Abstract

Cardiovascular diseases remain a leading cause of global mortality and morbidity for 6.9 million deaths in 2021 due to conditions like stroke and ischemic heart disease. The high cost of long-term treatment often leads to decreased medication adherence, spurring the development of new, more affordable yet effective drugs. To minimize risk, animal models are crucial in drug development before human trials. Therapy for cardiovascular diseases continues to evolve due to high costs and poor adherence. Zebrafish have emerged as a promising animal model due to their genetic similarity to humans, allowing for the testing of more efficient and affordable novel drugs. This study reviewed articles on cardiovascular disease in zebrafish embryos from MDPI, PubMed, Science Direct, and Springer Link databases, using specific keywords. Inclusion criteria included quantitative research articles in English or Indonesian, open-access, and published between 2015-2025. Following the PRISMA process (identification, screening, eligibility), 14 relevant journals were selected from 5.680 initial articles and subsequently analyzed using JBI Critical Appraisal Tools for further discussion. The findings indicate that various molecular techniques, such as immunostaining, PCR, and western blotting, enable cardiovascular disease research in zebrafish embryos. This model offers speed, accuracy, and cost-efficiency due to its transparent and replicable nature. Despite requiring specialized skills due to their small size, zebrafish embryos present a promising alternative for developing new cardiovascular drugs.

Abstract

Cardiovascular diseases such as pulmonary arterial hypertension can lead to increased workload on the right ventricle, which clinically manifests as peripheral edema. The exploration of herbal ingredients like butterfly pea flower (Clitoria ternatea L.) represents a preventive strategy due to its anti-angiogenic and cardioprotective activities. This study aimed to evaluate the cardioprotective effect of butterfly pea flower extract syrup in reducing pericardial edema in a zebrafish embryo model. Zebrafish embryos at 24 hours post-fertilization were exposed to butterfly pea flower syrup at various concentrations (1.56, 3.12, 6.25, 12.5, 25, and 50 µg/mL), along with a negative control (E3 medium) and a positive control (0.1% CHX) for 96 hours. The pericardial area was quantitatively analyzed using FishInspector software and computed using the Shoelace Formula in RStudio, followed by statistical analysis using SPSS. The results showed that the CHX group exhibited severe edema, with a pericardial area reaching 8,327.56 pixel². Concentrations of 50 µg/mL and 25 µg/mL of butterfly pea syrup induced increased fluid accumulation, potentially influenced by the osmolarity of the preparation. In contrast, concentrations 1.56 to 12.5 µg/mL maintained normal morphology, with pericardial area values ranging from 3,646.00 to 3,890.74 pixel², which were close to the negative control group (3,589.91 pixel²). Statistical analysis using ANOVA revealed a p-value of 0.448 (p > 0.05), indicating that the treatment groups exhibited a uniform cardioprotective effect without cardiotoxicity. Therefore, butterfly pea flower extract syrup at 5% concentration shows potential as a preventive therapeutic modality to reduce the risk of peripheral edema.
