toddlers at the health posts within the working area of the Wagir Health Center, Malang Regency, assess parents' knowledge, and analyze the factors that influenced parents' knowledge about phimosis. Material & Methods: This study was descriptive and observational analytical and involved 248 male toddlers. Data were obtained through questionnaire responses and physical Examination which were then analyzed using the SPSS software by Pearson’s Chi-Square comparative test and Binary Logistic regression. Results: Phimosis cases were found in 71.77% of 248 toddlers, with 82.6% of them having parents with a lack of knowledge. There was a significant association (p-value <0.05) between family history factors and parents' income level with parents' knowledge of phimosis. However, there was no significant relationship (p-value > 0.05) between the factors of parents' education history and the access to healthcare facilities with parents' knowledge of phimosis. Conclusion: Parents' knowledge of phimosis is low or still lacking. Keywords: Phimosis, parents' knowledge, sociodemographic factors, health factors, prevalence.

Abstract

Objective: This case report was aimed to present a lateral percutaneous nephrolitotomy (PCNL) approach for a patient with horseshoe kidney, few comorbidities, and the outcome of this procedure. Case(s) presentation: A 70 years old obese male came into our hospital with pain in the left flank. Upon CT scan examination, we found that the patient has Horseshoe kidney with left hydronephrosis. The hydronephrosis resulted from obstruction caused by left kidney pelvic stone with a size of 25 x 20 mm. We decided to do lateral PCNL for this case with an excellent result. Discussion: With newer advancements in urology, different minimally invasive treatment modalities are available to manage stone disease in HSKs. PCNL has shown the better result with fewer complications to remove calculi from HSKs and is accepted as a routine treatment of large calculi in HSK, showing a high SFR ranging from 71% to 88%. Lateral percutaneous access of the renal collecting system is safe, as it is gained on the opposite side to where the renal blood vessels enter the kidney. Conclusion: Stone management in patients with horseshoe kidneys is a challenging procedure in endourology, however percutaneous nephrolithotomy is considered a safe and effective treatment option in patients with renal stones in a horseshoe kidney with acceptable result. Abstract

Objective: To find out the description of the characteristics of Priapism patients. Material & Methods: This research used a total priapism patients in Arifin Achmad Hospital, Riau Province from January 2023 to February 2023. Results: From 7 patients with priapism who met the inclusion and exclusion criteria, most age group were from the 36-45 year age group in 3 (42.9%) patients. History of disease in patients with most priapism was leukemia in 3(42.9%) patients. Conclusion: Classification of ischemic priapism were in 4 (57.1%) patients and non-ischemic priapism was 3 (42.9%) patients and all management of priapism patients was carried out by surgery (100%). Abstract

Objective: This study aims to identify the characteristics of GPPPD in urologic patients. Material & Methods: The research design used descriptive cross-sectional study conducted at urology clinic patients of Arifin Achmad Hospital, Pekanbaru, Riau Province in October 2021 to January 2022 The patients were interviewed based on validated questionaire. The data collected were urology diseases and disorders, age, symptoms, risk factors of biology, interpersonal, psychological, socio-cultural, sexual satisfaction. The study had been approved by ethical committee of Medical Faculty, Riau University. Univariate analysis were used. Results: There were 22 patients in the study with most urology disease were urinary tract infection (54.5%), while the most common age group was 41-60 years(77.3%) and the common risk factors were biological (41%), psychological (13.6%) and socio-cultural ones (100%). Conclusion: The characteristics of GPPPD in urologic patients were mostly with UTI, age group of 41-60 years and socio-cultural factors. Keywords: GPPPD, urology, urinary tract infection.

Abstract

Objective: This study aims to evaluate the probability of stenosis that occurs in the ureter due to the use of electrocautery during ureteral incision procedures in Laparoscopic ureterolithotomy (LU). Material & Methods: This study was conducted in Fatmawati Hospital Jakarta, we collected the data retrospectively in 22 patients diagnosed with ureteral stones who underwent LU procedure from 2014 to 2021, are eligible and have completed the follow-up protocol. The patient was evaluated by assessing the condition of the kidney after the procedure using ultrasound and CT scan to evaluate the presence of ureteral stenosis. Result: Ureteral stenosis was found in 6 of 22 patients (27.3%). Median follow-up time was 45 months (3-4 years). All of them were asymptomatic. Conclusion: There is a 27.3% chance patient will develop ureteral stenosis after LU. Though LU is one of the best methods for extracting large ureteral stone, the late complication after LU should be considered. Abstract

Objective: This study aims to determine the effect of giving pelvic floor muscle biofeedback on improving erectile function in patients with LUTS. Improvement in erectile function was assessed by IIEF (International Index of Erectile Function), EHS (Erectile Hardness Score), and pelvic floor muscle strength. Improvement in LUTS symptoms was assessed by an IPSS (International Index of Prostate Symptoms). Material & Methods: This research was a randomized controlled trial study on outpatients at Cipto Mangunkusumo General Hospital. The research subjects were divided into 2 groups, the treatment group received 10 sessions of pelvic floor muscle biofeedback and home exercise. The control group only received home exercise. Results: There were 21 LUTS patients with ED. A total of 10 subjects (50%) had mild ED, 4 subjects (20%) had moderate ED, and 6 subjects (30%) had severe ED. From the results of the study, it was found that the scores for IIEF, IPSS, EHS, slow twitch, and fast twitch were improved in the treatment group (p-value <0.05), whereas in the control group there were only improvements in IIEF and IPSS scores (p-value <0.05). Conclusion: There was an improvement in erectile function (increased IIEF score, EHS score, decreased IPSS score, and increased pelvic floor muscle strength) after pelvic floor muscle biofeedback in ED patients with LUTS for 10 sessions. Abstract

Objective: This study aims to investigate post-operative complication rate of Tenckhoff catheter placement methods between open surgery and laparoscopic. Material & Methods: This is a case control comparative study reviewed from the medical records of patients who required insertion, removal, or repairment of Tenckhoff catheter for the provision of CAPD at Hasan Sadikin Bandung general hospital between January 2015 to December 2020. Clinical outcome and complication were compared between the open surgery and laparoscopic group. Results: We obtained 30 patients who required insertion of a Tenckhoff catheter for the provision of CAPD, 15 patients by open surgery and 15 patients by laparoscopic technique. Insertion of Tenckhoff catheter using open surgery have a higher risk of catheter migration (p=0.049; O R=3.25) and infection (p=0.014; OR=12.25) compared to laparoscopic. Discussion: Laparoscopic technique facilitates omentectomy, allows better fixation under direct visualisation, and for lysis of adhesions to increase peritoneal surface. Conclusion: Tenckhoff catheter insertion by using laparoscopic surgery tend to have better outcome compared to open surgery, with lower risk of catheter migration and infection. Abstract

Objective: Research on the distribution of urology in Indonesia, especially in East Java, has not been widely discussed. There are still several areas in East Java that do not have a urologist at all. We try to find out the factors underlying this distribution through several indicators. Material & Methods: Data from all hospitals in East Java were compiled with data of urologists in East Java obtained from the Indonesian Urological Association, supplemented by information from social media and the telephone of the hospital operator. The number of urologists and hospitals with urology services is then correlated with the human development index, the education index, purchasing power index, health index, and regional minimum wage. Results: The ratio of urologists in each district and city in East Java is not evenly distributed, and likewise with the hospitals that provide urology services in each district and city in East Java. The ratio of urologists and the number of hospitals providing urology services are significantly related to the human development index, the education index, the purchasing power index, the health index, and the regional minimum wage. Conclusion: The location of a urologist's practice is related to the development of an area, which in this study is assessed through indicators of human development, education, health, and economy. Abstract

Objective: This systematic review aims to determine the efficacy and safety of topical triamcinolone in reducing urethral stricture recurrence following internal urethrotomy procedures. Material & Methods: A systematic literature search was performed in Cochrane Library, PubMed and Google Scholar with the related keywords. The statistical analysis was conducted using a Review Manager v.5.4.1. Results: We identified 6 RCTs with a total of 316 patients diagnosed with urethral stricture undergoing internal urethrotomy. The statistical analysis appeared to lower the recurrence rate significantly in patients undergoing internal urethrotomy with the adjuvant therapy of topical triamcinolone (RR 0.55, 95% CI 0.38 – 0.78, p = 0.0008). Conclusion: Additional treatment with topical triamcinolone has the benefit of declining the recurrence rate of urethral stricture following internal urethrotomy.
