Abstract

Objective: This paper aims to report a case of penile cancer with multiple prepuce stones in a 46-year-old non-circumcised male. Case Presentation: A 46-year-old male patient came to the clinic with complaints of a slowly enlarging penis since the last 2 months. Complaints are accompanied by difficulty and pain when urinating, sometimes there are stones that come out when urinating. Physical examination revealed an enlarged penis, irregular shape, tenderness, unclear borders. Multiple prepuce stones, pus accumulation, and suspicion of malignancy in the penis were observed. Histopathology showed squamous cell carcinoma (SCC) of the penis. The patient was discharged 4 days postoperatively and prepared for follow-up treatment. Discussion: Penile cancer and prepuce stones are very rare cases. Smegma accumulation, chronic inflammation, and urinary stasis are the pathophysiology of penile cancer and prepuce stones. Conclusion: Penile cancer and preputial stones are very rare. We performed a surgical procedure to divert urine by urethral catheterisation, treated the infection and took penile tissue for biopsy, after which he was referred for further treatment. Abstract

Objective: To compare the effect of red ginger ethanol extract and Sildenafil on increasing the libido of diabetic rat models and determine the dose that significantly affects the increase in libido of diabetic rat models. Materials & Methods: This study is an experimental was conducted on 30 male Sprague dawley white rats divided into 6 groups, namely normal control group, negative control group, sildenafil positive control group, 200 mg ginger group, 400 mg ginger group and 600 mg ginger groupstudy consisting of test animal groups: normal control group; negative control; positive control with sildenafil administration, and treatment groups consisting of: dose I (200mg/kgBB); dose II (400mg/kgBB) and, dose III (600mg/kgBB). All groups except the normal control received alloxan induction at the initial stage of testing. The effect of increasing the libido of rats is seen from sexual activity including: latency of copulation, latency of riding (climbing) and frequency of riding. Results: The results of this study indicate that the administration of red ginger ethanol extract at a dose of 400mg/kgBB or 600mg/kgBB has the same potential as sildenafil in increasing the libido of diabetic rat models. Conclusion: The administration of red ginger ethanol extract at a dose of 400mg/kgBB or 600mg/kgBB has the same potential as sildenafil in increasing the libido of diabetic rat models. Abstract

Objective: This study aims to see the correlation between occupational burnout and sexual dysfunction in Indonesian female nurses. Material & Methods: A cross sectional study was conducted in Kardinah General Hospital, Tegal, Central Java, Indonesia between January 2022, and March 2022. An anonymous, self-administered online questionnaire was used. Our study subjects are female nurses from outpatient clinics, inpatient wards, high/intensive care unit, emergency unit, and operating theater. We distributed online questionnaires to female nurses who matched our inclusion and exclusion criteria. Occupational burnouts among nurses were assessed using Copenhagen Burnout Inventory (CBI), while Female sexual dysfunction (FSD) was assessed using Female Sexual Function Index (FSFI). Statistical analysis was conducted using IBM SPSS software ver 25.0. Results: A total of 285 nurses participated as samples of this study, 164 nurses (57.54%) were in the low workload group and 121 nurses (42.46%) in the high workload group. The prevalence of sexual dysfunction in female nurses in this study was as high as 87.7% While occupational burnout in high and low workload nurses in our study was 42.2% and 19.5%, respectively. The analysis shows a significant negative correlation between CBI score, sub scores, and burnout status to FSFI score (p < 0.05) although these correlations were weak. Our data proved that there were no independent variables that can become a predictor variable. Conclusion: Married female nurses have a relatively high occupational burnout and are prone to sexual dysfunction. This study showed statistically significant but weak correlation between occupational burnout with sexual dysfunction in married female nurses from the CBI total score, subscores and burnout status with FSFI total score and subscores in terms of lubrication, orgasm, satisfaction, and pain. Abstract

Objective: To evaluate the effectiveness and safety of intracavernosal injections of platelet-rich plasma (PRP) as an alternative therapy for the treatment of erectile dysfunction. Material & Methods: We conducted a comprehensive literature search on Pubmed, Cochrane Library, ScienceDirect, Scopus, and Proquest databases for randomized controlled clinical trials (RCTs) until April 2023. The study protocol has been registered in the International Prospective Register of Systematic Reviews (PROSPERO CRD42023424586). Results: Our results showed that PRP could improve MCID in IIEF-EF score (OR: 4.64; 95% CI: 2.20, 9.79; p < 0.0001; I2 = 77%) and PSV (MD: 9.63; 95% CI: 1.17, 18.09; p = 0.03; I2 = 73%). In addition, PRP administration was shown to be relatively safe, and there was no significant difference in post-injection VAS pain scores when compared to controls (MD: -0.09; 95% CI: -0.49, 0.31; p = 0.66; I2 = 84%). Conclusion: This systematic review and meta-analysis showed promising results regarding the effects of PRP administration on erectile dysfunction. However, due to the lack of quality and number of studies, these findings should be interpreted carefully, and further investigations are still needed. Abstract

Objective: The aim of this study is to determine the level of severity in hypospadias patients at Hasan Sadikin Hospital Bandung from 2019—2023. Material & Methods: This is a descriptive and observational study. The data collected includes information from all patients with hypospadias who underwent treatment at the hospital from January 2019 to November 2023, meeting the inclusion criteria and selected through total sampling. The data were presented using descriptive statistics in frequency and percentage. Results: The incidence of hypospadias at Hasan Sadikin Hospital in Bandung from 2019 to 2023 was 296 patients, with an average age of 8.6 ± 6.9 years, and the age group of 0–5 years being the most affected. Out of the total, 163 patients (56%) had moderate hypospadias, 99 patients (33%) had severe hypospadias, and only 34 patients (11%) had mild hypospadias. The average scores were G (glans) 2.2 ± 0.9, M (meatus) 3.4 ± 0.9, and S (shaft) 3.1 ± 1.0. Conclusion: In conclusion, early detection and surgical intervention faced no obstacles, and public awareness of this condition is good. Key words: Hypospadias, severity level, Hasan Sadikin.

Abstract

Objective: The purpose of this study was to characterize pediatric neurogenic bladder patients in order to recognize, treat, and avoid the related problems. Material & Methods: a retrospective study was conducted in the urology department's at Hasan Sadikin Hospital in Bandung, Indonesia. Data from pediatric neurogenic bladder patients who had urodynamic tests performed during the study period were evaluated. Results: Participants were predominantly female with 7 subjects (58.3%) while there were 5 male subjects (41.7%). The mean age of patients was 9.2 ± 5.44 years. Conservative management was the course of treatment for 10 patients (83.3%). The majority of this patient's bladder capacity—10 participants, or 83.33% of the total—have small capacities of less than 250. Seven patients (58.33%) had normal contractility, according to the bladder contractility index (BCI) inspection. Up to 7 participants (58.3%) had detrusor sphincter dyssynergia (DSD). Conclusion: Pediatric neurogenic bladder patients typically have small bladder capacities, according to urodynamic examination results. The most common symptoms that patients experience was those related to incontinence. The fundamental principle of treatment for children with neurogenic bladder is conservative therapy. Understanding the characteristics of neurogenic bladder patients and routinely carrying out urodynamic examinations allows early detection and appropriate treatment for this condition. Abstract

Objective: This case report was aimed to present an infected urachal cysts in an elderly male. Case(s) Presentation: We are presenting a case of an infected urachal cyst actively draining pus through the umbilicus in a 72-year-old male. He was managed by surgical excision of that urachal cyst completely along with omphalectomy after initial intravenous antibiotics and investigations. Discussion: In this instance, the patient complained of suprapubic discomfort, exudation of pus from the umbilical cord, and fever. Due to the variety of manifestations, the urachal cyst can frequently be interpreted as Meckel diverticulitis, pelvic inflammatory disorder, appendicitis, or infection of the urinary system. The probability of cancer ought to be weighed up while addressing an elderly with a urachal cyst. Urachal malignant tumor, which develops from the remaining epithelia of the urachal abnormalities, has an awful prognosis. The most typical presentation is hematuria. Conclusion: This case emphasizes the need for the inclusion of urachal anomalies in the differential diagnosis of abdominal discomfort with umbilical discharge even in the elderly patients. Abstract

Objective: This study aim to reveal the relationship preoperative neutrophil-to-lymphocyte ratio (NLR) with testicular germ cell tumors (GCT). Material & Methods: A total of twenty-six patients diagnosed with TGCT from 2017 to 2022 in Saiful Anwar Hospital, Malang were included in this retrospective study. Patients were grouped as localized and non-localized. Hematological and biochemical blood measured the days before radical orchiectomy. Results: Eleven patients (42.3%) had seminomas and fifteen patients (57.7%) had non-seminomatous testicular cancer. Six patients (23.1%) had localized and twenty patients (76.9%) had non-localized testicular cancer. There was a statistically significant difference between mean NLR of the localized patients and non-localized patients (localized NLR: 1.7±0.84, non-localized: 3.65±0.97, p=0.000). The optimal cut-off value for localized and non-localized TGCT was 3.05 with sensitivity 83.33% and specificity 75%. That was statistically significant between cut-off value NLR with localized and non-localized TGCT (correlation coefficient 0.505, p=0.008). Conclusion: NLR appears to be a useful marker for TGCT. It is successful in predicting localized and non-localized disease in early preoperative period. Keywords: Neutrophil-to-lymphocyte ratio, testicular cancer, testicular germ cell tumors.

Abstract

Objective: In this report, we aim to present a case of a prepubertal child with left varicocele who underwent successful microsurgical ligation. Case(s) Presentation: A 9-year-old boy was admitted to urology clinic due to his parents complaint about a mass in his left scrotum. We diagnosed this patient with grade III varicocele (Dubin-Amelar classification). The patient underwent microsurgical ligation surgery. After surgery, the mass such as "bag of worms" disappeared and the testicular volume increased by 53% from preoperative volume (1.8 to 2.3 cc). Discussion: The results of this case report indicate that microsurgical ligation of varicocele in children is a safe and effective therapeutic option. Conclusion: Microsurgical ligation of varicocele in pre-pubertal children represents a safe and effective therapeutic option, offering symptom relief and potential preservation of future fertility. Further studies are warranted to elucidate the long-term outcomes and reproductive implications of microsurgical varicocelectomy in this population. Abstract

Objective: This study aims to identify the complications associated with percutaneous cystostomy performed at tertiary hospital. Material & Methods: This study employed a retrospective descriptive design involving patients who underwent percutaneous cystostomy at Saiful Anwar General Hospital from January 2019 to December 2024. Data were collected by reviewing patient medical records, including information on average age, gender, diagnosis, and complications related to the procedure. Complications were classified into intra-procedural and post-procedural categories. This study was approved by the Health Research Ethic Commission of Saiful Anwar General Hospital No: 400/050/K.3/102.7/2025. Results: A total of 72 patients were included in the study (71 males, 1 female), with an average age of 58.58 years. Early complications observed in 7 patients included catheter obstruction in 6 patients (8.3%) and hematuria in 1 patient (1.4%). Late complications limited to surgical site infection, which occurred in 3 patients (4.2%). Conclusion: Percutaneous cystostomy is a safe and effective procedure with a low complication rate, making it a viable routine practice in clinical urology.
