Abstract

Background: Carpal tunnel syndrome (CTS) is a peripheral nerve lesion because of the nontraumatic mechanism or a pressure or entrapment of the median nerve under the transverse ligament (flexor retinaculum). Hydrodisection is used for adhesiolysis to eliminate adhesion and release the median nerve from the retinaculum and connective tissue around it and avoid injury to the nerves. Objective: Compare the effectiveness of hydrodisection injection therapy using an ultrasound guide between normal saline and triamcinolone in patients with carpal tunnel syndrome.Methods: Experimental study was carried out in the outpatient clinic of the hospital Dr. Saiful Anwar Malang from August 2018 to October 2018. A total of 30 patients. All study subjects were subjected to NPS, FSS, SSS scoring compared to before and 4 weeks after injection of hydrodisection and compared the effectiveness of injection agents.Results: NPS before and after triamcinolone injection (sig 0.000; p <0.05), FSS (sig 0.020; p <0.05), and SSS (sig 0.001; p <0.05). NPS before and after injection of normal saline 0.9% NaCl  (sig 0.001; p <0.05), FSS (sig 0.005; p <0.05), and SSS (sig 0,000; p <0.05). NPS between triamcinolone injection results and normal saline 0,9% NaCl  (sig 0.341; p <0.05), FSS (sig 0.425; p <0.05), SSS (sig 0.350; p <0.05).Conclusion: There no significant difference between injection of hydrodisection USG guiding with triamcinolone and normal saline 0,9% NaCl to reduce symptom CTS. Both of injection hydrodisection USG guiding has more or less the same effectiveness in treatment of carpal tunnel syndrome.Keywords: CTS, Hydrodisection, NPS, FSS, SSS

Abstract

Background: Epilepsy is the highest prevalence diseases in neurology. The decreased of cognitive function is one of the complications which is important. The purpose of this study is to determine the correlation between Mini Mental State Examination (MMSE) and Montreal Cognitive Assessment in Indonesian Version (MoCA-Ina) score on patient with epilepsy in Mataram. Method: This is an observational cross-sectional study with 56 subjects with epilepsy from West Nusa Tenggara 2 center hospital. Data that collected from the samples were MMSE and MoCA-Ina score. Beside of that patients’ characteristics data were also collected which is age, gender, education, seizure control, etiology of epilepsy, seizure type, antiepileptic drugs treatment, and smoking status from the subjects. Both instrument’s mean scores were analyzed for the correlation using Spearman correlation test. Result: This study showing that the subjects average age were 32.9 years old, female (55.4%), and low educated (62.5%). According to the clinical characteristics most subjects were not-depressed (67%), poor quality-controlled seizure (55.4%), idiopathic etiology (66.9%), generalized seizure (71.4%), and having monotherapy (85.7%). Most of the subjects were not a smoker (76.8%). In this study both the mean score of MMSE and MoCA-Ina were 25.3 and 21.3. MMSE and MoCA-Ina score mean have a strong correlation (r=0.766; p<0.001). Conclusion: MMSE and MoCA-Ina score mean on patients with epilepsy have a strong correlation. Therefore, both instruments could be used to evaluate cognitive function on patients with epilepsy

Abstract

Background: sleep is a physiological need for every human, including medical students. Studies have shown that poor sleep quality can lead to a decline in learning and memory abilities in medical students. The dual process theory explains that certain types of memory depend on common sleep conditions. This study aims to determine the association between sleep quality and short-term memory function, as measured by the digit span test, among medical students at the University of Mataram Objective: To determine the association between sleep quality and short-term memory function as measured by the digit span test among Medical Students at the University of Mataram. Methods: Observational analytic study with a cross-sectional design. The primary data included sociodemographic characteristics, Pittsburgh Sleep Quality Index, and the Digit Span Test. Data were analyzed using chi-square test and Fisher's exact test. Results: 62 participants were observed. Participants ranged from 18 to 23 years, with 42 students (67.7%) identified as female. 71% of the participants experienced poor sleep quality, while 29% reported good sleep quality. All 62 students showed normal performance in the forward digit span test. In the backward digit span test, 79% obtained normal results, while 21% performed abnormally. There is no relationship between sleep quality and short-term memory (p=0,739). Conclusion: sleep quality doesn’t influence memory function among medical students at the University of Mataram.

Abstract

Myasthenia gravis is one of the most common autoimmune diseases at the neuromuscular junction. Although this disease is rare, the incidence of this disease is believed to have increased in the last seven decades. This community service activity aims to provide education to the public regarding myasthenia gravis. This community service activity was carried out in the form of counseling at the Neurology Polyclinic of UNRAM Hospital on Friday, January 20 2023. All participants were asked to take part in all counseling activities which consisted of pre-test activities, delivery of material on the topic of myasthenia gravis, and post-test. The enthusiasm of the participants in this activity was presented in the form of photo documentation. Statistical analysis using paired t-test was carried out to analyze the difference between the pre-test and post-test mean values ​​and the results were considered significant if a p value <0.05 was obtained. As many as 23 participants who attended the Neurology Polyclinic at UNRAM Hospital enthusiastically participated in the whole series of activities. The average participant post-test score was significantly higher than the pre-test means score (p = 0.025). This activity was able to increase the participants' knowledge about myasthenia gravis.

Abstract

Cognitive impairment is one of the important complications of stroke with a high prevalence. If not detected and managed properly, this cognitive disorder can worsen into post-stroke severe cognitive impairment (vascular dementia) with social and economic impacts on the family. This community service activity aims to increase public understanding of the importance of recognizing memory loss in stroke patients. This activity is carried out in the form of counseling at the Neurology Polyclinic at UNRAM Hospital once a week for a period of 3 weeks involving visitors to the Neurology Polyclinic at UNRAM Hospital, both outpatients and their caregivers. The participants were asked to take part in a whole series of activities which included pre-test, counseling, discussion, and post-test. The number of participants who took part in this counseling activity were 36 subjects. The participants' relatively high interest in participating in this activity can be seen from the documentation of the entire series of activities and the significant increase in the post-test mean score compared to the pre-test average score at the statistical analysis stage (p <0.05). This activity showed high effectiveness in increasing participants' understanding of memory loss in stroke patients.
