Abstract

Methanol intoxication occurs after accidental or suicidal ingestion occasionally and the toxicity is due to the formationof formaldehyde and formic acid. Methanol ingestion can also lead to acute pancreatitis, a sudden in! ammation of thepancreas, and high metabolic acidosis as a complication. Brain injury in methanol intoxication could be ischemia orhemorrhage form. Therefore we?d like to present a report a male with methanol intoxication with several complicationsuch, acute pancreatitis, alcoholic ketoacidosis, and CVA-ICH. The patient presented with hemodynamic instability andsevere metabolic acidosis with pH 6.991. The anion gap was 30 mmol/l. Amylase was 187 U/l and CT scan showed intracerebralhemorrhage at right fronto temporo parietal lobe with peri-focal edema (volume 48 cc). Patient unfortunatelydied three days later after force discharge by his families. Brain injury in methanol toxicity is characterized by lesionsaffecting both basal ganglia and subcortical regions. The lesion could be ischemia/necrosis or hemorrhage, selectivelyaffecting the putamenal bilaterally.

Abstract

Recurrent esophageal varices bleeding  in  liver cirrhosis  increase  the morbidity and mortality. 50 ! 60% patients with Esophageal Varricess (EV) will experience recurrent bleeding, 30% or one third of them will experience recurrent bleeding one year after diagnosis of EV. Mostly recurrent bleeding will be found at 6 weeks until 6 months after the Þ rst bleeding. Prevention of recurrent bleeding is important for survival. The aim of this research to know the risk factors of recurrent EV bleeding in liver cirrhotic patients and the onset of recurrent bleeding after the Þ rst endoscopy. This is a  cross sectional study. Thirty Þ ve decompensated liver cirrhosis patients that fulÞ ll the inclusion and exclusion criteria participated in this study. After the Þ rst endoscopy, the risk factors are written, consist of sex, age, ascites, degree of varices, history of LVE/STE the severity of liver disease, and history of consuming gastric iritating drugs. The patients followed for six months to evaluated  the occurence of  recurrent bleeding. We used Pearson Chi-Square  test  for statistic analysis  (signiÞ cant  if p < 0.05). SPSS 17 were used to statistic calculation. Statistic analytical showed signiÞ cant correlation (p = 0.006; OR = 8.889; CI: 1.803 ! 43.820). On  the other hand sex, age, degree of EV, history of STE/LVE showed non signiÞ cant correlation. The main risk factor of recurrent EV in liver cirrhosis is the severity of liver disease.

Abstract

Cancer antigen (CA) 125 is a glycoprotein produced by epithelial ovarium tumors and mesothelial cells, its levels alsohave been shown to be elevated in patients with Non-Hodgkin!s Lymphoma (NHL). Several papers have reported anassociation of high CA 125 serum levels with advanced NHL as well as relationship between high CA 125 values and pooroutcome. This study aimed to determine association between CA 125 levels (> 35 U/ml) with normal CA 125 (  35 U/ml)to the respone of NHL patients receiving chemotherapy CHOP. An observational analytic prospective study was done in40 patients with NHL at Dr Soetomo Hospital Surabaya. The patient were recruited from both inpatient and outpatientclinic and the initial CA 125 level had determined before the patients received chemotherapy with Cyclophosphamide,Doxorubicin, Vincristine and Prednisone (CHOP). Of the 40 patients who included in this study, 62.5% were male, 37.5%were female, the average age 43,45 years, the most kind of histophatology result were diffuse large cell, cleaved or noncleaved cell (47.5%). There was a signiÞ cant association between CA 125 levels with therapy respone groups (responiveand unresponive), with signiÞ cancy 0.001 (OR 23.22; CI 95%), and with therapy respone groups (CR, PR, NC, PD) withsigniÞ cancy 0.013. The group with normal CA 125 levels (  35 U/ml), had better respone, it was around 95% responive(CR = 35%, PR = 60%) and 5% unresponive (NC = 5%, PD = 0%) than the group who had high CA 125 levels (> 35U/ml), it was only 45% responive (CR = 15%, PR = 30% and 55% unresponive (NC = 50%, PD = 5%). As a conclusion,levels of CA 125 (> 35 U/ml) perhaps could be a negative prognostic factor to predict the CHOP chemotherapy responein NHL patients.

Abstract

Magnesium is one of microminerals playing pivotal role in glucose homeostasis and insulin activities. Magnesium playsa very important role in phosphorylation of insulin receptor. This is an analytical cross-sectional study to determinethe prevalence of hypomagnesium and correlation between magnesium level with insulin resistance, performed inPedawa Village Bali at February 2010. Subjects for this study were comprised of 111 people, 45 males (40.5%) and 66females (59.5%). Prevalence of hypomagnesium is 17.12%. The prevalence of hypomagnesium is higher in geriatricsubjects compare with non geriatric subjects (38.48% vs 10.98%, p = 0.004), is higher in diabetes subjects compare withnon diabetes (60% vs 15.1%, p = 0.035), and higher in subject with metabolic syndrome compare with non metabolicsyndrome (34.78% vs 12.5%, p = 0.025). Because lack on sensitivity of insulin measurement, 87 subjects (78.38%) thathave insulin level < 2  IU/ml can not measured by machine so the level of HOMA-IR of this subjects can not calculated,and just 24 subjects (21.62%) have insulin level > 2  IU/ml which can measured. From this 24 subjects, there is nosigniÞ cant correlation between magnesium level and insulin resistance (HOMA-IR) (r = -0.1, p = 0.642). In this study,we found a high prevalence of hypomagnesium in residents of Pedawa Village. The prevalence of hypomagnesium ishigher in geriatrics, diabetes mellitus, and metabolic syndrome subjects. There is no signiÞ cant correlation betweenmagnesium and insulin resistance (HOMA-IR), may be because lack of subject in this study. Thus further studies withhigher sensitivity of insulin measurement and enough sample of diabetic or metabolic syndrome patients in hospitalsettingare needed to determine correlation between magnesium level and insulin resistance.

Abstract

Approximately  20%  of  the  world  populations  suffer  from  IgE-mediated  allergic  diseases. Although  genetic  factors play an  important  role  in  the manifestation of allergic disease,  the existence of other  factors  such as  lifestyle  factors, socioeconomic factors and presence of oro-fecal infection including Helicobacter pylori infection also take a part. This is  based  on  the  hygiene  hypothesis  that  infections  in  childhood  can  reduce  allergic  diseases. We  conduct  a  study  to analyze negative association between allergic diseases with IgG anti Helicobacter pylori. This is a matched case control study involved 52 patients in Internal Medicine outpatient ward Dr. Soetomo Hospital Surabaya consisting of 26 patients with allergic diseases as a group of cases and 26 non-allergic patients as controls. Allergic disease was diagnosed by anamnesis, physical examination, and skin prick tests, whereas IgG anti Helicobacter pylori was measured using immune-chromatography  technique. We  found  in  the allergic group  there were 19 patient  (73.08%) with positive Helicobacter pylori and 7 patient (26.92%) with negative result.Whereas in control group there were 21 patient (80.77%) with positive Helicobacter pylori  result and 5 patient  (19.23%) were negative. Helicobacter pylori serology had protective effect  to allergic disease (OR 0.67, CI 95%). Was the conclusion, there is negative association between atopic allergic diseases with IgG anti Helicobacter pylori

Abstract

Hyperlactatemia usually happen in Chronic Kidney Disease (CKD) patients and it can used as indicator how bad theoxygen perfusion to organs. Therapy for hyperlactatemia is Hemodialysis (HD). Some studies assume few factors thatcorrelate to the level of lactate post HD, such as improving of Glomerulus Filtration Rate (GFR), level of hemoglobin(Hb), oxygen saturation and accompany diseases. An analytic cross sectional study was carried out in Saiful AnwarHospital to deÞ ne correlation between GFR, level of Hb, oxygen saturation and accompany diseases with level of lactatepost HD. Forty three samples were included. There were signiÞ cant correlation between level of lactate post HD withlevel of Hb (p = 0.000) and GFR (p = 0.006). With multivariate models, there were signiÞ cant correlation between levelof lactate post HD with those factors (p = 0.000). Using linier regression entering those variables into model, it foundformula level of lactate = 3.719 ! 0.196 (Hb) ! 0.053 (GFR). There was signiÞ cant correlation between between level oflactate post HD with GFR, Hb, oxygen saturation and accompany diseases.

Abstract

Osteoarthritis (OA) is one type of arthritis which commonly found especially in elderly and become the common cause ofdisability in elderly people. Recently known that immunologic respons and homeostasis of cartilage metabolism have animportant role, and MMP-3 as one of degradative enzyme which has a pivotal role in OA patogenesis. Aim of this studyis to determine correlation between level of MMP-3 and radiographic grading of knee OA, in Sanglah Hospital, DenpasarBali. An analytical cross sectional study was carried out in Policlinic Sanglah Hospital, enrolled 76 samples with kneeOA. Of all sample, 32 (42.1%) were males and 44 (57.9%) famales. The mean of MMP-3 serum level is 25.2 ± 20.7ng/ml. Radiographic grading of knee OA based on Kelgren and Lawrence criteria 4 (5.3%) grade 1,29 (38.3%) grade 2,28 (36.8%) grade 3 and 15 (19.7%) grade 4. The mean level of MMP-3 on grade 1 group is (24.7 ng/ml), grade 2 (17.8ng/ml), grade 3 (28.3 ng/ml) and grade 4 is 32.8 ng/ml. Using Spearman!s analyses, there were signiÞ cant correlationbetween MMP-3 and radiographic grading of knee OA (r = 0.25 and p = 0.03).There were signiÞ cant positive correlation between MMP-3 and radiographic grading of knee OA.

Abstract

Bronchial asthma is a chronic inß ammation in the air way. There are some element of cell and cell also to interventionin the inß ammation process. Chronic inß ammation has caused hyper responsiveness in air way, which is episodic attacksign, like wheezing, cough, shortness of breath usually in the night. C-reactive protein is one of the protein which changesin acute infection and acute inß ammation. C-reactive protein is a acute phase response protein which responsibility todefense mechanism. Acute phase protein produced induction by injury or infection. Acute phase reactant synthesis byliver which induction by cytokine like IL-6. In acute asthma attack produced some cytokine pro inß ammatory like IL-1,IL-6 and TNF. All of that cytokine induced liver to produced CRP. Aim for this study, to know the level hs-CRP in asthmaattack patient and also to know correlation between the severity of asthma attack and level of hs-CRP in patient whotreated in Sanglah General Hospital. This study was enrolling 53 patient acute asthma attack, cross sectional study. Allthe data collected by interview, physical examination, blood examination which cbc, hs-CRP, also with peak ß ow meter.The data analyzed by SPSS. And the result is the median of level hs-CRP is 7.3 mg/l, and the correlation in odd ratio(OR) is 3.73; CI 95% (1.66 ! 8.41). Based on this study we have the conclusion there strong correlation between level ofhs-CRP and severity of asthma attack.
