Abstract

Caesarean delivery is a fetal birth by performing an incision on the abdominal/laparotomy and uterine/hysterotomy. The proportion of caesarean deliveries in the city of Pekanbaru continues to increase both in government hospitals and private hospitals. Syafira Hospital is a private hospital with the highest proportion of caesarean deliveries from six other private hospitals in the city of Pekanbaru. Data from January to December 2017 the number of caesarean deliveries was 495 (56.4%) from 877 deliveries. The aim of this research is to know the determinant factors of delivery of caesarean at Syafira Hospital in Pekanbaru City. This study uses quantitative observational analytical method with design case control.This research was conducted from August to September 2018. Using secondary data collection, from medical records. The population consisted of cases and controls, with a sample of 203 cases of caesarean section delivery and 203 normal delivery controls. Data analysis was carried out in univariate, bivariate and multivariate with multiple logistic regression using prediction models. The results of the study revealed that determinant factors were maternal medical indications at risk 35 times for delivery by caesarean (95% CI = 12,970-95,924).It is advisable for the hospital to promote the advantages and disadvantages of each type of childbirth, the use of antenatal care for early detection of labor complications, and to optimize the role of health workers in the handling of maternal neonatal emergencies.

Abstract

Pria juga dapat mengalami keganasan akibat infeksi Human Papillomavirus (HPV) serta bertindak sebagai reservoir virus. Metode skrining HPV pada wanita telah terstandardisasi, namun belum ada standar metode skrining pada pria di Indonesia. Beberapa studi pada populasi pria di luar negeri menunjukkan potensi sampling pada daerah genitalia eksterna untuk skrining HPV. Tujuan: mengoptimasi metode skrining HPV secara molekuler pada pria. Metode: Responden adalah partner seksual wanita pansien kanker serviks di RSUD Arifin Achmad Provinsi Riau. Apusan dari glans dan batang penis diambil menggunakan nylon-flocked swab yang kemudian dimasukkan ke dalam 350µl viral transport medium terpisah. DNA diisolasi dari sampel yang kemudian dianalisis untuk mendeteksi gen human β-globin dan HPV. Hasil: Optimasi awal menunjukkan gen β-globin dapat terdeteksi dari hasil ekstraksi dengan kit Zeesan Viral RNA Extraction. Pita HPV hasil PCR dengan primer MY09 dan MY11 dapat muncul namun masih tipis. Simpulan: Studi awal ini menunjukkan bahwa apusan glans dan batang penis dapat digunakan untuk deteksi HPV secara molekuler pada pria, namun proses pengambilan sampel, ekstraksi DNA, dan PCR masih perlu dioptimasi.Kata kunci: apusan, glans, HPV, penis

Abstract

Objective: To evaluate the accuracy of platelet parameters—platelet count (PC), mean platelet volume (MPV), and platelet distribution width (PDW)—as predictors of preeclampsia severity. Methods:A case-control study was conducted on 153 pregnant women with preeclampsia at Arifin Achmad Hospital from January 2019 to December 2020. Platelet indices (PC, MPV, PDW) were compared between 61 patients with severe preeclampsia and 92 with non-severe preeclampsia. ROC analysis determined predictive accuracy, with sensitivity and specificity calculated for selected cut-off values. Results:Significant differences were found in PC (p=0.000), PDW (p=0.049), and MPV (p=0.012) between groups. ROC analysis showed PDW >12.3% had a sensitivity of 60.7% and specificity of 59.8%, while MPV >10.65 fL had a sensitivity of 62.3% and specificity of 64.1%. AUC values for PDW and MPV were 0.62 and 0.66, respectively, indicating weak predictive accuracy. Conclusion:Although platelet parameters differ significantly between preeclampsia severity groups, their predictive value is limited. They should not be solely relied upon for assessing preeclampsia severity. Akurasi Parameter Trombosit (PC, MPV, dan PDW) sebagai Prediktor Tingkat Keparahan PreeklampsiaAbstrak Tujuan: Penelitian ini dilaksanakan pada pasien preeklampsia untuk mengevaluasi akurasi parameter trombosit—jumlah trombosit (PC), volume trombosit rata-rata (MPV), dan lebar distribusi trombosit (PDW)—sebagai prediktor tingkat keparahan.Metode: Studi kasus-kontrol dilakukan pada 153 ibu hamil dengan preeklamsia di Rumah Sakit Arifin Achmad dari Januari 2019 hingga Desember 2020. Indeks trombosit (PC, MPV, PDW) dibandingkan antara 61 pasien dengan preeklamsia berat dan 92 pasien dengan preeklamsia tanpa gejala berat. Analisis ROC digunakan untuk menentukan akurasi prediktif, dengan perhitungan sensitivitas dan spesifisitas berdasarkan nilai cut-off tertentu. Hasil: Terdapat perbedaan signifikan pada PC (p=0.000), PDW (p=0,049), dan MPV (p=0.012) antara kedua kelompok. Analisis ROC menunjukkan bahwa PDW >12.3% memiliki sensitivitas 60.7% dan spesifisitas 59.8%, sedangkan MPV >10.65 fL memiliki sensitivitas 62.3% dan spesifisitas 64.1%. Nilai AUC untuk PDW dan MPV adalah 0.62 dan 0.66, menunjukkan akurasi prediktif yang lemah. Kesimpulan: Meskipun parameter trombosit menunjukkan perbedaan signifikan antara kelompok preeklamsia berat dan tidak berat, nilai prediktifnya terbatas. Parameter ini tidak dapat digunakan secara mandiri untuk menilai tingkat keparahan preeklamsia. Abstract

Endometrial cancer (EC) is the fourth most frequent gynecological cancers, and its incidence and mortality rates have increased over the last decade. Cytotoxic therapy with carboplatin and paclitaxel is the recommended first-line treatment for EC patients. However, the options for following therapy are limited. The latest advances in molecular studies have uncovered the nature of genetic alterations in EC, compelling methods for further research into the treatment of EC since they may disclose to tailored pharmacological therapy. The aim of this study to identify novel drug candidates in treating EC using genomics variants and biological pathway. The genomic variants of  EC were downloaded from cBioportal database. We established connection between the biological EC risk genes from cBioportal database and the DrugBank database. Finally, we used Connectivity Map (CMap) analysis to identify possible drugs whose mechanisms coincided with therapeutic targets and rank them based on the scoring system. We identified novel potential candidate drugs for EC, they are Bosutinib and Nilutamide which exhibit robust scores in the CMap analysis compare to paclitaxel. We also discovered BCR-ABL1 and AR as potential biomarker-driven therapy in EC. This study demonstrates the possibility of using genetic network analysis combined with bioinformatics to repurpose drugs for the treatment of EC. Further study will investigate the mechanisms of using BCR-ABL1 and AR targeting in the treatment of EC.
