Abstract

The importance of the completeness of filling in the Initial Medical Assessment is one indicator of the quality of information in the medical record file, which according to Kepmenkes 129 / Menkes / SK / II / 2008 states that the completeness of filling in medical record files must be filled in 24 hours after completion of medical services in the hospital is 100%. Based on the results of preliminary studies conducted, the authors took a sample of medical record files of surgical patients in February as many as 30 files, out of 30 files there were 20 files (66.66%) of the patient's initial medical assessment form that had not been completely filled in. The purpose of this study was to determine the completeness of filling out the initial medical assessment of surgical patient hospitalization. The research method used is descriptive method with quantitative approach. The population in this study was the initial medical assessment form for inpatient surgical patients in November-January 2023 at Al Ihsan Hospital, West Java Province, totaling 209 files. The sample taken from the population was 68 samples. The data collection techniques used were observation, interview, and documentation. The results of the research conducted by the author, showed that of the 68 samples of initial medical assessment sheets in November-January 2023, namely for the completeness of filling out the initial medical assessment in the patient identification variable of 50 files (74%). The variable of filling in reports / important notes amounted to 35 files (51%). Author authentication variable of 55 files (81%). The variable of good and correct recording was 67 files (98%) and the average completeness of filling out the initial medical assessment form for inpatient surgical patients in November-January 2023 was 76%.

Abstract

This study is entitled Review of Medical Information Release Services and Filling in Raharja Service Claim Files for Traffic Accident Victims at Al Ihsan Hospital, Bandung Regency, West Java Province. The purpose of this study is to determine the Medical Information Release Service and Filling in Jasa Raharja Claim Files for Traffic Accident Victims at Al Ihsan Hospital, Bandung Regency, West Java Province. The method used in this study is: using a descriptive method with a qualitative approach. The subject of this study is the Medical Information Release Service and Filling in the Raharja Service Claim File at Al-Ihsan Hospital, Bandung Regency, West Java Province. The object of this study is the standard operating procedure for releasing medical information and Jasa Raharja claim forms for traffic accident victims who claim Jasa Raharja insurance at RSUD AlIhsan Bandung Regency. Data collection techniques by observation, interview and documentation. Research results: medical information release services at Al-Ihsan Hospital are in accordance with SOPs. There is no written procedure for filling Jasa Raharja's claim file but the implementation is in accordance with the SOP.

Abstract

Berdasarkan studi pendahuluan yang dilakukan oleh penulis pada bulan Februari 2021, diambil dari sampel bulan Januari 2020 sebanyak 3.729 pasien dirawat inap dengan ketidaklengkapan kode diagnosa sebanyak 67 berkas dengan persentase 1,8%, ketidakakuratan kode diagnosa sebanyak 10 berkas dengan persentase 0,3%, dan ketidaktepatan waktu pelaporan sebanyak 67 berkas dengan persentase 1,8%. Tujuan penelitian ini adalah untuk mengetahui kelengkapan kode diagnosa pada jurnal pasien rawat inap tahun 2020 guna menunjang pelaporan rl 4a di Rumah Sakit TK II Dustira. Metode penelitian yang digunakan penulis adalah metode deskriptif dengan pendekatan kuantitatif. Hasil penelitian penulis dari sampel bulan Januari s/d Desember 2020 dengan total 23.006 sampel pasien rawat inap dengan jumlah hari perawatan sebanyak 89.800 hari. Dengan kelengkapan kode diagnosa pasien rawat inap yang terisi lengkap sebanyak 98%, keakuratan kode diagnosa pasien rawat inap sebanyak 99,85%, dan ketepatan waktu pelaporan pasien rawat inap sebanyak 98%, sehingga ketidaklengkapan kode diagnosa mempengaruhi ketepatan waktu pelaporan rl 4a pasien rawat inap tahun 2020. Kesimpulan dari penulis yaitu ketidaklengkapan kode diagnosa, ketidakakuratan kode diagnosa mempengaruhi ketepatan waktu pelaporan rl 4a pasein rawat inap tahun 2020, dimana masih terdapat ketidaklengkapan kode diagnosa mencapai diangka 500 berkas rekam medis. Abstract

The results of a preliminary study at the Bandung Regional General Hospital Kiwari in the March – December 2022 Service found 78 duplicated medical records unsustainable medical record Data and inefficient resources are prone to duplication of medical records so that it can hinder the service process in the filling section. The purposeof this study is to determine the factors that cause duplication of medical records in order to support the quality of Service in the filling of hospitals in Bandung Kiwari.The research method used is a descriptive method with a qualitative approach. Data collection techniques with observation, literature studies and documentation. Instruments in the study using interview guidelines, recording equipment, stationery, Cheklist sheet.The results ofthe implementation of medical record storage using centralization and numbering of medical records using the unit system (unit numbering system) but for the implementation process is not good enough. Factors causing duplication of medical records in hospitals in Bandung Kiwari is the lack of communication between officers, limited inventory of medical records document folder and medical records of patients who were not found. The effort made by the hospital is to increase the accuracy of the officers through the training of checking back on the expedition SIMRS, and if the medical record is not found then a temporary file is made. It is recommended that communication be improved for coordination and improving the knowledge of employees whose qualifications are not suitable through formal and non-formal education.

Abstract

The results of the Preliminary Study when searching for medical record documents there were missfile incidents of 10 medical record documents, with 4 medical record files in March, 3 medical record files in April, and 3 medical record files in May. This is because the storage room at Sulaiman Air Force Air Force Base is still a mess The research method used is descriptive method with a qualitative approach. The subjects in this study were medical record officers, the object of this study was the procedure for using medical record file tracers. Data collection techniques used are observation, interviews, and documentation. The results of the research conducted by the author, show that the data for taking medical record files in March were 150 medical record files, the occurrence of missfiles was 4 medical record files due to an error in the sequence of numbers and file storage errors, in April there were 120 medical record files, 3 medical record files due to errors in the order of numbers, reading errors, and mistaking numbers, while in May there were 100 medical record files with missifles as many as 3 medical record files due to sequence errors and file storage errors. The tracer used is 10.5 x 32 cm in size, yellow and orange. The constraints on using a tracer to prevent missfile incidents are the time factor, the accuracy factor, the expedition book and the storage rack. Efforts were made to overcome the problem of using a tracer in preventing missfile incidents, namely if the medical record file is not found, the storage officer will create a medical record file to replace the missing medical record file.

Abstract

The increase in BPJS patients at Bandung Kiwari Hospital is something that needs to be considered in their services, the inhibiting factors are employees who still cannot correct computer system errors, officers still rely on IT employees to justify them, so the registration service process becomes hampered and late due to waiting for the team Different IT floors, apart from that there are still employees who are lacking in conveying information to patients. The purpose of this study was to find out how the flow of procedures for outpatient registration services for BPJS users in order to improve the quality of health services at Bandung Kiwari Hospital. The research method used is descriptive method with a qualitative approach with the subject, namely the registration officer, and the object, namely the BPJS registration flow. Data collection techniques are by interview, observation and documentation study. The research instrument uses stationery, stop watch, and interview guidelines. Based on the Hospital SPO for outpatient registration of BPJS participants, it is explained that employees must greet, but employees are still normal to receive patients, and officers are still not reliable enough to correct computer system errors, besides that hospitals are lacking in conveying information to patients so there are still patients lack information for BPJS registration which must register with predetermined conditions. The author suggests that the Hospital provide facilities that new patients can see regarding the requirements for registering to use BPJS and provide motivation to employees regarding empathy for patients.

Abstract

Health services as a sub-component of the national health system that have direct contact with the community. In order to realize good and quality health services, the government is responsible for the implementation of public health insurance through the National Health Insurance through claims pending or delayed claims from the BPJS to the hospital. In July to September there were pending claims of 68 out of 1912 files resulting in delays in payment of Rp. 83,561,200. The purpose of this study is to find out the pending BPJS claims to support the smooth running of JKN claims at the Sulaiman Air Force Hospital in Bandung. The research method used is descriptive method with a quantitative approach. Data collection techniques used are observation, interviews and documentation studies. The population in this study is the pending BPJS claim files for 2021, with a sample of 211 outpatient pending files and 80 inpatient files. The sampling technique is saturated sampling. The results of the author's research, there is no written SOP regarding the implementation of claims at the hospital. The sample of pending BPJS claim data on outpatient care was 211 (3%) from 7016 files and inpatient care was 80 (10.16%) from 787 files. The occurrence of pending claims will affect the smoothness of the conformity of the payment amount submitted to the JKN. The average suitability of payment of outpatient claims was 96.8% and 89.84% for inpatient care. The cause of pending is due to coding, medical, and administrative factors. The author's conclusions and suggestions are that there is no written SOP regarding the implementation of claims, it is better to make a procedure for implementing claims in writing. Pending claims occur due to incomplete claim requirements so that they can hinder the smooth payment of claims submitted. It is better to conduct socialization and regular meetings to all relevant parties regarding claim procedures and claim settlement.

Abstract

Hasil observasi di bagian pendaftaran RSUD Bandung Kiwari, terdapat petugas merangkap pekerjaan di luar tugas pendaftaran, dan karyawan pada shift siang tidak cukup melakukan tugas pendaftaran ketika volume pekerjaan banyak kondisi tersebut menyebabkan masalah ketidakpuasan kerja yang berefek kepada kinerja karyawan. Tujuan penelitian ini untuk mengetahui Hubungan Kepuasan Kerja dengan Kinerja Karyawan Pendaftaran di Bagian Pendaftaran Rawat Jalan RSUD Bandung Kiwari. Metode penelitian yang digunakan adalah metode mixed methods dengan pendekatan kuantitatif dan kualitatif. Teknik pengumpulan data dengan observasi, wawancara, kuesioner, studi kepustakaan. Teknik pengolahan data yaitu editing, coding, prosessing, cleaning. Sampel dalam penelitian ini berjumlah 19 orang responden petugas rekam medis di bagian pendaftaran rawat jalan. Data dianalisis secara univariat dan bivariat menggunakan analisis chi square. Proses perhitungan menggunakan program IBM SPSS Statistics 29. Hasil penelitian berdasarkan uji chi square menunjukkan bahwa terdapat hubungan yang signifikan antara kepuasan kerja dengan kinerja karyawan pendaftaran rawat jalan. Berdasarkan hasil uji chi square di peroleh nilai p=0,000 (p value <0,05). Ini termasuk hubungan yang memadai dengan arah positif yang bahwasanya semakin banyak kebahagiaan kerja, semakin tinggi kinerjanya. Penulis menyarankan pihak Rumah Sakit bisa mempertahankan kepuasan kerja serta kinerja petugas rekam medis, melengkapi sarana dan prasarana seperti menambahkan loket dan petugas pendaftaran rawat jalan pada shift pagi.
