Abstract

Vascular Cell Adhesion Molecule-1 (VCAM-1) is a vascular adhesion molecule that plays a crucial role in vascular inflammation, endothelial dysfunction, and the development of atherosclerosis. Increased VCAM-1 expression occurs in response to various inflammatory stimuli and cellular stressors that involve the activation of specific molecular signaling pathways. Various factors such as chronic inflammation, oxidative stress, activation of the Nuclear Factor-kappa B (NF-κB) pathway, the Janus Kinase-Signal Transducer and Activator of Transcription (JAK-STAT) pathway, obesity, leptin resistance, genetic factors, and changes in vascular hemodynamics are known to contribute to increased VCAM-1 expression. This study aims to analyze various factors that influence VCAM-1 changes molecularly using a narrative review method. Article searches were conducted through the PubMed, Scopus, and Google Scholar databases using the keywords "VCAM-1", "NF-κB", "oxidative stress", "LEPR", "leptin resistance", "endothelial dysfunction", and "atherosclerosis". The articles used were published between 2020 and 2026, relevant to the research topic, and available in full text. The review results indicate that NF-κB activation is the primary mechanism of VCAM-1 elevation triggered by pro-inflammatory cytokines, increased reactive oxygen species (ROS), leptin resistance, and LEPR activation via the JAK-STAT pathway. Furthermore, genetic factors such as LEPR polymorphisms and epigenetic mechanisms also contribute to increased vascular inflammation and VCAM-1 expression. Thus, VCAM-1 has great potential as a biomarker of vascular inflammation and a therapeutic target in cardiovascular disease, particularly atherosclerosis.

Abstract

This study developed a multi-algorithm machine learning prototype for multiclass breast cancer survival prediction using 1,980 patient records, classifying patients as Living, Died of Disease, or Died of Other Causes. The framework integrated NN, SVM, RF, NB, and KNN algorithms within a decision-support monitoring application, with preprocessing steps including data cleaning, normalization, feature preparation, and dataset partitioning. To prevent target leakage, survival-related variables were excluded from the predictor set. The revised evaluation results indicated that NB and KNN delivered the strongest performance, achieving weighted average F1-scores of 0.93 and 0.92, respectively, while NN and RF showed comparatively lower results. These findings highlight the potential of machine learning for breast cancer survival status monitoring, although the proposed system is designed as a decision-support prototype rather than a clinical diagnostic tool. Therefore, before actual healthcare deployment, more research incorporating explainable AI techniques, external validation, and real-world clinical testing is needed.

awal, namun hubungan tersebut belum banyak dibuktikan pada populasi laboratorium di Indonesia. Tujuan: Menganalisis hubungan kadar HbA1c dengan estimasi laju filtrasi glomerulus (eGFR) dan kadar mikroalbumin urine. Metode: Penelitian analitik dengan desain potong lintang pada 40 subjek yang menjalani pemeriksaan HbA1c, kreatinin serum, dan mikroalbumin urine. Nilai eGFR dihitung menggunakan persamaan CKD-EPI 2021. Analisis statistik meliputi korelasi Spearman, korelasi parsial, uji Mann-Whitney, Kruskal-Wallis, uji eksak Fisher, regresi linear berganda, regresi logistik, dan analisis kurva ROC. Hasil: Rerata usia subjek 54,77 ± 12,96 tahun dengan 55,0% perempuan. Median HbA1c sebesar 6,20% (IQR 5,51–8,47), rerata eGFR 89,25 ± 19,89 mL/menit/1,73 m², dan median mikroalbumin urine 30,24 mg/dL (IQR 16,12–73,70). Albuminuria ditemukan pada 70,0% subjek, lebih tinggi dibandingkan peningkatan kreatinin serum (37,5%). HbA1c tidak menunjukkan korelasi bermakna dengan eGFR (ρ = +0,044; p = 0,789) maupun mikroalbumin urine (ρ = +0,048; p = 0,769), dengan kemampuan diskriminasi yang rendah (AUC 0,525 dan 0,522). Setelah penyesuaian usia dan jenis kelamin, HbA1c berhubungan positif dengan eGFR (b = +3,64; p = 0,005) dan negatif dengan kreatinin serum (b = −0,058; p = 0,002). Penurunan eGFR dipengaruhi oleh usia ≥60 tahun dan jenis kelamin laki-laki. Kesimpulan: HbA1c tidak dapat digunakan sebagai indikator tunggal penurunan fungsi ginjal tahap awal. Pemeriksaan albuminuria bersama eGFR tetap diperlukan untuk mendeteksi kerusakan ginjal dini.
