Abstract

Restorasi akhir pada gigi setelah perawatan endodonti sering menggunakan pasak dan inti sebagai fondasi penahan mahkota penuh. Saat ini penggunaan pita fiber resin reinforced yang terbuat dari bahan ultra high polyethylene fiber sebagai pasak menjadi popular. Resiko terjadinya fraktur akar dapat berkurang karena sistem pasak ini tidak memerlukan perluasan saluran akar lebih banyak. Retensi pasak ini didapat dari sistem adhesif sehingga dapat menambah kekuatan struktural dari gigi yang akan direstorasi. Laporan kasus ini membahas perawatan yang dilakukan pada dua orang pasien yang mengalami fraktur mahkota pada gigi insisivus maksila dan membutuhkan segera restorasi estetis. Laki-laki berusia 28 dan 19 tahun. Setelah perawatan endodonti dilakukan, bahan polyethylene fiber digunakan sebagai pasak-inti dengan menggunakan semen luting resin untuk mempertahankan struktur gigi tersisa. Komposit resin micro hybrid diaplikasikan sebagai restorasi veneer untuk membentuk restorasi akhir. Sebagai kesimpulan, bahan polyethylene fiber reinforced dapat membuat restorasi estetis dan memperkuat pasak dan inti secara struktural dalam satu kali kunjungan tanpa harus melalui proses laboratorium.

Abstract

Penggunaan bahan adhesif pita serat penguat polietilen dengan resin komposit sebagai pasak buatan semakin banyak diminati. Pemasangan pasak ini tidak membutuhkan pelebaran saluran akar yang lebih banyak sehingga dapat mengurangi risiko terjadinya fraktur akar. Wetting resin dipakai sebagai wettability untuk meningkatkan perlekatan interfasial. Terkadang para klinisi sering menggunakan flowable resin sebagai penggantinya. Penelitian ini bertujuan untuk mengetahui ketahanan fraktur penggunaan pita serat penguat poli etilen anyaman braided dan locked-sticthed threads sebagai pasak buatan pada restorasi pasca perawatan endodonti dengan aplikasi wetting resin dan flowable resin. Empat puluh gigi premolar mandibular yang telah diekstraksi dan memiliki ukuran sama dibagi menjadi empat kelompok. Kelompok A (locked-sticthed+wetting resin), B (locked-sticthed+flowable resin), C (braided+wetting resin) dan D (braided+flowable resin). Setelah pemotongan mahkota dan perawatan endodonti dilakukan pemasangan pasak buatan. Pita dibenamkan ke dalam semen resin dual cured dan dilanjutkan dengan pembentukan mahkota dari resin komposit. Setelah thermocycling, dilakukan uji tekan searah aksial gigi dengan kecepatan 0,5mm/detik sampai terjadi fraktur menggunakan Universal Testing Machine. Nilai beban fraktur dan pola fraktur yang terjadidicatat. Hasil uji statistik Anova menunjukkan tidak terdapat perbedaan signifikan pada ketahanan fraktur pasak buatan pita serat penguat dengan pola anyaman dan wettability yang berbeda (p= 0,367). Dengan adanya keterbatasan penelitian ini, pola fraktur pasak fiber anyaman locked-sticthed threads dan braided dengan wetting resinmenunjukkan 80-90% mudah direstorasi kembali. Sebagai kesimpulan, penggunaan pita serat penguat polietilen sebagai pasak buatan dengan anyaman berbeda memiliki ketahanan fraktur yang sama. Akan tetapi, pola fraktur pada pasak yang menggunakan wetting resin lebih mudah direstorasi kembali dibandingkan dengan flowable resin.

Abstract

This paper explains the community service activities in Suka Makmur Village, Binjai District, Langkat Regency. The aim is to improve the quality of life of the community through oral health, which is to encourage the emergence of creativity and initiative of each individual or community group in the village of Suka Makmur. Communities actively participate in health programs in their villages, and determine program priorities according to the needs and desires of the community. Overall it can be seen that 95.4% of the population experienced a low caries experience category, 3.8% of them had a moderate caries category and there were already 0.9% of the population who had a high caries risk category.

Abstract

Mandibular second molar is often associated with variations in root canal morphology. Despite the intricate association, the occurrence of middle distal canal in endodontic literature is limited, with an incidence of 0.2% to 3%. Therefore, this study presents a case report describing a clinical situation in mandibular second molar that shows four canals, opening into different apical foramens. A 30-year-old male patient came to the Rumah Sakit Gigi dan Mulut of Universitas Sumatera Utara with the main complaint of intermittent pain in the lower right tooth persisting for two days. The results of clinical examination of the lower right second molar labeled as #47, showed a temporary filling, while radiography indicated widened periodontal ligament space and termination of lamina dura. After receiving informed consent, root canal therapy was carried out, and a direct resin composite onlay was selected for final restoration. To locate and treat hidden and unusual anatomy, dentists are often required to possess a comprehensive understanding of internal dental anatomy and use modern instruments, such as the dental operating microscope and ultrasonic tips. In certain clinical, direct resin composite onlay restorations can represent a valid alternative to traditional indirect restorations. In conclusion, this study shows that a comprehensive understanding of the potential variances in the internal anatomy of human teeth is essential to achieving successful outcomes in endodontic therapy.

Abstract

Introduction: Teeth with furcation involvement lesions present a significant challenge for clinicians regarding treatment options. Various methods can be employed to restore mutilated teeth for a longer period of function, including resective surgery (hemisection, bicuspidization and root amputation/root resection). Case and Management: This case series presents clinical and radiographic data collected from two patients who were referred to the dental and oral hospital at the University of Sumatera Utara. The patients were examined and treated by a dental conservation resident, who diagnosed a periodontal case in the patient with a chief complaint of pain and swelling associated with class II and III furcation involvement, as well as endo perio lesions on the radiograph of the first lower left and right mandibular molars. The class II and III furcation was complicated with endodontic involvement. Patients were provided with endodontic treatment in conjunction with the initial stage of periodontal treatment, which includes dental health education, root scaling and debridement. Following three months of endodontic treatment, resective surgery, including hemisection and bicuspidation, were performed. Radiographic analysis revealed that the teeth exhibited satisfactory regeneration following resective surgery, with the teeth becoming the supporting teeth for the fixed denture. Conclusion: It is not always necessary to extract teeth with conditions involving furcation. However, a multidisciplinary approach is essential to ensure effective treatment of the lesion.

Abstract

Dental trauma may lead to tooth loss despite various efforts to retain and maintain the compromised tooth. Several studies have shown that the upper anterior teeth are more likely to suffer from trauma. In addition, their loss can lead to significant aesthetic and functional problems that are difficult to manage. A 22-year- old patient came to RSGM Universitas Sumatera Utara with chief complaint of fractured tooth that impaired appearance. Clinical and radiographic examination showed complicated crown and root fracture on anterior maxillary teeth (Andreason classification). Diagnosis of the fractured tooth revealed the presence of necrotic pulp with asymptomatic apical periodontitis. Treatment plan for tooth 11 was endodontic treatment, while tooth 21 was extracted due to poor prognosis. Subsequently, aesthetic rehabilitation was restored with the use of cantilever bridge. Management of tooth 11 was carried with endodontic treatment using a rotary file under rubber dam isolation, followed by retraction with dental floss. Cantilever bridge was then used as final restoration to optimize the aesthetic result. In addition, 1 month follow up showed good outcome in the patient. Increased patient demand for optimal aesthetics had led to the widespread use of alternatives to restore aesthetic following post- traumatic endodontic treatment, such as cantilever bridge. Lithium disilicate was an indirect restoration material that could be bonded using the latest universal adhesive system and had excellent aesthetic for use in the repair of aesthetic disorders. Root canal treatment and cantilever bridge could serve as treatment options to manage anterior tooth fracture and anterior tooth loss caused by dental trauma.

Abstract

Background: The polishing procedure is an important stage in producing a successful restoration. Polishing involves the use of abrasive materials, and blood cockle shells contain these natural components, such as calcium carbonate, which can be used as a polishing paste. One of the factors that influences the polishing quality is the polishing duration. Purpose: This study evaluated the polishability of a nanohybrid composite using blood cockle shell polishing paste by measuring the surface roughness and gloss value, and by observing the surface qualities based on different polishing times. Methods: Ninety samples of maxillary premolars were immersed in artificial saliva in an incubator at 37°C for 24 h. A total of 45 samples were subjected to a profilometer to observe surface roughness, and the rest were subjected to a glossmeter to observe the composite resin gloss. These included blood cockle shell paste, Prisma Gloss polishing paste, and without paste for 30, 60, and 90 seconds. The data were statistically analyzed using repeated measures ANOVA and post hoc least significant difference (LSD). Results: It showed that the group of 90 seconds had the highest gloss and the lowest surface roughness in the blood cockle shells. The difference was statistically significant (p>0.05). The results of the surface roughness test of the blood cockle shells with a duration of 30 and 60 seconds were found to be not significant (p<0.05). Conclusion: There were significant differences by adding the polishing duration, which decreased the surface roughness and increased the surface gloss, in which the lowest roughness and highest glossiness was found with a polishing time of 90 seconds. The polishability of composite resin restorations in this study obtained the best gloss and smoothest surface results from the longest polishing time duration of 90 seconds with the blood cockle shell polishing paste.

Abstract

Fiber reinforced composite post often used to increase the retention of the crown restoration after endodontic treatment. Retention of fiber reinforced composite post it was obtained through a system adhesif of cement luting resin. The problem of luting cement based resin is often occurs polymerization shrinkage thus a microleakage. This study attempts to look at the differences micro gap of glass prefabricated fiber reinforced post and ribbon polyethylene fiber reinforced post by using ahesif system total etch. Root canal treatment on 20 of the premolars first mandible have extracted for the orthodontic treatment. Coronal part of the teeth were banished, then used glass prefabricated fiber reinforced post and ribbon polyethylene fiber reinforced post. To evaluate the micro gap on both the post adhesif technique was used penetration dyestuff methylene blue 2 %. The entire sample diveded into three parts horizontally namely coronal, middle, and apical. The upper part of peces of these sample observed under stereomicroscop with enlargement 20 x, were a score 0-4 in the area expansion of penetration dyestuff. The result of statistical test Kruskal Wallis Test show there are significant differences (p 0,05) in group glass prefabricated fiber reinforced post and ribbon polyethylene fiber reinforced post. However the result of statical test Mann- Whitney test show significant differences on the coronal and middle (p0,083) while on the apical there is no significant difference (p 0,083).Keywords: glass prefabricated fiber reinforced post, polyethylen fiber reinforced, micro gap

Abstract

Background: Biomimetic restoration with multilayered composite gives the highest aesthetic result with the least invasive tooth structure. The goal of these techniques is to restore teeth while closely mimicking the micro- and macrostructure, chromatic properties, and tooth symmetry of the original dentition. These techniques need extra instruments and the clinician's experience. Purpose: This case study aims to describe a technique for repairing anterior defects in just one appointment. Case: A 24-year-old man complained about the color, asymmetry, and fracture of his anterior teeth. However, he does not have much time to wait for the indirect restoration procedure. A single-visit treatment was preferred for this case. Case Management: A manual wax-up for the palatal index followed by multilayered composite restoration was nicely done in a single appointment. Defects in anterior teeth could be repaired in a single appointment using multilayer direct composite restoration. This method could effectively replicate the teeth's morphology, symmetry, micro-macro structure, and chromatic properties. The quick manual wax-up and layering steps are critical to this procedure. Discussion: Currently, direct restorations for anterior teeth are completed in three stages: incisal edge and palatal wall shaping; interproximal wall creation utilizing a pre-contoured matrix technology; and buccal surface application of multilayered composite. Multilayered direct composite restoration technique with manual palatal index can be an alternative procedure to create immediate natural-looking anterior restoration. This technique offered a reduction of chairside time in a single-stage appointment, laboratory cost savings, and more predictable results.

Abstract

Selain kesehatan umum lainnya, kebersihan gigi dan mulut yang buruk dapat mengganggu sistem pengunyahan, menyebabkan masalah fonetik ketika berbicara, menurunkan kepercayaan diri, dan memengaruhi kesehatan umum dan kualitas hidup seseorang. Prevalensi penyakit gigi terbesar adalah Karies (lubang gigi) yang proses kerusakannya dapat meluas ke dalam pulpa gigi sehingga menyebabkan rasa sakit atau nyeri gigi. Kurangnya pengetahuan tentang kesehatan gigi, rendahnya motivasi untuk mengobati sakit gigi dan jauhnya akses lokasi pesantren ke puskesmas mendorong Tim pengabdian masyarakat Fakultas Kedokteran Gigi untuk membentuk Pos Kesehatan Gigi Pesantren (POSKESGITREN) sebagai salah satu unit pendukung tempat pelayanan kesehatan gigi untuk warga pesantren. Sebagai lembaga pendidikan agama Islam yang beroperasi dalam bentuk asrama atau pondok, pondok pesantren telah terbukti mampu membantu masyarakat dalam hal pendidikan keagamaan, sosial budaya, dan ekonomi, tetapi tidak dalam hal kesehatan, khususnya kesehatan gigi dan mulut. Program pelatihan untuk membentuk kader kesehatan gigi dan mulut yang terdiri dari santri, guru, dan karyawan puskesmas adalah bukti kerjasama antara Fakultas Kedokteran Gigi Universitas Sumatera Utara dan Pesantren Modern Al Mukhlishin Deli Serdang. Tujuan kegiatan pengabdian masyarakat ini adalah untuk meningkatkan derajat kesehatan gigi dan mulut kepada siswa di lingkungan Pesantren Modern Al Mukhlishin Deli Serdang melalui program pemeriksaaan screening secara berkala, pelatihan kader sebagai motivator edukasi kesehatan gigi dan mulut dan melatih kader untuk dapat memberikan obat sakit gigi pertolongan pertama sebelum perujukan ke fasilitas kesehatan masyarakat,. Agar program dapat dijalankan dengan baik tim pengabdian ini membentuk Pos Kesehatan Gigi Pesantren (POSKESGITREN) dengan memberikan sumbangan fasilitas berupa alat dental unit portable dan bahan untuk menunjang Kesehatan Gigi. Pembentukan Pos Kesehatan Gigi Pesantren (POSKESGITREN) sebagai salah satu bentuk Pengabdian Fakultas Kedokteran Gigi khususnya dalam bidang Konservasi Gigi kepada masyarakat  dalam rangka meningkatkan kualitas kesehatan gigi dan mulut kepada warga santri di lingkungan Pesantren Modern Al Mukhlishin Deli Serdang.
