Abstract

Exclusive breastfeeding has a major impact on a baby's growth and development, as breast milk contains immune system proteins and high levels of bactericides, and breastfeeding can reduce the risk of a baby dying. At the 65th World Health Assembly, member countries of the World Health Organisation (WHO) set a target for at least 50% of babies under six months to be exclusively breastfed by 2025. With massage stimulation that stimulates the release of endorphins and oxytocin, as well as suggestions from the SPEOS method, it will create feelings of calm and strong confidence in the mother, which will influence the let-down reflex. The combination of endorphin massage, oxytocin and suggestive massage can lead to a smooth flow of breast milk, which will help the mother to meet the nutritional needs of the baby for its growth and development. The aim of this research is to determine the success of the let down reflex and the smooth production of breast milk (ASI) after a combination of endorphin massage, oxytocin and suggestive massage intervention. This research uses a post-test only control design, which is a type of true experimental design methodology. The study population consisted of thirty mothers after caesarean section on the first and second day in the delivery and postpartum room of Dr. RSUD. H. Slamet Martodirdjo Pamekasan. The sampling technique used in this study was purposive quota sampling. Data analysis in this study used bivariate and multivariate analysis, namely T-test. In this research design, there were 2 groups, namely the intervention group, namely post caesarean mothers who were given SPEOS intervention and the control group, namely post caesarean mothers who were not given SPEOS intervention. The results of statistical tests using a paired sample t-test on the control group showed results of sig-2 tailed = 1,000 > 0.05, meaning that there was no difference in the let down reflex and smoothness of breast milk production in the control group. The results of statistical tests using a paired sample t-test on the treatment group showed results of sig-2 tailed = 0.000 < 0.05, meaning that there was a difference in the let down reflex and smoothness of breast milk production in the treatment group after receiving the SPEOS intervention, so the hypothesis was accepted, meaning that there was a combination effect of SPEOS (stimulation). endorphin massage, oxytocin and suggestive massage) on the success of the let down reflex and the smoothness of the mother's breast milk production after caesarean section at RSUD Dr H. Slamet Martodirdjo Pamekasan in 2023.

Abstract

Stunting is not only a problem of malnutrition, but has a major impact on the quality of future human resources. To overcome this, a comprehensive approach involving various sectors is needed, one of which is a community-based convergence approach. The implementation of a community-based convergence strategy in Pamekasan Regency aims to reduce the stunting rate to achieve the Zero Stunting target. This strategy will strengthen cross-sector collaboration, actively involve the community, and pay attention to health, nutrition, sanitation and education interventions for families at risk of stunting. This strategy is in line with the Sustainability Development Goals (SDGs) target of “Zero Hunger”, namely eliminating all forms of malnutrition by 2030, Presidential Regulation No. 72 of 2021 concerning the acceleration of stunting reduction. East Java Province ranks 25th with the highest prevalence of stunting, while Pamekasan Regency has reached 6% by 2023.  The achievement of reducing the prevalence of stunted children has not been evenly distributed to areas in Pamekasan Regency. Therefore, a holistic and multisectoral approach is needed, including the government, academics, industry, society and the media. Collaboration from the five parties can be optimally beneficial if all parties understand their roles and contributions in efforts to accelerate stunting reduction. That way the Zero Stunting target can be achieved. In addition, some of the policies and implementation strategies in this policy brief can be used as a basis for determining future policies.

Abstract

The “zero Hunger” target of the Sustainability Development Goals (SDGs) is to eliminate all forms of malnutrition by 2030 and reduce the prevalence of stunting in children by 50% (United Nations, 2015). In 2022, the Pamekasan Health Office succeeded reducing the incidence of stunting in a significant time, namely from 38.6% in 2021 to 8.1% in 2022 (SSGI, 2022). The next urgency is to maintain this positive achievement and ensure that there are no more additional cases of stunting, by looking at the results of the evaluation of the implementation of the strategy, namely optimizing the active role of the community to support stunting prevention actions (LKJIP Health Office, 2021). Thus, one of the programs was created, namely the Revitalization Of PIC Stunting in 10 stunting village locations in Pamekasan Regency 2023. The purpose of this activity is to improve the knowledge and skills of PIC Stunting in efforts to prevent and manage reporting of stunting cases. This activity was attended by 23 participants consisting of 17 PIC Stunting, 4 people from the Pamekasan Health Office, and 2 people from DP3AP2KB. The core activity was in presentation of material from analytical speakers in the form of problems, strategies to accelerate stunting reduction, socialization of the SOBAT GESIT application, discussion, and closed with the provision a module " Langkah Mewujudkan Generasi Emas Bebas Stunting". This PIC Stunting Revitalization Program produced outputs in the form of a joint commitment in efforts to accelerate stunting reduction in Pamekasan Regency.
