Abstract

Introduction: Charity circumcision is common in Indonesia. It is usually done by general practitioners or medical students under supervision. Procedures are often held on a mass scale and rarely followed with post-monitoring. We developed a telemedicine follow-up protocol to educate and address post-procedure complications. This pilot study evaluates the acceptability of telemedicine follow-up protocol for charity circumcision in Indonesia. Method: Data were collected from operators and caregivers of children attending charity circumcision. The protocol consisted of text interviews (pain, urinary retention) and surgical sites' four-angle photographs (epithelialization, bleeding, infection, edema, and glans excoriation). The protocol was conducted two-way between caregivers and operators on days 1, 3, 7, and 30 after circumcision. The Ethics Committee of the Faculty of Medicine, Universitas Indonesia, has approved this study. Result: Twenty-five boys with a mean age of 8.02±1.53 years old have similar pre-operative, operative, and postoperative protocols. Twenty-one caregivers completed four times follow-ups, one missed two times follow-ups, and three missed the last follow-up. Mild pain was the most reported complication, with 32% (8/25), 12% (3/25), 8% (2/24) incidence on days 1, 3, and 7, respectively. By the end of the follow-up, there were no persisting complications, and 80% (20/25) of boys showed complete epithelialization. All caregivers and most operators were satisfied with telemedicine for post-mass circumcision monitoring. Conclusion: The telemedicine-based follow-up is acceptable and feasible to be applied in mass charity circumcision. This protocol is beneficial for monitoring the results and adverse events, therefore, should be routinely adopted in mass charity circumcision programs.

Abstract

Background : Hypospadias is a genital congenital abnormality with a variable prevalence. There are risk factors associated with hypospadias. This study analyse the characteristics of hypospadias patients, hypospadias with other genital anomalies, and family history of hypospadias. Method : We used the National Hospital Cipto Mangunkusumo Medical Record Database, which contains all the patient's data from 2013 to 2017. All the information of the diagnosis, type of hypospadias, and the family history are provided. After collecting all the data, we analysed the total cases of hypospadias and the characteristic of hypospadias patients. Result : Based on the data, 6,254 patient that registered in the Plastic Surgery Division, forty-nine patients were diagnosed with hypospadias. Among them, 16% are penoscrotal type hypospadias. Furthermore, there is only 3% of the patients who has a history of hypospadias running in the family. Approximately 29.4% of the patients associated with other genital anomalies. However, only 35% of the sample population underwent surgical repair at the age of 0-5 years. Conclusion: The number of hypospadias cases treated by Plastic Surgery Division at The National Hospital Cipto Mangunkusumo is parallel with the global prevalence of hypospadias. The most common hypospadias type treated in Cipto Mangunkusumo Hospital is penoscrotal, which is similar to the previous study in Europe. The prevalence of hypospadias in Indonesia remains unknown therefore, the data management of hypospadias patients ought to be improved.

Abstract

Background : A Surgeon should have good leadership. Leadership plays an important role in improving health services. Leadership can shape a better future. Method : A literature search was conducted in November 2019. Related keywords were applied to Pubmed, Medline, and SCOPUS for studies published in the last five years. Relevant research is taken to be used as the discussion material. Result : The key to surgical leadership is collaboration and cooperation, humanism and mentorship, and operational efficiency. Conclusion: Effective leadership in a surgical team has the following characteristics: (1) Defines the role of a leader clearly, especially in critical situations; (2) Leadership style that suits the clinical situation; (3) Clear directions to team members; (4) Consistently seeking input from team members; (5) Involving members in decision making.

Abstract

One of the Indonesian plants that can be used as medicine is the dicotyledonous plant Cassia alata L., commonly known as Chinese ketepeng. Chinese ketepeng (Cassia alata L.) is a member of the Fabaceae family. Chinese ketepeng is traditionally used to treat gastrointestinal disorders, skin diseases, allergic reactions, internal and external infections, and inflammatory conditions. This plant is a popular herbal medicine in natural anti-diabetic treatment in Africa. This extension aims to formulate Chinese ketepeng leaf extract (Cassia alata L.) into an ointment and test the effect of Chinese ketepeng leaf ethanol extract on inflammation in the paws of white rats, using 1% (w/v). Carrageenan solution as an inflammation inducer, with a dose of 0.1 ml every 60 minutes for 6 hours. Changes in the volume of the rat's paw pads were observed using a plethysmometer. Chinese ketepeng leaf ointment extract was given in three doses: 5 mg/kgBW, 10 mg/kgBW, and 15 mg/kgBW. The negative comparator used was the ointment base without the extract, and the positive comparator used was 1% hydrocortisone acetate ointment. The chemical composition of the extract showed the presence of alkaloids, flavonoids, saponins, steroids, and tannins, which are effective in inhibiting the cyclooxygenase pathway, which can inhibit inflammation. Data on the percentage of edema inhibition were analyzed statistically using the Statistical Package for the Social Sciences (SPSS). The results of the extension showed that Chinese ketepeng leaf extract (Cassia alata L.) can be formulated as an ointment. The best anti-inflammatory effect was achieved from the three doses.

Abstract

This study aims to analyze the short story writing skills of third-grade students at SD HKBP Sei Mati. A descriptive qualitative method was employed,involving 20 student-written short stories.the analysis focused on five aspects :originality,relevance of content to the title,text coherence,vocabulary choice,and grammar usage.The results indicated that students’weiting abilities ranged from good to very good,The highest scores were foud in content relevance,coherence,and grammar,while originality and vocabulary require further improvement.these findings highlight thr nrrd for more creative and interactive writing instruction to enhance studentd’idea development and vocabulary use.this research is expected to serve as a reference for teachers in improving the effectiveness of weiting lessons in elementary schools
