Abstract

Diabetes mellitus is a metabolic symptom caused by an increase in blood glucose due to impaired insulin secretion, insulin resistance, or both. Blood glucose levels above the normal limit have an influence on the quality of life of diabetes mellitus patients. The study was conducted with a cross sectional design. Data collection was carried out prospectively during the period from May to June 2022. The sample in this study was patients with diabetes mellitus in one of the primary health care centers in Banjarmasin City. Quality of life data was collected using the EQ-5D questionnaire, while blood glucose levels data was obtained from medical record patient. A total of 75 samples participated in this study. The results showed that the mean fasting blood sugar level was 162,95 mg/dL. The mean value of the sample's quality of life score based on the EQ-5D index was 0,739. There is a significant correlation between blood glucose levels and the quality of life of patients with diabetes mellitus. The results of the correlation indicated that the lower the blood glucose level of patients with diabetes mellitus, the higher their quality of life.

Abstract

Background: Pasak Bumi (Eurycoma longifolia Jack) is an endemic plant that has been utilized as a medicinal agent by communities in Kalimantan and Sumatra for an extended period. The entirety of E. longifolia, inclusive of its leaves, stems, and roots, has been utilized as a medicinal agent. The primary compound present in E. longifolia is eurycomanone, which is commonly found in the roots of the plant. Objective: This study aims to optimize the extraction of eurycomanone from E. longifolia roots using imidazolium-based ionic liquids (1-butyl-3-methylimidazolium chloride [BMIM][Cl]) through microwave-assisted extraction (MAE), applying response surface methodology (RSM) for statistical meodeling. Methods: A Box-Behnken design was employed to evaluate the effects of three independent variable including [BMIM][Cl]concentration, extraction time, and solid-liquid ratio on eurycomanone content. Quantification was performed using a validated HPLC method, and model performance was assessed via analysis of variance (ANOVA). Results: The concentration of [BMIM][Cl] significantly influenced eurycomanone content (p<0.001), while extraction time and solid-liquid ratio showed no significant individual effects. The optimized condition yielded 0.4061 µg/mg of eurycomanone, closely matching the predicted value (0.3835 µg/mg). The model demonestreated good fit (R2 = 0.8135) and non-significant Lack of Fit (p = 0.6566), indicating reliable predictive capability. Conclusion: MAE using [BMIM][Cl] is an effective and environmentally friendly approach for extracting eurycomanone. The developed model provides a robust framework for process optimization and potential scale-up in phytopharmaceutical applications.

Abstract

Geriatric patients with congestive heart failure (CHF) face an elevated risk of medication errors due to physiological changes, polypharmacy, and comorbidities. Integrated Medicine Management (IMM) is a multidisciplinary model designed to minimize drug-related problems through structured interventions during admission, hospitalization, and discharge. This study aimed to validate a medication error checklist developed for the IMM model, specifically for hospitalized older adults with CHF. A descriptive, quantitative design was applied, with expert-based content validation. A purposive sample of eight healthcare professionals, including physicians, clinical pharmacists, and senior nurses, was recruited. Each checklist item was assessed for relevance using a 4-point Likert scale. Content validity was evaluated using the Item-Level Content Validity Index (I-CVI) and the Scale-Level CVI (S-CVI/Ave). All nine items obtained I-CVI values of ≥0.875, with seven items achieving a full consensus (I-CVI = 1.00). The overall S-CVI/Ave was 0.97, indicating excellent content validity. The checklist integrates global safety frameworks, including the STOPP/START criteria, the WHO Medication Without Harm initiative, ISMP's high-alert medication guidance, and The Joint Commission's reconciliation protocols. Core components address reconciliation, therapy duplication, high-risk drugs, and discharge education. The validated checklist demonstrates strong content validity and clinical relevance for detecting medication-related problems in hospitalized geriatric CHF patients. Integration into digital platforms such as MINE (Medicine IN gEriatric) may enhance interprofessional collaboration, reduce preventable errors, and promote safer pharmacotherapy across care transitions.
