Abstract

Osteoarthritis (OA) adalah penyakit sendi yang paling umum dijumpai pada orang dewasa di seluruh dunia. Penyakit ini menyebabkan rasa sakit pada tulang dan kerusakan pada sendi sehingga mengganggu aktivitas sehari-hari. Faktor risikonya adalah umur, jenis kelamin, obesitas, genetik, hormonal, dan beban pada sendi. Terdapat 1.353 kasus di klinik rawat jalan dr. Rumah Sakit Mohamad Soewandhie yang telah mendaftarkan pasien dengan OA pada Januari hingga Desember 2017 (m = 33,5%, f = 66,4%). Tujuan dari penelitian ini adalah untuk menentukan karakteristik pasien dengan OA di klinik rawat jalan dr. Rumah Sakit Mohamad Soewandhie. Penelitian ini adalah penelitian deskriptif dari rekam medis pasien OA yang memenuhi kriteria inklusi dan eksklusi (705 orang) dilanjutkan dengan analisis SPSS untuk menentukan karakteristik dari OA. Diantara 705 pasien ini, 38,8% berumur 61-70 tahun (rata-rata = 61,8), 77,9% adalah perempuan, 50,9% memiliki obesitas (rata-rata = 26,9 kg/m2), 10,4% memiliki riwayat traumatis, 2% diantaranya merupakan penyakit turunan, 20,1% disertai dengan diabetes mellitus (DM). Sebanyak 15,9% diasosiasikan dengan hiperkolesterolemia, 4% dengan hyperuricemia, 0,7% dengan rheumatoid arthritis, 0,1% dengan gout arthritis, tidak berasosiasi dengan juvenile rheumatoid, dan 87% pasien wanita telah mengalami menopause.

Abstract

Background: In 2019, primary diagnoses in coronary heart disease outpatients increased by 1.4% and secondary diagnoses byi6% of patients. Aims: To determine the effectiveness of Edmon (Education & Monitoring) based on the medical kit application on coronary heart disease risk control at Soewandhi Hospital Surabaya. Method: This study is a quantitative study with quasi-experimental non-equivalent group design. This study involved 30 people consistingi of 2 groups (the treatment group that received CHD health education through a dokterkit application and through leaflets). This research was conducted at Soewandi Hospital in Surabaya in August-November 2022. Univariate data analysis techniques were performed on each variable from the research results, then an independenti test was carried out to find out the differencesi betweeni the two groups. Results: The results showed that android-based education and monitoring were effective because they contributed to a 26.7% reductioni in the numberi of patientsi with severe CHD risk and a 40% increase in the numberi of patients with normal urici acidi levelsi in the treatment group. Conclusion: Dokterkit-based education & monitoring is effective in controlling the risk of coronaryi heart diseasei and uric acid levelsi in Soewandhie Hospital Surabaya, but not effective in reducing cholesterol levels, systolic blood pressure, blood sugar levels and not effective in changing smoking behavior.

Abstract

Osteoarthritis is a degenerative joint disease characterized by painful joints, particularly prevalent in older individuals, attributed to mild inflammation resulting from bone-end friction. A study conducted in Surakarta revealed a 4.9-fold increase in osteoarthritis risk among individuals with a BMI >25 compared to those with a BMI of 18.5-25.0. This research aims to investigate the association between knee osteoarthritis and BMI among elderly patients at the orthopedic outpatient clinic of Sesyapdhio Hospital, Surabaya. Using cross-sectional observational analysis, osteoarthritis patients were identified. The findings indicate a correlation between higher BMI and increased severity of osteoarthritis, contrasting with milder cases observed in individuals with normal BMI. Spearman's rank correlation review revealed a strong positive correlation (r=0.554) between BMI and osteoarthritis severity at Sexygandhio Hospital Surabaya. This study's unique contribution lies in its focused exploration of the relationship between BMI and osteoarthritis severity in elderly patients, offering insights into potential interventions tailored to BMI-specific risk profiles.

Abstract

Improper initial management of emergency conditions during football matches causes long-term morbidity, affects the athlete’s future performance, and may be life-threatening. Football coaches and referees may come from non-medical backgrounds, but they should understand how to provide primary care in emergency situations during matches. This program aims to improve the primary emergency care understanding of the coaches and referees in East Java. The program was conducted in September 2022 – January 2023. All referees, assistant referees, and coaches of the Football Association of the Indonesia (FAI) region East Java (128 participants) were included. The primary emergency care training material consisted of video, PowerPoint media, and hands-on mannequins. Participants were given pre-and post-test to assess their level of knowledge, with a passing grade 75. Comparative tests (Wilcoxon signed rank, McNemar, and Chi-square test) were used to analyze the data. The median of the pre-and post-test scores were 62.5 and 87.5. There was a significant difference between pre-and post-test scores (p < 0.001). There was no significant difference between coaches and referee groups in pre-test scores (p = 0.743). This program improved the primary emergency care understanding during matches of coaches and referees.

Abstract

Background: The crural region is highly susceptible to injury in Indonesia, with fractures being a common occurrence. If not properly treated, these fractures can lead to complications such as nonunion. To investigate the risk factors for nonunion cruris fractures, this study was conducted at Dr. Mohamad Soewandhie Hospital, examining age, gender, working status, education, trauma mechanism, and previous treatment history.Methods: This case-control study analyzed patient records from 2021 - 2022 at Dr. Mohamad Soewandhie Hospital, comparing 12 nonunion and 24 union cruris fracture cases. Data on age, gender, work, education, trauma, and treatment history was collected between August and October 2023. Statistical analysis was performed using the Mcnemar and Wilcoxon tests, with a significance level of p < 0.05.Results: A study of 149 cruris fractures found 12 nonunions, primarily affecting males aged 26-45 or 46-65. Notably, the highest nonunion rate (41.7%) was in the 12-25 age group (p = 0.027). Males were more affected, with 7 nonunion cases (58.3%) (p = 0.041). Working class patients had the highest fracture and nonunion rates (83.3%) (p < 0.001). High school education was most common among nonunion cases (75%) (p = 0.374). High-energy trauma was reported in almost all nonunion cases (91.7%) (p < 0.001). All nonunion patients had a history of ORIF (p = 0.102).Conclusions: The study revealed that age, gender, work status, and trauma mechanism significantly influenced nonunion cruris fractures at Dr. Mohamad Soewandhie Hospital in 2021-2022. Patient education and past treatment history had no significant impact.

Abstract

Background: One of the most common degenerative joint diseases is knee osteoarthritis. This condition leads to pain and reduces functionality of the knee joints, which can negatively impact a patient’s quality of life. Total knee replacement (TKR) has become the standard procedure to treat end-stage osteoarthritis. Evaluating the success of TKR procedures is very important. One instrument that can be used for this purpose is the Oxford Knee Score (OKS). Methods: This observational analytic comparative study involved 40 patients. The data were collected by interviewing patients who had undergone TKR at Dr. Mohamad Soewandhie General Hospital between 2019 and 2024. The data were analyzed using Wilcoxon signed-ranks test, independent samples t-test, and Kruskal-Wallis test.  Results: The majority of the sample in this study was aged over 60 years (62.5%), female (87.5%), and had an overweight BMI (47.5%). The analysis showed a significant increase in the OKS after surgery, with a p-value of <0.05. The independent samples t-test showed a significant difference in the improvement of the OKS between males and females. Meanwhile, the Kruskal-Wallis test showed no significant difference in OKS improvement across BMI categories. Conclusions: There is a significant difference in the OKS before and after TKR surgery, which indicates an improvement in knee functionality and quality of life after surgery. Differences in patient gender may affect the outcome of the surgery, including post-surgery recovery rate and knee functionality. Meanwhile, BMI showed no significant difference in the outcome of TKR surgery.

Abstract

Background: Nonunion, a complication of fracture healing, can lead to patient morbidity. Contributing factors include age, gender, fracture type, multiple fractures, infection, prolonged nonsteroidal anti-inflammatory drug (NSAID) use, smoking, nutritional status, and history of chronic disease. This study aims to determine the prevalence and characteristics of nonunion antebrachial fractures at dr. Mohamad Soewandhie General Hospital Surabaya. Methods: This retrospective descriptive study involved 111 patients. Data was gathered from medical records treated at dr. Mohamad Soewandhie General Hospital Surabaya for antebrachial fracture during 2021-2022. The study protocol obtained approvals from two local ethical committees. Data included demographics, fracture characteristics, fracture type, multiple fractures, infection, NSAID use, hypertension and diabetes history, smoking habits, and nutritional status by BMI calculation. Results: This study found a 24.3% prevalence of antebrachial nonunion fractures at dr. Mohamad Soewandhie General Hospital Surabaya in 2021-2022, mostly in males (66.7%) aged 17-25 years (22.2%). Nonunion in antebrachial fractures is marked by high rates in closed (92.6%) and single fractures (92.6%). Notably, infections (22.2%), smoking (7.4%), and hypertension history (7.4%) are common contributors. Despite ideal BMI in many cases (44.4%), nonunion still occurred. Conclusions: The study conducted at Dr. Mohamad Soewandhie General Hospital in Surabaya revealed a high prevalence of antebrachial nonunion fractures at 24.3%, particularly among young males. It was found that closed, single fractures were the most common type, with infections, smoking, and a history of hypertension being significant contributing factors. Interestingly, even individuals with an ideal BMI were still at risk for nonunion.
