Abstract

AbstrackBackground : There are issues that Health Center is still considered as a middle to lower health service, unsatisfactory service, and the attitude of officers who arbitrarily provide services that cause the Health Center got a bad view. Observations on 5 nurses were still found the nurses who were still passive towards work, rarely smiled, and did not care about their appearance. Transformational service as a service at the highest level that is expected to be able to change customer character because it is able to provide solutions to customer problems. Purpose: The purpose of this study was to know the Transformational Service with Imogene King interpersonal system approach and the effect to the quality of health service at Tanjung palas Health Center, Bulungan District, North Kalimantan in 2018 2018. Method: This was a quantitative study with a quasi-experimental research method with pre-post and control group design. The sample of this study were patients at Tanjung Palas Health Center and Bumi Rahayu Health Center, with total sample in the intervention group were 40 respondents and in the control group were 40 respondents, used a simple random sampling technique and analysis method of General Linear Model Repeated Measure (GLM-RM). Result: The results of this study showed that there were significant figures at the level of health services at Tanjung Palas Health Center. The mean values from measurement 1 to 4 were 76.20 from measurements 1 and d 88.29 in measurement 4 with a difference of 12.09. While the average value in the control group between measurements 1 and 4 were 75.93 and 83.44 with a difference of 07.51. From the difference in the average value between the differences in quality of health services in measurements 1 and 4 in the intervention group compared to the control group made conclusion that Ha accepted. Conclusion: The Transformational Service model with Imogene King interpersonal system approach was effective to improve the quality of health service at Tanjung palas Health Center, Bulungan District, North Kalimantan in 2018. Abstract

Dismenorea adalah keadaan nyeri yang hebat dan dapat mengganggu aktivitas sehari-hari pada perempuan khususnya remaja putri. Dismenorea merupakan suatu fenomena simptomatik meliputi nyeri abdomen, kram dan sakit punggung. Untuk mengatasi dismenorea adalah dengan cara mengkonsumsi air kelapa hijau sebanyak 1 hari 1x dalam waktu 30 menit rasa nyeri haid berkurang, air kelapa hijau di konsumsi selama 3 hari dalam siklus menstruasi. Tujuan dari studi kasus ini untuk mengetahui penurunan skala nyeri dari pemberian air kelapa hijau terhadap penurunan nyeri haid (dismenorea). Penelitian ini menggunakan desain studi kasus komparatif yaitu perbandingan dua subyek dengan pemberian air kelapa hijau terhadap penurunan nyeri haid (dismenorea). Setelah dilakukan pemberian air kelapa hijau selama 3 hari didapatkan hasil yaitu subyek 1 pre skala nyeri 5 post skala nyeri 1 dan subyek 2 skala nyeri pre 6 dan post menjadi skala nyeri 2. Dari hasil yang didapat maka penulis dapat menyimpulkan bahwa pemberian air kelapa hijau dapat menurunkan nyeri haid (dismenorea).

Abstract

Penelitian ini mengkaji implementasi Pembelajaran Teaching Factory mengenai Teaching Factory (Tefa) di lembaga pendidikan vokasi. Teaching Factory adalah pendekatan pembelajaran yang mengintegrasikan kegiatan produksi nyata ke dalam proses pendidikan, dengan harapan meningkatkan keterampilan kerja peserta didik sesuai dengan kebutuhan industri. Berdasarkan kajian literatur menunjukkan bahwa implementasi Teaching Factory di lembaga pendidikan telah berhasil memberikan pengalaman belajar yang relevan dan praktis ke arah hasil bagi siswa. Selain itu, para siswa lebih terampil dalam aspek teknis maupun non-teknis seperti kerjasama tim, pemecahan masalah, dan manajemen waktu. Kendala yang ditemukan selama pelaksanaan Teaching Factory meliputi keterbatasan sarana produksi, keterlibatan industri yang belum optimal, serta kesiapan tenaga pendidik dalam memfasilitasi proses pembelajaran berbasis produksi ini. Secara keseluruhan, pembelajaran Teaching Factory dinilai efektif dalam mempersiapkan peserta didik memasuki dunia kerja, meskipun diperlukan peningkatan dalam beberapa aspek untuk optimalisasi hasil pembelajaran.

Abstract

Background: This study is an application of Evidence-Based Nursing Practice in the prevention of diabetic foot ulcers (DFU) in vulnerable adult communities through the JULAKWAKRI intervention tools (seven steps of self-care foot care). Objective: To assess the effectiveness of JULAKWAKRI tools in preventing foot injuries in diabetic patients.  Methods: This quasi-experimental study with a pre- and post-test control design was conducted in the Harapan Mulya RW 4, 5, and 6 community areas in Kemayoran, Central Jakarta. The instruments used included the Spoken Knowledge in Low Literacy Diabetes (SKILLD) questionnaire, the Diabetes Empowerment Scale–Short Form (DES-SF), the Diabetes Problem-Solving Skills (DPSS) scale, and blood glucose measurements for glycemic control. Data were analyzed using the paired t-test to determine changes before and after the intervention. Results: The intervention significantly improved knowledge (MD: 28.44; p: 0.007), empowerment (MD: 16; p: 0.006), and problem-solving (MD: 34; p: 0.008). In addition, objective outcomes based on glycemic control also showed a significant decrease of up to 132.56 points (p: 0.008). Conclusion: DFU prevention is an essential component of empowering communities to become more independent and improving the health status of diabetic patients vulnerable to foot ulcers. The JULAKWAKWARI tools can be recommended as an effective intervention in community-based nursing practice.

Abstract

Education on how to follow up care after a patient is discharged is crucial to maintaining the level of health achieved during treatment, preventing recurrence of future care. Patient discharge planning can foster collaboration with providers of advanced nursing services. Patient discharge planning can improve knowledge, foster awareness of care management, knowledge of medications, and awareness of danger signs that indicate potential complications. This community service activity was conducted at Sawerigading Regional Hospital, Palopo City, South Sulawesi. The target group was 32 nurses working in the inpatient ward. The results showed that the nurses' knowledge was predominantly good (56.3%), sufficient (31.2%), and inadequate (12.5%). Furthermore, the nurses' attitudes were predominantly positive (65.6%), and a minority (34.4%). Nurses' understanding of discharge planning is crucial to maximize patient discharge preparation and readiness for home care to maintain long-term health.

Abstract

Supervision by the ward head is one of the factors that influence nurses' compliance in carrying out their duties in the hospital. Supervision is the direct and periodic observation by superiors of the work carried out by subordinates. Then, if problems are found, direct assistance is immediately provided to resolve them. Objective: To determine the relationship between the supervision of the ward head and the implementation of the five moments of hand hygiene for nurses in the ICU Room of Tgk Chik Di Tiro Sigli Regional Hospital. Method: Using a correlation research design to see the relationship between the variables studied. The sample used was 38 patients. The results of the study: the ward head's supervision variable was predominantly good, namely 19 patients (50.0%), and the implementation of the five moments of hand hygiene variable was predominantly not implemented, namely 33 patients (86.8%). Based on the results of the statistical test, it was found that the P value <∝ (0.05) was 0.002, meaning there was a significant relationship between direct supervision and the implementation of the five moments of hand hygiene. Furthermore, the statistical test results showed a P value < ∝ (0.05), namely 0.002, indicating a significant relationship between indirect supervision and the implementation of the five moments of hand hygiene. Hospital management needs to promote handwashing to prevent nosocomial infections.

Abstract

The skill of performing bedside handover is a major strength during bedside handover, because competent skills lead to the assurance of more accurate and up-to-date information regarding the patient's condition, which will affect the degree of patient safety. This study aims to determine the relationship between nurses' skills in performing bedside handover and patient safety. The research design is explanatory research with a cross-sectional approach. The population is all nurses working in the inpatient ward of Tgk Chik Di Tiro Regional Hospital, totaling 212 nurses. The technique for determining the total sample in this study uses the Rule of the Thumb formula with the formula: 5-10 x total indicators so that a total sample of 155 nurses is obtained. The research instrument is a questionnaire on skills in performing bedside handover and patient safety. All question items have been tested for validity and reliability. The data analysis used is descriptive correlation analysis using the chi-square test. The results show that the value in the Pearson chi-square column with Asymptotic Significance is 0.000 <0.05, so it can be concluded that there is a significant relationship between nurses' skills in performing bedside handover and patient safety. It is recommended that hospital management continue to strive to improve the skills of nurses in carrying out bedside handovers so that patient safety increases towards complete health.

Abstract

Background: Perineal wounds following childbirth are common, particularly among women who experience vaginal or perineal tears. When healing is delayed, these wounds may lead to serious postpartum complications, including infection, bleeding, and increased maternal morbidity. Kegel exercises are widely recognized as a simple, non-pharmacological intervention that strengthens pelvic floor muscles and may support the healing process of perineal wounds. However, limited attention has been given to postpartum mothers’ personal experiences in performing these exercises during the recovery period. Purpose: To explore postpartum mothers’ experiences in practicing Kegel exercises as part of perineal wound healing at the Gadingrejo Community Health Center. Method: A qualitative study with a phenomenological approach was employed to capture mothers’ lived experiences. Participants were selected through purposive sampling, involving eight postpartum mothers who met the inclusion criteria. Data were collected through in-depth interviews and direct observation. Thematic analysis was conducted to identify recurring patterns and meanings related to the implementation of Kegel exercises. Results: The findings revealed five main themes: (1) timing of Kegel exercise initiation, (2) initial physical complaints after childbirth, (3) perceived benefits of the exercises, (4) vaginal care practices, and (5) dietary patterns during the postpartum period. Observations of wound healing using the REEDA assessment showed a gradual improvement from day one to day seven. Initially, most participants demonstrated poor wound healing characteristics, which progressed to good wound healing by the end of the observation period. Conclusion: Postpartum mothers who practiced Kegel exercises experienced better perineal wound healing compared to their initial condition. These findings suggest that Kegel exercises may contribute to accelerating the perineal wound healing process. In addition, proper vaginal care and adequate nutritional intake appear to play supportive roles in promoting recovery among postpartum women.
