Abstract

Abstract: Claim files that are returned / pending can result in a decrease in hospital income due to a mismatch in service costs with the number of claims billed. The results of submitting inpatient claims at Sentra Medika Cibinong Hospital in September-November 2023 were 327 (10%) pending files out of 3,250 files billed. The purpose of the study was to determine the effect of coding accuracy on the results of submitting BPJS Health inpatient claims. With a quantitative approach and cross sectional design. The population was all inpatient claim files, a sample of 300 claim files with the Accidental Sampling technique. Data analysis using chi square test and observation data collection. The results showed inaccurate coding files 116 (38.7%) and accurate coding 184 (61.3%) and pending files 49 (16.3%) and not pending 251 (83.7%). While the results of the chi square test obtained a P-value of 0.002 <0.05 OR value of 2.733 It was concluded that there was an influence between coding accuracy on the results of submitting BPJS Health inpatient claims at Sentra Medika Cibinong Hospital. Furthermore, inaccurate coding accuracy files are 2.7 times more likely to be pending than accurate files. Accurate coding files were pending 20 (10.9%) and not pending 164 (89.1%), inaccurate coding files were pending 29 (25%) and not pending 87 (75%). Suggestions for conducting regular evaluations of cases that have the potential to experience pending claims. Keyword: Coding Accuracy, Submission Results, Pending Claims Abstrak: Berkas klaim yang dikembalikan/pending dapat mengakibatkan penurunan pendapatan rumah sakit akibat ketidaksesuaian biaya layanan dengan jumlah klaim yang ditagihkan. Hasil pengajuan klaim rawat inap di RS Sentra Medika Cibinong pada bulan September–November 2023 Sebanyak 327 (10%) berkas pending dari 3.250 berkas yang ditagihkan. Tujuan penelitian untuk mengetahui pengaruh akurasi koding terhadap hasil pengajuan klaim rawat inap BPJS Kesehatan. Dengan pendekatan kuantitatif dan desain cross sectional. Populasi seluruh berkas klaim rawat inap pada bulan Maret 2024, sampel 300 berkas klaim dengan teknik Accidental Sampling. Analisa data menggunakan uji chi square dan pengumpulan data observasi. Hasil penelitian menunjukan berkas koding tidak akurat 116 (38,7%) dan koding akurat 184 (61,3%) serta berkas terpending 49 (16,3%) dan tidak terpending 251 (83,7%). Sedangkan hasil uji chi square diperoleh nilai P-value 0,002 < 0,05 nilai OR 2,733 Maka disimpulkan ada pengaruh antara akurasi koding terhadap hasil pengajuan klaim rawat inap BPJS Kesehatan di RS Sentra Medika Cibinong. Selanjutnya berkas akurasi koding tidak akurat berpeluang 2,7 kali akan terpending dibandingkan berkas yang akurat. Berkas koding akurat yang terpending 20 (10,9%) dan tidak terpending 164 (89,1%), berkas koding tidak akurat yang terpending 29 (25%) dan tidak terpending 87(75%). Saran melakukan evaluasi secara berkala terhadap kasus yang berpotensi mengalami pending klaim. Kata kunci: Akurasi Koding; Hasil Pengajuan; Pending Klaim

Abstract

Abstract: Perceptions of ethics and work behavior are two things that cannot be separated because perceptions of ethics influence the work behavior of the medical recording profession so that medical recorders can run well. The aim of this research is to determine the relationship between perceptions of professional ethics and the behavior of medical recorders and health information in DPD PORMIKI DKI Jakarta.  The sample consisted of 91 people using quota sampling. Analysis uses the Spearman Correlation Test. The research results show that PMIK's work behavior has a minimum value of 54 and a maximum of 80, mean +/std.deviation 69.76+/7.473 and 95% CI lower-upper 68.20-71.31 and perception of professional ethics a minimum value of 30 and a maximum of 52 , mean+/-std.deviation 45.68+/5.938 and 95%CI lower-upper 44.44- 46.92, r results (0.290) level of strength is sufficient, the positive correlation coefficient value of the relationship between the two variables is said to be in the same direction and the p-value 0.005, the relationship between variables is significant, there is a relationship between perceptions of professional ethics and PMIK work behavior. Training facilities for work ethics and behavior, internal research on perceptions and collaboration with educational institutions. Keywords: Medical Recorder and Health Information, Work Behavior, Perception of                  Professional Ethics Abstrak: Persepsi etika dengan perilaku kerja merupakan dua hal yang tidak dapat dipisahkan karena persepsi etika mempengaruhi perilaku kerja dari profesi perekam medis sehingga perekam medis bisa menjalankan tugasnya dengan baik. Tujuan dari penelitian ini adalah untuk mengetahui hubungan persepsi etika profesi dengan perilaku perekam medis dan informasi kesehatan di DPD PORMIKI DKI Jakarta.  Sampel terdiri dari 91 orang menggunakan kuota sampling. Analisis menggunakan Uji Korelasi Spearman. Hasil penelitian menunjukkan bahwa perilaku kerja PMIK nilai minimum 54 dan maximum 80, mean +/std.deviasi 69,76+/7,473 dan 95%CI lower-upper 68,20-71,31 dan persepsi etika profesi nilai minimum 30 dan maximum 52, mean+/-std.deviasi 45,68+/5,938 dan 95%CI lower-upper 44,44- 46,92, hasil r (0,290) tingkat kekuatan cukup, bernilai koefisien korelasi positif  hubungan kedua variabel dikatakan searah dan p-value 0,005 hubungan antar variabel bernilai signifikan ada hubungan antara persepsi etika profesi dengan perilaku kerja PMIK. Abstract

Background: The results of the 2023 Indonesian Health Survey show that the prevalence of hypertension in Indonesia based on measurements of the population aged ≥18 years is 30.8%, but only 8.6% are diagnosed by doctors, while cases of hypertension at the Sukasari Health Center between 2020 and 2023 have doubled from 3.428 to 6.229 people. Integrated non-communicable disease development posts are one strategy in dealing with hypertension. On the other hand, hypertension sufferers who routinely visit integrated non-communicable disease development posts are still low at 45.7%. Purpose: To determine the factors that influence visits by hypertension sufferers to integrated non-communicable disease development posts. Method: An observational quantitative study using a cross-sectional method was conducted at the Sukasari Health Center, Sukasari Village, Tangerang District, Tangerang City, Banten in March-June 2024. The sampling technique used was total sampling and 155 samples were obtained. The research data collection was carried out by reviewing documents (secondary data) and the instrument used was a document review sheet to review data on visits to the integrated non-communicable disease development post. Results: This study shows that the most influential factors on visits by hypertension sufferers to the integrated non-communicable disease development post are primary education level (p-value = 0.007 OR = 11.140), secondary education (p-value = 0.026 OR = 7.266), prehypertension blood pressure (p-value = 0.041 OR = 2.698), and hypertension blood pressure level 2 (p-value = 0.045 OR = 0.369). Meanwhile, the variables that had no effect were gender (p-value=0.205), occupation (p-value=0.743), and comorbidity (p-value=0.497). Conclusion: The most influential factors on visits by hypertension sufferers to the integrated non-communicable disease development post were education level and blood pressure status. Suggestion: There should be efforts to increase the active role and support of health cadres and village officials in promoting the activities of the integrated non-communicable disease development post, through social media groups for residents in each region so that the community can diligently visit the integrated non-communicable disease development post.   Keywords: Hypertension; Visit; Integrated Development Post; Non-Communicable Diseases.   Pendahuluan: Hasil Survei Kesehatan Indonesia Tahun 2023, menunjukan bahwa prevalensi hipertensi di Indonesia berdasarkan hasil pengukuran pada penduduk umur ≥18 tahun sebesar 30.8%, namun hanya sebesar 8.6% yang terdiagnosis dokter, sedangkan kasus penyakit hipertensi di Puskesmas Sukasari antara tahun 2020 dan 2023 meningkat dua kali lipat dari 3.428 menjadi 6.229 jiwa. Posbindu PTM merupakan salah satu strategi dalam penanggulangan hipertensi. Di sisi lain, penderita hipertensi yang rutin berkunjung ke posbindu PTM masih rendah dengan persentase 45.7%. Tujuan: Untuk mengetahui faktor yang mempengaruhi kunjungan penderita hipertensi di posbindu penyakit tidak menular (PTM). Metode: Penelitian kuantitatif observasional menggunakan metode cross sectional yang dilaksanakan di Puskesmas Sukasari, Kelurahan Sukasari, Kecamatan Tangerang, Kota Tangerang, Banten pada Bulan Maret-Juni 2024. Teknik pengambilan sampel yang digunakan adalah total sampling dan didapatkan sebanyak 155 sampel. Pengumpulan data penelitian dilakukan dengan cara telaah dokumen (data sekunder) dan instrumen yang digunakan adalah lembar telaah dokumen untuk melakukan tinjauan data kunjungan posbindu PTM. Hasil: Penelitian ini menunjukkan bahwa faktor yang paling berpengaruh terhadap kunjungan penderita hipertensi di posbindu PTM adalah jenjang pendidikan dasar (p-value=0.007 OR=11.140), pendidikan menengah (p-value=0.026 OR=7.266), tekanan darah prehipertensi (p-value=0.041 OR=2.698), dan tekanan darah hipertensi tingkat 2 (p-value=0.045 OR=0.369). Sementara itu, variabel yang tidak ada pengaruh adalah jenis kelamin (p-value=0.205), pekerjaan (p-value=0.743), dan komorbiditas (p-value=0.497). Simpulan: Faktor yang paling berpengaruh terhadap kunjungan penderita hipertensi di posbindu PTM adalah jenjang pendidikan dan status tekanan darah. Saran:  Adanya upaya untuk meningkatkan peran aktif dan dukungan kader kesehatan serta perangkat desa dalam mempromosikan kegiatan posbindu PTM, melalui grup media sosial warga pada tiap wilayah agar masyarakat dapat rajin berkunjung ke posbindu PTM.   Kata Kunci: Hipertensi; Kunjungan; Penyakit Tidak Menular (PTM); Pos Pembinaan Terpadu (Posbindu).

Abstract

Setiap RS wajib mendokumentasikan penyebab kematian ke dalam SMPK, dan diisi lengkap 100% oleh dokter dalam waktu < 24 jam setelah selesai pelayanan. Pada SMPK terdapat beberapa indikator penyebab kematian seperti : penyebab langsung, penyebab antara, dan penyebab dasar. Tepat atau tidak tepat penentuan kode penyebab kematian pada SMPK merupakan tanggung jawab coder, ketidaktepatan kode penyebab kematian dapat menyebabkan laporan mortalitas menjadi tidak akurat. Penelitian ini dimaksudkan untuk mengetahui hubungan kelengkapan penulisan penyebab kematian pada SMPK dengan ketepatan kode penyebab kematian di RSIJ Sukapura Kelapa Gading. Penelitian ini dilakukan di RSIJ Sukapura Kelapa Gading. Jenis penelitian yaitu kuantitatif dengan desain obsevasional anatilik. Metode yang digunakan yaitu cross sectional. Populasi pada penelitian ini 166 SMPK dengan besar sampel 115 SMPK. Pengumpulan data dilakukan menggunakan daftar tilik. Teknis analisis data menggunakan analisis univariat dan bivariat dengan uji chi square. Hasil penelitian menunjukkan bahwa tidak ada hubungan antara kelengkapan penulisan penyebab kematian pada SMPK dengan ketepatan kode penyebab kematian, dengan nilai P value yaitu 0,442.

Abstract

Abstract: Hospitals are required to minimize errors made by medical and non-medical staff to build patient trust in the services provided. Data show that patient visits at Jakarta Islamic Hospital Pondok Kopi from January to August 2023 reached 36,188 with an average of 4,523 visits per month, with the lowest number recorded in April 2023. This study aims to describe the influence of outpatient trust in doctors services on their interest in repeat visits. The research employed a quantitative approach with a cross-sectional design, involving 100 non-BPJS outpatients at the Outpatient Unit, with data collected through questionnaires. Data were analyzed using a Simple Logistic Regression test. The findings indicate that patients trust doctors services and are interested in returning for subsequent visits. Bivariate analysis revealed that outpatient trust in doctors services simultaneously had a significant effect on repeat visit interest, as shown by the wald test result (f count 11.170 > f table 1.984467) with a significance level of 0.001 < 0.05. Thus, patient trust plays a crucial role in fostering loyalty through repeat visits at Jakarta Islamic Hospital Pondok Kopi.Keyword: Patient Trust, Doctor Services, Repeat Visits, Hospital, OutpatientAbstrak: Rumah sakit dituntut untuk meminimalkan kesalahan tenaga medis maupun nonmedis agar tercipta kepercayaan pasien terhadap pelayanan yang diberikan. Data menunjukkan kunjungan pasien di Rumah Sakit Islam Jakarta Pondok Kopi pada JanuariAgustus 2023 sebanyak 36.188 orang dengan rata-rata 4.523 kunjungan per bulan, di mana angka terendah terjadi pada April 2023. Penelitian ini bertujuan menggambarkan pengaruh kepercayaan pasien rawat jalan terhadap pelayanan dokter dalam meningkatkan minat kunjungan ulang. Penelitian menggunakan pendekatan kuantitatif dengan desain cross sectional, melibatkan 100 responden pasien non-BPJS di Unit Rawat Jalan, dan data dikumpulkan melalui kuesioner. Analisis dilakukan menggunakan uji Regresi Logistik Sederhana. Hasil penelitian menunjukkan bahwa pasien menaruh kepercayaan pada pelayanan dokter dan berminat melakukan kunjungan ulang. Analisis bivariat mengungkapkan kepercayaan pasien secara simultan berpengaruh signifikan terhadap minat kunjungan ulang, dibuktikan dengan hasil uji wald (f hitung 11.170 > f tabel 1.984467) pada tingkat signifikansi 0.001 < 0.05. Dengan demikian, kepercayaan pasien menjadi faktor penting dalam membangun loyalitas melalui kunjungan ulang di Rumah Sakit Islam Jakarta Pondok Kopi.Kata kunci: Kepercayaan Pasien, Pelayanan Dokter, Minat Kunjungan Ulang, Rumah Sakit, Rawat Jalan

Abstract

The medical recorders at the registration site have an important role because the initial documentation of the patient begins in the registration process. In general, the registration place can be divided into outpatient, inpatient, and emergency patient registration places. Several studies have shown the obstacles experienced by the medical recorders in the registration section, including queues, the medical record documents were not found, and the patient did not carry an identity card for treatment. From these problems, the purpose of this study is to analyze the need for medical recorders in the registration section based on the workload in the literature review. The design of this research is a literature review. The article in this study uses a time span of the last 5 years (2014 - 2019) and focuses on the area of research conducted in the hospital. Furthermore, from the screening results according to the inclusion and exclusion criteria of research articles, 6 research articles were obtained. The results of the review of the article show that the average available work time is 2,156 hours/year, the average standard time workload is 215,595 minutes/year, the average allowance standard is 0.15, and the need for available medical recorders that has been met is 34%. From the results of the research, there were still those that did not meet the need for medical recorders because they did work outside their basic duties. In addition, the process of searching for medical records that hampers services raised queuing problems.

Abstract

Problems related to work discipline among Medical and Health Information Recorders (PMIK) observed during fieldwork practice in hospitals include non-compliance with punctuality, delays in completing tasks, and ineffective use of break time. These conditions indicate that the level of work discipline among PMIK personnel is suboptimal, which may affect productivity and service quality. Job satisfaction is one of the factors presumed to influence work discipline. This study aimed to examine the effect of job satisfaction on work discipline among PMIK members within the DPD PORMIKI DKI Jakarta area. A quantitative approach with a cross-sectional design was employed. Using a quota sampling technique, 94 respondents were selected from a total population of 1,490. The results showed that job satisfaction had a positive and significant effect on work discipline (p-value = 0.000). The regression equation obtained was Y = 5.288 + 0.606X. Job satisfaction accounted for 75.1% of the variance in work discipline, while the remaining 24.9% was influenced by other factors not examined in this study. Therefore, efforts to enhance job satisfaction are essential to improve work discipline among PMIK professionals.

Abstract

Kinerja suatu organisasi bergantung pada kinerja dari sumber daya manusianya. Karakteristik tiap karyawan mempengaruhi kinerja yang dihasilkan, termasuk dalam pelayanan di unit rekam medis. Oleh karena itu, tujuan penelitian ini untuk menganalisis hubungan karakteristik karyawan dengan kinerja di Unit Rekam Medis di RSUD Tengku Rafi’an Kabupaten Siak. Jenis penelitian ini menggunakan pendekatan kuantitatif analitik dengan desain penelitian potong lintang. Penelitian ini dilakukkan di Rumah Sakit Umum Daerah Tengku Rafi’an Kabupaten Siak Riau. Populasi pada penelitian ini adalah seluruh karyawan di unit rekam medis. Pengambilan sampel dilakukan dengan mengambil seluruh populasi sehingga didapatkan jumlah sampel sebanyak 49 orang. Pengumpulan data dilakukan menggunakan kuesioner. Uji statistik yang digunakan adalah uji chi square. Hasil penelitian ini menunjukkan mayoritas karyawan memiliki kinerja kurang. Analisis bivariat menunjukkan tidak terdapat hubungan antara usia dengan kinerja karyawan di Unit Rekam Medis di RSUD Tengku Rafi’an Kabupaten Siak (p value=0,414). Terdapat hubungan antara status pegawai (p-value=0,026), tingkat pendidikan (p-value=0,007), dan pengalaman kerja (p-value=0,012) dengan kinerja sumber karyawan di Unit Rekam Medis di RSUD Tengku Rafi’an Kabupaten Siak.

Abstract

Abstract: Workload is the accumulation of tasks and responsibilities that must be carried out by employees or staff in accordance with the targets set by their supervisors within a specific period of time. The transition from a manual system to an electronic system has the potential to affect the workload of Medical Record and Health Information (MRHI) personnel. This study aims to measure the workload of MRHI staff after the implementation of Electronic Medical Records (EMR) in the Medical Records Department, using job analysis and workload analysis methods based on Regulation of the Ministry of Administrative and Bureaucratic Reform (KEMENPAN RB) No. 1 of 2020. This is a descriptive study with a purposive sampling approach, involving 10 out of 34 MRHI staff with a D3-level education. Data were collected through interviews and direct observation.The results show that the workload for new outpatient registration is 4 minutes, returning outpatients 3 minutes, inpatients 13.4 minutes, and self-registration via APM 5 minutes—requiring 16 staff. Assembling takes 10 minutes per medical record, requiring 2 staff; outpatient coding takes 1 minute and inpatient coding 3.5 minutes per record, requiring 3 coders. Filing and distribution take 6 and 5 minutes per record respectively, requiring 6 staff. Retention and digitization of active and inactive records take 10 minutes per record, requiring 5 staff. Reporting requires 15,240 minutes in total, with 3 reporting staff. Analysis of EMR completeness requires 15 minutes per record, needing 2 expert MRHI staff, while EMR monitoring requires 3 expert MRHI staff. The total workforce required is 35 skilled MRHI personnel and 5 expert MRHI professionals. Keyword: Electronic Medical Records, Workload, Medical Record and Health Information, Job Analysis Abstrak: Beban kerja merupakan akumulasi tugas dan tanggung jawab yang harus dilaksanakan oleh karyawan atau petugas sesuai dengan target yang telah ditentukan oleh atasan dalam periode waktu tertentu. Transisi dari sistem manual ke sistem elektronik berpotensi mempengaruhi beban kerja Perekam Medis dan Informasi Kesehatan (PMIK) Penelitian ini bertujuan untuk mengukur beban kerja PMIK pasca penerapan RME di Instalasi Rekam Medis menggunakan metode analisis jabatan dan analisis beban kerja sesuai Peraturan KEMENPAN RB No. 1 Tahun 2020. Penelitian ini merupakan penelitian deskriptif dengan pendekatan purposive sampling, melibatkan 10 orang dari total 34 petugas di instalasi rekam medis dengan pendidikan D3 PMIK. Pengumpulan data dilakukan melalui wawancara dan observasi. Hasil pengukuran menunjukkan bahwa beban kerja pendaftaran rawat jalan pasien baru 4 menit,pasien lama 3 menit untuk pasien rawat inap 13,4 menit sedangkan APM 5 menit dibutuhkan sebanyak 16 petugas, Beban kerja assembling 10 menit/BRM dibutuhan 2 tenaga assembling, Beban kerja koding pasien rawat jalan 1 menit , koding rawat inap 3,5 menit/BRM di butuhkan 3 tenaga koding, beban kerja filling 6 menit/ BRM dan distribusi 5 Menit/BRM memerlukan 6 petugas, Beban kerja petugas retensi dan alih media berkas rekam medis in akif dan aktif 10 menit/BRM dan dibutuhkan 5 tenaga, Beban kerja pelaporan 15.240 menit kebutuhan tenaga pelaporan 3 petugas , beban kerja analisa kelengkapan rekam medis elektronik 15 menit/RME di butuhkan 2 tenaga PMIK ahli dan monitoring rekam medis elektroni k sebanyak 3 tenaga PMIK ahli, total kebutuhan tenaga PMIK adalah 35 tenaga PMIK terampil dan 5 orang PMIK ahli. Abstract

Patient satisfaction is an important thing that must be maintained in the hospital. Intense competition from all hospitals to shape patient satisfaction in the era of globalization depends on the quality of services provided. The purpose of this study was to measure the effect of service quality on patient satisfaction in hospitals based on tangible, reliability, responsiveness, assurance, and empathy dimensions. This study used the literature review method with the topic of patient satisfaction. As for the 126 articles recorded, then 27 articles were selected that match the research criteria. The results showed the tangible dimension (23 articles), the dimensions of reliability, responsiveness, and empathy (22 articles) and the assurance dimension (20 articles which stated that there was an effect on patient satisfaction. Simultaneously, the magnitude of the effect of the service quality dimension was more than 50% on patient satisfaction.
