Abstract

Rotasi klinik di bagian Psikiatri dapat memberikan tantangan berbeda bagi dokter muda (DM) yang nantinya dapat memengaruhi proses dan hasil pembelajaran serta pandangan mereka terhadap psikiatri. Penelitian ini bertujuan untuk mengeksplorasi pengalaman pembelajaran selama stase psikiatri pada dokter muda. Analisis dilakukan terhadap naskah refleksi diri yang dituliskan dokter muda Fakultas Kedokteran Universitas Jember (FK UNEJ) yang telah menyelesaikan stase psikiatri di RSD Dr. Soebandi Jember pada bulan Juli 2019-Juni 2021. Naskah dianalisis secara tematik untuk mendapatkan beberapa tema. Didapatkan 102 naskah refleksi diri yang ditulis oleh 27 DM laki-laki dan 74 perempuan. DM memiliki pandangan yang berbeda-beda terkait anggapan awal terhadap psikiatri/rotasi klinik di bagian psikiatri serta hal-hal yang dipelajari selama pelaksanaan stase baik secara personal maupun . Meskipun secara umum DM menganggap pelaksanaan stase sudah berlangsung baik, mereka berharap agar pembelajaran bisa dilakukan pada latar yang lebih variatif dan dilakukan secara lebih aplikatif. Peneliti selanjutnya dapat memfokuskan pembahasan pada topik tertentu dan melakukan kajian kualitatif secara lebih mendalam atau menggunakan metode kuantitatif dengan jumlah sampel lebih besar untuk meningkatkan kekuatan dan generalizability penelitian.

Abstract

Increased use of pesticides can have detrimental health consequences, one of which is chronic neurotoxicity. The symptoms include degenerative and neurobehavioral issues. Chronic neurotoxicity occurs through oxidative stress, inflammation, and neurotransmitter disturbances. This study aimed to determine chronic neurotoxicity and test malondialdehyde and cholinesterase levels as neurotoxicity biomarkers among agricultural workers in Wuluhan, Jember, Indonesia. The 60-person research sample was divided into two groups: agricultural and non-agricultural workers. The interview utilized a mini-mental score examination, Chan's questionnaire, and the Patient Health Questionnaire to analyze the cognitive impairment, Parkinsonism, and depressive symptoms. The examination of serum malondialdehyde levels was performed using the TBARS method and cholinesterase levels by photometric kinetic method at a biochemistry laboratory from October to November 2022. Results showed cognitive impairment and depressive symptoms in agricultural workers, as well as high levels of malondialdehyde and low cholinesterase levels. This study concludes the presence of chronic pesticide neurotoxicity among agricultural workers in Jember, Indonesia, and that malondialdehyde and cholinesterase levels might serve as biomarkers of pesticide-induced neurotoxicity.

Abstract

Introduction: Challenges in specialist medical education in Indonesia can increase stress, but the expression of stress may be hampered or appear as physical complaints. The behavior of self-prescribing medication often occurs among specialist medical education participants to deal with stress-related complaints. This study aims to analyze the relationship between stress and self-prescribing behavior among specialist medical education participants in Indonesia. Methods: This cross-sectional study involved 39 respondents who completed an anonymous online survey. The perceived stress scale (PSS-10) was used to measure stress levels, and questions about medication self-prescribing behavior were asked. Multinomial logistic regression was used to analyze the relationship between PSS scores and self-prescribing behavior. Results: Most respondents (74.4%) had PSS scores higher than age and gender norms. Antibiotics were the most frequently self-prescribed medication (92.3%). No association was found between demographic characteristics, total PSS score, and self-prescribing behavior. Conclusion: The high level of stress in specialist medical education participants may not be directly correlated with self-prescribing behavior. This may be related to Asian culture where psychological distress tends to manifest as somatic complaints.

Abstract

The most common problem in orthopedics that becomes the main cause of disability worldwide is musculoskeletal injuries. It may develop into chronic conditions that interfere with social function, quality of life, and productivity. The Return to Work (RTW) Rate is used for measuring the success of intervention. Provision of social support becomes the psychosocial interventions that may reduce the negative impact. This study aimed to learn whether social support may increase the RTW rate of musculoskeletal injury, by conducting a systematic review based on Preferred Reporting Items for System and Meta Analysis (PRISMA). Literature search using three databases, Springer, PubMed, ScienceDirect, and manual searching. The critical appraisal tool used was the Joanna Briggs Institute (JBI) checklist. There were seven included articles with high and medium quality. All studies were conducted in developed countries. Social support from supervisors and colleagues increases the chances of achieving partial and sustained RTW, and reduces the rate of interrupted RTW, while social support from family and friends can reduce the chances of achieving a partial RTW. More studies analyzing provision of social support to patients with other medical conditions should be made in the population of working age in developing countries, to assess for possible generalization of the results of the present systematic review.

Abstract

Introduction: Multidrug-resistant tuberculosis (MDR-TB) is a form of TB that shows resistance to both isoniazid and rifampicin. Globally, the number of new MRD-TB cases has continued to rise since 2020. Indonesia is positioned among the top ten countries with the highest incidence of MDR-TB worldwide. Specifically, Jember ranked second in the number of MDR-TB cases in East Java. This study aimed to assess the comparison between the administration of individual MDR-TB regimens and short-term regimens concerning anxiety levels in MDR-TB patients at Dr. Soebandi Regional General Hospital, Jember. Methods: This was an analytical observational study with a cross-sectional design. It was conducted at Dr. Soebandi Regional General Hospital, Jember, using interviews and medical record data from August to September 2023. The sample size was 69 participants, who were obtained through consecutive sampling. Analysis was performed using the Statistical Package for the Social Sciences (SPSS) and the Chi-Square test for group comparisons. Results: This study found that 64% of subjects received individual regimens. In the individual regimen group, 80% of subjects reported anxiety. Among the short-term regimen group, 60% of subjects showed no anxiety. The significance value of this study was 0.007 in the Chi-Square test for group differences. Conclusion:  There was a difference in anxiety levels between individual and short-term regimens among MDR-TB patients at Dr. Soebandi Regional General Hospital, Jember. Patients with anxiety were more common in individual regimens compared to short-term regimens.
