Abstract

Latar Belakang: Penyakit Arteri Perifer (PAP) adalah tersumbatnya pembuluh darah perifer sebagian atau total akibat oklusi aterosklerosis terutama pada ekstremitas bawah. Hiperhomosisteinemia merupakan salah satu tanda penyakit kardiovaskular (PKV) yakni menunjukkan proses terbentuknya aterosklerosis. Penelitian sebelumnya telah melaporkan bahwa terdapat peningkatan level homosistein pada pasien dengan Diabetes Mellitus (DM). Namun, data mengenai level homosistein pada penderita PAP dengan DM tipe 2 terbatas. Oleh karena itu, penelitian ini bertujuan untuk mencari hubungan antara DM dengan level homosistein pada pasien PAP di Kota Mataram. Metode: Penelitian ini merupakan penelitian potong lintang yang dilakukan di Rumah Sakit Islam Siti Hajar Kota Mataram. Sampel penelitian adalah subjek dengan PAP dengan nilai Ankle Brachial Index (ABI) <0,90 dan DM tipe 2. Hasil penelitian ini adalah level homosistein pada pasien DM dengan PAP dan non DM dengan PAP. Analisis data menggunakan uji T tidak berpasangan. Hasil: Lima puluh dua pasien PAP berpatisipasi dalam penelitian ini, dengan 39 sampel (75%) berjenis kelamin wanita dan rata-rata usia 57,4 tahun. Rerata level homosistein pada pasien DM tipe 2 dengan PAP (5,7±0,9 µmol/L) lebih tinggi secara signifikan dibandingkan level homosistein pada pasien non DM tipe 2 dengan PAP (4,8±1,0 µmol/L) (p=0,001). Kami menemukan bahwa riwayat hipertensi (p=0,012) dan nilai ankle brachial index (p=0,0049) berhubungan dengan DM dan PAP. Jenis kelamin, usia dan merokok tidak berhubungan dengann DM dan PAP. Kesimpulan: Terdapat perbedaan yang signifikan antara rerata level homosistein pada kelompok subjek PAP dengan DM dan non DM. Kata Kunci: Penyakit Arteri Perifer, Diabetes Mellitus Tipe 2, Level Homosistein.

Abstract

BACKGROUND: In ST-segment elevation myocardial infarction (STEMI) management, some patients undergo spontaneous coronary reperfusion (SCR) with a better prognosis than those without SCR, but predictors for SCR remain unclear.OBJECTIVE: To investigate several potential predictors of SCR, including smoking status, BMI, DAPT loading time, hemoglobin levels, platelet count, random blood sugar levels, uric acid levels, creatinine clearance, and the Syntax score.METHODS: This case-control study was conducted at the West Nusa Tenggara General Hospital in Indonesia from December 2022 to September 2023. Data collection encompassed various patient demographics and clinical parameters, including name, medical record number, age, gender, smoking status, BMI, DAPT loading time, hemoglobin levels, platelet count, random blood sugar, uric acid, creatinine clearance, the Syntax score, and the occurrence of SCR. Statistical analysis for this study involved multivariate logistic regression analysis.RESULTS: A total of thirty-two patients was included, evenly divided into 16 subjects allocated to the SCR group and 16 to the non-SCR group. The analysis indicated that only BMI demonstrated a statistically significant association with SCR occurrence. However, the study did not yield conclusive evidence regarding the influence of smoking status, DAPT loading time, hemoglobin levels, platelet count, random blood sugar levels, uric acid levels, creatinine clearance, and the Syntax score on the likelihood of SCR.CONCLUSION: A normal BMI is identified as a robust predictor for the incidence of SCR in patients diagnosed with STEMI.

Abstract

Decompression sickness is considered as an important complication of deep diving. Since the trend of diving activities is increasing throughout the world and is dominated by novice divers, the incidence of decompression sickness should be high. Severe form of decompression sickness can cause damage to the central nervous system with varying manifestations of mild to severe neurological deficits. Recent studies show that sleep disorders need to be considered as an important consequence of decompression sickness. However, the pathophysiology of decompression sickness-associated sleep disorders is currently not well understood. Since sleep disorders are associated with dysfunction of anatomical structures and the neurotransmitter systems in the brain that regulate the sleep-wakefulness cycle, decompression sickness-associated sleep disorders can be postulated to be related to dysfunction of these two regulators of sleep-wakefulness cycle. Sleep disorders found in patients with decompression sickness are more likely to occur in type 2 decompression sickness and may be found in the form of insomnia, hypersomnia, circadian rhythm disorders, obstructive sleep apnea, and narcolepsy. This review aims to discuss the pathophysiology of sleep disorders and decompression sickness as well as possible mechanisms underlying sleep disorders in decompression sickness through theoretical approaches and the results of available studies. In conclusion, decompression sickness-related sleep disorders are thought to involve two pathological conditions in the brain, namely structural lesions and dysfunction of the neurotransmitter system in the brain.

Abstract

BACKGROUND: Trans-radial access has become the new standard and is increasingly in demand in coronary angiography. However, this method can fail due to tortuosity of the right subclavian artery. OBJECTIVES: This study aims to identify the predictors of right subclavian artery tortuosity. METHODS: This retrospective case-control study was conducted at two of the largest hospitals in West Nusa Tenggara, Indonesia. All research data were obtained from secondary sources, including video recordings of coronary angiographies, angiography reports, and patient medical records. Predictors included age, gender, smoking, height, body mass index, hypertension, diabetes mellitus, systolic and diastolic blood pressure, and random blood sugar. RESULTS: The study involved 80 subjects, divided into right subclavian artery tortuosity and normal group, with 40 subjects each. The prevalence of tortuosity was 8.1%, while the success rate of trans-radial access was 70.0%. Four significant predictors were identified: smoking (adjusted OR 0.26; p = 0.019), hypertension (adjusted OR 4.83; p = 0.020), diastolic blood pressure (adjusted OR 1.05; p = 0.044), and body mass index (adjusted OR 1.16; p = 0.035). The optimal cutoff points were determined to be a body mass index of ≥ 24.4 (sensitivity 60.0%; specificity 60.0%; p = 0.006; AUC = 0.665) and diastolic blood pressure of ≥ 78.5 mmHg (sensitivity 67.5%; specificity 67.5%; p = 0.002; AUC = 0.685). CONCLUSION: This study has identified smoking, hypertension, diastolic blood pressure, and body mass index as significant predictors of right subclavian artery tortuosity in trans-radial coronary angiography access.

Abstract

Depresi merupakan salah satu komplikasi penting dari stroke iskemik. Prevalensi depresi pada pasien stroke secara global cukup tinggi, yaitu sekitar 55%. Pengabaian kondisi depresi pada pasien stroke akibat rendahnya pemahaman mereka mengenai pentingnya penanganan depresi dan faktor-faktor risikonya akan menyebabkan terjadinya penurunan kualitas hidup pasiennya. Oleh karena itu, pengabdian masyarakat yang ditujukan untuk deteksi dini depresi pasca stroke dan edukasi terkait pentingnya penatalaksanaan depresi dan faktor-faktor risiko penyertanya pada pasien stroke iskemik penting untuk dilakukan. Sebanyak 22 pasien stroke iskemik dengan waktu awitan stroke 3 bulan pertama yang datang ke Poli Saraf Rumah Sakit Umum Daerah Provinsi Nusa Tenggara Barat selama Bulan Agustus 2019 sampai dengan Maret 2020 turut berpartisipasi sebagai subjek dalam kegiatan ini. Frekuensi depresi pada subjek dengan stroke iskemik yang berpartisiapasi dalam kegiatan pengabdian ini cukup tinggi (45%). Sebagian besar subjek memiliki rentang usia produktif, berjenis kelamin laki-laki, memiliki tingkat pendidikan rendah, memiliki lokasi infark pada hemisfer kiri, dan memiliki hipertensi. Seluruh karakteristik demografik dan klinik yang teridentifikasi tidak berhubungan secara signifikan dengan frekuensi kejadian depresi pasca stroke (p<0.05). Seluruh subjek dan anggota keluarga yang mendampinginya menunjukkan antusiasme yang tinggi dalam kegiatan ini. Kegiatan deteksi dini depresi ini sangat bermanfaat bagi subjek dan edukasi terkait pentingnya penatalaksanaan depresi dan faktor-faktor risikonya ini bermanfaat dalam meningkatkan pengetahuan mereka terkait tindak lanjut yang harus dilakukan untuk mencegah atau mengatasi depresi pasca stroke.

Abstract

Myasthenia gravis is one of the most common autoimmune diseases at the neuromuscular junction. Although this disease is rare, the incidence of this disease is believed to have increased in the last seven decades. This community service activity aims to provide education to the public regarding myasthenia gravis. This community service activity was carried out in the form of counseling at the Neurology Polyclinic of UNRAM Hospital on Friday, January 20 2023. All participants were asked to take part in all counseling activities which consisted of pre-test activities, delivery of material on the topic of myasthenia gravis, and post-test. The enthusiasm of the participants in this activity was presented in the form of photo documentation. Statistical analysis using paired t-test was carried out to analyze the difference between the pre-test and post-test mean values ​​and the results were considered significant if a p value <0.05 was obtained. As many as 23 participants who attended the Neurology Polyclinic at UNRAM Hospital enthusiastically participated in the whole series of activities. The average participant post-test score was significantly higher than the pre-test means score (p = 0.025). This activity was able to increase the participants' knowledge about myasthenia gravis.

Abstract

Cognitive impairment is one of the important complications of stroke with a high prevalence. If not detected and managed properly, this cognitive disorder can worsen into post-stroke severe cognitive impairment (vascular dementia) with social and economic impacts on the family. This community service activity aims to increase public understanding of the importance of recognizing memory loss in stroke patients. This activity is carried out in the form of counseling at the Neurology Polyclinic at UNRAM Hospital once a week for a period of 3 weeks involving visitors to the Neurology Polyclinic at UNRAM Hospital, both outpatients and their caregivers. The participants were asked to take part in a whole series of activities which included pre-test, counseling, discussion, and post-test. The number of participants who took part in this counseling activity were 36 subjects. The participants' relatively high interest in participating in this activity can be seen from the documentation of the entire series of activities and the significant increase in the post-test mean score compared to the pre-test average score at the statistical analysis stage (p <0.05). This activity showed high effectiveness in increasing participants' understanding of memory loss in stroke patients.

Abstract

Background: Cardiovascular disease (CVD) is the leading cause of death, which affects 17.9 million people every year. This study aimed to determine trends in CVD distribution and case fatality rate (CFR) at the West Nusa Tenggara Province General Hospital, the major referral hospital in West Nusa Tenggara Province of Indonesia Methods: This study was descriptive with a retrospective approach using medical records of patients with CVD at the West Nusa Tenggara General Hospital from January 2014 to December 2022. Consecutive sampling was performed. Data based on diagnosis, patient age, gender, and case fatality rates were analyzed descriptively and expressed in frequency and percentage distribution. Results: Of the 11,742 patients included, more than half of the patients (58.92%) were male. In this study, the most common CVD was ischemic heart disease (IHD) (44.23%, n=5,194), followed by patients with hypertension (24.55%, n=2,883), and heart failure (19.30%, n=2,265) which were dominated by males, 68.34%, 57.25%, and 50.53%, respectively. Females were lower in those CVD due to estrogen hormone which has role as cardio protector that maintains the elasticity of blood vessels and prevents atherosclerosis. Meanwhile, mitral valve disease is the fourth most common CVD, approximately 6.69%, followed by atrial fibrillation or atrial flutter at 5.97%, which were dominated by females, 57.78% and 54.17%, respectively. Our data showed that the highest CFR was in patients with atrial fibrillation or atrial flutter, which was 17.5% in 2022. Conclusion. IHD is the most commonly found in the majority of patients with CVD. The highest case fatality rate is in atrial fibrillation or atrial flutter. CVD prevalence, or CFR, is essential to focus on treatment and prevention strategies.
