Abstract

Background: Puskesmas (Primary Health Care) is the entry point or basic services accessible to all levels of society at the village level. However, the utilization of health services in Indonesia, including Madura, remains low. Objectives: To analyze community perceptions of the quality of health services and their influence on the utilization of health services at Puskesmas Jaddih, Bangkalan Regency, Madura. Methods: This was an explanatory research with a cross-sectional design. The population included all residents in the working area of Puskesmas Jaddih, with an average of 500 patient visits per month. A total of 149 patients who used health services at the Puskesmas Jaddih and met the inclusion criteria participated in the study. Data were analyzed using descriptive statistics and the Spearman test with a significance level of p<0.05. This study was conducted from January to December 2024. Results: Most patients had a primary education. More than half of the patients were not working. The majority rated the competence of health workers as very good. In terms of affordability, effectiveness, continuity, safety, convenience, and information, most rated it as excellent or good. In the SERVQUAL assessment, it was found that almost half of the total patients rated it as excellent (45.3%), just under half of the total patients rated it as good (44.1%), and a small proportion reported it as poor (10.6%). Most patients reported health care utilization in the order category (73.2%). A small proportion reported repeat orders (8.9%) and recommendations (17.9%). The results of statistical analysis showed that competence, affordability, effectiveness, continuity, safety, comfort, and information were related to health care utilization with p<0.05. Overall, in the SERVQUAL assessment, it was found that the results of SERVQUAL analysis with health care utilization showed p<0.05. Conclusion: The quality of health services at Puskesmas Jaddih is perceived by the community, effectiveness, and convenience. However, service utilization remains limited to initial use, with low levels of repeat visits and recommendations. All quality dimensions showed a significant relationship with healthcare utilization, indicating that community perceptions of service quality strongly influence decisions to use, re-use, and recommend health services.

Abstract

ABSTRAK Tuberkulosis masih menjadi masalah kesehatan masyarakat yang serius dalam masalah nasional. Indonesia menempati peringkat kedua di dunia untuk kasus TB, sebagai respon terhadap epidemi TB, pemerintah mengeluarkan kebijakan strategi penanggulangan TB yang komprehensif salah satunya adalah penguatan komitmen di pemerintahan pada semua tingkatan. Mengoptimalkan peran masyarakat merupakan salah satu upaya pendukung dalam eliminasi TB. Kader merupakan barisan pertama yang bisa menjangkau masyarakat ketika masyarakat sakit dan akan mengakses layanan kesehatan sehingga peningkatan pengetahuan dan ketrampilan sangat diperlukan untuk menunjang peran kader dalam implementasi program TB. Tujuan kegiatan ini adalah meningkatkan pengetahuan kader kasehatan tentang TB dan penatalaksanaannya dan edukasi pemanfaatan media sosial TikTok bagi kader untuk penyebaran informasi kesehatan di Desa Kramat Bangkalan. Metode yang digunakan adalah memberikan edukasi tentang TB dan penatalaksanaanya selain itu kader juga diberikan edukasi terkait pemanfaatan media sosial bagi penyebaran informasi kesehatan dan bagaimana membuat video promosi kesehatan yang kreatif dan menarik. Hasil kegiatan menunjukkan bahwa terjadi peningkatan pengetahuan kader posyandu tentang TB sebesar 21,8% dan kemampuan kader dalam membuat video promosi kesehatan tentang TB yang diunggah di akun media sosial TikTok kader desa Kramat yang sudah banyak pengikut atau follower dan banyaknya kesukaan (like) dari postingan tersebut. Kegiatan pengabdian ini adalah bentuk nyata jika dengan peningkatan pengetahuan dan skill dapat diupayakan dengan pemberian edukasi dan pelatihan sesuai dengan tujuan yang akan dicapai.  Kata Kunci: Tuberkulosis, Media, Sosial, TikTok.  ABSTRACT Tuberculosis remains a serious public health problem in the national context. Indonesia ranks second in the world for TB cases. In response to the TB epidemic, the government has issued a comprehensive TB control strategy policy, one of which is strengthening commitment across all levels of government. Optimizing the role of the community is one of the supporting efforts in TB elimination. Cadres are the first line of defense who can reach the community when they are sick and will access health services, so increasing knowledge and skills is essential to support the role of cadres in implementing TB programs. The purpose of this activity is to increase the knowledge of health cadres about TB and its management, and educate cadres on the use of TikTok social media to disseminate health information in Kramat Bangkalan Village. The method used is to provide education about TB and its management. In addition, cadres are also given education regarding the use of social media for disseminating health information and how to create creative and engaging health promotion videos. The results of the activity showed a 21.8% increase in Posyandu cadres' knowledge about TB and their ability to create health promotion videos about TB, which were uploaded to the Kramat village cadre's TikTok social media account, which already has a large following and many likes. This community service activity is a concrete example of how increasing knowledge and skills can be achieved through education and training in accordance with the objectives to be achieved. Keywords: Tuberculosis, Media, Social, TikTok.
