Abstract

Hospital malnutrition is known as a risk factor for mortality and morbidity in children, adolescents, and adults. Critically ill child more prone to suffer from malnutrition during treatment. This is because protein metabolism in critically ill children in different to the healthy children. Protein metabolism in critically ill children influenced by catabolic hormones triggered by stressful state resulting in increased protein breakdown. Therefore the protein requirement for critically ill children is different from healthy children. There are several methods of evaluation that can be used to evaluate the protein metabolism status, among others anthropometry, nitrogen balance, and tracer isotope. The importance of maintaining the balance of formation and protein breakdown must be one of the targets of nutritional therapy in the pediatric intensive care unit, thus improving treatment outcomes.Keyword : critically ill, protein metabolism, children

Abstract

World Health Organization (WHO) recommends to continue breastfeeding until two years of age. There are few studies assessing the correlation between breastfeeding duration and motor development. A cross-sectional study was conducted in April 2016 in Muara Batang Gadis District. The sample were children 3-6 years of age. Breastfeeding duration was obtained by interview, and motor development was assessed using Denver Developmental Screening Test II. Chi-square test was used to evaluate the correlation between breastfeeding duration and motor development, with P-value <0.05 and 95% Confidence Interval (CI). Among 204 children, 5.4% had the fine motor delay, and 1% had the gross motor delay. Children with breastfeeding duration for at least for two years had better fine motor development compared to breastfeeding duration for less than two years (P<0.001, PR=1.139, 95% CI=1.055-1.231) and there is no difference in gross motor development (P=0.193, PR=1.023, 95% CI=0.991-1.055). There is a correlation between breastfeeding duration and fine motor development.

Abstract

WHO recommends the supplementation of 200,000 IU vitamin A into 400 mg albendazole to cut down its reinfection. A Randomized Controlled Trial was conducted in SD N 384 Sikapas, North Sumatera from April - June 2016. Inclusion criteria were children with ascariasis. Sample divided into intervention and control group. Kato-Katz was used to identify egg worms and faecal egg count intensity (FECs). P value < 0.05 was consider significant. After three months follow up, there were 51 and 56 samples in intervention and control group with average age 9.6 years (SD 1.99) and 9.7 years (SD 2.20). Prevalence of ascariasis was 75.5%. There was no significant difference in Ascaris lumbricoides reinfection but instead of in FECs intensity. The NNT was 22.31. The addition of vitamin A in albendazole was not suggested according to this study.

Abstract

Luka tembak tembus kepala adalah luka otak disebabkan trauma fisik berupa penetrasi massa tajam yang menembus tengkorak dan jaringan otak. Luka tembak tembus memiliki luka masuk dan luka keluar. Dilaporkan kasus luka tembak tembus kepala. Penetrating gunshot wound was caused by projectiles. Peneterating gunshot have an entrance wounds and exit wounds. This is a case report of cranial penetrating gunshot wounds.

Abstract

Disomnia merupakan gangguan tidur yang sering dijumpai pada remaja, disebabkan oleh faktor internal dan eksternal, serta faktor medis dan nonmedis. Disomnia dapat berdampak buruk pada kesehatan remaja. Disomnia dapat didiagnosis secara objektif maupun subjektif. Salah satu komplikasi disomnia adalah peningkatan tekanan darah. Peningkatan tekanan darah pada remaja akan menyebabkan hipertensi saat dewasa serta berbagai masalah kardiovaskuler lainnya.Dyssomnia is a common sleep disturbance in adolescents; it is caused by internal and external factors along with medical and nonmedical factors. Dyssomnia may have negative impact on adolescent’s health. One of it’s complications is increased blood pressure. Increased blood pressure in adolescent will lead to hypertension in adult together with other cardiovascular problems.

Abstract

Abstrak. Sepsis adalah kondisi klinis disfungsi organ yang berpotensi mengancam nyawa. Skor Pediatric Logistic Organ Dysfunction 2 (PELOD-2) sering digunakan untuk memprediksi kematian pasien sepsis. Sebagian besar penelitian menemukan peningkatan nilai MPV pada pasien sepsis, baik neonatus maupun dewasa. Nilai MPV dapat dijadikan salah satu parameter untuk menilai prognosis pasien sepsis.Abstract. Sepsis is a clinical condition of organ dysfunction that potentially life-threatening. Pediatric Logistic Organ Dysfunction 2 (PELOD-2) is used to predict mortality in patients with sepsis. Many studies found increased MPV value in sepsis (neonates and adults). MPV can become a prognostic parameter in sepsis.

Abstract

Enuresis adalah keluarnya air kemih yang tidak disadari dan berulang pada anak berusia 5 tahun atau lebih yang diharapkan sudah dapat mengendalikan proses berkemihnya. Istilah enuresis primer dipakai untuk anak yang belum pernah berhenti mengompol sejak bayi. Salah satu penyebab enuresis primer adalah keterlambatan maturasi sistem saraf pusat. ASI merupakan nutrisi yang ideal untuk bagi perkembangan sistem saraf. Pemberian ASI eksklusif menunjukkan manfaat besar dalam perkembangan visual, kognitif, dan neurologis anak sehingga berperan dalam mencegah enuresis pada anak. Hal ini didukung oleh beberapa penelitian yang melaporkan hubungan antara pemberian ASI eksklusif dengan enuresis pada anak.Enuresis is repeated involuntary discharge of urine in children 5 years of age or older expected to be able to control the micturition. Primary enuresis is when children 5 years of age or older still bed-wetting. One of the causes of primary enuresis is delayed maturation of the central nervous system. Exclusive breastfeeding has beneficial effect in visual, cognitive, and neurological development in children; it may have a role in preventing enuresis. Several studies report the correlation between exclusive breastfeeding and enuresis prevention in children.

Abstract

BACKGROUND: Early detection and treatment of sepsis can prevent septic shock and reduce mortality rate. Troponin can become a prognostic factor in sepsis. However, not all health facilities are equipped to assess troponin levels. Vasoactive-inotropic score (VIS) is a simpler and more accessible method to describe hemodynamic status. The aim of this study was to assess the suitability of VIS score as early prognosis and mortality predictor of sepsisMETHODS: A retrospective study was conducted to determine the correlation between VIS and troponin levels for sepsis cases in Pediatric Intensive Care Unity (PICU) Haji Adam Malik Hospital, Medan from January 2018 to December 2019. VIS score at 48 hours, maximum VIS score, pediatric logistic organ dysfunction-2 (PELOD-2) score, cardiac troponin levels at 48 hours were taken from medical records.RESULTS: There were 54 samples analyzed. VIS scores were positively correlated (p<0.001) to troponin T and troponin I levels at 48 hours (r=0.670 and r=0.606, respectively). VIS at 48 hours and maximum VIS were related to mortality (p=0.001 and p<0.001, respectively). VIS score at 48 hours could be used as a predictive factor for mortality (area under the curve (AUC): 79.7%, p<0.001) with a cut-off point at 11 (74.4% sensitivity and 80% specificity). High VIS at 48 hours indicated poor outcomes of sepsis in children with odd ratio (OR) value: 1.99 (95% confidence interval (CI): 1.25-3.19).CONCLUSION: Vasoactive-inotropic score was suitable as an alternative to cardiac troponin T and troponin I levels at 48 hours to early detect myocardial dysfunctions and mortality in children.KEYWORDS: troponin, vasoactive-inotropic score, sepsis, children, mortality

Abstract

BACKGROUND: Sepsis in children is a significant cause of morbidity and mortality in the pediatric intensive care unit (PICU). Assessment of pediatric sepsis using serial Pediatric Logistic Organ Dysfunction (PELOD)-2 score can be used as a prognostic factor. The use of biomarkers of sepsis is also used for diagnosis and predicting outcomes. Many studies have suggested that C-reactive protein (CRP) and procalcitonin (PCT) can be used to predict mortality.METHODS: A prospective cohort study was conducted to evaluate CRP, PCT, PELOD-2 score and its combination as a predictive factor of mortality in sepsis. All patients admitted to PICU Haji Adam Malik General Hospital, Medan, from April to November 2019 with suspected sepsis were included in this study. Blood examination and PELOD-2 scores were examined in the first 24 hours.RESULTS: A total of 79 children were included with a mortality rate 55.7%. The CRP, PCT, and PELOD-2 score were higher in nonsurvivor (2.8 (0.5-22.4) mg/dL; 9.36 (0.13-79.8) ng/mL; 9 (3-21), respectively). In multivariate logistic regression analysis, neither CRP nor PCT values could be independent predictors of mortality. The PELOD-2 score can be an independent predictor for mortality at a cut-off score of 7 (OR: 3.47 (95% CI: 1.68-7.19)). The combination of PELOD-2 and CRP scores as predictors of mortality showed lower values than PELOD-2 and PCT scores (0.80 vs. 0.95). The combination of all parameters only adds 1% of the predicted mortality value.CONCLUSION: PELOD-2 score with PCT value are recommended to predict mortality children with sepsis.KEYWORDS: sepsis, mortality, C-reactive protein, procalcitonin, PELOD-2 score

Abstract

Latar belakang. Angka morbiditas dan mortalitas pada anak yang disebabkan penyakit kritis masih tinggi, terutama pada usia di bawah 5 tahun. Hingga saat ini belum ada data mengenai profil pasien anak dengan penyakit kritis yang dirawat di ruang rawat intensif anak RSUD Dr. Zainoel Abidin (RSUDZA), Banda Aceh.Tujuan. Untuk mengetahui bagaimana profil pasien anak dengan penyakit kritis yang dirawat di ruang rawat intensif anak RSUD Zainoel Abidin (RSUDZA), Banda Aceh pada tahun 2019.Metode. Penelitian deskriptif retrospektif di ruang rawat intensif anak RSUDZA, Banda Aceh. Data dari rekam medis usia 1 bulan - 18 tahun sejak Januari 2019 sampai Desember 2019.Hasil. Diperoleh data pasien anak dengan sakit kritis yang dirawat di ruang intensif anak RSUDZA berjumlah 316 subjek. Mayoritas usia di bawah 5 tahun. Penyakit utama terbanyak adalah disfungsi organ respirasi (28,5 %). Jumlah Skor PELOD-2 terbanyak pada studi ini < 7 (69,6%) dengan lama rawat 2-7 hari (64,9%) dan angka mortalitas 21,8%.Kesimpulan. Profil penyakit kritis pada anak di RSUDZA Banda Aceh Tahun 2019 menunjukkan mayoritas subjek berusia di bawah 5 tahun, skor PELOD-2 < 7 dengan disfungsi organ terbanyak adalah respirasi.
