Abstract

BACKGROUND: Sepsis in children is a significant cause of morbidity and mortality in the pediatric intensive care unit (PICU). Assessment of pediatric sepsis using serial Pediatric Logistic Organ Dysfunction (PELOD)-2 score can be used as a prognostic factor. The use of biomarkers of sepsis is also used for diagnosis and predicting outcomes. Many studies have suggested that C-reactive protein (CRP) and procalcitonin (PCT) can be used to predict mortality.METHODS: A prospective cohort study was conducted to evaluate CRP, PCT, PELOD-2 score and its combination as a predictive factor of mortality in sepsis. All patients admitted to PICU Haji Adam Malik General Hospital, Medan, from April to November 2019 with suspected sepsis were included in this study. Blood examination and PELOD-2 scores were examined in the first 24 hours.RESULTS: A total of 79 children were included with a mortality rate 55.7%. The CRP, PCT, and PELOD-2 score were higher in nonsurvivor (2.8 (0.5-22.4) mg/dL; 9.36 (0.13-79.8) ng/mL; 9 (3-21), respectively). In multivariate logistic regression analysis, neither CRP nor PCT values could be independent predictors of mortality. The PELOD-2 score can be an independent predictor for mortality at a cut-off score of 7 (OR: 3.47 (95% CI: 1.68-7.19)). The combination of PELOD-2 and CRP scores as predictors of mortality showed lower values than PELOD-2 and PCT scores (0.80 vs. 0.95). The combination of all parameters only adds 1% of the predicted mortality value.CONCLUSION: PELOD-2 score with PCT value are recommended to predict mortality children with sepsis.KEYWORDS: sepsis, mortality, C-reactive protein, procalcitonin, PELOD-2 score

Abstract

Climate change that contributes to the destruction of the earth is one of the impacts of human behavior. One example of damage to the earth due to human behavior is global warming. One of the effects of global warming is the melting of permafrost. Melting of glaciers due to global warming will result in an isostatic rebound phenomenon, where melting glaciers will cause the weight of the earth's crust to decrease and it is easy to move and bounce, resulting in the movement of earth faults and increased activity in the magma chamber (increased seismic activity). The big impact is the occurrence of natural disasters that not only occur in parts of the world where glaciers melt but in other parts of the world far from where glaciers melt. Natural disasters that can occur are earthquakes, volcanic eruptions, tsunamis, and landslides. Global warming will also have an impact on the emergence of disease outbreaks. Indonesia is the meeting place of three major tectonic plates in the world, namely the Eurasian plate, the Indo-Australian plate and the Pacific plate. In addition, Indonesia is also located in the Pacific ring of fire, making Indonesia an area with high volcanic activity and seismic activity. Climate change will increase the risk of disasters in Indonesia. The long experience of the Indonesian people in dealing with natural disasters that often occur should be used as a strategy to prevent and reduce the impact of disasters in the future.

Abstract

Luka tembak tembus kepala adalah luka otak disebabkan trauma fisik berupa penetrasi massa tajam yang menembus tengkorak dan jaringan otak. Luka tembak tembus memiliki luka masuk dan luka keluar. Dilaporkan kasus luka tembak tembus kepala. Penetrating gunshot wound was brain injury caused by projectiles in the form of sharp mass penetrations through the skull and brain tissue. Peneterating gunshot have an entrance wound and exit wound. This is a case report of cranial penetrating gunshot wounds.

Abstract

Climate change has a close relationship with the greenhouse effect. The greenhouse effect is needed by the earth, but if there are too many greenhouse gases in the atmosphere this will increase the earth's temperature. Children are a group of people who have a high vulnerability to climate change. Climate change will affect children's health through 4 impacts, namely direct impacts on children's health, impacts on children's health through ecosystems, impacts on children's health through human behavior, and health impacts on children due to natural disasters. To anticipate the impact of climate change on children's health, parents and the government must take anticipatory steps so that children can be saved from the extreme dangers of climate change.

Abstract

Diabetes mellitus is a chronic metabolic disease that leads to oral complications generally, including changes and damage to the gingiva due to increased inflammation. The incidence of periodontal disease is two to three times higher compared to individuals without diabetes, which exacerbates gingival tissue damage and slows down healing. Hyperglycemia triggers the formation of advanced glycation end-products (AGEs), which increase the production of pro-inflammatory cytokines. This article aims to identify changes in the gingiva associated with diabetes mellitus. The method used is a literature review of articles published online through databases such as Google Scholar and PubMed. The analyzed articles are publications from 2019 to 2024, with a total of 21 articles meeting the inclusion criteria and relevant to the research objectives. The study results indicate that the interaction between hyperglycemia causes changes in the gingiva of diabetic patients, increased oxidative stress, and dysfunction in the immune system, which worsens inflammation and gingival tissue damage. Factors such as increased free radicals, impaired tissue healing due to vascular dysfunction, and decreased collagen production also contribute to the exacerbation of tissue damage, thereby increasing the risk of periodontal disease in patients with diabetes mellitus. Gingivitis and periodontitis are two common forms of periodontal disease caused by pathogenic biofilms around the teeth.
