Abstract

Background: The most common complications in Diabetes Mellitus patients are foot problems and high risk for amputation. Factors that trigger problems in diabetic feet in the form of neuropathy factors, namely loss of sensory nerve function and circulatory disorders. One of the non-pharmacological therapies to prevent these problems is safe Neuromuscular Taping (NMT) which is applied to the skin with elastic plaster through a decompression method to prevent a decrease in peripheral blood circulation and improve blood circulation. Objectives: The purpose of this study was to determine the comparison of the effectiveness of Neuromuscular Taping (NMT) intervention on microvascular circulation which was assessed by Ankle Brachial Pressure Index (ABPI) measurement in type 2 Diabetes Mellitus patients. Methods: This study used a quasi-experimental research design with a pretest-posttest control group approach. The number of respondents to this study was 60 type 2 DM patients divided into treatment group respondents and control group respondents. The intervention group was given NMT intervention for 2 weeks with daily plaster changes while the control group was given standard foot care education. The data collected is an ABPI score that describes microvascular circulation using a sphygmomanometer. Analysis was carried out using paired sample t-tests which were used to determine the effectiveness of the intervention. Results: Based on the results of the study, significant changes were obtained in the microvascular circulation of the treatment group (p = 0.000) α 0.05 which is the category of normal microvascular circulation or no disturbance. Increased microvascular circulation is caused by the application of NMT for 2 weeks. Conclusion: There was an increase in microvascular circulation seen through changes in Ankle Brachial Pressure Index (ABPI) values before and after Neuromuscular Taping (NMT) intervention. There were significant differences between the intervention and control groups in changes in ABPI scores.

Abstract

Background: Adherence to the recommended dietary patterns for individuals with Type 2 Diabetes Mellitus (T2DM) is a major challenge in effectively managing the disease. One reason is the varying levels of knowledge among T2DM patients regarding appropriate dietary patterns. Limited knowledge makes it difficult for patients to understand appropriate eating patterns, impacting the effectiveness of diet management. As an educational digital media, the Embodied Conversational Agent (ECA) has proven effective in improving patient knowledge and adherence to T2DM management. Purpose: Developing and evaluating the feasibility of peunajoh.id as an ECA-based application designed to strengthen knowledge of the DMT2 diet. Methods: This study used a Research and Development (R&D) design with the Borg & Gall development model. Validation was conducted by 3 material experts and 3 media experts, followed by an initial trial on 15 T2DM patients in the prolanis community to assess the content's understandability and ease of use of the application, and an operational test on 45 T2DM outpatients to test the effectiveness of the widespread use of ECA media in a regional hospital. Data were analyzed using the Content Validity Index (CVI) to assess the appropriateness of the content and the percentage of user satisfaction with the use of ECA media in dietary education for T2DM patients. Results: Expert validation yielded an average I-CVI of 0.88 (material) and 0.83 (media). The results exceeded the ideal feasibility limit of ≥ 0.78, indicating that the material and media display had high relevance. Operational test results showed an average user satisfaction of 92%, with aspects of ease of navigation at 33.7%, ease of application use at 33.3%, and interaction with virtual agents at 33%. Conclusion: Peunajoh.id is an interesting application and is suitable for use as a diet education digital media for T2DM patients through the results of an average user satisfaction level reaching 92%.

Abstract

Background: Blood pressure changes during hemodialysis are a common complication and can profoundly impact patient safety and quality of life. Various factors such as individual characteristics, technical aspects of the dialysis procedure, and psychological conditions are suspected to contribute to these blood pressure variability. Objective: This study aims to analyze the relationship between age, dialysis vintage, interdialytic weight gain (IDWG), blood flow rate (BFR), and stress levels with blood pressure variability in hemodialysis patients. Methods: This study uses a cross-sectional design with a quantitative approach. Fifty hemodialysis patients who met the inclusion criteria were selected as respondents. Data were collected through clinical dialysis observation sheets, pre-post dialysis blood pressure measurements, and stress level questionnaires. Data analysis was performed using the chi-square test. Data analysis was performed using the chi-square test. Results: The results showed that 54% of patients experienced changes in blood pressure after hemodialysis. There was a significant relationship between age (p = 0.008), IDWG (p = 0.003), BFR (p = 0.027), and stress levels (p = 0.002) with changes in blood pressure. Meanwhile, the dialysis vintage did not show a significant relationship (p = 0.569). Conclusion: Variability in blood pressure during hemodialysis are influenced by physiological, technical, and psychological factors. Managing hemodialysis patients requires a holistic and individual approach, including IDWG management, BFR adjustment, and stress management. Further research is recommended with a longitudinal design and a larger sample coverage.
