Abstract

Background: Stroke risk factors disparities in race-ethnic. Bataknese is the most stroke rates compared to non-Bataknese at H. Adam Malik General Hospital Medan. This may be due to genetics and lifestyles that have a link to increase stroke risk factors in Bataknese. Bataknese has a characteristic lifestyle that are more eating, typical foods also contain lots of cholesterol, and have a habit of drinking traditional alcoholic beverages.Subjects and Method: This was a cross-sectional study selected by consecutive sampling tech­niques, clinically proven stroke patients and computed tomography (CT) scan studied at H. Adam Malik General Hospital Medan from June to November 2018. This study aimed to know the difference of stroke risk factor between stroke patient of Bataknese and non-Bataknese.Results: This study recruited 70 samples consisting of 45 Bataknese and 25 non-Bataknese. There were no significant differences in risk factors for hypertension, dyslipidemia, heart disease, dia­betes mellitus, smoking, and obesity among stroke patients in the Bataknese and non-Bataknese. But the distribution of hypertension was higher in Bataknese (50%) compared non-Bataknese (30%) (PR=0.67, 95%CI= 0.19 to 2.40, p= o.755). Dislipidemia was higher in Bataknese (38.6%) compared non-Bataknese (18.6%) (PR=1.39, 95% CI= 0.52 to 3.71, p=0.692). Heart disease was higher in Bataknese (17.1%) compared non-Bataknese (8.6%) (PR=1.15, 95% CI= 0.37 to 3.57, p=1.000).  Diabetes mellitus was higher in Bataknese (17.1%) compared non-Bataknese (10%) (PR=1.10, 95% CI= 0.31 to 2.87, p=1.000). Smoke was higher in Bataknese (27.1%) compared non-Bataknese (14.3%)(PR= 1.59, 95% CI= 0.38 to 6.61, p=1.000). Obesity was higher in Bataknese (11.4%) compared non-Bataknese (4.3%) (PR=1.59, 95% CI= 0.38 to 6.61, p=0.735). There was significant differences in risk factors for alcohol consumption between stroke patients in the Bataknese and non-Bataknese, where alcohol consumption was higher in the Bataknese (15.7%) compared non-Bataknese (1.4%) (PR=7.77, 95% CI= 0.94 to 64.22, p=0.045).Conclusion: A significant difference in stroke risk factors between stroke patients in Bataknese and non-Bataknese was alcoholic consumption.Keywords: stroke, risk factor, Bataknese, Non-BatakneseCorespondence: Laura Panca Susila Tambunan. Abstract

Stroke is the third highest cerebrovascular disease in the world with high mortality and disability rate that is mostly dominated by ischemic stroke. Genetic factor that had been reported to have an indirect effect in increasing the incidence of ischemic stroke is ACE gene polymorphism. ACE gene polymorphism is characterized by the insertion marked by letter (I) or deletion marked by letter (D) on intron 16, chromosome 17. ACE gene polymorphism has drawn a lot of attention from scientists and had been reported to have an indirect effect in increasing the ischemic stroke incidence through pathogenesis of hypertension and atherosclerosis. In this study, 78 subjects of ischemic stroke consist of 43 subjects with hypertension and 35 subjects with normotension. I allele of ACE gene polymorphism was more dominant than D allele in hypertensive ischemic stroke patients (72.1% > 27,9%) and this dominance was also seen in the incidence of hypertension vs normotension (55.4% > 44.6%). However, the correlation of ACE gene polymorphism with the incidence of hypertension was not statistically significant when compared based on its genotype (p=0.280) and allele (p=0.948).

Abstract

Introduction: The Alberta Stroke Program Early CT Score (ASPECTS) is a scoring system assessed from CT scans to investigate the involvement of brain regions experiencing early ischemic changes. Lower scores are known to be associated with worse conditions and the occurrence of complications such as dysphagia. This study aimed to assess the relationship between ASPECTS scores and the occurrence of dysphagia in patients with acute ischemic stroke. Method: This cross-sectional analytical study was conducted at RSUP H. Adam Malik Medan from November 2022 to January 2023. ASPECTS scores were assessed based on CT scan results, while dysphagia was evaluated using the GUSS score. The Mann-Whitney U test was performed to assess the relationship between the two variables. Results: : A total of 34 subjects were included in this study. Among them, 11 (32.4%) subjects experienced dysphagia. The median ASPECTS score was 8 (range 3-9). The Mann-Whitney U test showed a significant association between ASPECTS scores and the occurrence of dysphagia (p<0.001). Conclusion: There is a significant relationship between ASPECTS scores and the occurrence of dysphagia.

Abstract

Introduction: Stroke is a global health problemwhose incidence is increasing every year throughout the world. Stroke can cause disorders of the autonomic system in sufferers including cardiovascular disorders. Several studies examining cardiovascular disorders after acute stroke using electrocardiography (ECG), one of which is the QT Dispersion. The aim of this study was to examine the relationship between the National Institute of Health Stroke Scale and the QT Dispersion. Method: This research is analytic correlation with cross-sectional design. The study sample consisted of 50 acute ischemic stroke patients. QT Dispersion were assessed based on the results of the ECG examination on the first day of treatment. Stroke outcome assessment using the NIHSS score was assessed on the first day of treatment. Results: The mean age of the 50 subjects is 57,28±12,216. The majority were male (64%), the majority were Batak (72%), 58% Subjects graduated fromsenior high school and the majority works as housewives 32%. The mean NIHSS value obtained was 8,74±6,599.81 and the mean QTd value was 142,40±73,250 ms. The most common infarct site in this study were basal ganglia (21%). There was a significant correlation between NIHSS scores and QTd in acute ischemic stroke patients (r= 0,668, p<0.05). Conclusion: There was a significant relationship between NIHSS scores and QT dispersion in acute ischemic stroke patients (p<0.05).

Abstract

Seizure yang terjadi setelah stroke dan tidak memiliki riwayat epilepsi sebelumnya disebut sebagai post stroke seizure (PSS). Post stroke seizure dapat dibagi menjadi early seizure dan late seizure. Post stroke seizure meningkatkan mortalitas pada pasien, disabilitas pada saat keluar dari rumah sakit, dan juga perpanjangan masa rawat inap di rumah sakit. Pasien dengan post stroke seizure menunjukkan hasil fungsional yang buruk (mRS >2). Mengetahui hubungan antara gambaran head CT scan dan elektroensefalogram (EEG) dengan luaran pada pasien post stroke seizure di RS Adam Malik Medan. Penelitian ini bersifat analitik observasional dengan metode penelitian secara kohort prospektif dengan sumber data primer yang diperoleh secara konsekutif dari semua pasien PSS yang dirawat inap di RS Adam Malik Medan dan telah dilakukan pemeriksaan head CT scan dan EEG. Luaran pasien PSS dinilai dengan skor mRS pada hari ke-14 sejak seizure. Terdapat 24 subjek penelitian yang memenuhi kriteria inklusi dan eksklusi yang berusia antara 18-74 tahun dan terdiri dari 18 subjek laki-laki dan 6 subjek perempuan. Terdapat hubungan antara gambaran head CT Scan dengan luaran klinis pasien post stroke seizure berdasarkan hasil uji chi square yang dilakukan dengan nilai p = 0,041(<0,05) untuk lokasi lesi dan p = 0,018 (<0,05) untuk luas lesi. Terdapat hubungan antara EEG dengan luaran klinis pasien post stroke seizure berdasarkan hasil uji Chi Square yang dilakukan didapati nilai p = 0,001 (<0,05). Terdapat hubungan bermakna antara gambaran Head CT Scan dan EEG terhadap luaran pada pasien.

Abstract

Infectious diseases, especially those transmitted by mosquitoes, are health problems that are often experienced by people in coastal areas. Cases of infection such as dengue hemorrhagic fever (DHF) and even malaria, were reported to be found in partner areas. The partner's location which is located on the beach with swamps around it allows breeding places for mosquitoes, especially Anopheles sp. which acts as the main vector of malaria and filarial. Breeding sites for Aedes aegypti and Aedes albopictus mosquitoes as dengue vectors are also possible to be found in partner locations because they are located not too far from the city of Medan and include tourist attractions. Cerebrocardiovascular disease, especially stroke and coronary heart, has high morbidity and mortality rates. This disease has almost the same risk factors because both are related to blood vessels, five main risk factors can be controlled, namely hypertension, diabetes mellitus, dyslipidemia, obesity, and smoking. The importance of public understanding of how to prevent and control these risk factors. Public Health Clinic Center as a front-line health service facility plays an important role in improving the health status of the community. One of the community service activities carried out by the Tanjung Beringin Health Center is counseling to cadres from community members this is related to stroke and coronary heart disease because these diseases are diseases that have a very close relationship and become a problem that requires proper and careful management. The risk factors for these two diseases, including hypertension, cause heart attacks, heart failure, and strokes and often lead to new DM in many cases. Due to the increase in the disease due to lifestyle changes, with this education, we can add to the public's knowledge to be responsive in conducting treatment and regular health checks.
