Abstract

Background: Pityriasis sicca is a non-inflammatory skin disorder. The symptoms such as white or gray scales that accumulate on the surface of the scalp or in a localized place, peels easily and usually with itching. There are three main factors that cause Pityriasis sicca. Stress stimulates the body to increase sebum production in the sebaceous glands. Objective: To know the correlation between stress levels and incidence of Pityriasis sicca in the final year students of the Faculty of Medicine, Universitas Sumatera Utara class of 2017. Methods: This was an analytical study with a cross-sectional design, started from July 2020 until October 2020. Sampling was carried out using a total sampling technique. The data were obtained using univariate and bivariate statistical analysis. Results: Univariate analysis showed 102 respondents were at a normal stress level (40.3%) and there were 105 respondents (41.5%) who suffered from Pityriasis sicca, total samples were 253 respondents. From the bivariate analysis, it was found that there is a significant correlation between stress levels and the incidence of Pityriasis sicca. Conclusion: There is correlation between stress levels and the incidence of Pityriasis sicca in class 2017 students of the Faculty of Medicine, Universitas Sumatera Utara.

Abstract

Introduction: Wound healing involves complex interactions between cells and mediators that occur immediately after the wound occurs and depends on nutritional factors and wound closure. Snakehead murrel fish (Channa striata) extract has been extensively researched to speed up the wound healing process because of its high amino acid content. Patch tests were carried out to assess the allergen potential of the Channa striata extract biopolymer in the form of wound dressings. Methods: Experimental study of 40 subjects who had admit patch test with various consentration of Channa striata extract biopolymer. The patch test materials divided of 5 grups. They were aquadest solution, Eucheuma cottonii biopolymer, Channa striata extract biopolymer concentrations of 5.4 g/mL, 10.8 g/mL, and 21.6 g/mL. All subjects had been assessed for skin reactions that occured after 48 hours, 72 hours, and 96 hours. Results: A total of 40 subjects met the inclusion criteria and completed informed consent.  There was minimum allergen reaction (+) in slight subject in all grups except aquadest grup after 48 hours. The allergic reaction dissolve in 72 and 96 hours. An irritant reaction (IR) occurred in among all grups but indicated to the  hypafix plaster. Conclusion: Patch test containing biopolymer Channa striata extract showed a minimal allergic reaction

Abstract

Rhinophyma is the most severe and disfiguring manifestation of phymatous rosacea, characterized by progressive hypertrophy of sebaceous glands and connective tissue on the nasal skin. It often leads to both aesthetic and functional impairment. Various treatment options are available, with surgical excision considered the most definitive for advanced cases. This case report aims to highlight the clinical presentation, diagnostic approach, and the effectiveness of surgical excision combined with electrocautery as a treatment modality for rhinophyma. We report a case of a 49-year-old male presenting with progressive nasal enlargement, nodularity with sharply demarcated borders, an irregular surface, and a firm, slightly compressible consistency. Clinical and histopathological examination confirmed the diagnosis of rhinophyma. The patient underwent surgical excision followed by electrocauterization. The procedure was well-tolerated without intraoperative or postoperative complications. Follow-up showed satisfactory cosmetic improvement and no signs of recurrence. The management of rhinophyma varies depending on disease severity. In advanced cases, surgical excision remains the treatment of choice. It provides immediate debulking of hypertrophic tissue and restoration of nasal contour. Combined with electrosurgical techniques, this method offers good cosmetic and functional outcomes, minimal scarring, and a low recurrence rate. It addresses both functional obstruction and cosmetic disfigurement, contributing to improved quality of life.

Abstract

Pyogenic granuloma is a benign vascular tumor that commonly arises following minor trauma, irritation, or hormonal influences. Although frequently found on the gingiva, hands, or face, localization on the upper lip is considered uncommon. This case report aims to describe the clinical presentation, diagnostic features, and therapeutic outcome of pyogenic granuloma in an unusual location. A 16-year-old girl presented with a two-month history of a red, painless nodule on the upper left lip. The lesion initially appeared as a wound that gradually enlarged and occasionally bled following minor trauma. The patient had been using orthodontic braces for the past four months. Dermatological examination revealed a solitary erythematous papule measuring 0.3 cm with a stalk and crust. Histopathological examination revealed slit-like proliferative blood vessels and inflammatory infiltrates, confirming the diagnosis of pyogenic granuloma. The lesion was treated by shave excision followed by electrocautery. Clinical improvement was observed, with complete resolution of the lesion at the 10-day follow-up. This case underscores the importance of clinical vigilance in recognizing pyogenic granuloma at atypical anatomical sites and demonstrates the effectiveness of shave excision combined with electrocautery as a treatment modality.

Abstract

ABSTRACT Background: Xanthelasma palpebrarum is a most common form of xanthoma characterized as lesion with yellowish colour, flat, soft, and mostly found in medial corner of eyelids. Most cases of xanthelasma are associated with hyperlipidemia. Case Reports: A male aged 37 presented with yellow patches on both upper eyelids since ±2 years ago. Vital sign within normal limid. Dermatological examination found yellowish plaques with clear boundaries sized 0.5 cm located on medial canthus of the supraorbital dextra and sized 1 cm located on medial canthus of the supraorbital sinistra. Laboratory test showed that hypercholesterolemia. The patient then underwent excision surgical procedure. Discussion: Xanthoma is a plaque consisted abnormal deposition of lipid and foam cells. Generally, this condition is found in aged 40–60 years old. The condition is found more in females than males, on superior palpebrae as the most common location. Diagnosis is established based on history and dermatological evaluation. The aim of xanthelasma management is mostly cosmetic-related. Elliptical or fusiform excision is the most method applied. Conclusion: A 37 years old male with xanthelasma palpebrarum on bilateral upper eyelids with hypercholesterolemia was treated with surgical excision. The surgical wound appeared well later on the 6th days and a month post-surgery. Latar Belakang: Xantelasma palpebrarum merupakan bentuk xantoma yang paling umum dengan lesi berwarna kekuningan, datar, lunak, dan sebagian besar terletak di sudut medial palpebra. Sering dikaitkan dengan hiperlipidemia. Laporan Kasus: Seorang laki-laki 37 tahun datang dengan bercak kekuningan pada kedua sudut kelopak mata atas sejak ±2 tahun lalu. Tanda vital dalam batas normal. Pemeriksaan dermatologis ditemukan plak berwarna kekuningan, dengan diameter berukuran 0,5 cm berbatas tegas pada regio kantus medial supraorbital dekstra dan 1 cm berbatas tegas pada regio kantus medial supraorbital sinistra. Pemeriksaan laboratorium menunjukkan kolesterol total 215 mg/dL, HDL 45 mg/dL, LDL 126 mg/dL, dan trigliserida 110 mg/dL. Pasien kemudian direncanakan untuk dilakukan bedah eksisi. Diskusi: Xantoma adalah plak yang terdiri dari endapan lipid abnormal dan sel busa. Umumnya kondisi ini muncul pada usia 40-60 tahun, ditemukan lebih banyak pada perempuan dibandingkan dengan laki-laki dan palpebra superior sebagai lokasi tersering. Diagnosis umumnya ditegakkan berdasarkan anamnesis dan pemeriksaan dermatologis. Penatalaksanaan xantelasma ditujukan untuk mengatasi gangguan kosmetik. Eksisi elips merupakan metode yang paling banyak digunakan. Kesimpulan: Seorang pria 37 tahun dengan xantelasma palpebrarum pada palpebra superior bilateral dengan hiperkolesterolemia ditangani dengan bedah eksisi. Luka tampak baik saat kontrol 6 hari dan satu bulan pasca tindakan.   Â
