Abstract

Fibroadenoma mammae (FAM) is the most common benign breast tumor that occurs at the age of 14-35 years. About 10% of the female population has FAM. FAM manifests as solitary masses that are easily movable, well-defined, with a smooth surface. The aim of this study was to determine the characteristics, clinical features, and management of FAM patients at Dustira Hospital using secondary data from medical records. A total of 68 FAM patients were selected as samples during the period 2018-2023. The research variables consisted of the number of cases, age, body mass index(BMI), location, topography, size, quantity, and management of FAM. Univariate analysis was used in this study and the results were presented in narrative and tabular forms. The results of the study showed that the highest number of FAM cases occurred in 2018, reaching 28 (41.1%). Characteristics of FAM patients included 49 individuals (72%) in the 16-30 age group, 32 individuals (47%) with normal body mass index. Clinical features of FAM were found in 31 individuals (45.6%) on the right breast, 24 individuals (35.3%) in the upper medial quadrant, with a size of 3-5 cm in 24 individuals (35.3%), and 49 individuals ( 72.1%) limited solitary masses. Management included surgery in all 68 individuals (100%), with 67 individuals (98.5%) underwent excision, while 1 individual (1.5%) underwent incision. Age and BMI are suspected to play a role in FAM formation related to estrogen hypersensitivity and mutations in the mediator complex subunit 12 (MED12). Lesion size is influenced by estrogen exposure. Management of fibroadenoma depends on the patient's preferences and clinical conditions. Women at risk or suspecting breast masses are advised to perform self-breast examination (BSE) and efficiently consult with healthcare professionals.

Abstract

Breast cancer, the most common type of cancer diagnosed in women, is characterised by the growth of abnormal cells in the breast tissue. Chemotherapy is one of the main treatment methods for breast cancer, aimed at destroying cancer cells. Although effective, chemotherapy may cause various side effects that affect patients' quality of life. This study aimed to determine the quality of life of breast cancer patients undergoing chemotherapy at Hasan Sadikin Hospital Bandung. The study design was descriptive observational of breast cancer patients undergoing chemotherapy. The sample consisted of 35 individuals who met the inclusion criteria, selected using a consecutive technique. The instrument for measuring quality of life used the standard EORTC QLQC-C30, which measures global health status, functional scales, and symptom scales. The research results show that the quality of life of breast cancer patients undergoing chemotherapy has a global health status in the good category (73.8), functional scale in the good category (78.2), and symptom scale in the low category (28.6). The average quality of life of the patients is in the moderate category with a score of 685.8. Discussion, although global health status and functional scale are considered good, symptoms such as insomnia, vomiting, and decreased appetite remain a challenge. Ini mempengaruhi aktivitas sehari-hari dan hubungan sosial, yang dapat memperburuk ketidakseimbangan emosional dan psikologis pasien.

Abstract

The transition of medical professional students from the preclinical stage to the clinical stage is a period filled with critical academic and psychological challenges. The concept of an integrated clerkship is needed to support experiential learning, improve self-confidence, and minimize the risk of medical errors due to system unpreparedness. Evaluation of the Integrated Surgical Clerkship (ISC) program at the Faculty of Medicine, Jenderal Achmad Yani University (FK Unjani) is an essential step to ensure effective curriculum implementation. This study aimed to describe the perceptions of professional-year students of the LXXVI cohort regarding the effectiveness and implementation of the ISC program at the Faculty of Medicine, Unjani. A descriptive quantitative study with a cross-sectional design was conducted on all professional-year students of Cohort LXXVI (N = 120) using a total sampling technique. Data were collected using a validated questionnaire consisting of 26 items. Data analysis was performed using descriptive statistics. The instrument demonstrated very high reliability, with a Cronbach’s alpha of 0.982. The respondents consisted of 83 female students (69.2%) and 37 male students (30.8%). Students’ perceptions of the ISC program were in the very good category, particularly in the domains of clinical readiness and mentor quality. The ISC program was found to be effective in supporting the transition to clinical education and is recommended for continued implementation.

Abstract

Pre-operative tachycardia is a frequent clinical manifestation commonly attributed to pain, anxiety, or systemic infection.1,2 However, uncommon endocrine emergencies such as pheochromocytoma may present with nonspecific cardiovascular instability and remain clinically unrecognized until catastrophic perioperative deterioration occurs.3,4 This case report describes a 26-year-old female who presented with a painful hyperemic right breast mass accompanied by persistent tachycardia and respiratory distress. In accordance with CARE case report principles, clinical data were retrospectively collected from medical records, including serial hemodynamic monitoring, laboratory investigations, electrocardiography, thoracic imaging, perioperative management, and intensive care interventions. Initial evaluation demonstrated persistent tachycardia (153 bpm), tachypnea, and hypertensive episodes that were disproportionate to the localized breast pathology. Despite surgical intervention and initial postoperative stabilization, the patient developed progressive hemodynamic instability characterized by persistent tachyarrhythmia, a Burch-Wartofsky Point Scale score of 20, severe hyperglycemia (858 mg/dL), ketonuria, and electrolyte imbalance, which eventually culminated in asystolic cardiac arrest. This constellation of cardiovascular and metabolic abnormalities strongly suggested a pheochromocytoma crisis precipitated by surgical stress and excessive catecholamine release.1,3,4 Catecholamine-induced insulin resistance and impaired peripheral glucose utilization likely contributed to the profound metabolic derangement observed in this patient.5,6 This case underscores the importance of recognizing unexplained perioperative tachycardia and hyperglycemia as potential indicators of occult pheochromocytoma.7–9 Early endocrine evaluation and appropriate perioperative preparation are critical to prevent fatal cardiovascular complications and improving surgical outcomes.   DOI : 10.35990/amhs.v4n3.p151-158   REFERENCES Nazari MA, Rosenblum JS, Haigney MC, Rosing DR, Pacak K.
