Abstract

Osteoarthritis (OA) is the leading cause of chronic disability in population. Because individuals with early stages of thedisease are often asymptomatic, a biomarker of early OA may be useful. YKL-40, a major secretory protein of humanchonrocytes and synovial cells, is increased in the serum and synovial uid of individuals with joint or cartilage disease.This study evaluated serum levels YKL-40 in patients with knee OA and its relation with join space narrowing.YKL-40 wereassayed in serum samples from 44 patients (36 women, 8 men, mean age 50 ? 60) with symptomatic OA of the knee joint.YKL-40 assayed by immunoassay and joint space narrowing by ultrasound. OA severity was assessed by the measurementdegree of joint space width with ultrasound. Statistical analysis was perfomed to determine association between serummarker and radiological join space width.The mean (standard error) YKL-40 level was 82.66 ng/ml in patients with knee OA.The serum levels of YKL-40 and joint space narrowing were signi!cantly correlated (Spearman test r = 0.769; p < 0.01).inpatients with symptomatic knee OA. Serum YKL-40 may be a marker of joint inflammation in symptomatic OA.

Abstract

Malignant ascites is the abnormal accumulation of !uid in the peritoneal cavity associated with several intra-pelvicand intra-abdominal malignancies. Among the gynecologic malignancies, ovarian carcinoma predominates. Of thegastrointestinal malignancies, ascites can occur with advanced colon, pancreas, gastric carcinoma, and esophagealcarcinoma. Recent reports in the cytological literature suggest that 6% to 11% of malignant peritoneal effusions aredue to tumors of unknown origin.1 We reported a 62 years old female presenting adenocarcinoma ascites, which shownconfusing of site of primary origin from imaging examination. First ultrasound imaging showed mass on colon area, butre ultrasound of abdominal imaging showed the possibility of endometrium carcinoma. The CA 125 level was high (2.294u/ml). Open laparotomy showed milliar metastatic several nodule on omental, liver, and abdominal wall. There wasattachment of vesica urinaria, endometrium, colon, sigmoid, and rectum. All those result conclude this patient sufferedfrom malignant ascites of adenocarcinoma of unknown primary origin, with highly suggestive from malignant ascites ofadenocarcinoma of unknown primary origin, with highly suggestive from gynecologic malignancy that difcult to differwhether from ovarian cancer or endometrium cancer. Somehow, the epidemiology, pathophysiology, clinical presentationlead the diagnosis for ovarian cancer.

Abstract

Vascular complications develop progressively from 5 ? 40 years after the onset of diabetes, although they may appearearlier in type 2 diabetes when associated with a periode of undiagnosed diabetes and impaired glucose tolerance.Endothelial dysfunction is the proposed site of both micro vascular and macro vascular complications in diabetes.Endothelial dysfunction has been documented in both type1 and type 2 diabetes and is present in the insulin resistancesyndrome and in pre diabetes. There were will likely be a subsequent increased burden of vascular complications ofdiabetes, including diabetic nephropathy (DN). It is predicted that 25% of newly diagnosed patient with type 2 diabeteswill develop early stage DN within the next 10 years. Advanced Glycation End Products (AGEs), which accumulate indiabetic vasculature, result in enhanced expression of endothelial cell-associated Vascular Cell Adhesion Molecule-1(VCAM-1) as well release of a soluble form of VCAM-1 (sVCAM-1) into culture supernatants. We hypothesized thatsVCAM-1 in diabetic plasma might reect early vascular perturbation in diabetic vasculopathy. The aims of this studyis to know the prol of sVCAM-1 and albuminuria in type 2 diabetes. A cross sectional study to measure sVCAM-1andalbuminuria. Descriptive statistical analysis used to know the prol of sVCAM-1 and albuminuria. Of the 69 diabeticsubjects (31 males, 38 Females) with mean age of 57.4 ± 4.8 years, normoalbuminuria 39 subjects, microalbuminuria 23and macroalbuminuria 7 patients, and sVCAM-1 was 0.614 ± 0.177. The levels of sVCAM-1 plasma is increased alongwith stage of albuminuria. There was an increase of sVCAM-1 level along with stage of albuminuria.

Abstract

Extended Spectrum !-Lactamases (ESBL) is plasmid enzyme which catalyzing hydrolysis and inactivation !-lactamantibiotic, including third generation of Cephaloporin, penicillin and aztreonam. This enzyme is mutation of !-lactamaseTEM-1, TEM-2, and SHV-1 which commonly found in Enterobacteriaceae and normally have resistance to penisillin andrst generation celaphorin. The incidence of infection cause by ESBL bacterial is increasing worldwide. AntimicrobialResistance in Indonesia: prevalence and prevention (AMRIN study) is showed incidence of infection ESBL bacterial 29%E. coli, and 36% K. pneumonia. This study is descriptive and restrospectively learn about prole of DMK with septicemiapatient caused by ESBL bacterial in Internal Medicine Ward RSUD Dr Soetomo within 1st January 2010 until 30th June2011. Sixty four patient were recruited and 48 patients with ESBL bacterial positive. Mortality rate is 16.7%, severe septiccondition 27.08%, septic shock 14.58%. The result of positive ESBL bacterial was found from urine culture (37.5%).Sensitivity of antibiotics results are meropenem (98.07%), amikacin (88.46%).

Abstract

Saiful Anwar Hospital (SAH) doesn?t have standardized Fresh Frozen Plasma (FFP) liquefying tools. Standard OperatingProcedure (SOP) to liquify FFP at SAH is put the FFP on the table until diluted. Recently a new Thawer of FFP wasdeveloped in SAH. In this study we compared between the two methods which are convensional and the new one. Thirttytwo bag of FFP were divided in two different treatments which is group of new Thawer and the convensional. Eachliqueed bag of FFP was recorded the time of liquefaction, aPTT, PPT and INR. Statistical analyses using independent Ttest or Mann-Whitney as alternative. In this study found mean time of liquefaction of FFP on Thawer group were shorterthan conventional method 25.32 ± 3.4 vs 50.38 ± 5.52 minutes and statistically signicant (p < 0.05). Mean of aPTT onnew Thawer group is 36.79 ± 5.94; but on conventional method is 36.94 ± 5.53 second (p = 0.67). Mean of PPT 11.71 ±0.95 second on new Thawer group and 11.84 ± 2.22 second on conventional method (p = 0.249). Mean INR each groupwere 1.02 ± 0.08 on new Thawer group and 1.03 ± 0.19 on conventional method (p = 0.234). This study shows that newFFP Thawer liquifay FFP more quickly than conventional method without decreasing the quality of FFP.

Abstract

Atopic allergic disease related with genetic factor and environment. Some studies found parents and environment factorhave an important role on emerging of clinical manifestation. This study try to evaluated wheather atopic parents andenvironment can inuanced clinical manifestation of allergic disease in their children. This information is important foreducation and in intervention of treatment for patients with allergic disease. This study is an observasional analytic casecontrol study involved all student range from junior until high school level and their parents at Surabaya region. Datawas taken by quisioner, physical and Skin Prick Test (SPT). Statistical analyses using multivariat logistic regression test.Threehundreed forthy eight student was enrolled where 91 (26.1%) with positive SPT have atopic father 26 (56.9%) andatopic mother 25 (54.3%) on the other hand 249 student with negative SPT have atopic father 22 (43.1%) and atopicmother 21 (45.7%). Logistic regression test found that sequency number of birth and explanation dust mate assosiatedwith occurence of clinical manifestation of atopic allergic disease in children.

Abstract

Prevalence of knee Osteoarthritis (OA) in the community is quite high. Diagnosis and assessment of severity of knee OAbased on X-ray examination is not so sensitive yet. Various attempts have been made to nd markers of cartilage damage.One potential marker is Cartilage Oligomeric Matrix Protein (COMP). Based on animal studies, the more severity damageof joint cartilage, the more COMP is released. Based on this, researchers want to know whether there is a correlationbetween the severities of knee OA with serum concentration of COMP in human.This study is a cross sectional analyticstudy, whose performed in Outpatient clinic RSUP Sanglah Denpasar. The diagnosis of knee OA based on ACR criteriaand the degree of knee OA severity is determined based on the Kellgren and Lawrance criteria. COMP patients arechecked by using ELISA method. The correlation between the severities of knee OA and serum COMP was analyzed bySpearman correlation test. Level of statistical signicance used was p < 0.05. Sixty-six patients who meet the inclusioncriteria are examined, male was 28 (42.4%), women was 38 (57.6%). The mean age is 63.9 ± 7.0 years. The severity ofknee OA are: 4.5% for grade one, 35.8% grade two, 38.8% grade three and 20.9% at grade 4. The mean serum COMPconcentration is 1084.0 ng/ml. There is a trend of increasingly grade of severity of knee OA, the higher the concentrationof serum COMP, but not statistically signicant (r = 0.127, p = 0.31). In conclusion, there is no positif correlation thatstatistically signicant between severity of knee OA and serum COMP concentration.

Abstract

Acute Kidney Injury (AKI) is a common clinical nding that produces some serious health problem. Since the previouslydecades, the identication of AKI clinical practically based on laboratory test as Blood Urea Nitrogen (BUN) and Serum Creatinin (SC). Unfortunatelly, SC is not a reliable indicator of AKI in acute phase. Neutrophil Gelatinase-AssociatedLipocalin (NGAL) is promisingly for an early biomarker of ischemic kidney injury. After performing the inclusion criteriaand exclusion criteria, samples are underwent the operation then admitted to the ICU ward. Samples were followed andunderwent serially laboratory examination of urine NGAL, SC and urine output according to the timetable of the research.Mean concentration of SC before operation and hour 0 hour, 6 hours, and 24 hours after operation for non AKI patientswere as follow 0.83 ± 0.19 mg/dl; 0.83 ± 0.19 mg/dl; 0.89 ± 0.18 mg/dl; and 0.88 ± 0.25 mg/dl. And for AKI patients were0.86 ± 0.21; 0.81 ± 0.20 mg/dl; 1.01 ± 0.20 mg/dl; and 1.84 ± 0.75 mg/dl. Mean concentration of urine NGAL on 0 hour, 3hours, and 6 hours post operation non AKI patients were as follows 26.0 ± 27.7 ng/l; 24.4 ± 23.0 ng/l; and 32.6 ± 42.4 ng/l.While in AKI patients were 809.4 ± 379.0 ng/l; 732.0 ± 450.0 ng/l; and 651.4 ± 432.0 ng/l. The elevated concentration ofurine NGAL among cases of AKI occurred earlier than the elevated concentration of SC.

Abstract

Ambient air pollution often associated with allergic diseases and impaired respiratory function. Total serum IgE levels was  one  of  the  diagnostic  tools  to  establish  an  allergic  diseases while  lung  function  test was  standard  examination to establish alteration in lung function. Trafc police ofcers were often assigned on the highway is one of the mostvulnerable population against exposure to air pollution. Aim of this study isto know the association between air pollution on total serum IgE levels and lung function in trafc police ofcers compared to staff police ofcer as control.This is anobservational analysis study with cross sectional design.There were 60 police ofcers as a subject, divided in two groups.Thirty subjects as exposured groups and 30 subjects as control group. Air pollution was measured by Pollutant StandardIndex (PSI)/ Indeks Standar Pencemar Udara (ISPU) method. Independent  t-test and Mann-Whitney  test were used  to analyze differences between variabel. Spearman test was used to analyze correlation and strength between variabel.There were no signicant differences in total IgE serum levels between groups (p = 0.301). There were signicant differencesin FEV1 (%p) (p = 0.015), FVC (%p) (p = 0.000) and spyrometry result (p = 0.001). There was no signicant differencein FEV1/FVC ratio (p = 0.573). There were negative weak association between air polution and FEV1 (%p) (r = -0.298;p=0.021) and between air pollution and spyrometry result (r = -0.380; p = 0.003). There was negative moderate associationbetween air pollution and FVC (%p) (r = -0.409; p = 0.001). There was no association between air pollution and FEV1/FVC ratio (r = 0.058; p = 0.662). There was signicant association between air pollution and impaired respiratory functionbut there was no association between air pollution and total serum IgE levels.

Abstract

Iron is an essential substance needed for metabolism process, such as for transporting oxygen to the tissues for energyproduction in cells. Iron is also important for transporting electrons in the mitochondria for cell respiration process, andfor synthesizing Deoxyribo Nucleic Acid (DNA). In addition to its role in oxygen transport and metabolic pathways, ironplays a pivotal role in immune function by promoting lymphocyte activation and proliferation. On the other hand, ironover-load can have immune disorders and yet pathogenic microorganism require iron for their growth and proliferation.Therefore iron homeostasis must be maintained through careful regulation of duodenal absorption and recycling of ironstores since human body has no ideal excretory system for iron. Discovery of hepcidin peptide help clarify molecularbased of cellular interaction between duodenal iron absorption capacity and iron stores cells. Hepcidin a putative negativehumoral regulator of iron homeostasis by inhibiting iron absorption and couple with ferroportin preventing iron exportedfrom intracellular iron stores. Hepatic hepcidin expression is increased in iron overload and decreased in iron deciency. Inaddition to regulation by body iron needs, hepcidin is increased during an acute phase respone of inlammation processed.When an in!ammation is initiated, liberation of proin!ammatory cytokine IL-6 stimulate hepcidin expression causingiron absorption and iron release from the macrophage of the reticuloendotheleal system are decreased. Many other studieson humans and experimental animals have shown signicant change of the levels of certain cytokines in iron deciencystate such as IL-2; IL-4; IL-6; IL-10; IL-12; IFN gamma. As summarized, cytokines in!uence iron homeostasis and in theother way iron in!uences activity of cytokines and immune effector mechanism of macrophages, and all these affect thelevel of immune defence against invading pathogens.
