Abstract

Diabetes mellitus (DM) is the fifth most disease in Semarang, but the achievement of Minimum Service Standard (SPM) only reached 50.64% in 2017. The district with the highest morbidity in Semarang is South Semarang which has two health centers. Puskesmas Pandanaran has lower achievement than the other central health in South Semarang, which is 89%. The purpose of this study is to analyze the implementation of SPM for DM in Puskesmas Pandanaran. The research method is qualitative research with in-depth interviews. Research subjects include the head of puskesmas, the program in charge, the program implementer, and the community. The research was conducted in August-October 2020. The results showed that the implementation of SPM services is still constrained at the stage of logging and early detection of DM patients. Unfulfilled performance targets are affected by standard and policy objectives that are difficult to achieve due to differences in the amount of target with the actual population, lack of infrastructure resources and funding sources, there is no Standar Operational Procedure (SOP) in the implementation of SPM for DM patients, and community knowledge factors. The conclusion of this study is variable that affects the performance of SPM are standards and policy objectives, resources, characteristics of implementing organizations and the social environment of the community. Coordination between health services and health centers regarding the use of data as targets for achievement is required, addition and fulfillment of infrastructure and funds, creating SOPs, forming team to record DM patients, and giving education.

Abstract

In response to policy changes, the hospital as an organization is expected to establish a strategy to stay afloat in business competition. However, the health industry, especially hospitals, has certain determinants that are different from other industries. This article is a literature review that focuses on business strategy management, namely sustainable competitive advantage in the hospital industry. This review is aimed at managers nowadays in building and maintaining their hospital positions. In conclusion, changes in strategy can be made specifically according to the type of hospital and positioning results.

Abstract

Adolescent hypertension is emerging as a significant health challenge. Peer-led interventions utilizing engaging multimedia platforms have demonstrated the potential to enhance prevention behaviors among adolescents. However, there remains a gap in the literature comparing the effectiveness of multimedia intervention with traditional lecture-based approach in adolescent hypertension prevention in Indonesia. The aim of this study was to compare the traditional lecture-based approach and interactive multimedia intervention (e-module, animated videos, and infographics) on adolescents' knowledge, attitudes, and practices regarding adolescent hypertension prevention. A quasi-experimental study with pre-test and post-test control group study was conducted from June to December 2023 in two senior high schools in districts of Pedurungan and Kedungmundu, Semarang, Central Java, Indonesia. To prepare the multimedia model, the Analysis, Design, Development, Implementation, and Evaluation (ADDIE) framework and mapping intervention approach were employed to ensure effective decision-making at each step. A total of 100 high school students were involved, with 50 assigned to the intervention group and 50 to the control group. The intervention group received a multimedia model from the peer educators, while the control group received a traditional PowerPoint-based presentation from the community healthcare workers. The hypertension prevention knowledge, attitude, and practice were assessed using validated questionnaires with 20, 15, and 15 items, respectively. This study found that, after model implementation, significant improvements were observed in students’ knowledge, attitudes, and practices regarding hypertension prevention (all had p<0.001). Specifically, knowledge scores increased by 4.78 points, attitude scores by 10.64 points, and practice scores by 2.9 points. This study highlights that multimedia interventions could significantly improve hypertension prevention behaviors among adolescents by enhancing their knowledge, attitudes, and practices through engaging and interactive multimedia materials.

Abstract

Anemia pada wanita hamil sering disebabkan karena perubahan fisiologis saat kehamilan serta diperparah dengan keadaan kurang gizi. Anemia yang sering dijumpai pada kehamilan yaitu akibat kekurangan zat besi. Pengetahuan tentang anemia pada ibu hamil dapat ditingkatkan melalui informasi yang diberikan oleh tenaga kesehatan atau melalui pendidikan kesehatan. Salah satu faktor yang mempengaruhi proses pendidikan kesehatan yaitu sarana atau media yang dipakai untuk memberikan informasi. Media Motion Video Education (MVE) yaitu video animasi yang berisikan pemahaman tentang pengertian anemia, penyebab anemia, jenis-jenis anemia, gejala anemia, dampak anemia pada ibu dan janin, dan pencegahan anemia. Kebaruan penelitian ini karena meneliti tentang pengembangan media MVE untuk meningkatkan pengetahuan dan sikap inu hamil tentang anemia. Tujuan penelitian ini yaitu Mengembangkan Media Motion Video Education Sebagai Upaya Meningkatkan Pengetahuan dan Sikap Ibu Hamil tentang Anemia. Metode Penelitian menggunakan Research and Development (RnD) yaitu metode penelitian untuk menghasilkan produk tertentu dan untuk menyempurnakan suatu produk yang sesuai dengan kriteria dan standar dari produk yang dibuat. Sampel penelitian yang dihitung menggunakan Teknik Purposive sampling didapatkan sebanyak 44 ibu hamil. Hasil penelitian ini berdasarkan Analisis uji Wilcoxon terdapat perbedaan tingkat pengetahuan dan perbedaan sikap responden antara sebelum dan sesudah diberikan  edukasi sosialiasasi tentang anemia melalui media motion. Kesimpulannya penggunaan media motion dalam pemberian sosialiasi edukasi tentang anemia terbukti signifikan untuk meningkatkan pengetahuan dan sikap ibu hamil terkait anemia, sehingga masalah kesehatan akibat anemia dapat diminimalisir.Kata kunci: Anemia; Ibu hamil; MVE.

Abstract

Background: Strengthening the quality of primary health care is one way to improve public health. For this reason, an appropriate primary care assessment tool (PCAT) is necessary to evaluate and improve primary health care. Objective: This study aims to provide an overview of the development of primary care assessment tools. Methods: The study was conducted through a scoping review of articles written in English that were published in the PubMed, Scopus, Science Direct, and Semantic Scholar databases from 2010 to 2023. Results: A total of 10 selected articles describe the development of primary care assessment tools, namely PCAT Malawi, Vietnam PCAT-PE, Vietnam PCAT-AE, CR-PCAT (China), PCAT-TI (Tibet), UG-PCAT (Uganda), PCAT-C (China), PCAT-Facility (Spain), KS-PCAT (Standard Korean), PCAT-10 (Spain), and Colombia PCAT. Each tool has a different development focus according to the context of each country. Conclusion: This study provides an overview of the development of PCAT. In general, the development of PCAT in each country demonstrates different characteristics. The selection of a health service assessment tool must align with the needs of the element or aspect to develop. Abstract

The development of human resources (HR) is strongly influenced by nutritional factors. The impact of malnutrition on pregnant women leads to low birth weight babies (LBW), malnutrition in toddlers and even stunting. This study aims to determine whether there are differences in the perceptions of pregnant women with chronic energy deficiency on nutritional fulfilment in stunting prevention based on stunting cases in Semarang City. This type of research is quantitative research with analytical observational research using a comparative study approach. The data collection method in this study was interview using a questionnaire that has been prepared by the researcher based on the Health Belief Model theory. The sample in this study were pregnant women with Chronic Energy Deficiency (CED) who were in the health centre with the highest stunting cases and the health centre with the lowest stunting cases, a sample of 50 respondents was taken for each health centre with stunting cases using total sampling. Data were collected by meeting pregnant women with Chronic Energy Deficiency through pregnant women's classes and door to door accompanied by health workers. Based on the results of the analysis, the data were not normally distributed so the data analysis used the Mann-Whitney u test and the significance level (p = 0.05) The results showed that there were differences in the perceived barrier to action of pregnant women with Chronic Energy Deficiency (CED) on the fulfilment of nutrition in preventing stunting based on differences in stunting cases namely (p value 0.033), then other variables did not show differences.

Abstract

This study examines the readiness of public health laboratories in Indonesia's primary healthcare services, focusing on resource distribution, critical result reporting, and health policy implementation. A review of several studies reveals that laboratory readiness is limited by facility constraints, drug availability, and healthcare workers' understanding of critical result handling. Policies like BLUD and lean hospital approaches can enhance operational flexibility in laboratories. Recommendations are directed toward strengthening resource distribution and staff training to improve laboratory service quality in primary healthcare.

Abstract

Hipertensi merupakan suatu kondisi medis yang memerlukan perawatan jangka panjang. Dukungan sosial dan keluarga berperan penting dalam pengobatan hipertensi. Pada pengobatan hipertensi, dukungan sosial keluarga merupakan elemen penting yang membantu pasien untuk meningkatkan kualitas hidup dan mengurangi risiko komplikasi. Penelitian ini bertujuan untuk mengetahui dampak dari dukungan sosial dan keluarga terhadap kepatuhan pengobatan hipertensi pada pasien hipertensi. Penelitian ini menggunakan metode scoping review dengan menggunakan pendekatan PRISMA. Database yang digunakan pada penelitian ini adalah dari Google Scholar, Pubmed dan Proquest dengan seleksi artikel kurun waktu pada 5 tahun terakhir. Artikel ini menunjukkan bahwa dampak dukungan sosial dan keluarga mempengaruhi kepatuhan pengobatan hipertensi pada pasien hipertensi. Dukungan sosial keluarga memainkan peran penting dalam mengurangi tekanan psikologis dan meningkatkan kepatuhan pasien. Dukungan sosial dan keluarga dapat mempengaruhi kepatuhan pasien hipertensi. Dukungan sosial dan keluarga dapat membuat pasien hipertensi tetap termotivasi dan memperkuat kesejahteraan psikologis mereka dalam meningkatan kepatuhan pengobatan. Dukungan social dan keluarga merupakan komponen yang sangat bermanfaat dan sebagai faktor yang mendorong perubahan kepatuhan.

Abstract

The aim of this research is to find out the use of the TRIZ application to build a systematic innovation in solving problems in hospitals in an innovative way. Research method used literature review method with qualitative approach. The data obtained in carrying out this research is secondary data that comes from various studies. Data is taken from international-based articles with a research range from 2017-2023. The data obtained is known to be a total of 11 international journal articles spanning 2017-2023. The data were analyzed by identifying the existence of identities, methods, research results, and categories in the form of services, infrastructure, and facilities. Finally, the findings of this study can be seen that services have a total of 9 of 11 data literature, infrastructure has a total of 3 of 11 data literature, and facilities have a total of 3 literature of 11 data. So from when the three were totaled it was 15/11 the data obtained from the research results. In terms of services, infrastructure and facilities, the problems in hospitals can be solved innovatively and systematically using the TRIZ method.

Abstract

ABSTRACT A Public Health Laboratory (Labkesmas) is a health laboratory that provides clinical specimen examination and sample testing services to support disease prevention and control efforts and improve public health, adhering to WHO standards. This research is motivated by the importance of laboratory service requirements in the integration of primary health care services, where community health center laboratories serve as Level 1 Labkesmas in Pekalongan City. This study aims to analyze the implementation of the Level 1 Labkesmas implementation policy in Pekalongan City. Grindle's (1980) policy model theory is used as a research framework to assess the extent of implementation of the Level 1 Labkesmas implementation policy in Pekalongan City. This research method used a qualitative approach through in-depth interviews with respondents representing those who were "ready to implement a health laboratory" and those who were "not ready to implement a health laboratory." This research was conducted at Community Health Center (Puskesmas) units (UPT) throughout Pekalongan City, with a population of 14 respondents. The results showed that the policy for implementing a Level 1 Health Laboratory (Labkesmas) within the integration of primary health services was ready to be implemented by the Community Health Centers in Pekalongan City in accordance with health laboratory standards. Data analysis concluded that the policy for implementing a Level 1 Health Laboratory in Pekalongan City has been implemented in accordance with the mandate of the Ministry of Health's policy on implementing a health laboratory according to standards. Keywords: Readiness, Standards Compliance, Public Health Laboratory, Community Health Center.  ABSTRAK Laboratorium Kesehatan Masyarakat (Labkesmas) merupakan laboratorium kesehatan yang melaksanakan pelayanan pemeriksaan spesimen klinik dan pengujian sampel untuk menunjang upaya pencegahan dan pengendalian penyakit serta peningkatan kesehatan Masyarakat dengan mengacu pada standar WHO. Penelitian ini dilatarbelakangi adanya persyaratan pelayanan laboratorium dalam integrasi layanan kesehatan primer dimana laboratorium puskesmas menjadi Labkesmas Tingkat 1 di Kota Pekalongan. Penelitian ini bertujuan menganalisis implementasi kebijakan penyelenggaraan Labkesmas tingkat 1 di Kota Pekalongan. Teori kebijakan model grindle (1980) digunakan sebagai kerangka penelitian untuk menilai sejauh mana implementasi kebijakan penyelenggaraan Labkesmas Tingkat 1 di Kota Pekalongan. Metode penelitian ini menggunakan pendekatan kualitatif dengan wawancara mendalam dengan responden yang mewakili hasil wawancara “siap impelementasi labkesmas” dan “tidak siap implementasi labkesmas”. Penelitian ini dilakukan di UPT Puskesmas se-Kota Pekalongan dengan populasi seluruh puskesmas sebanyak 14 responden. Hasil penelitian yang diperoleh bahwa Kebijakan penyelenggaraan Labkesmas tingkat 1 dalam integrasi layanan kesehatan primer sudah siap dilaksanakan oleh Puskesmas di Kota Pekalongan sesuai standar laboratorium kesehatan. Hasil analisis data dapat disimpulkan bahwa kebijakan penyelenggaraan labkesmas Tingkat 1 di Kota Pekalongan sudah diimplementasikan sesuai dengan amanat kebijakan Kementerian Kesehatan tentang penyelenggaraan labkesmas sesuai standar. Kata Kunci: Kesiapan, Pemenuhan Standar, Laboratorium Kesehatan Masyarakat, Puskesmas.
