Abstract

Background: Diabetic foot ulcers are becoming a major health problem worldwide. And patients with diabetic foot ulcers are on the rise, ulcers are chronic and difficult to heal, infections and limb ischemia with the risk of amputation and even life-threatening. As well as these cases require substantial health resources, then providing a socio-economic burden for patients, communities, and countries. Method: This research is a Case Control Study, to determine the effect of risk factors on the occurrence of diabetic foot ulcers. The sampling technique used is concecutive sampling. The sample of this study consisted of 60 respondents, treatment group of 30 respondents that is diabetic foot ulcer patient and control group of 30 respondents, that is diabetic mellitus patient without diabetic foot ulcers. The instruments used were questionnaires and observation sheets. Bivariate analysis technique used Chi Square test (X²) and multivariate analysis using multiple logistic regression. Results: The results of this research indicate that the variable blood pressure has an OR value of 8.887 with p value of 0.031. In conclusion among the five independent variables, blood pressure variables had the most dominant effect on diabetic foot occurrence in Diabetes Mellitus patients at Genteng and Blambangan Hospitals. Conclusions: Patient with Diabetes Mellitus faced a high blood pressure problem, so they have a greater risk of complication. The compication is diabetic foot ulcer. So the patients is recommended to control his health to decrease the incidence of the diabetic foot ulcers Keywords: Diabetes Mellitus, Diabetic Foot Ulcer

Abstract

Background: Period toddlers are vulnerable malnourished. One of the nutritional problems that are often encountered in toddlers is stunting. Stunting in children under five is an indicator of nutritional status that can give you an idea disruption overall socio-economic situation in the past. Based on the results Riskesdas 2013 the prevalence of stunting in children under five years of 37.2% Indonesia. There are various factors related to the occurrence of stunting. This study aims to know risk factors associated with the incidence of stunting among children aged 6-36 months in the district of Jember. Methods: This research is an observational research with quantitative methods and using cross-sectional design. The research sample was 120 toddlers in Kalisat districts of Jember regency that met the inclusion criteria. The data obtained and in the bivariate analysis using chi-square then continued right with multivariate analysis by using logistic regression. Results: The results showed that stunting children come from families who have parents with lower education and income family, parenting and poor diet, and not given breast milk exclusive, never suffered an infection, have parents were classified as short. Conclusions: The most dominant risk factors on the incidence of stunting that family income and father's height. Keywords: stunting, family income, father's height.

Abstract

Background: Identify a complaints that the community needs as a service user to health services. Is an important factor to improve the improvement of health services at public health center (Puskesmas). Provision of excellent service to the community which is the embodiment of the obligations of the government apparatus as a public servant. In its implementation the service providers should implement access to public service accesses consisting of transparency, accountability, participatory, and responsiveness. The purpose of this study is to explore the community's experience in receiving puskesmas services. Methods: The research design used was qualitative with interpretive phenomenology approach. The data were collected through in-depth face-to-face interviews with 10 community members from community leaders, cadres and religious leaders. Data were collected and analyzed using thematic analysis based on Braun & Clarke approach. Results: The research yield 3 theme that is the expectation of facility convenience, less optimal of puskesmas service, and lack of service officer accessibility. Conclusions: Community experience in receiving puskesmas services is a complex experience related to the expectation of facility convenience, lack of optimal service of puskesmas, and the lack of service access of officers so as to result in improvement efforts for improvement of service based on public complaints to improve public health service quality. Exploration of community experience combined with staff experience is necessary to obtain material for policy proposals on improving the quality of public services in the health sector. Abstract

Background: Low Back Pain is one of the disturbances that occur in the body due to the activities of the human body during the move with an unsafe position. The agricultural sector is one type of work that has a high risk for its workers. Extreme environmental conditions and traditional equipment determine the level of health and safety of farmers. The aim of this study was to determine the relationship between a work position and low back pain in the elderly farmer, Methods: This research was quantitative research with an analytic observational method, used crossectional design with Simple random sampling technique. The sample size was 45 farmers. This research was conducted in March 2017 in Sumengko village Mojokerto. Results: The result showed that most of the respondents are at the high-risk level (73,3%), meaning that the elderly farmers work in nonergonomic position. Low back pain experienced by most farmers is on the level of moderate complaints with 40 respondents (88.89%). Based on Spearman rank statistical test with a value of α = 0.05 significance level obtained result of 0.02, so there is a significant relationship between the working position of elderly farmers with the risk of low back pain in the Sumengko Mojokerto. Conclusions: Elderly farmers have low back pain complaints at moderate level, this is influenced by the unergonomic working position of farmers. So the farmers should keeping their body in good condition and also minimize the risk of getting low back pain. Keywords: work position, elderly farmers, low back pain

Abstract

Background: The problems of elementary school age learning include low interest in reading, shyness and lack of confidence in conveying arguments, weaknesses in discussions, and inability to address problems encountered. The aim of this study was to analyze the long association of biblioterapy with verbal creativity in school-aged children. Methods: The research design was crossectional correlation. The population in this study were all grade 4 and 5 students of SDN Bangunsari 02 Mejayan with sample size 25 respondents. Sampling technique in this research was probability sampling with proportionate stratified random sampling. Data collection used checklist and questionnaire. Data processing included with editing, coding, scoring and tabulating. Data analysis used simple linear regression with error rate 5% (0,05). Results: The results showed that all linear regression assumptions were met ie normal distributed error, no autocorrelation problem and no heteroscedasticity problem. Pearson Correlation analysis results obtained p value 0.000 <alpha 0.05 so H1 accepted means there is a long relationship of giving biblioterapy with verbal creativity in school-aged children. Linear regression results obtained overall testing using Statistical F obtained p = 0,000 <0.05 (alpha) means that the fit model shows that the model is able to explain the empirical data as a whole. Partial test results using Statistics t obtained p value 0.000 <0.05 means that the duration of giving biblioterapy has a significant effect on verbal creativity. Conclusions: The conclusion of this study is there is a long relationship of giving biblioterapy with verbal creativity in school-aged children. Abstract

Background: Cataract is the turbidity of the eye lens that causes impaired vision. This occured because of the damage on the eye lens so it reduced eye vision. Indonesia is still recorded as the highest number of patients with cataract in South East Asia, Therefore, surgical cataract or cataract surgery becomes the most surgical done by an ophthalmologist. However there is a gap between the incidences of cataract with the number of surgeries in a year. The gap between the number of cataract patients who were operated on and were not operated because it was lack of intention to have surgery from cataract patients themselves. The lack of intention from the patients to have cataract surgery could be caused by several factors, such as the lack of access to information and knowledge. The increasing of knowledge was closely related to the provision of health education, and the health education was provided, patients with cataract can improve his or her intention to have cataract surgery. Methods: This research used a pre-experimental research design by using study design One-group pre-post test design. This sampling technique was a non-probability sampling, total sampling (sampling saturated) with a total sample of 25 respondents. Results: The result of data analysis used a dependent t-test and p-value was 0,000. P value indicated <α (0,05) which meant there was the effect of health education about cataract to the intention on having cataract surgery. Conclusions: The result was expected to increase information and improve knowledge of cataract that could increase the client's intention to have cataract surgery. Abstract

Background: Profession nurse is one of the profession as a health worker who plays an important role in achieving health development goals. In producing competent nursing student graduates, educators need to design learning that can generate students' potential in using their critical thinking skills to solve problems. One of the learning approaches is to use the learning model of "Problem Based Learning (PBL)". Methods: This study uses a systematic method of review which combines several research journals to be reviewed and analyzed and taken a conclusion. Results: Exposure to research conducted by Williams and Beattie (2008), Shin and Kim (2013), as well as research conducted by Hernando et al. (2014) it can be seen that the application of PBL in nursing education can encourage students to have the initiative to learn independently. This experience is indispensable in everyday life where the development of one's mindset and work patterns depends on how he or she taught himself. Conclusions: Problem Based Learning (PBL) should be used in learning, especially in nursing learning because with PBL there will be meaningful learning. The PBL learning model also encourages students to integrate knowledge and skills on an ongoing basis and apply them in a relevant context. Abstract

Background: Pain treatments are closely related to pain assessment. Pain is probably one of the most common clinical phenomenon faced by nurses. Therefore, nurses are required to have adequate knowledge, skills and attitudes about pain, pain assessment and pain management. The objective of this descriptive explorative study was to identify the nurse's knowledge of pain, Methods: The samples were 33 nurses selected using convenience sampling method from medical surgical wards in selected hospital in Medan. Data were collected by using demography questionnaires and pain knowledge question sheet 30 items, Results: Subjects knowledge of pain ranged from 5 to 20 with average knowledge 11,33 (SD=3,33). Based on pain knowledge categories, more than half subjects (63,7%) were in moderate level, low level (33,3%), and only 3,0% in high level. Furthermore, based on subscales of pain knowledge, it was found moderate level of pain categories (M=3,82;SD=1,33) and pain mechanism’s subscales (M=2,03;SD=1,05). However, this study also found that pain definition (M=0,97;SD=0,98), pain behaviours (M=1,52;SD=1,09), and pain assessment’s subscales (M=3,00;SD=1,54) were in low level. In order to improve nurses knowledge of pain as well as the accuracy of pain assessment assessed by nurses, therefore nurses knowledge of pain need to be updated, Conclusions: this study found that nurses knowledge of pain was sufficient in moderate level, however need to be improved especially in three areas of knowledge including: pain definition, pain behaviours, and pain assessment. Abstract

Background: Central obesity of elderly has been a health problem. Central obesity is related to a high risk of cardiovascular disease and metabolic disorders. While somatotype with endomorph type is significantly correlated to the waist circumference where waist circumference is one kind of central obesity measurement. The aim of this research is to determine whether there is a waist circumference difference among somatotype groups of elderly. Methods: The design of this research is cross sectional. The research population was elderly in RW 5 Sawunggaling Surabaya. The number of the sample was 80 respondents with simple random sampling technique. Waist circumference and 10 anthropometric measurements were done to the respondents. Somatotype calculation used Heath-Carter method. Basic characteristic data of the research subject used univariate analysis. Hypothetic test used Kruskal-Wallis test continued with Mann Whitney U test. Dependent variable of this research is waist circumference value. Independent variable in this research is somatotype elderly. Results: Kruskal-Wallis statistic test is p=0.003 where p<α (0.05) means there is a significant waist circumference difference among somatotype. Mann Whitney U test result shows a waist circumference difference in central somatotype group with endomorph p=0.019, central with endomorph-mesomorph p=0.006, central with mesomorph p=0.049, endomorph-mesomorph with mesomorph-ectomorph p=0.017, endomorph-mesomorph with ectomorph p=0.041, mesomorph with mesomorph-ectomorph p=0.023. Conclusion: Somatotype endomorph, mesomorph and endomorph-mesomorph waist circumference tend to be higher than another type because there is more body fat as a result of visceral digestion domination in endomorph type.
